Face-name associative memory in temporal lobe epilepsy: Region-specific insights in right-hemisphere onset.

Mapping memory function in temporal lobe epilepsy (TLE) is important for understanding surgical risk, yet practical noninvasive tools remain limited. We tested whether a face-name associative memory fMRI task could detect subregional dysfunction within the amygdala-hippocampal complex (AHC) in TLE. Forty-nine individuals with unilateral TLE (29 left-, 20 right-onset; 51% MTS-negative) and 25 controls from three centers were scanned during a face-name encoding task and completed delayed recognition and neuropsychological tests of memory. The hippocampal head, body, tail, and amygdala were anato

Ictal Switch Pattern among TEMPORAL LOBE SEIZURES and its Association with Other Markers of Bilaterality - A Retrospective Multicentre Study from North Kerala.

BACKGROUND: Ictal switch in the temporal lobe is a rarely described phenomenon where ictal rhythm lateralized to one hemisphere shifts to the contralateral hemisphere and remains for a period of at least 10 seconds. It is important to recognize this electrographic event as it can indicate bilateral epileptogenicity, which is associated with poor surgical outcomes. Objective: This study was aimed to determine the prevalence of ictal switch of lateralization on scalp electroencephalogram (EEG) during temporal lobe seizures and its relationship with interictal spike lateralization and Magnetic Res

Multidimensional Profiling of MRI-Negative Temporal Lobe Epilepsy Uncovers Distinct Phenotypes.

OBJECTIVE: Although hippocampal sclerosis (TLE-HS) represents the most frequent cause of temporal lobe epilepsy (TLE), up to 30% of patients show no lesion on visual MRI inspection (TLE-MRIneg). These cases pose diagnostic and therapeutic challenges and are underrepresented in surgical series. We investigated whether TLE-MRIneg constitutes a distinct clinical and neuroanatomical entity compared to TLE-HS and aimed to identify subtypes within the TLE-MRIneg group. METHODS: We analyzed MRI and clinical data from 209 patients with TLE and 102 healthy controls from the multicenter "3TLE project".

Role of video-electroencephalography in the presurgical evaluation of patients with temporal lobe epilepsy due to mesial temporal sclerosis: A retrospective reliability study.

PURPOSE: To retrospectively examine whether prolonged video electroencephalography (vEEG) is a necessary diagnostic tool to help localize the seizure focus among patients who have evidence of mesial temporal sclerosis (MTS) on MRI and other clinical and ancillary findings of temporal lobe epilepsy. METHODS: Seventy-nine patients with evidence of MTS on presurgical MRI testing were included in the study. Two independent epileptologists determined whether the provided workup information, including routine EEG, MRI, semiology, and neuropsychological assessment, was sufficient to localize and pro

MR Imaging Protocols for Epilepsy.

MR imaging is the imaging modality of choice in epilepsy for detecting structural brain abnormalities, particularly those being considered for surgery. A dedicated epilepsy protocol typically includes high-resolution 3 dimensional (3D) T1-weighted imaging (eg, magnetization prepared rapid acquisition gradient echo) and 3D T2 fluid-attenuation inversion recovery (FLAIR) of the whole brain for anatomic detail and characterization of suspected malformations of cortical development including subtle focal cortical dysplasias. Coronal T2-weighted and/or T2/FLAIR sequences perpendicular to the hippoc

Cerebellar GABA and executive function among adult female carriers of the fragile X messenger ribonucleoprotein 1 premutation: a pilot study to examine neural underpinnings of the clinical phenotype.

The FMR1 premutation is associated with a complex clinical phenotype, with increased risk for outcomes across the domains of psychological disorders, motor functioning, and reproductive health. A key gap is understanding intermediate processing that may help to explain how the FMR1 premutation alters brain functioning to confer increased risk across these domains. The current study begins to address this gap by focusing on two candidate, interrelated processes: cerebellar GABA and executive function. Data were collected from a cohort of 15 adult female FMR1 premutation carriers and a compariso

Concurrent Unilateral Temporal Glioma and Contralateral Temporal Arachnoid Cyst: Case Series and Review of Literature.

Arachnoid cysts are benign, congenital, cerebrospinal fluid-filled lesions, most often located in the middle cranial fossa, and usually discovered incidentally. Gliomas, in contrast, are the most common primary brain tumors of glial origin. The coexistence of dual intracranial pathologies is rare, and when reported, typically involves arachnoid cysts associated with other congenital or vascular abnormalities. To date, there are no published reports describing the coexistence of a glioma with a contralateral arachnoid cyst. We present two cases of concurrent unilateral temporal glioma with cont

Is temporal lobe epilepsy a progressive disease? Insights from a 5-HT1A receptor partial volume correction study.

BACKGROUND AND OBJECTIVES: Clinical, magnetic resonance (MR) and [18F]fluorodeoxyglucose positron emission tomography (PET) studies suggest that temporal lobe epilepsy (TLE) might be a progressive disease. Whether epilepsy progression affects 5-HT1A receptor concentration is still unknown. METHODS: We used PET with the 5-HT1A ligand [18F]FCWAY in 22 TLE patients. Dynamic frames were acquired for 120-min and modeled with a simplified 3-parameter 2-tissue compartment. An automated, MR-segmentation-based partial volume correction (PVC) algorithm was applied. Contralateral-ipsilateral asymmetry i

Associations between thyroid hormones and biochemical metabolism in major depressive disorder patients with and without comorbid eating disorders.

