Suzetrigine (Journavx) as a Non-Opioid Adjuvant for Cancer-Associated Pain: A Case Series.

Opioids remain central to managing cancer-associated pain (CAP) but are often limited by constipation, nausea, sedation, and concerns about maladaptive use amid the ongoing opioid epidemic. Suzetrigine (Journavx), a first-in-class, non-opioid, selective NaV1.8 sodium channel inhibitor, was approved by the U.S. Food and Drug Administration in January 2025 for moderate-to-severe acute pain; its role in chronic pain syndromes such as CAP has not been established. This case series describes two patients with CAP of differing pathophysiology started on suzetrigine as an off-label adjuvant analgesic

Chronic pelvic and related genital pain in transgender and gender-diverse people: a scoping review.

BACKGROUND: Chronic pelvic pain (CPP) and related genital pain are disabling and multifaceted, but understudied in transgender and gender-diverse (TGD) people. METHODS: In this scoping review, we mapped evidence on prevalence, aetiology, assessment, and management across gender-affirming hormone therapy (GAHT), surgical, and community contexts. Following Joanna Briggs Institute methodology and Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) reporting guidance, we searched ten databases from database inception to 30 July 2025, suppl

Perioperative analgesic effects of opiranserin in canine castration: a pilot study.

INTRODUCTION: Opiranserin, a non-opioid analgesic acting as a glycine transporter 2 inhibitor and 5-HT2A receptor antagonist, was evaluated for reducing intraoperative rescue analgesia and postoperative pain in dogs undergoing castration under isoflurane anesthesia. MATERIALS AND METHODS: Eight healthy male Beagle dogs were randomized to receive a continuous rate infusion of opiranserin (5 mg·kg-1·h-1 for 1 h before induction, then 0.33 mg·kg-1·h-1 for 9 h; n = 4) or 0.9% saline (n = 4). Anesthesia was induced with alfaxalone and maintained with isoflurane (1.5% end-tidal); all dogs received

Structured Analgesia Pathways, Including Nurse-Initiated Approaches, for Acute Pain in Emergency Departments: A Systematic Review and Meta-Analysis.

Acute pain is a common reason for emergency department (ED) attendance, yet delayed analgesia remains a persistent challenge in emergency care. Structured analgesia pathways, including nurse-initiated approaches, may reduce treatment delays by enabling earlier pain assessment and analgesic administration according to predefined criteria. This systematic review and meta-analysis evaluated the effects of structured analgesia pathways on acute pain management in ED settings, focusing on time to analgesia, pain reduction, analgesia administration, patient satisfaction, adverse events, and ED flow.

Beyond the Ladder: A 10-Domain Cognitive and Operational Toolkit to Humanise and Implement Acute Pain Guidelines at the Bedside.

BACKGROUND: Although pain is among the most common reasons for emergency department (ED) attendance, oligoanalgesia remains a persistent global issue. While the 2026 EUSEM guidelines provide evidence-based pharmacological recommendations, translating these into the crowded ED environment is often constrained by systemic overcrowding and a traditional reliance on technical skills, with limited integration of human factors and nontechnical skills relevant to suffering. The objective of this position paper is to propose a practical, human-centred operational framework that bridges scientific guid

Thoracic Epidural Analgesia: Do the Alternatives for Abdominal Surgical Pain Control Measure up?

Thoracic epidural analgesia (TEA) is well-known as the gold standard for perioperative somatic and visceral analgesia in abdominal surgery. Recently, there has been a shift away from TEA with the emergence of alternative pain control techniques such as fascial plane blocks (FPBs). Proponents of these alternatives often note the high failure rate of TEA, which can reach up to 25%. The alternatives to TEA are not a panacea. Important techniques have emerged to improve upon the reliability of TEA. In cases where TEA is contraindicated or impractical, alternatives including intrathecal morphine, p

[FALSE PENILE FRACTURE CAUSED BY SEXUAL INTERCOURSE: A CASE REPORT].

Penile fracture is caused by rupture of the tunica albuginea and typically presents with acute pain, a snapping sound, and rapid detumescence. False penile fracture mimics these clinical features but occurs without tunical rupture, most commonly due to dorsal venous injury. Distinguishing between these two conditions is often challenging, particularly in emergency settings where imaging modalities may not be readily available. Current guidelines recommend surgical exploration when the diagnosis is uncertain. A man in his 50s presented with penile swelling during sexual intercourse. Progressive

A non-inferiority randomized controlled trial comparing nebulized fentanyl and intravenous morphine for acute pain management in the emergency department.

