Assessment of the Damage Index and Its Dynamics in Patients with Non-Systemic Variants of Juvenile Arthritis during the Treatment with Biologics.

UNLABELLED: Juvenile idiopathic arthritis (JIA) is characterized by a high risk of joint destruction, extra-articular changes, and rapidly progressive disability. The CHAQ (Childhood Health Assessment Questionnaire) and  X-ray are often used to assess functional impairment and the degree of irreversible damage, but both methods have limitations. The Juvenile Arthritis Damage Index (JADI) has been developed to comprehensively assess the biological prognosis of JIA, which includes articular (JADI-A) and extra-articular (JADI-E) damage evaluation. The use of the JADI may be an effective tool for

Erythema Induratum as a rare manifestation of cutaneous tuberculosis in a young woman: a case report.

Cutaneous tuberculosis (TB) is a rare form of extrapulmonary TB, often leading to diagnostic challenges due to its varied clinical presentation. Erythema induratum of Bazin (EIB), a type of lobular panniculitis, is a recognized manifestation of cutaneous TB. This report describes a 19-year-old woman who developed EIB, presenting with recurrent fever, night sweats, weight loss, and hyperpigmented nodular lesions on both lower extremities. Laboratory tests showed an elevated erythrocyte sedimentation rate and a positive tuberculin skin test, while histopathology confirmed lobular panniculitis. P

Unmasking the Great Mimic: An Atypical Presentation of Giant Cell Arteritis With Recurrent and Isolated Diplopia.

Giant cell arteritis (GCA) is a well-recognized vasculitis, typically presenting with headaches, scalp tenderness, jaw claudication, polymyalgia rheumatica, or constitutional symptoms. In stroke and transient ischemic attack (TIA) clinics, GCA is primarily considered in cases of visual loss due to ophthalmic artery involvement. However, diplopia as an isolated manifestation of GCA is exceedingly rare, making early recognition challenging. We present the case of a 75-year-old man who attended our TIA clinic with seven recurrent episodes of transient binocular diplopia. Notably, he lacked the cl

Unmasking primary thyroid tuberculosis - A rare but deceptive diagnosis: A case report with review of literature.

INTRODUCTION AND IMPORTANCE: Thyroid tuberculosis is a rare condition, even in regions where tuberculosis (TB) is more common, possibly due to the thyroid gland's relative immunity. Diagnosing thyroid TB can be challenging as the clinical signs are often nonspecific. CASE PRESENTATION: A 75-year-old male presented with a swelling in the midline of the neck with an abscess in the lower portion of the swelling. Thyroid function tests were normal, and the tuberculin skin test returned positive. USG of the neck showed heterogeneous hypoechoic collection in the left lobe of thyroid, with surroundi

A Case of CRP-Negative Giant Cell Arteritis Detected by Contrast-Enhanced Orbital MRI.

Giant cell arteritis (GCA) is a vision-threatening emergency that can cause irreversible visual loss due to arteritic ischemic optic neuropathy. We report a case in which C-reactive protein (CRP) blood tests and whole-body imaging showed no signs of inflammation, but an abnormal signal in the superficial temporal artery on MRI led to the diagnosis. A 90-year-old man presented with a two-week history of decreased visual acuity in his left eye. At the initial visit, his visual acuity was 20/25 in the right eye and no light perception in the left eye. Bilateral optic disc swelling and right-sided

[Rheumatoid arthritis combined with IgG4-related disease successfully treated with rituximab: A case report].

Rheumatoid arthritis is an autoimmune disease primarily characterized by chronic symmetric arthritis. While rheumatoid arthritis with elevated eosinophils exhibits distinct clinical features, the potential coexistence of immunoglobulin (Ig)G4-related disease warrants clinical vigilance. This article reports a case of a patient presenting with polyarticular swelling and pain accompanied by morning stiffness. Positive tests for anti-keratin antibody, anti-cyclic citrullinated peptide antibody, anti-perinuclear factor, and rheumatoid factor were observed with a significantly elevated erythrocyte

Navigating Diagnostic Delays: A Case of Takayasu Arteritis Complicated by Acute Heart Failure and Vascular Compromise in a 37-Year-Old Woman From Cameroon.

Takayasu arteritis (TAK) is a rare form of large-vessel vasculitis that significantly impacts cardiovascular health, leading to severe complications such as acute heart failure (AHF). Its rarity in Cameroon may be attributed to limited awareness, underdiagnosis, and a lack of published data on the condition. We report the case of a 37-year-old female patient who presented with AHF and secondary hypertension, chest pain, and AHF after undergoing umbilical hernia repair two weeks prior. Initial symptoms included dyspnea at rest (New York Heart Association (NYHA) stage 4), orthopnea, paroxysmal n

Takayasu's Arteritis with Systemic Lupus Erythematosus: A Case Report.

Takayasu arteritis (TA) is a rare granulomatous vasculitis affecting the aorta and its major branches, while systemic lupus erythematosus (SLE) is a multisystem autoimmune disorder known for vascular and serological involvement. Although both diseases share certain immunopathological pathways, their co-occurrence is exceedingly rare. We report a case of a 44-year-old Indian female with a prior diagnosis of SLE who presented with breathlessness, palpitations, and nodular episcleritis. Cardiovascular evaluation revealed carotid bruit, diminished peripheral pulses, blood pressure discrepancies, a

Diverse Vascular Manifestations of Takayasu Arteritis: Coronary Artery Stenosis and Aneurysmal Dilation in pediatric patients.

Takayasu arteritis (TA) is a rare, chronic inflammatory disease that primarily affects large arteries, and in childhood (c-TA), it often presents with diverse and delayed manifestations that complicate diagnosis. We describe 2 pediatric cases that highlight the importance of early recognition and intervention. The first was a 3-month-old girl who presented with fever, respiratory distress, and peripheral cyanosis. Laboratory investigations revealed leukocytosis, anemia, elevated inflammatory markers, and hypercoagulability, while Doppler ultrasound and computed tomography angiography (CTA) dem

Oligocentric Castleman Disease (OligoCD): A Novel Diagnostic Entity in the Spectrum of Castleman Disease.

Castleman disease (CD) is a rare lymphoproliferative disorder traditionally classified as unicentric (UCD) or multicentric (MCD). However, some cases exhibit features that do not fit either category. These atypical cases warrant recognition as a distinct subtype, proposed here as oligocentric Castleman disease (OligoCD). A 38-year-old woman presented with bilateral lacrimal gland swelling and cervical lymphadenopathy, fatigue, and low-grade fever. Laboratory investigations, including hemogram, liver and kidney function tests, inflammatory markers, and interleukin-6 (IL-6), were within normal l

Chronic Nonbacterial Osteomyelitis in a Young Child: A Case Report of a Diagnostic Challenge Mimicking Malignancy.

Chronic nonbacterial osteomyelitis (CNO) is a rare, autoinflammatory bone disorder that primarily affects children and adolescents. It typically presents with localized bone pain and lacks systemic symptoms, making diagnosis difficult. Radiologic features often mimic malignant or infectious etiologies, contributing to delays in diagnosis and the use of unnecessary invasive procedures. We report the case of a 5-year-old female patient with a history of sickle cell trait who presented with intermittent left arm pain. The initial physical examination was unremarkable, and symptoms were attributed
