Cervico-thoracic neurenteric cyst: clinicoradiological correlation with embryogenesis.

Two cases of cervico-dorsal neurenteric cysts are presented, one located dorsal and the other ventral to the spinal cord, with associated Klippel-Feil anomaly, meningocele, spinal dysraphism and possibly spinal cord malformation. One patient was operated on as a neonate for a cervico-thoracic meningocele and presented during adolescence with spastic quadriparesis. Imaging revealed spina bifida, a dorsally located neurenteric cyst and a tendency towards split cord malformation. The other child presented with cervico-thoracic kyphoscoliosis and paraparesis. Imaging demonstrated persistence of a

Hemianopic visual field defects in children with intracranial shunts: report of two cases.

OBJECTIVE AND IMPORTANCE: Intracranial shunts are commonly placed in children and frequently require replacement during the child's growing years. Severe signs of increased intracranial pressure often are the first indication of shunt displacement and malfunction. Subtle neuro-ophthalmic signs in children are usually overlooked. CLINICAL PRESENTATION: Two cases are described in which homonymous hemianopsia resulted from unsuspected movement of the shunt catheter in growing children. In one male patient, the catheter tip impaled the optic tract, and in the other male patient the shunt became e

Development and applications of in vivo clinical magnetic resonance spectroscopy.

4.1 CURRENT STATUS. While an extensive clinical literature of MRS of muscle, brain, heart and liver has been achieved, the MRS technique is not considered essential for routine diagnosis because it is inherently insensitive and metabolic changes tend to be small. However, MRS techniques have proven to be of considerable value for prognosis in some circumstances, notably for predicting outcome following hypoxic-ischaemic injury in the newborn and also in predicting graft viability following organ transplantation. The chemical specificity of MRS has been illustrated, and exploiting the non-invas

Magnetic resonance imaging of the brain in adolescents with phenylketonuria and in one case of 6-pyruvoyl tetrahydropteridine synthase deficiency.

White matter abnormalities on MRI have been observed in phenylketonuria (PKU) patients with late onset neurological symptoms as well as in neurologically inconspicious patients. We investigated 14 early treated adolescents at an age between 12 and 17 years (mean age 14.3 years) with classical PKU as well as one retarded patient with atypical PKU by cranial MRI with spinecho T1-, T2- and proton density sequences. Clinical examination was normal. Visual evoked potential (VEP) examination showed a prolonged latency of peak P100 (mean 122.6 ms; control mean 115.9) and IQ testing showed a mean IQ o

Evolution of different therapeutic strategies in the treatment of cranial dural arteriovenous fistulas--report of 30 cases.

30 cases of cranial dural arteriovenous fistuals, treated between 1983 and 1992, are reported. Twelve presented with an aggressive clinical course including intracranial haemorrhage, progredient neurological deficit, medically intractable seizures, and cerebellar symptoms. The other 18 patients had a more benign clinical presentation with audible bruit, exophthalmus, chemosis, and cranial nerve dysfunction. One of the latter had symptoms of pseudotumour cerebri due to sinus occlusion with contralateral sinus stenosis. The most common location was at the transverse sinus, followed by the cavemo

Clinical and MRI correlates in 27 patients with acquired pendular nystagmus.

Clinical and MRI investigations were carried out on 27 patients with acquired pendular nystagmus in an attempt to delineate possible sites of lesions responsible for pendular nystagmus and mechanisms underlying the frequent ocular disconjugacy of this nystagmus. The aetiologies were multiple sclerosis (n = 21), brainstem stroke (n = 3) and other neurological conditions. In at least 59% of the patients, pendular nystagmus appeared > 1 year after the first symptom of the disease. Patients MRIs were characterized by multiple areas of abnormal signal and were analysed statistically to identify are

[Evaluation of cerebral perfusion imaging with N-isopropyl-p-[123I]iodoamphetamine (IMP) in the cases of antiphospholipid syndrome].

Five cases of antiphospholipid syndrome with mild headache, but without any neurological deficits and abnormal findings by CT and MRI, were examined by cerebral blood perfusion SPECT using N-isopropyl-p-[123I]iodoamphetamine (IMP). Although three cases were performed quantification of cerebral blood flow with a microsphere method simultaneously, their values were within normal limits. Two of them showed focal low perfusion areas. One case had relatively low perfusion areas in the bilateral occipital lobes and the right temporal lobe, which improved after treatment. One of two had low perfusion

[Phase 1 clinical study of 123I-iomazenil: a new probe to evaluate central-type benzodiazepine receptor with SPECT].

A Phase 1 clinical study of 123I-iomazenil (IMZ), a new radiopharmaceutical developed for evaluation of central-type benzodiazepine receptor (BZR) with SPECT, was performed to examine its safety and biodistribution in six healthy volunteers. The brain uptake of 123I-IMZ reached a maximum value of 11.7 +/- 1.6% of the injected dose at 10-20 min after i.v. administration, and then decreased slowly. The regional tracer distribution immediately after injection (0-10 min) was considered to be a reflection of the cerebral blood flow. From 2 to 3 hours post-injection, washout of the radioactivity fro

Diffusion, perfusion and functional magnetic resonance imaging.

