Evaluating Costs and Efficacy of rFVIII Prophylaxis Using Matching-Adjusted Indirect Comparisons in Hemophilia A.

Introduction Prophylaxis with factor VIII (FVIII) is the standard of care for individuals with severe hemophilia A, but it is associated with a substantial economic burden. This analysis compared the efficacy, dosing, and annual treatment costs of simoctocog alfa (Nuwiq, a recombinant FVIII [rFVIII]) with five extended half-life (EHL) rFVIII concentrate products. Methods Matching-adjusted indirect comparisons (MAICs) were performed to compare simoctocog alfa with efanesoctocog alfa (ALTUVIIIO) and turoctocog alfa pegol (Esperoct), and results were integrated with a previously published MAIC co

Facility Factors Associated With Candida Central Line-Associated Bloodstream Infections in Neonatal Intensive Care Units.

BACKGROUND: Candida species are a leading cause of central line-associated bloodstream infections (CLABSIs) in neonatal intensive care units (NICUs) with high associated morbidity and mortality. Data about facility-level factors associated with Candida CLABSI incidence in NICUs are lacking. METHODS: We analyzed Candida CLABSIs reported through the Centers for Disease Control and Prevention's National Healthcare Surveillance Network (NHSN) bloodstream infection module and assessed facility characteristics using NHSN and American Hospital Association annual surveys during 2015-2022. We assessed

Small nucleolar RNA host gene 3 regulates sepsis-induced acute lung injury and inflammation by targeting microRNA-186-5p.

In the process of sepsis-induced acute lung injury (ALI), reducing the inflammatory response is crucial for restoring cellular and barrier function. The expression of small nucleolar RNA host gene 3 (SNHG3) is increased in the serum of sepsis patients, yet whether SNHG3 is involved in regulating the pathophysiology of ALI remains unclear. This study aims to investigate the effect of SNHG3 on sepsis-induced acute lung injury and explore its underlying mechanism. Sepsis-induced ALI models were established using lipopolysaccharide (LPS)-treated mice and lung endothelial cells. SNHG3 expression wa

Investigating Real-World Tolerance and Dose Reductions of Oncology Multikinase Inhibitors in a VA Population.

BACKGROUND: Oral multikinase inhibitors (MKIs) are widely used and effective targeted therapies for patients with cancer. These agents can have substantial drug-drug interactions and adverse effects (AEs), which can make them difficult to tolerate. This study sought to evaluate the real-world tolerance of MKIs prescribed by the hematology/oncology clinic at the Veterans Affairs North Texas Health Care System (VANTHCS) to guide clinicians in the selection of starting doses and in setting expectations for the management of dose titrations. METHODS: A retrospective chart review of veterans manag

Current Management of Serious Infections due to Vancomycin-resistant Enterococci.

Vancomycin-resistant enterococci (VRE) are major health care-associated pathogens and significantly contribute to patient morbidity and mortality, particularly in immunocompromised and critically ill patients. The clinical management of VRE infections is challenging due to limited therapeutic options and the paucity of high quality clinical data to guide management. Delays in initiating appropriate therapy has proven to have significant impact in VRE-associated bloodstream infections. This review summarizes the current evidence on treatment strategies for serious VRE infections, incluiding bac

Direct identification and susceptibility testing of positive blood cultures: a large-scale evaluation of an ammonium chloride-based in-house workflow.

PURPOSE: Previously, we reported an ammonium chloride-based in-house rapid workflow for direct pathogen identification and antibiotic susceptibility testing from positive blood cultures, eliminating the need for the following overnight subculture. Here, we aimed to assess the workflow’s reliability, accuracy, and potential for integration into routine clinical practice. METHODS: From 2018 to 2022, 1,686 monomicrobial positive blood cultures were processed by the rapid workflow: ammonium chloride lysis/centrifugation/wash followed by MALDI-TOF MS identification and, when indicated, same-day VIT

Effects of dexmedetomidine-remifentanil on hemodynamics and inflammatory responses in patients undergoing atrial fibrillation ablation.

This randomized controlled trial evaluated dexmedetomidine combined with remifentanil versus remifentanil alone for monitored anesthesia care during radiofrequency catheter ablation for atrial fibrillation. Between January and December 2023, 100 patients were stratified by age, sex, BMI, and atrial fibrillation type, then randomly assigned (1:1, block size 4, sealed envelopes) to Group A (dexmedetomidine 1 μg/kg loading then 0.2-0.4 μg/kg/h plus remifentanil, n = 50) or Group B (saline control plus remifentanil, n = 50). Baseline characteristics were comparable (all P > 0.05). Hemodynamic para

Communication through autoattractants can enhance and limit collective migration of immune cells.

Many eukaryotic cells produce attractant molecules to which they themselves are also attracted. For example, neutrophils produce leukotriene B4 while swarming. These autoattractants create a secondary signalling layer that coordinates collective cell behaviour. The principles by which autoattractants shape migration, however, remain poorly understood. Here we use a hybrid agent-based computational model to dissect the effects of autoattractants on the collective chemotaxis of immune cells. We find that autoattractant signals strongly enhance cells' responses to primary attractant, but only if

Association between S100 calcium-binding protein A12 and sepsis associated-acute kidney injury: a prospective cohort study.

