Ciprocopan: First Approval.

Ciprocopan (®) is a small molecule inhibitor of complement factor B, which is a key component of the alternative complement pathway. It is being developed by Haisco Pharmaceutical Group for the treatment of complement-mediated disorders. In July 2026, ciprocopan received its first approval for treatment of adult patients with paroxysmal nocturnal haemoglobinuria (PNH) who have not previously received treatment with a complement inhibitor therapy in China. This article summarizes the milestones in the development of ciprocopan leading to this first approval.

Allogeneic transplantation versus non-transplant treatment for classical paroxysmal nocturnal haemoglobinuria without complement inhibitors.

To describe outcomes after haematopoietic stem cell transplantation (HSCT) and non-transplant treatment in patients with classical paroxysmal nocturnal haemoglobinuria (cPNH) without access to complement inhibitors, we analysed 227 patients with cPNH treated at 10 centres between January 1976 and April 2022. Among them, 51 underwent HSCT and 176 received non-transplant treatment without complement inhibitors. At 6 months post-treatment, 96.08% of HSCT patients achieved normal complete blood count, compared to 3.41% in the non-HSCT cohort (p < 0.001). Estimated 5-year overall survival (OS) did

PNH clone distribution in aplastic anaemia: heterozygous germline carriers exhibit immune-mediated clone patterns rather than inherited bone marrow failure phenotype.

Distinguishing immune-mediated aplastic anaemia (AA) from inherited bone marrow failure syndromes (IBMFS) is critical but challenging. The diagnostic value of paroxysmal nocturnal haemoglobinuria (PNH) clones in this context remains uncertain. We prospectively studied 222 AA patients (median age 21.5 years, range 0.8-67) who underwent high-sensitivity PNH flow cytometry (≥0.01%) and next-generation sequencing. Patients were classified as category 1 (pathogenic/likely pathogenic variants fully explaining phenotype; confirmed IBMFS), category 2 (single heterozygous variants), and categories 3/4

Phase II study of the triple combination of rabbit ATG, ciclosporin and eltrombopag in patients with transfusion-dependent aplastic anaemia: West Japan Hematology Study Group (W-JHS) AA02 trial.

The efficacy of a triple combination of rabbit anti-human thymocyte immunoglobulin (rATG), ciclosporin and eltrombopag (EPAG) was prospectively evaluated in patients with severe or transfusion-dependent non-severe aplastic anaemia (SAA) across 29 institutions in Japan. Sixty patients were enrolled, of whom 48 had SAA. The primary end-point, the haematological overall response rate at 12 weeks, was 52.6% (95% confidence interval, 39.0%-66.0%), increasing to 67.9% at 26 weeks. The most frequent grade 3/4 adverse event was febrile neutropenia (20.0%). One elderly patient with severe neutropenia d

MUC4 mutations promote a thrombotic phenotype in patients with paroxysmal nocturnal haemoglobinuria by increasing the deposition of terminal complement.

BACKGROUND: Thrombosis is a common complication in paroxysmal nocturnal haemoglobinuria (PNH) patients, but primary prevention remains controversial. Identifying high-risk individuals could enable risk-stratified prophylactic anticoagulation strategies. METHODS: We analyzed clinical data from PNH patients with or without thrombosis, including MUC4 mutation status and serum complement C5b-9 levels. Complement deposition assays and a murine lower limb deep vein thrombosis model were used to investigate the role of MUC4 mutation in thrombotic risk and explore the underlying mechanism involving t

Comparative Efficacy of Complement Inhibitors in Complement Inhibitor-Naïve PNH: A Systematic Review With Supportive Exploratory Network Meta-Analysis of Randomized Trials.

INTRODUCTION: Paroxysmal nocturnal haemoglobinuria (PNH) is an uncommon, life-threatening disease, caused by intravascular haemolysis by the complement system. In this review, we aim to compare the efficacy of the available agents across patient-centred outcomes in complement inhibitor-naive patients. METHODS: Following PRISMA guidelines, a comprehensive literature search was conducted on PubMed, Cochrane CENTRAL and ClinicalTrials.gov for studies published up to 30th May 2025. A frequentist model network meta-analyses were conducted in RStudio (v5.4.1) using a common-effects model. RESULTS:

Paroxysmal nocturnal haemoglobinuria: Considerations for optimal pregnancy care. A case report.

