Efficacy of Transcranial Alternating Current Stimulation (tACS) for Treating Hallucinations: A Systematic Review and Meta-analysis.

PURPOSE OF THE REVIEW: Transcranial alternating current stimulation (tACS) is a non-invasive technique that modulates brain oscillations and may help treat hallucinations when conventional treatments fail. This systematic review evaluated the efficacy of tACS in reducing hallucinations in psychiatric disorders. COLLECTION AND ANALYSIS OF DATA: A systematic search of PubMed, Scopus, EMBASE, and clinical trial registries was conducted. Studies included adult psychiatric populations receiving tACS with hallucinations measured using standardized scales. Three randomized controlled trials (RCTs) w

Clinical characteristics of auditory verbal hallucinations by spatial location: objective and subjective comparisons in schizophrenia.

BACKGROUND: The clinical characteristics of the spatial location of auditory verbal hallucinations (AVHs) has shown inconsistent results. This study compared AVHs characteristics by spatial location using both objective and subjective assessments. METHODS: Eighty-five schizophrenia patients scoring 3 + on the hallucination item (P3) of the Positive and Negative Syndrome Scale for Schizophrenia (PANSS) were included. The spatial location of AVHs was categorized using the clinician-rated Psychotic Symptom Rating Scales - Auditory Hallucinations (PSYRATS-AH) and the self-report Hamilton Schizophr

What's Written Down.

"What's Written Down" explores the lived experience of schizophrenia through Nari, a young Korean American woman in San Francisco whose auditory hallucinations blur the boundaries between Korean and English, past and present. The story examines how intergenerational mental illness intersects with immigrant silence, cultural stigma, and the complexities of medication adherence. It asks what it means to coexist with an illness that one's family could never name, and whether recovery can exist without cure.

Virtual Reality for the Treatment of Auditory Hallucinations in Schizophrenia Spectrum Disorders: a Systematic Review and Meta-Analysis.

BACKGROUND AND HYPOTHESIS: Auditory hallucinations (AHs) are debilitating symptoms of schizophrenia spectrum disorders (SSDs) associated with several negative outcomes. AHs are often resistant to existing pharmacological and psychological interventions. Virtual reality (VR) has emerged as a promising intervention for AHs. This systematic review and meta-analysis aimed to assess the effectiveness of VR interventions in treating AHs in SSDs. STUDY DESIGN: A comprehensive literature search was conducted on Embase, APA PsycINFO, and MEDLINE via the Ovid Database. Studies with a randomized control

Severe Nondiabetic Hypoglycemia After Risperidone Initiation in an Adult With Schizophrenia: A Case Report and Systematic Review.

Hypoglycemia is a rare but potentially serious adverse effect of second-generation antipsychotics (SGAs). Most studies have focused on SGA-induced hyperglycemia and metabolic syndrome, while hypoglycemia in non-diabetic adults remains underreported. We describe a case of severe hypoglycemia following risperidone initiation, demonstrating diagnostic challenges and safety concerns. A 41-year-old woman with treatment-resistant schizophrenia, Asperger's syndrome, and attention-deficit/hyperactivity disorder was admitted for worsening commanding auditory hallucinations and persecutory delusions. Brugmansia suaveolens Abuse in a Patient With Multiple Substance Use Disorders: A Case Report.

Anticholinergic pharmacological agents are commonly misused because of their euphoric effects, increased energy, improved mood, and enhanced social interactions. Some herbal products with anticholinergic properties can also be abused for their psychoactive effects. One such ornamental plant is Brugmansia suaveolens, which contains alkaloids including atropine and scopolamine and can produce toxic effects. This case report describes a male patient with Brugmansia suaveolens abuse and coexisting multiple substance use disorders. The patient reported consuming a decoction of Brugmansia suaveolens

Persistent auditory hallucinations despite hearing aid use in bilateral sensorineural hearing loss without evidence of psychosis.

Auditory hallucinations are frequently linked to the onset of psychotic disorders, especially when accompanied by delusions or disorganized thinking. However, hallucinations may also arise from non-psychiatric causes. Hearing loss is often underrecognized as a contributing factor, since reduced auditory input can produce perceptual experiences that mimic psychosis. This case report presents a woman in her 50s with bilateral sensorineural hearing loss who experienced persistent auditory hallucinations for more than 3 years. She maintained insight, normal daily functioning, and exhibited no othe

Investigating the acceptability and validity of a novel VR paradigm that simulates auditory hallucinations.

There is a growing use of Virtual Reality (VR) technologies in psychosis research and intervention. Validated VR simulations of psychotic-like experience have the potential to allow controlled manipulation of stimuli, reproducibility, and to isolate mechanisms of change in an ethical research framework. Although initial developments appear to be safe, qualitative reporting of participants' experiences of VR simulated psychotic-like experiences is lacking. This paper proposes and demonstrates an ethically grounded framework for evaluating the acceptability and validity of an immersive VR experi

Is emotion regulation a mechanism of change in virtual reality-assisted therapy for auditory hallucinations? Secondary analyses of the CHALLENGE randomized clinical trial.

Virtual reality (VR)-assisted avatar therapy has shown promising effects for reducing voice-related distress and overall severity of persistent auditory verbal hallucinations (AVH) in psychosis, yet the mechanisms underlying these improvements remain unclear. Emotional processes may play a role during therapeutic encounters with simulated voices, but it is unknown whether treatment effects involve broader changes in emotion regulation. We conducted secondary analyses of the CHALLENGE randomized clinical trial (N = 270), comparing a seven-session immersive VR-based avatar therapy with enhanced

Cognitive profiles in psychosis: comparing those experiencing uni- or multisensory versus multimodal hallucinations.

Introduction: A vast body of cognitive research in psychosis has focused on auditory hallucinations, though more recent studies are turning towards other sensory modalities. This study aimed to compare the cognitive profile of persons experiencing uni- or multisensory versus multimodal hallucinations. Methods: Participants with primary diagnosis of a psychotic disorder were subdivided into those experiencing uni- or multisensory (UMS; n = 31) versus multimodal (MM; n = 28) hallucinations relative to non-clinical controls (NC; n = 32). Cognitive assessment comprised the MATRICS Consensus Cognit

The Multisensory Hallucinations Schedule to assess hallucinations across multiple sensory modalities in psychosis: A pilot validation study.

OBJECTIVE: Hallucination studies in psychiatry have typically focused on the auditory domain, despite mounting evidence that non-auditory hallucinations are prevalent, and elicit distress and functional interference. Proponents have called for more in-depth phenomenological enquiries into hallucinations across multiple sensory modalities, but there are no comprehensive, validated assessment tools to date. This study aimed to document the design and pilot validation of such a transdiagnostic instrument - the Multisensory Hallucinations Schedule. METHODS: Leveraging off a precursor measure, the

Predictors of postoperative visual and auditory hallucinations in hypertensive vs. non-hypertensive cardiac surgery patients: insights from the prospective multicenter VAACS study.

BACKGROUND: Hallucinations are common yet under-recognized after cardiac surgery and contribute to postoperative morbidity. Hypertension (HTN), the most prevalent comorbidity in surgical candidates, may alter susceptibility through impaired cerebrovascular autoregulation. Whether HTN modifies predictors of postoperative visual and auditory hallucinations remains unclear. METHODS: In the prospective, multicenter VAACS study, 1,332 adults undergoing CABG or valve surgery were stratified by hypertension (HTN, n = 517; non-HTN, n = 815). Visual and auditory hallucinations within 7 days were asses