BACKGROUND: Sepsis-associated acute kidney injury (SA-AKI) is a common and severe complication in critically ill patients, yet early diagnostic biomarkers remain limited. S100A12, a pro-inflammatory calcium-binding protein, may contribute to sepsis-induced renal injury through oxidative and immune-mediated mechanisms. This study aimed to investigate the association between plasma S100A12 levels and the risk of SA-AKI in a prospective sepsis cohort. METHODS: In this prospective cohort study, 202 adult patients with sepsis were consecutively enrolled. Plasma S100A12 levels were measured at enrol

Reducing Inpatient Pediatric Central Line-associated Bloodstream Infections by Proactive Risk Assessment and Escalation.

INTRODUCTION: Central line-associated bloodstream infections (CLABSIs), despite the application of best practices, are still prevalent and cause significant morbidity and mortality in pediatric patients. We describe a quality improvement project with an aim to reduce inpatient CLABSIs focused on standardization and proactive risk assessment. METHODS: The aim was to reduce inpatient CLABSI rates by 30% within 3 years beginning in January 2021. An apparent cause analysis in 2018 suggested that the lack of escalation of concerns was a potential focus area. We used Plan-Do-Study-Act cycles starti

Challenges of monocyte HLA-DR targeted immunomodulation in sepsis-a prospective observational cohort study.

INTRODUCTION: Reduced monocyte HLA-DR expression, a hallmark of immunosuppression in sepsis, is associated with infectious complications and mortality. Therapeutic strategies, including IFN-γ, have been used to restore monocyte HLA-DR and immune function, but have not consistently improved clinical outcomes. Therefore, we conducted an iterative series of experiments to re-examine the core assumptions and address the key gaps in the current understanding. METHODS: We conducted a prospective cohort study of patients admitted to the intensive care unit (ICU) with sepsis (n = 55, 36% mortality) t

Revealing Causal Proteomic Signatures of Hypertension: Systematic Insights From Proteome-Wide Mendelian Randomization.

BACKGROUND: Hypertension is a major cardiovascular risk factor with an incompletely understood cause. We aimed to unravel its proteomic landscape and investigate potential molecular mechanisms underlying its risk related to modifiable exposures. METHODS: Leveraging protein quantitative trait loci from 3 biobank-scale studies (deCODE, Fenland, and UKB-PPP [UK Biobank Pharma Proteomics Project]) based on distinct proteomic platforms (SomaScan or Olink), we performed proteome-wide Mendelian randomization to identify and cross-platform validate proteins causally associated with hypertension and b

Cultivating moral sensitivity and restraint-free practice in nurses and midwives.

Nursing and midwifery students are frequently exposed to physical restraint during clinical training, often presented as necessary for patient safety. However, growing evidence challenges this assumption, highlighting the risks associated with restraint and the limited implementation of effective alternatives in practice. Studies show that alternatives are considered in only a minority of cases, while organisational and educational factors contribute to the persistence and normalisation of restraint. Although professional codes define restraint as an exceptional, last-resort measure, this prin

m-EASIX (better than EASIX) predicts severe CAR T-cell toxicities, worse overall survival, and discriminates cytokine release syndrome from sepsis.

BACKGROUND: Cytokine Release Syndrome (CRS) and Immune Effector Cell-Associated Neurotoxicity Syndrome (ICANS) are life-threatening complications that often arise after CAR T-cell immunotherapy. Endothelial dysfunction is believed to play a central role in their development, leading to the interest in biomarker-based tools for diagnosis and differentiating these toxicities from sepsis. This study aimed to evaluate the Endothelial Activation Stress Index (EASIX) and its modified version (m-EASIX, which replaces creatinine with C-reactive protein [CRP] (mg/dL)) as early predictors of severe CRS

Targeting Immunologic Pathways in Eosinophilic Granulomatosis With Polyangiitis: Translating Emerging Evidence Into Clinical Practice.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare and potentially life-threatening systemic, inflammatory disease with multi-organ manifestations, variable presentation and complex pathology. Multiple interconnected immunological pathways are implicated in EGPA pathology, including a type-2 immune response driving predominantly eosinophilic inflammation, B-cell mediated autoimmunity, neutrophil activation, and the generation of pathogenic anti-neutrophil cytoplasmic antibodies, all of which can contribute to tissue/organ damage. High-dose glucocorticoids are the mainstay treatment

Interstitial glucose during sanctioned competition and during high-intensity training in highly trained collegiate male endurance athletes: A real-world continuous glucose monitoring study.

Real-world evidence on glucose behavior during competition in highly trained athletes is scarce. We characterized interstitial glucose dynamics in collegiate endurance athletes during sanctioned competitions using continuous glucose monitoring (CGM), with within-subject comparisons to high-intensity training performed at the same prescribed race pace. Twenty male runners and race walkers (Tier 3: Highly Trained/National) wore CGM during 36 races, contributing 20 paired race-versus-training comparisons. Glucose rose from baseline to 8.3 ± 1.5 mM 1 h pre-race, peaked at 11.2 ± 2.2 mM during comp
