Polyethylene terephthalate (PET) microplastics (MPs) are pervasive environmental contaminants with documented human exposure, transporting across intestinal epithelium into the circulatory system and accumulating in multiple organs. This study demonstrates that digestive transformation confers a surface modification on PET MPs, altering their bio-identity and initiating a multilevel pathogenic cascade leading to intestinal dysfunction. Using physiologically relevant particles derived from commercial bottles and a simulated gastrointestinal tract, digested PET MPs inhibit the GLUT2 causing intr

The management of chronic obstructive lung diseases, particularly severe asthma, chronic obstructive pulmonary disease (COPD) and non-cystic fibrosis bronchiectasis, is complicated by frequent overlap syndromes such as asthma-bronchiectasis overlap and bronchiectasis-COPD overlap syndrome. These overlapping phenotypes are characterized by severe symptoms, frequent exacerbations, accelerated lung function decline and increased mortality, driven by a common, destructive endotype: persistent, neutrophil-dominant airway inflammation. This inflammation is fuelled by the overactivity of neutrophil s

INTRODUCTION: It is believed that anticholinergics alleviate tremor and rigidity in Parkinson's (PD) by counteracting a relative cholinergic overactivity in motor regions, but this remains unclear. This study aimed to investigate possible cholinergic alterations in tremor dominant (TD) PD that may back use of anticholinergics.
METHODS: Twelve PD patients [six with TD and six with non-tremor dominant (nTD) phenotype] and six healthy controls (HC) underwent cholinergic positron emission tomography with [11C]-donepezil. Participants were assessed with the Movement Disorder Society-Unified Parkin

OBJECTIVES: In this study, we aimed to investigate the impact of vibegron, a β3-adrenergic receptor agonist, on lower urinary tract dysfunction in a diabetic rat model.
METHODS: Female rats were categorized into three groups: non-diabetic controls (N), streptozotocin-induced diabetic rats receiving vehicle (D), and diabetic rats treated with vibegron (DV). In the DV group, vibegron was administered orally at 30 mg/kg/day starting 7-8 weeks after diabetes induction. At 8 weeks post-induction, voiding assays, urinary 8-hydroxydeoxyguanosine (8-OHdG) measurements, cystometry, and analysis of bla

BACKGROUND: Sacral neuromodulation (SNM) is a treatment used to manage urinary symptoms of both overactive bladder and non-obstructive urinary retention. The use of SNM in managing these symptoms has yielded positive results, contributing to a reduction in symptoms and an improvement in quality of life (QoL) for patients. This article aims to report on the electrical neurostimulation of sacral nerves to treat idiopathic non-obstructive urinary retention.
CASE DESCRIPTION: A 27-year-old female presented with chronic non-obstructive idiopathic urinary retention, using intermittent bladder cathe

The autonomic nervous system (ANS) is a central regulator of cardiovascular function, coordinating involuntary control of heart rate, vascular tone, and blood pressure through its sympathetic (SNS) and parasympathetic (PNS) subdivisions. The SNS mediates the "fight or flight" response via catecholamines, increasing heart rate, contractility, and vasoconstriction, whereas the PNS promotes restorative processes through acetylcholine, decreasing heart rate and enhancing vasodilation. Nitric oxide further modulates vascular tone and autonomic balance, serving as a key neuromodulator. Assessment of

Hypertension is highly prevalent among patients with chronic kidney disease (CKD), contributing significantly to cardiovascular morbidity and progressive renal decline. This overview explores the intricate pathophysiologic mechanisms driving hypertension in renal insufficiency, including volume overload, renin-angiotensin-aldosterone system (RAAS) activation, sympathetic overactivity, and vascular dysfunction. Diagnostic challenges such as white-coat hypertension and the underuse of ambulatory monitoring are discussed, along with the importance of volume assessment and target organ evaluation.

