[Clinical course following a neonatal E.E.G. recording reported as severely abnormal (author's transl)].

The purpose of this study was to evaluate the prognostic value of neonatal E.E.G. tracings in children born at term. The clinical course of 45 children was followed and related to E.E.G. abnormalities reported during the first 5 days of life. Essentially the findings confirmed those previously reported by others. However some differences were noted: paroxysmal tracings were not associated with a poor clinical state, and moderately abnormal tracings (the prognostic significance of which has never been defined) led on sometimes to a severe encephalopathy. We wish to stress certain aspects of our

Integral storage and voiding reflexes. Neurophysiologic concept of continence and micturition.

It is a common clinical misconception to regard the spinal micturition reflex center as fundamentally overactive and dependent on cerebral inhibition. Initiation and cessation of micturition is simplistically viewed as a manifestation of voluntary withdrawal and resumption of inhibitory corticospinal "regulation''. This view is in conflict with basic neurophysiologic experimental data. Actually, the organization of the micturition reflex is extremely complex. It is affected by multiple sources of facilitative and inhibitory influence, peripheral as well as central. During the past half century

Overactivity of the integral storage and voiding reflexes in the juvenile urinary incontinence syndrome.

The physiologic importance of 12 integral storage and voiding reflexes has not been generally recognized and appreciated by clinicians. Pathologic overactivity of reflexes No. 9 and No. 10, the urethrodetrusor facilitatory reflexes, appears to be a very common factor in the etiology of the juvenile incontinence syndrome. Mild but urodynamically significant obstructive lesions of the distal urethra are a common predisposing factor to chronic urethritis and are potentially remediable by simple surgical procedures. Cystometric as well as symptomatic improvement is commonly observed postoperativel

Adrenergic blockade in subarachnoid haemorrhage.

Over the last 15 years progressive interest has developed in the relationship between subarachnoid haemorrhage, sympathetic nervous system over-activity and the genesis of cerebral artery spasm. Following on from earlier work, vide infra, suggesting that adrenergic blockade might influence the course and prognosis of patients with subarachnoid haemorrhage, two double blind trials have recently been completed in Southampton. The first trial attempted to establish whether or not a single intracarotid injection of phenoxybenzamine improved the mortality and late morbidity of patients with subarac

Behavioral characteristics of children with chronic nonspecific diarrhea.

The authors obtained early case histories of 16 children with chronic nonspecific diarrhea and 16 control subjects and found that the index subjects had a significantly higher frequency of sleep problems, crying and irritability, digestive problems excluding diarrhea, overactivity, resistance to discipline, and family history of gastrointestinal problems. They conclude that these behavioral characteristics represent temperamental traits based on physiological factors, perhaps an imbalance of autonomic nervous system function. They speculate that chronic nonspecific diarrhea may occur in associ

Gout and the regulation of purine biosynthesis.

Overproduction of purine nucleotides de novo is the cause of hyperuricemia in a substantial portion of the gouty population. Specific enzyme abnormalities--deficiency of hypoxanthine-guanine phosphoribosyltransferase (an enzyme of the purine "salvage" pathway) and overactivity of 5- phosphoribosyl-1-pyrophosphate (PP-ribose-P) synthetase--result in hyperuricemia, and are associated with increased de novo purine synthesis and increased intracellular concentrations of PP-ribose-P. The latter is a common substrate for the first enzyme of the de novo pathway (phosphoribosyl amidotransferase) and t

Angiotensin II blockade in normal man and patients with essential hypertension. Blood pressure effects depending on renin and sodium balance.

1) Saralasin was administered to 9 normotensive volunteers and 13 patients with essential hypertension after sodium depletion and sodium repletion. 2) In standing normotensive volunteers, angiotensin II inhibition induced significant hypotension if previously a cumulative sodium loss of at least 160-200 mEq had been induced. 3) In patients with essential hypertension, saralasin infusion induced either blood pressure reduction, no change or even significant blood pressure increase, depending on the prevailing state of sodium balance. 4) Following vigorous and prolonged sodium depletion induced

Progressive facial hemiatrophy (Parry-Romberg syndrome).

Three cases of progressive facial hemiatrophy are presented. In all three cases there is evidence of localised scleroderma or morphea in association with the facial hemiatrophy. This would seem to support the contention that the two disorders are closely related. In two cases, ocular complications are prominent and in one Raynaud's phenomena provide clear asymmetrical dilatation of the lateral ventricle suggest that there may be a central rather than a peripheral cause for the sympathetic overactivity.

Effect of cardioselective beta-blockade on heart function and chest pain in acute myocardial infarction.

Systolic time intervals and the a/H ratio were recorded in 20 patients with uncomplicated acute myocardial infarction over a period of five days. The initial high heart rate and systolic blood pressure and the short PEP and ICT indicating a sympathetic overactivity were spontaneously normalized during the first week of infarction. LVET was reduced indicating a fall in stroke volume and the a/H ratio was unchanged at the high levels suggestive of elevated preload or LVEDP. In 10 patients with acute myocardial infarction and recurrent chest pain recordings on noninvasive parameters were made bef

The inexhaustible beta cell.