OBJECTIVES: The underlying mechanism of major depressive disorder (MDD) with eating disorder (ED) remains unclear. We aim to clarify the characteristic changes of THs and brain neurometabolic alterations in ED, and to explore their relationships. METHODS: The study included 28 individuals with MDD and ED, 81 individuals with MDD without ED, and 37 age-matched healthy controls (HCs). Serum TH levels were assessed, and 1H proton magnetic spectroscopy was utilized to determine the N-acetylaspartic acid to creatine (NAA/Cr) and choline-containing compounds to creatine ratios in the prefrontal cor

Evidence for early, clinically silent medial temporal lobe inflammation after CAR-T cell therapy.

OBJECTIVE: Immune effector cell-associated neurotoxicity syndrome (ICANS) is the most frequent neurological complication following chimeric antigen receptor (CAR)-T cell therapy. Long-term cognitive effects remain controversial, even in the absence of clinical neurotoxicity. CAR-T cell-associated neurotoxicity often involves the medial temporal lobe (MTL), yet conventional structural magnetic resonance imaging (MRI) frequently remains normal. There is a need for markers of subclinical neurotoxicity that may predict long-term cognitive outcomes. METHODS: Therefore, we established an imaging pr

Regional brain metabolism and metabolic connectivity alterations in corticobasal syndrome assessed with [18F]FDG-PET.

PURPOSE: Corticobasal syndrome (CBS) is a clinically defined phenotype with different underlying neuropathological substrates most commonly the 4-repeat tauopathy corticobasal degeneration (CBD). 2-[18F] fluoro-2-deoxy-D-glucose Positron Emission Tomography ([18F]FDG-PET) studies have described regional metabolic abnormalities but detailed subcortical and cerebellar structures involvement is less known and metabolic connectivity remains unexplored. This study combined voxel-based, region-of-interest (ROI) analyses and connectivity approaches to further characterize metabolic alterations and to

Transcranial pulse stimulation modulates spectral signatures of Alzheimer's disease in the 3×Tg-AD mouse model.

BACKGROUND: Large-scale brain network dysfunction is increasingly recognized as an important feature of Alzheimer's disease (AD), offering insight into disease mechanisms and opportunities for targeted therapeutic intervention. The spectral features of this dysfunction remain poorly understood, and how neuromodulatory interventions interact with and reshape these frequency-resolved network signatures has yet to be explored. METHODS: Triple-transgenic (3×Tg-AD) mice underwent resting-state functional MRI to assess functional connectivity, signal power, and variance across frequency bands after

Amyloid-related imaging abnormalities after immunotherapy: How might brain microvascular basement membrane components be involved?

Basement membrane components are integral to the physiologic function of cerebral microvessels. Immunotherapy (including by the use of lecanemab) in Alzheimer's disease patients may result in vascular complications identified by neuroimaging (ARIA-E and ARIA-H). This pilot study (on a relatively small number of human brain specimens) suggests one mechanism might be through the effects of lecanemab on a collagen component of said microvessel walls. It utilizes microvessels isolated from human brains (of Alzheimer's disease patients) and maintained in a viable state, to examine this mechanism us

Neuroimaging studies of non-suicidal self-injury in young adults with major depressive disorder.

Non-suicidal self-injury (NSSI) is highly prevalent in adolescents with major depressive disorder (MDD), elevating suicide risk and indicating poor prognosis. While neuroimaging studies have examined NSSI and MDD separately, systematic reviews focusing on major depressive disorder with non-suicidal self-injury (nsMDD) remain scarce. In accordance with PRISMA guidelines, this study systematically reviewed 24 neuroimaging studies (2013-2025) involving 2379 participants (mean age ≤23 years) from PubMed, PsycInfo, and Embase. Using the Newcastle-Ottawa scale for quality assessment, we found 20 fun

Choroidal-ventricular system abnormalities are linked to amyloid-β aggregation in Alzheimer's disease.

INTRODUCTION: Enlargement of the choroidal-ventricular system occurs in aging and Alzheimer's disease (AD), but emerging evidence links these abnormalities to amyloid beta (Aβ) aggregation. We tested this hypothesis by assessing associations between AD pathophysiology and choroidal-ventricular system measures across the AD continuum. METHODS: Ventricular volume, choroid-plexus volume, and ventricular radioactivity after positron emission tomography (PET) tracer injections were analyzed in 385 Translational Biomarkers in Aging and Dementia (TRIAD) and 282 Alzheimer's Disease Neuroimaging Initi

Cerebrospinal fluid metabolites and brain imaging in epilepsy subtypes: Mendelian randomization and multi-omics analysis.

BACKGROUND: Epilepsy is a complex neurological disorder significantly influenced by genetic factors. Observational studies can identify associations, but cannot establish causality. Mendelian randomization (MR) offers a robust tool for identifying etiology. Therefore, using multi-omics, we explored genetically proxied associations among cerebrospinal fluid metabolites, neuroimaging phenotypes, and epilepsy to prioritize candidate genes and potential therapeutic targets. METHODS: Leveraging data from genome-wide association studies, gene expression databases, and the UK Biobank, we performed t
