Developmental Convergence Across Diverse Genetic Architectures in Childhood Apraxia of Speech.

Childhood apraxia of speech (CAS) is a neurodevelopmental disorder of speech-motor planning and programming, associated with literacy, academic, and psychosocial difficulties. We performed whole-exome sequencing in 11 families comprising 21 affected and 15 unaffected individuals with phenotyping spanning speech, language, cognition, literacy, and motor function. We identified 60 prioritized variant observations representing 22 unique variants across 20 genes with family-based segregation analysis assessed in informative relatives; 5 arose de novo . Our cohort included multiple affected sibling

Effortful Eyelid Opening With Enhanced Mechanoreceptor Stretch in the Superior Tarsal Muscle Accelerates Inferred Microsaccade Frequency and Velocity, Exacerbating Reflexive Frequent Blinking: A Report of Two Cases.

To maintain eyelid opening and arousal without proprioceptive or visual fading, microsaccades of fast-twitch fibers (FTFs) in the levator palpebrae superioris muscle (LPSM) and the global layer of the superior rectus muscle (GLSRM) continually stretch mechanoreceptors in the superior tarsal muscle (STM). This stretch generates proprioceptive signals that induce reflex contractions of LPSM slow-twitch fibers (STFs) and regulate arousal via the mesencephalic trigeminal nucleus (MTN) and locus coeruleus (LC). Concurrently, counteracting microsaccades of FTFs in the global layer of the inferior re

What does it take to grasp a tool? A fine-grained multiple-case analysis of hand-tool and tool-object interaction errors in apraxia.

Apraxia of tool use is a neuropsychological syndrome characterized by difficulties in selecting or using familiar tools that cannot be better explained by primary motor, sensory, attentional, or language deficits. The neuropsychological literature indicates that tool use is a complex, multifaceted ability relying on heterogeneous cognitive processes and neural substrates. Patients may commit errors at different stages of the grasp-to-use sequence, including hand-tool interaction errors (e.g., holding tools upside-down), tool-object association errors (i.e., incorrect matching of the functional

Progressive Cognitive Decline and Pyramidal Signs in a Patient With a Novel Homozygous c.395A>T; p.Lys132Met Mutation in CHCHD2.

Biallelic CHCHD2 variants are rare. We report a consanguineous man with a novel homozygous CHCHD2 c.395A>T (p.Lys132Met) variant who developed progressive cognitive decline, apraxia, oculomotor impairment, and pyramidal signs without parkinsonism. Clinicians should consider CHCHD2 in familial dementia with pyramidal signs, even when motor parkinsonism is absent.

Heidenhain Disease: Challenging Diagnosis of a Creutzfeldt-Jakob Variant.

Creutzfeldt-Jakob disease (CJD) is a rare disorder caused by prions, characterized by rapid deterioration of cognitive and neurological functions. The Heidenhain variant is an uncommon form in which visual symptoms are prominent at onset due to involvement of the occipital cortex, leading to a delayed diagnosis. We present the case of a 67-year-old woman who initially exhibited decreased visual acuity, followed by difficulties in reaching for and identifying objects, experiencing optic apraxia, psychomotor slowing, perseverative behavior, and impaired judgment. Brain magnetic resonance imaging

MZI-switched dual-input SOI-AWG interrogator for neural-network-assisted FBG interrogation.

A dual-input silicon-on-insulator (SOI) arrayed waveguide grating (AWG) fiber Bragg grating (FBG) interrogator, enabled by an on-chip Mach-Zehnder interferometer (MZI) switch, is proposed and experimentally demonstrated. The device integrates a polarization beam splitter (PBS), a thermo-optic MZI, and an asymmetric dual-input AWG. The MZI enables on-chip selection of two AWG input ports, generating complementary spectral responses without increasing the number of output channels. Due to fabrication-induced non-idealities in SOI-AWG responses, conventional fitting-based interrogation methods su

Gesture Impairments in Schizophrenia: Distinct Roles of Negative Symptoms and Formal Thought Disorder.

BACKGROUND AND HYPOTHESIS: Gesture impairments are increasingly recognized as a core feature of schizophrenia, apparent across different domains and gesture categories. However, the potential differential effects of specific symptom domains on distinct types of gestures are unknown. The current study aimed to investigate whether specific domains of negative symptoms (NS) and formal thought disorder (FTD) relate to different aspects of gesture performance in patients with schizophrenia. STUDY DESIGN: Gesture accuracy was assessed using the well-established test of upper limb apraxia, which exa

Assessing Limb Apraxia in Quebec: Normative Study of the Brief Battery for the Evaluation of Praxis in Healthy French-Speaking Adults.

OBJECTIVE: This study presents normative data for the Batterie Brève d'Évaluation des Praxies (BBEP), a brief screening tool developed to assess symbolic gestures, pantomimes, and imitation of meaningless gestures-three domains commonly impaired in limb apraxia. While the BBEP was originally validated in a French population, no normative data have been available for French-speaking adults in Quebec. Given that cultural, linguistic, and demographic factors may influence gesture production and interpretation, the current study aimed to generate region-specific reference values to support accurat

Refining the phenotypic spectrum of PNKP-related microcephaly: a study of 27 new patients.

