Novel LAMA1 Mutations in a Pedigree With Poretti-Boltshauser Syndrome: Implications for Hypomyelination.

BACKGROUND: Poretti-Boltshauser syndrome (PBS) is an autosomal recessive disorder caused by biallelic pathogenic variants in the LAMA1 gene, typically presenting with cerebellar dysplasia and fourth ventricle abnormalities. While neurological and ocular manifestations are well recognized, an association between LAMA1 variants and central nervous system (CNS) dysmyelination has not been previously established, and the underlying mechanisms remain unexplored. CASE PRESENTATION: We report two pediatric siblings from a single family with novel compound heterozygous LAMA1 variants [c.505C>T (p.Arg

Limb apraxia and active inference in the visuomotor pathways.

This study presents a unified neurocomputational account of apraxia within the visuomotor system using the active inference framework. In this framework, the brain encodes a generative model of the causes of sensory observations and continually reduces the surprise (or, more formally, variational free energy) associated with these observations. Our goal is to illustrate how the neuroanatomical and neurofunctional organization of the visuomotor system are emergent consequences of this minimization under the appropriate generative model, while apraxia arises from incorrect parameterizations of t

Differences in non-word repetition, oral-motor performance and auditory discrimination in children with childhood apraxia of speech (CAS) and speech motor delay (SMD).

Non-word repetition (NWR) is a frequently used task in speech and language assessments. This study investigated NWR performance in children with childhood apraxia of speech (CAS) and speech motor delay (SMD), examining its relation to auditory discrimination (AD) and oral motor performance. Participants were 54 children with persistent speech-sound disorders (SSD) (31 with CAS and 23 with SMD) and 39 children with typical speech development (TSD) aged 6-16 years. Assessments of word naming, NWR, AD, and oral-motor performance were conducted. Percentage consonants correct (PCC), and percentage

Activity-dependent development of vocal circuits in the neonatal rodent forebrain.

Vocal communication is fundamental for social interaction across species, yet the neural mechanisms that shape vocal circuit development remain poorly understood despite their relevance to neurodevelopmental disorders. Here, we investigate vocal circuit development in mice using isolation-induced ultrasonic vocalizations (USVs) in neonates. An activity-tagging approach identifies the ventromedial prefrontal cortex (vmPFC) as a cortical region strongly activated during USV emission. We find a predictable temporal correlation between vmPFC activity and USV emission using in vivo fiber photometry

Frontotemporal dementia: does structural MRI-based clustering match clinical syndromes?

BACKGROUND: Frontotemporal dementia is an umbrella term that encompasses several clinical syndromes with impaired behavioral, language, and motor functions. These syndromes show considerable overlap in clinical features and imaging patterns. Therefore, there is a need to investigate the syndromic heterogeneity in FTD using unbiased data-driven approaches. METHODS: We used data-driven clustering analysis of structural magnetic resonance imaging (MRI) data on 400 patients with clinical FTD diagnoses [behavioral variant of frontotemporal dementia (bvFTD), semantic variant of primary progressive

Dissociation between physical reasoning and tool use in individuals with left hemisphere brain damage.

Tool use and physical reasoning are often assumed to rely on shared cognitive and neural mechanisms. At some level, this correspondence is expected: using an object typically requires understanding its physical properties. However, both capacities are complex and multicomponential, so the relationship between them may vary across levels of representation and task demands. Here, we asked whether third-person physical reasoning about object dynamics can dissociate from tool use (i.e., performing a tool's typical action) in individuals with left-hemisphere stroke. We tested 11 patients, five of w

Atypical cortical presentation of anti-Ri paraneoplastic encephalitis mimicking tumor.

