BACKGROUND: Parkinson's disease (PD) patients often report seeing persons or animals rather than objects but this phenomenon remains poorly understood. Here, we use three experimental tasks to confirm such observation and to explore its cognitive mechanisms.
METHOD: Fourteen PD patients with visual hallucinations (PD-VH), 14 PD patients without visual hallucinations (PD-NVH) and 14 controls with similar cognitive performance were tested using ambiguous stimuli. Ambiguous stimuli were morphs in which visual features from faces and flowers were melted together (Experiments 1 and 2) and a black

Charles Bonnet syndrome (CBS) is characterised by the presence of visual hallucinations following visual loss in many patients. The neuropathophysiology of CBS is poorly understood. We used electroencephalography (EEG) in individuals with frequent hallucinations in order to identify changes in neural activity that co-occur with hallucination onset. We found reduced α power in occipital electrodes at the onset of the hallucinations compared to offset (p < 0.05), suggesting that intermittent periods of low endogenous alpha may create the neural conditions for hallucination to emerge in CBS patie

BACKGROUND: Delirium occurs in about one fourth of hospitalized older adults and is associated with multiple adverse outcomes. Despite efforts to improve the prevention and treatment of delirium, it remains a burden for patients, their relatives, and healthcare professionals. Currently, little is known about how people experience delirium episodes. METHODS: A sample of 77 inpatients who had been recently treated for delirium were recruited from acute geriatric care units at four hospitals. A semi-structured interview was conducted to explore memories and feelings associated with the delirium e

OBJECTIVE: Dementia with Lewy bodies (DLB) is the second most common neurodegenerative dementia after Alzheimer's disease but remains underdiagnosed because of its heterogeneous symptomatology. Network analysis may capture interrelationships among symptoms, potentially improving diagnostic precision. The authors applied network analysis to a cohort of patients with DLB to map symptom connections, identify central features, and explore clustering patterns.
METHODS: Clinical records of 107 patients with a confirmed DLB diagnosis from Brugmann University Hospital, Brussels, were retrospectively

Emerging evidence suggests that hallucinations may arise because of an over-reliance on prior knowledge during perception. While best established in psychosis-spectrum illness, data also support the presence of this abnormality in other hallucination-prone neuropsychiatric illnesses that vary in their association with disruption of sensory circuits. In this piece, we ask whether an over-weighting of expectations may be conceived of as a compensatory response to degraded incoming sensory information. We make the case that visual hallucinogenesis across a wide array of neuropsychiatric disorders

Cerebral venous sinus thrombosis (CVST) is a rare type of stroke with a variable presentation. Without a prompt diagnosis, subacute presentation can delay treatment with devastating results. Our case presents an unusual clinical pattern of a young patient presenting with audio-visual hallucinations who was diagnosed with CVST. A 26-year-old man with no neurological or psychological history presented with audio-visual hallucinations, transient uncontrolled movements of his left upper extremities, and right-sided headache. Physical examination was nonfocal, and he demonstrated normal awareness,

Pediatricians and all other pediatric primary care clinicians are likely to encounter youth who report symptom concerns of psychosis, such as auditory or visual hallucinations. It is essential for clinicians to be comfortable evaluating and triaging these symptoms, including assessment of co-occurring safety concerns, to ensure access to appropriate treatment. Due to limited specialty mental health resources nationwide, pediatricians may also be tasked with initiating medications or referring directly to evidence-based psychosocial interventions in the community. This article reviews different

Altered lipid metabolism has been implicated in neurodegenerative disorders; however, its relevance to dementia with Lewy bodies (DLB) remains incompletely characterized. This study examined associations between plasma lipid profiles, lipid-related inflammatory indices, and clinical and neurodegenerative features in drug-naive patients with DLB, with comparisons to age-matched healthy controls. Plasma levels of high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol did not differ significantly between patients with DLB and controls, whereas triglyceride (TG) level

INTRODUCTION: Patients with psychiatric concerns often present to an emergency department (ED) for medical evaluation prior to inpatient psychiatry placement. One diagnosis to consider prior to disposition is autoimmune encephalitis (AIE). This report describes a pediatric patient who presented with psychiatric symptoms that required inpatient admission and workup to diagnose a rare form of AIE.
CASE REPORT: A 16-year-old female with no known past medical history presented as a transfer from an outside hospital for medical evaluation of two days of auditory and visual hallucinations. Initial

We herein report an 82-year-old man with post-stroke epilepsy (PSE) who presented with persistent complex visual hallucinations (CVH) involving past episodic scenes. Electroencephalography revealed epileptic discharges in the right posterior temporal and parietal regions. The symptoms resolved with levetiracetam and were maintained with zonisamide. A literature review suggested that CVH may occur when epileptic activity spreads to non-dominant temporal or parietal regions. In conclusion, CVH may reflect visual memory and retention and can be accompanied by psychiatric symptoms. Improved recogn