INTRODUCTION: Intravenous (IV) morphine is the standard treatment for acute pain in emergency department (ED), but is limited by invasiveness and adverse effects. Nebulized fentanyl is a noninvasive alternative. This study aimed to assess the efficacy of nebulized fentanyl compared with IV morphine for acute pain relief 15 min after administration. Secondary outcomes included pain score trends, adverse effects, and rescue analgesia requirements at 30, 45, and 60 min. METHODS: We conducted a double-blind, randomized controlled trial in patients aged 18 to 65 years who had severe pain (numeric

Use of Intranasal Analgesia for Acute Pain in Pediatric Emergency Departments: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Pain is one of the most common reasons for presentation to pediatric emergency departments, yet timely and effective analgesia remains inconsistently delivered. Intranasal (IN) analgesics offer a noninvasive alternative to injectable routes, but their comparative effectiveness, safety, and acceptability across agents and routes remain incompletely defined. METHODS: We conducted a systematic review and network meta-analysis of randomized controlled trials evaluating IN analgesics for acute pain management in children aged 0-18 years presenting to emergency or prehospital settings.

Bilateral lower limb dysfunction due to acute hematoma in the lumbar ligamentum flavum: A case report and literature review.

RATIONALE: There have been several reports of lumbar yellow ligament hematomas leading to cauda equina and/or nerve root compression. However, based on clinical history and histological findings, most of these cases involve chronic hematomas. Acute lumbar yellow ligament hematomas causing nerve compression are relatively rare. PATIENT CONCERNS: A 71-year-old male presented with an 8-day history of sudden-onset lower back pain without an identifiable trigger, which progressively worsened with new-onset pain and numbness in both lower extremities. Physical examination revealed muscle strength o

Novel 5-Phenylmorphans as Opioid Receptor Antagonists: Synthesis, Structure-Activity Relationship, Biological Evaluation and Computational Modeling.

The widespread emergence of ultra-potent synthetic opioids in illicit drug supplies and the limited effectiveness of naloxone in reversing certain synthetic opioid overdoses highlight the urgent need for potent high-affinity opioid antagonists. This study describes an improved synthetic approach and structure-activity relationship (SAR) study of N-substituted analogs of a potent and active MOR antagonist, 3-((1S,5R,9R)-9-methyl-2-phenethyl-2-azabicyclo[3.3.1]nonan-5-yl)phenol ((1S,5R,9R)-1). Oliceridine for acute pain management: pharmacology, clinical efficacy, safety, and future perspectives: a narrative review.

Opioids are a cornerstone in the management of moderate-to-severe acute pain; however, their clinical utility is limited by well-recognized challenges, including respiratory depression, gastrointestinal (GI) dysfunction, and the potential for misuse and abuse. Oliceridine is a novel μ-opioid receptor agonist with G-protein-biased signaling and reduced β-arrestin recruitment. It exhibits potent analgesic effects while potentially reducing opioid-related adverse events. In preclinical investigations, oliceridine was associated with lower rates of GI dysfunction and respiratory depression than mo

A Difference-in-Differences Analysis of Low-Value Opioid Utilization for Acute Low Back Pain, 2019-2021.

INTRODUCTION: The use of opioids for acute low-back pain (LBP) is considered low-value in most clinical scenarios. Understanding trends in low-value opioid utilization can inform interventions to support guideline-concordant care. This study evaluated the utilization of low-value opioids for acute LBP, and variation by rurality, payer, and sex, before and during the COVID-19 pandemic. METHODS: In this retrospective cohort study, we examined opioid claims from Virginia's All-Payer Claims Database among adults with commercial, Medicare Advantage (MA), or Medicaid insurance during 2019-2021. Psychometric validation of the Short Form of the Unesp-Botucatu Pig Composite Acute Pain Scale (UPAPS-SF) for assessing pain in piglets following surgical castration.

Piglets' surgical castration is a painful procedure leading to behavioural alterations that can be used to diagnose pain. The Unesp-Botucatu Pig Composite Acute Pain Scale (UPAPS) is a validated scale widely used in research. Previous refinement studies have proposed that UPAPS could be shortened to only presence/absence of five behaviours. Therefore, our aim was to test the psychometric properties and validate the Short Form of the UPAPS (UPAPS-SF) for diagnosing acute pain in piglets undergoing surgical castration. Forty-eight 2-8 days-old piglets were video-recorded 24h before, immediately