PURPOSE: This report expands the known clinical phenotype of anti-Ri (ANNA-2)-associated paraneoplastic syndrome to include isolated neocortical involvement. METHODS AND RESULTS: We report the case of a 54-year-old woman who presented with transient apraxia, confusion, and headache. Brain MRI revealed a tumor-like lesion in the left frontal cortex. Stereotactic brain biopsy showed inflammatory changes without evidence of malignancy. Serum testing was strongly positive for anti-Ri antibodies, and systemic workup identified breast cancer with axillary lymph node metastasis. The patient received

A case report of crossed aphasia, grasping, and intermanual conflict with the left hand.

There are few reports describing the coexistence of alien hand syndrome (AHS) and crossed aphasia. The patient was a right-handed woman in her seventies. Following an infarction involving the medial right frontal lobe and the genu of the corpus callosum, she developed groping, grasping, and intermanual conflict (IC) in the left hand, along with crossed aphasia. The groping and grasping may have resulted from impaired inhibition due to supplementary motor area damage, whereas IC may have arisen from disrupted interhemispheric information transfer. The absence of apraxia in the right hand and th

Improving Intelligibility in Childhood Apraxia of Speech Through Communication Partner Training.

PURPOSE: Perceptual learning, improved understanding, of a degraded speech signal is driven by speech predictability. A hallmark feature of childhood apraxia of speech (CAS) is speech variability and inconsistency. This initial case study examined whether both naive and familiar listeners can improve their understanding of a child with CAS through perceptual training. METHOD: Speech recordings were collected from a 4-year-old boy with CAS and presented to listeners within a structured pretest-training-posttest paradigm. In Experiment 1, naive listeners (n = 44) received training using speech

A Novel Treatment of Error Awareness for Acquired Apraxia of Speech and Aphasia: Effects of a Single-Case Experimental Design Investigation.

PURPOSE: Error awareness (EA) is the ability to identify errors in one's speech productions. Persons with aphasia and apraxia of speech (A-AOS) have been found to vary widely in their EA abilities. EA, as measured by the accuracy of self-judgments of word production accuracy, has been found to be moderately associated with response to AOS treatment. EA training has rarely been included in AOS treatments, and the impact of EA as a treatment ingredient has not been studied. This study was designed to examine the effects of a novel treatment for impaired EA in individuals with A-AOS-the Treatment

Novel tool use does not depend on mechanical reasoning: evidence from apraxia.

A hallmark of human intelligence is the ability to use tools. Yet the cognitive processes supporting this ability remain debated. One contemporary view holds that mechanical reasoning is central for tool use, especially in the case of tools with which we have no prior experience. However, previous support for the role of mechanical reasoning often relies on circular logic, wherein poor performance on novel tool-use tasks is taken as evidence that impaired mechanical reasoning causes tool-use deficits in limb apraxia. To address this limitation, we independently assessed mechanical reasoning an

Cerebellar Ataxia and Chorea: Genetic Etiologies and Key Considerations for Diagnosis and Management.

BACKGROUND: The differential diagnosis of progressive ataxia is broad. Multiple degenerative, inherited, and acquired causes of ataxia must be considered. The presence of chorea as part of the clinical phenotype provides invaluable clinical information which may help to narrow the differential. OBJECTIVES: There are a limited number of conditions with both prominent chorea and cerebellar ataxia, and it is essential for clinicians to recognize these disorders to direct early disease-specific treatment. We aim to identify these conditions and propose a diagnostic approach. METHODS: We conducte

Progressive Supranuclear Palsy Unmasked After Post-COVID-19 Functional Decline in an Elderly Patient: A Diagnostic Challenge.

Progressive supranuclear palsy (PSP) is a rare neurodegenerative tauopathy characterized by early postural instability, vertical supranuclear gaze palsy, axial rigidity, and poor response to levodopa therapy. Diagnosis remains challenging, particularly in the early stages, because of overlap with other Parkinsonian syndromes. We report the case of an 83-year-old woman who developed progressive gait instability, severe functional decline, dysphagia, and marked weight loss following a mild SARS-CoV-2 infection. Extensive investigations initially excluded metabolic, infectious, neoplastic, and st