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare haematological disorder characterised by an acquired mutation in haematopoietic stem cells leading to intravascular haemolysis and thrombotic events. In untreated women, pregnancy amplifies the risks of these manifestations, leading to increased maternal and neonatal morbidity and mortality. In the UK, eculizumab is the only approved treatment for PNH in pregnancy. Eculizumab was found to be safe in pregnancy and reduces PNH-related complications. Multidisciplinary team care between haematologists specialising in PNH, obstetricians, anaesthe

Aplastic anaemia with small paroxysmal nocturnal haemoglobinuria clones developing during osimertinib therapy for non-small cell lung cancer.

We report two cases of aplastic anaemia (AA) with small populations of paroxysmal nocturnal haemoglobinuria (PNH)-phenotype cells that developed during osimertinib (OSIM) therapy for epidermal growth factor receptor mutation-positive lung adenocarcinoma (EGFR-LUAD). The first case was a 76-year-old woman with EGFR-LUAD accompanied by pleural dissemination. She developed pancytopenia 9 months after starting OSIM therapy. We diagnosed the patient with moderately severe AA and observed hematopoietic recovery following treatment with anabolic steroids and eltrombopag. The second case involved a 63

Thrombotic Events in Patients With Paroxysmal Nocturnal Haemoglobinuria Using Eculizumab: A Post-Marketing Surveillance Sub-Analysis.

BACKGROUND: Thrombotic events (TEs) account for approximately 40%-67% of deaths in untreated paroxysmal nocturnal haemoglobinuria (PNH). C5 inhibitor treatment (e.g., eculizumab and ravulizumab) greatly reduces the incidence of TEs in patients with PNH, but data on the characteristics of PNH patients experiencing TEs are limited. METHODS: This post-marketing surveillance sub-analysis evaluated the incidence of TEs in patients with PNH in Japan. RESULTS: During eculizumab treatment, 54 TEs occurred in 44/794 patients (5.5%), an incidence of 1.50/100 patient-years. The most frequent TEs were d

Exploring individualized crovalimab dosing in PNH through in silico modelling: Potential for improved convenience and cost efficiency.

AIM: Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, acquired haematopoietic stem cell disorder. Crovalimab, a complement C5-inhibitor, is approved for PNH and can be self-administered subcutaneously every 4 weeks, offering a more convenient route than intravenous C5-inhibitors. Since exposure often exceeds pharmacokinetic targets and varies with body weight, we aimed to explored individualized dosing of crovalimab using clinical trial simulations to optimize therapy. METHODS: We compared standard, weight-based and therapeutic drug monitoring (TDM)-guided regimens in treatment-naïve PNH

Paroxysmal Cold Haemoglobinuria: A Chilling Case of Acute Renal Failure.

We describe a rare case of a 64-year-old man with acute renal failure secondary to paroxysmal cold haemoglobinuria (PCH), presenting with haemoglobinuria, jaundice, and fevers. PCH is an intravascular haemolytic anaemia with polyphasic IgG antibody binding of red blood cells at cold temperatures, with complement-mediated haemolysis on rewarming. Renal biopsy confirmed acute tubular necrosis without thrombotic microangiopathy. The patient received supportive management including intermittent haemodialysis with a warm circuit to prevent further haemolysis and achieved full renal recovery. Optima

A fusion protein of the two tick-derived C5 inhibitors OmCI and RaCI: complete C5 inhibition despite strong complement activation.

The complement system must be tightly regulated to prevent tissue damage and disease, yet current C5 inhibitors like the anti-C5 monoclonal antibody Eculizumab - though clinically approved (e.g., for paroxysmal nocturnal haemoglobinuria, PNH) - fail to achieve complete terminal pathway blockade under conditions of strong complement activation. To address this limitation, we investigated tick-derived C5 inhibitors and discovered that combinatorial targeting of non-overlapping C5 epitopes enables full inhibition. Leveraging this insight, we engineered a bifunctional fusion protein RaCI-gly-OmCI