ObjectiveWe aimed to present a patient who developed a severe chorea after orthopedic surgery, plausibly related to multiple anesthetic agents.CaseAn otherwise healthy 11-year-old girl developed choreiform movements and distressing behaviors (screaming, visual hallucinations) 24-36 hours after ankle-fracture surgery. With urgent stabilization needed and haloperidol not yet at an effective dose, continuous midazolam infusion provided prompt control. Other potential causes of chorea were excluded: brain magnetic resonance imaging and cerebrospinal fluid were normal; routine biochemistry and comp

Alien hand syndrome is an uncommon yet recognized complication of ischemic stroke. It is characterized by involuntary yet seemingly purposeful limb movements that occur without the patient's volitional control. The syndrome is most commonly associated with injury to the corpus callosum or adjacent medial frontal regions, where interhemispheric motor integration takes place. We report the case of a 50-year-old woman with multiple vascular risk factors who developed an acute infarction of the right corpus callosum. She presented with difficulty initiating movement, confusion, transient visual ha

Visual mental imagery (VMI) refers to the ability to "see with the mind's eye", in the absence of adequate sensory input, while visual hallucinations (VH) involve perceiving stimuli in their absence. This review investigates the relationship between VMI and VH across neurodegenerative and psychiatric conditions, focusing on Parkinson's disease (PD), Alzheimer's disease (AD), Dementia with Lewy bodies (DLB), and schizophrenia (SCZ). A relationship between VMI and VH was identified, with greater VMI vividness associated with increased VH occurrence in all conditions, except for DLB, for which no

BACKGROUND/AIMS: The emotional response to Charles Bonnet Syndrome (CBS) (visual hallucinations in individuals with sight loss) is associated with negative affect, suggesting a link between psychological measures and hallucination characteristics. This study set out to investigate whether the association extends to a broader range of hallucination attributes and psychological measures, taking into account clinical factors likely to influence such associations.
METHODS: 70 participants with self-reported CBS completed an online survey assessing hallucination attributes of frequency, duration,

Anti-GAD65 encephalitis-like other autoimmune encephalitis conditions-poses a significant diagnostic challenge; its heterogeneous neuropsychiatric phenotypes mimic primary psychiatric disorders. The broad pathophysiological spectrum, ranging from limbic dysfunction to catatonia, underscores the need for systematic approaches that integrate immunotherapy with neuromodulation when standard interventions fail. We present a 78-year-old woman with no prior psychiatric history who developed acute-onset catatonia with visual hallucinations. Neurological evaluation revealed markedly elevated anti-GAD6

BACKGROUND AND OBJECTIVES: Alzheimer's disease pathology (ADP) and Lewy body pathology (LBP) are traditionally associated with distinct cognitive profiles. However, growing evidence highlights the role of behavioral and psychological symptoms of dementia (BPSD) in shaping clinical presentations. The combined influence of cognitive and behavioral symptoms across neuropathologically confirmed ADP, LBP, and mixed AD-LBP has not been systematically examined. This study aimed to identify clinically meaningful subtypes by jointly analyzing cognitive performance and BPSD profiles in individuals with

Visual hallucinations are common non-motor symptoms of Parkinson's disease (PD) that substantially impair quality of life, particularly in advanced stages. However, the neural substrates underlying visual hallucinations in PD remain incompletely understood. Here, we report a rare case of a 72-year-old right-handed man with PD whose visual hallucinations completely resolved after a right occipitoparietal subcortical hemorrhage. He developed PD at 58 years of age and later experienced wearing-off, dyskinesia, cognitive decline, visual hallucinations, and a false sense of presence. At 72 years of

IntroductionNeuropsychiatric symptoms are often considered late manifestations of dementia and can be neglected during early clinical assessments. Besides visual hallucinations, less is known about other neuropsychiatric symptoms in mild dementia with Lewy bodies (DLB). We aimed to 1) describe neuropsychiatric symptoms and associated caregiver distress in mild DLB; 2) describe neuropsychiatric symptoms stratified by dementia stage within the included participants.MethodsIndividuals with probable DLB and their caregivers underwent clinical and neuropsychiatric assessments. Charles Bonnet syndrome (CBS) is characterized by complex visual hallucinations in visually impaired individuals who maintain intact cognitive function. Despite significant progress in understanding this condition, the precise neural mechanisms underlying CBS remain incompletely understood. This review synthesizes current evidence regarding the pathophysiology of CBS, with particular emphasis on emerging neurobiological models that extend beyond simple cortical hyperexcitability. Recent neuroimaging, neurophysiological, and computational modeling studies suggest that CBS hallucinations may ari