Repeated intensive pancreatic beta-cell stimulation was carried out in 42 subjects, comprising 22 normal controls, 10 mild to "severe" maturity-onset diabetics, and 10 chronic pancreatitis patients. Each subject received 75 gm. oral glucose twice and 1 mg. glucagon plus 0.5 gm. tolbutamide intravenously three times at short intervals. Each of the three combined stimuli caused almost equivalent marked spikes of insulin release in all experimental groups. The total calculated output of insulin was equivalent to the total daily insulin output in normal subjects. Pancreatitics and those with sever

Influence of prostaglandins on plasma glucagon levels in the rat.

The effect of PGE1and PGA1 intravenous infusion (2/mug/min) on plasma glucose and glucagon levels was investigated in normal, sympathectomized and propranolol-treated rats. PGE1 infusion significantly increased glucose and glucagon levels, while PGA1 had no effect. Since the dose of PGE1 used in this study was able to reduce the arterial blood pressure by about 20%, the possibility that PGE1 acted indirectly through a reflex sympathetic overactivity was tested. The increases in plasma glucagon induced by PGE1 occurred also in sympathectomized or in beta-blocked animals. Thus, it was possible t

Activity of sympathetic nerves in heart during cardiopulmonary bypass in patients.

The sympathetic nerve activity in heart during cardiopulmonary bypass was studied in 20 patients. The rate of release of dopamine-beta-hydroxylase from cardiac nerves was used as an indicator. It was found that the thoracotomy alone does not represent such a stimulus which would lead unambiguously to sympathetic overactivity in heart. Cardiopulmonary bypass represents, on the other hand, an intensive sympathomimetic stimulus, as it was demonstrated by significnat increase of coronary A-V difference negativity of dopamine-beta-hydroxylase as early in 15th minute of its duration. The washout of

Continuous recording of local myocardial function in normal and ischemic myocardium.

Experiments in anaesthetized open chest dogs are described in which intramyocardial pressure was determined by two methods, both suitable for long time experiments. By means of a Fogarty catheter in connection with a normal transducer the intramyocardial pressure can be measured. With this method the pressure gradient from epicardial to endocardial muscle layers is demonstrated. Insertion of catheter tip manometers leads only to small trauma of the myocardium. This method allows the determination of systolic and enddiastolic tissue pressure together with the dp/dtmax and dp/dtmin. Measurements

[Bronchospasm, a sign revealing pulmonary embolism].

As a clinical feature, and sometimes as a feature of the special mechanical tests of ventilation, bronchospasm has been found by the authors to be associated with pulmonary embolus. Bronchospasm was found to be the commonest feature in the 45 patients studied. The sudden onset of bronchospasm must be regarded as strong evidence for pulmonary embolus, especially in patients with a past history of bronchitis. The mechanism of these asthma-like features is discussed. The authors impugn the adrenergic system both to explain the bronchial overactivity of the patients with pulmonary embolus, and to

Pathogenesis of essential hypertension.

It is proposed that essential hypertension is an exaggeration of the tendency for blood-pressure to rise with age, that it results from a repeated sequence in which a small rise in pressure, possibly resulting from an autonomic nervous overactivity, produces changes in the kidney which maintain the rise of pressure and become the basis for a further rise.

Bromocriptine in Huntington chorea.

Following a recent report that apomorphine hydrochloride alleviates the involuntary movements of Huntington chorea, we have investigated another dopamine receptor agonist, bromocriptine, in this disease. A double-blind crossover study in six patients showed that rather than improving chorea, bromocriptine induced an exacerbation. This finding supports the view that choreatic movements correlate with overactivity in dopaminergic systems.

Results in treating 210 patients with detrusor overactivity incontinence of urine.

Eighty (72.7 per cent) of 110 patients with detrusor overactivity (in controlled and uncontrolled series) showed improvement with parasympatholytic drug (propantheline bromide or dycyclomine HC1) therapy. Twenty per cent of patients on placebo therapy showed improvement. When standard surgery improved bladder neck funnelling, detrusor overactivity was improved in 77.3 per cent of cases. Surgery failed to improve detrusor overactivity in 54.5 per cent of patients who showed no significant bladder neck funnelling. Bladder neck funnelling appears to be a cause of detrusor overactivity in some wom

The relationships between obsessional personality, obsessions in depression, and symptoms of depression.

The relationships between obsessional personality, obsessions in depression, and symptoms of depression were investigated by means of a retrospective study of case notes and item sheets. One hundred and sixty-eight cases of depression, aged 20 to 29 years, were rated for obsessional personality as defined by Ingram (1961). The presence of previous obsessions, of obsessions in depression and of eight symptoms of depression was assessed from the item sheets. Obsessional personality was found to be significantly associated only with a decreased frequency of objective apathy, although it seemed to

Pulmonary oedema in meningococcal meningitis.

Two cases of meningococcal meningitis complicated by pulmonary oedema are described. The pulmonary arterial wedge pressure was raised in the one case studied. Profound sympathetic over-activity may be the cause of the pulmonary oedema occurring in this situation. If this is so, adrenergic blockade would appear to be a rational approach to therapy.