Hypertension remains a major public health concern globally. Accumulating evidence suggests that dietary phosphate (Pi) and fibroblast growth factor 23 (FGF23), a phosphaturic hormone, are involved in blood pressure regulation. Experimental studies have shown that excess Pi consumption, largely from inorganic Pi used as a preservative or flavor enhancer in processed foods, and increased FGF23 may contribute to vascular abnormalities, thereby promoting hypertension. Importantly, recent animal studies have demonstrated that peripheral FGF23 can cross the blood-brain barrier and stimulate FGF rec

Sudden cardiac death in young individuals with structurally normal hearts represents a critical unresolved clinical challenge and typically occurs in patients with inherited arrhythmia syndromes due to cardiac channelopathies. Catecholaminergic polymorphic ventricular tachycardia (CPVT) can cause fatal arrhythmias triggered by adrenergic stimulation. Therapeutic interventions primarily target cardiac myocytes (CMs) despite robust clinical evidence demonstrating the life-saving efficacy of cardiac sympathetic denervation. To understand this therapeutic paradox, we developed human induced plurip

Polycystic ovary syndrome (PCOS) is intricately linked to both reproductive and metabolic health, and its pathogenesis remains unclear. Previous studies suggest that exposure to endocrine disrupting chemicals (EDCs) may contribute to PCOS. However, there is still a lack of systematic researches and analyses on this topic. In this study, we investigated the association between PCOS and EDC exposure and established a mechanistic link between the two by analyzing hormone levels. The meta-analysis pointed out that the levels of EDCs in patients with PCOS were significantly higher than those in the

The sympathetic nervous system plays a pivotal role in the pathophysiology of hypertension. Although the contribution of the sympathetic nervous system to an elevation of blood pressure is well established, the determinants of persistent sympathetic overactivity remain incompletely understood. This review summarizes the findings of recent basic research that have expanded the understanding of sympathetic regulation in hypertension, highlighting emerging mechanisms and providing insights into the development of novel therapeutic strategies.

BACKGROUND: Severe lower urinary tract dysfunction (LUTD) in neurologically and anatomically normal children is uncommon and frequently underdiagnosed. When severe, functional voiding disorders may closely mimic obstructive or reflux pathology, leading to diagnostic errors, unnecessary invasive procedures, and potential risk to the upper urinary tract.
CASE PRESENTATION: We present three pediatric cases (aged 3-10 years) referred for recurrent febrile urinary tract infections, incontinence, or acute urinary retention in the absence of neurological or structural abnormalities. Urodynamic evalu

BACKGROUND: Hyperactivity of the orexin system has been implicated in hypertension, yet the long-term impact of orexin 1 receptor (OX1R) overexpression in the paraventricular nucleus (PVN), a key brain region involved in arterial blood pressure (ABP) regulation, remains unclear. This study examined whether chronic OX1R overexpression in the PVN alters cardiovascular, neuroendocrine, and oxidative functions.
METHODS: Adult male Sprague-Dawley rats received bilateral PVN injections of AAV2-OX1R or control virus AAV2-GFP. Their ABP and heart rate (HR) were monitored for eight weeks via radiotele

BACKGROUND: Sympathetic overactivity is associated with hepatic steatosis. Renal denervation (RDN) is an approved treatment for uncontrolled hypertension through sympathetic modulation; however, its hepatic effects are unknown. This study aimed to assess the effects of RDN on noninvasive tests for hepatic steatosis.
METHODS: This single-center study included patients with uncontrolled hypertension and cardiometabolic comorbidities undergoing RDN (n=32) or a sham procedure (n=10). Noninvasive tests for hepatic steatosis, including the hepatic steatosis index (HSI) and fatty liver index (FLI),

Background: Obstructive sleep apnea syndrome (OSAS) is associated with sympathetic overactivity, intermittent hypoxia, and increased vascular resistance, leading to cardiovascular morbidity. Electrical cardiometry (EC) is a novel, non-invasive technology that continuously measures hemodynamic parameters such as systemic vascular resistance (SVR), systemic vascular resistance index (SVRI), cardiac output (CO), and cardiac index (CI). The aim of this study was to compare SVR and SVRI values, measured by EC, between patients with OSAS and age- and sex-matched healthy controls. Methods: In this re

Diabetic kidney disease (DKD) is a major microvascular complication of diabetes mellitus and the leading global cause of end-stage renal disease. While chronic hyperglycemia and classic metabolic pathways such as protein kinase C activation, advanced glycation end product (AGE) accumulation, and renin-angiotensin system overactivity elucidate many features of DKD, they do not fully account for its complex inflammatory pathology. Recent studies have highlighted neutrophil extracellular trap (NET) formation, or NETosis, as a critical mechanism linking metabolic stress to immune dysregulation in

Metabolic health and physical performance rely upon skeletal muscle adaptation that is a result of exercise. Recent advancements in high-throughput sequencing and functional genomics have successfully identified a vast landscape of exercise-responsive circRNAs, providing critical insights into the molecular complexity of muscle adaptation. While these studies have established a foundational framework for understanding the circRNA-RBP axis, there are serious issues related to current research.