Recent developments in the use of Magnetic Resonance Imaging (MRI) to measure and image molecular diffusion and blood microcirculation (perfusion) hold significant promise in the noninvasive evaluation of normal brain function and functional disorders. Molecular diffusion is the result of spontaneous random motion that involves all molecules and probes molecular motion at microscopic level. Using diffusion MRI, information on tissue geometry and compartmentation effects can be obtained. Diffusion MRI has been used to map myelin fiber orientation in brain with high accuracy. Diffusion MRI is al

Positron-emission tomography in tardive dyskinesia.

Cerebral glucose metabolic rates were determined in normal control subjects (n = 26) and schizophrenic patients with tardive dyskinesia (n = 14). Globus pallidus and primary motor cortex (precentral gyrus) metabolic values divided by those of the cerebral hemispheres were significantly increased in the patient group. A similar metabolic pattern has not been reported for schizophrenic patients without tardive dyskinesia, and the abnormalities were demonstrated despite the normal appearance of the basal ganglia on X-ray CT. The findings differed markedly from the reduced metabolic rates of the b

Isolated adrenocorticotropin deficiency as a rare cause of hypoglycaemia in children. Further studies and report of an additional case.

An 11.5-year-old boy presented with recurrent attacks of hypoglycaemic coma. Endocrine investigations indicated adrenocortical insufficiency secondary to isolated ACTH deficiency, how ACTH and cortisol plasma levels with normal secretory reserve of other anterior pituitary hormones. The absence of ACTH response after corticotropin releasing factor and insulin-induced hypoglycaemia suggested a failure of primary pituitary ACTH-secreting cells. Among other hormone-secreting cells, pituitary cell circulating antibodies were negative. CT scan and MRI failed to reveal any anatomical abnormality of

The diagnosis of hippocampal sclerosis: other techniques.

Pathologically, hippocampal sclerosis (HS) is characterized by neuronal loss and gliosis affecting particularly the pyramidal neurons of CA1, CA3, and CA4 with relative sparing of the CA2 neurons. This can be identified in vivo with magnetic resonance (MR) imaging techniques that can reveal both morphological and signal abnormalities. The morphological changes are atrophy and loss of the normal internal architecture of the hippocampus as seen in coronal section. There is also T1- and T2-weighted signal abnormality in the hippocampus. Quantitative techniques are very good at measuring any singl

Clinical correlations: MRI and EEG.

Main structural correlates of epileptogenesis include hippocampal sclerosis, cortical dysgenesis, foreign tissue lesions, gliosis, and dual pathology (a combination of any two). These structural abnormalities are now increasingly defined with MRI, enabling systematic EEG correlative analyses. Hippocampal atrophy (HA) and increased T2 signal in medial temporal structures predict the presence of mesial temporal sclerosis with a high degree of sensitivity and specificity. In 50 patients with clinical evidence of temporal lobe epilepsy and isolated HA, ictal scalp EEG was concordant to the atrophi

Clinical applications: MRI, SPECT, and PET.

MRI, PET, and SPECT are all used to image abnormalities in the epileptic brain. Comparison of the techniques is difficult because they measure different aspects of the epileptic process--structure, metabolism, and perfusion. SPECT is the only one that can be systematically applied during seizures, while all three are used to image interictal abnormalities. Literature review suggests that of interictal techniques, PET has the highest diagnostic sensitivity in temporal lobe epilepsy (TLE) (84% vs. 66% for SPECT, 55% for qualitative MRI, 71% for quantitative MRI) while SPECT has the highest sensi

Magnetic resonance imaging and epilepsy: neurosurgical decision making.

Advances in magnetic resonance imaging (MRI) techniques have had an important impact on the decision-making process leading to surgical resection for chronic seizures. The MRI is now obtained relatively early in the work-up, and, when it shows abnormality, it assumes a crucial role in the detection of specific surgically remediable syndromes. These syndromes, when diagnosed by MR and other confirmatory studies such as electroencephalography (EEG), positron emission tomography (PET), magnetoencephalography (MEG), and neuropsychological testing, define the essential part of the surgical plan; th

Structure-function relationships in rat brainstem subnucleus interpolaris: XII. neonatal deafferentation effects on cell morphology.

In the developing whisker-barrel neuraxis, it is known that pattern formation, receptive fields, axon projections, and even cell survival are under the control of peripheral signals transmitted through the infraorbital nerve. However, afferent influences upon the development of single-cell morphologies have not received thorough study. Intracellular recording, antidromic activation, receptive field mapping, dye injection, and computer-assisted cell reconstruction methods were used to assess the morphology of trigeminal (V) brainstem neurons in adult rats whose infraorbital nerves were transect

Microarchitectural findings in sleep EEG in depression: diagnostic implications.

A 10-year review of sleep electroencephalogram (EEG)-frequency analysis in depression reveals several consistent microarchitectural abnormalities. Decreased delta amplitude or incidence, particularly in the first 100 min of sleep, has been reported. Elevated fast-frequency EEG has been shown in both remitted and symptomatic depressed patients, especially in the right hemisphere. Further, interhemispheric coherence is reduced in both depressed groups. These microarchitectural features may not be present in narcolepsy, obsessive-compulsive disorders, or schizophrenia, despite similarities in sle