BACKGROUND: Biallelic pathogenic variants in PNKP are associated with microcephaly and early-onset seizures (MCSZ), ataxia with oculomotor apraxia type 4 and Charcot-Marie-Tooth disease type 2B2. METHODS: We describe the clinical and neuroimaging features of 27 new patients with PNKP variants. All patients presented with early-onset seizures, congenital microcephaly and intellectual disability. In addition, we compared our results with data in the literature. RESULTS: Twenty-five patients presented with the classic MCSZ phenotype, while two showed a more severe clinical phenotype. The brain

What Is in a Label? The Importance of Clinical Specificity and Challenges of Diagnostic Evolution in Progressive Speech-Language Disorders.

PURPOSE: This viewpoint addresses the complexity of the nomenclature of progressive speech-language disorders, which often exists at the intersection of descriptive symptoms, clinical syndromes, and underlying neuropathology. Using primary progressive apraxia of speech (PPAOS) as an example, we explore how overlapping diagnostic frameworks employed by speech-language pathologists, neurologists, and other clinicians and researchers can lead to different diagnostic labels that may evolve throughout disease progression. These discrepancies and changes can complicate patient understanding, clinica

Comparing Word Elicitation Strategies for Individuals With Apraxia of Speech and Aphasia.

INTRODUCTION: Following a stroke, some individuals experience aphasia, apraxia of speech (AOS), or both. When these disorders are severe, it can be challenging to determine whether the observed deficits stem from disruptions in linguistic-symbolic processes, as seen in aphasia, or from impairments in motor planning, as characteristic of AOS. Sentence completion may be a strategy for improving word production that may facilitate both symbolic-linguistic and motor processes for persons with acquired aphasia and AOS. AIMS: To evaluate word production in individuals with aphasia and AOS using two

Childhood motor speech disorders: who to prioritise for genetic testing.

The aetiology of childhood motor speech disorders of dysarthria and apraxia has been poorly understood. Recent evidence suggests a moderate genetic contribution for these rare and severe speech disorders. To date, however, no studies have examined genetic diagnostic yield for childhood apraxia of speech (CAS) and dysarthria in a clinical setting. Here, we used a clinically accredited genomics pipeline to investigate genetic diagnostic yield and variables predictive of a genetic diagnosis in a tertiary hospital speech clinic. A cohort of 153 children (range 2;7-16;5 years, 42 female) ascertaine

[Polypharmacy in patients with neuropsychiatric symptoms].

Dementia, delirium, and depression are the main geriatric psychiatric syndromes, and their prevalence is increasing significantly due to demographic aging. At the same time, multimorbidity and polypharmacy lead to increased interaction rates and a higher frequency of side effects, as well as reduced adherence. In Germany, the number of dementia cases is projected to rise from the current 1.8 million to 2.8 million by 2050. The most common etiologies are Alzheimer's disease and vascular dementia. Progressive cognitive and motor function loss often results in apraxia and dysphagia, which complic

Ideational and conceptual apraxia by cerebral infarction in the left basal ganglia and right pons: a case report.

We report a rare and uncommon case of ideational apraxia (IA) and conceptual apraxia (CA) in a 70-year-old woman after concurrent cerebral infarctions of the left basal ganglia (BG) extending to the corona radiata and the right pons. The patient abruptly developed difficulty operating devices such as the car's gear shift and smartphone. The brain MRI on admission disclosed infarction in the area involving the left BG and right pons. During rehabilitation therapy, she exhibited features of IA and CA, including sequencing failures when using technological devices and a profound loss of functiona

Reliability of Lexical Stress Transcription in Childhood Apraxia of Speech.

PURPOSE: Lexical stress errors are a reported characteristic of childhood apraxia of speech (CAS) in American English-speaking children. However, the ability to reliably identify stress errors may vary given several factors. This study examines the interrater reliability of lexical stress accuracy ratings derived from phonetic transcriptions. METHOD: Fifty-six General American English-speaking participants with CAS aged 9-16 years engaged in picture-naming and multisyllabic word imitation tasks at up to four different time points. Some participants (n = 37) engaged in face-to-face data collec

Underreported Demographic Factors in Apraxia of Speech Treatment Studies: Recommendations for Inclusive Research.

PURPOSE: While acquired apraxia of speech (AOS) has been the focus of intervention research for decades, little information is reported about the demographic representation of study participants. This article examines the extent to which published AOS treatment studies report participants' age, sex/gender, race/ethnicity, language background, and socioeconomic status; identifies critical gaps in the literature; and considers implications for representativeness. This information supports the important call to capture these patient factors systematically and thereby advance equity in research an

Diagnosing Apraxia of Speech in Children and Adults When There Is No Gold Standard: A Scoping Review About the Diagnostic Process.

PURPOSE: Childhood apraxia of speech (CAS), stroke-induced apraxia of speech (AOS), and progressive apraxia of speech (PAOS) cannot be diagnosed with an error-free gold standard test or measure. Reference standards are sensible approximations but are vulnerable to bias. In this scoping review, we characterize the diagnostic process for reference standards that have been used in recent diagnostic literature and evaluate the extent to which bias-reducing strategies have been implemented. METHOD: We extracted data about the diagnostic process for studies of speech characteristics in CAS, AOS, an
