Extensive Aplasia Cutis Congenita with Calvarial Defect and Superior Sagittal Sinus Exposure in a Term Neonate: A Case Report of Successful Healing with Conservative Management.

BACKGROUND: Aplasia cutis congenita (ACC) is a rare congenital disorder characterized by localized absence of skin, most commonly affecting the scalp. Extensive lesions involving the calvarium and dura, particularly when the superior sagittal sinus is exposed, are uncommon and may be complicated by hemorrhage, infection, thrombosis, neurological injury, and mortality. Management may be conservative or surgical and must be individualized according to defect size and depth, the structures exposed, the clinical condition of the neonate, and the resources and expertise available. CASE PRESENTATIO

Vertical Maxillary Movement Is Associated With Incisal Display in Orthognathic Surgery.

BACKGROUND: Maxillary incisal display (MID) is a key feature in orthodontic and orthognathic surgical planning that critically impacts esthetics and forms the basis for the vertical positioning of the maxilla. In orthognathic surgical planning, several approaches are taken to position the maxilla to determine the MID, but little evidence exists in the relationship between maxillary movement and MID. PURPOSE: The purpose of this study was to measure the association between MID and maxillary movements in maxillary orthognathic surgery. STUDY DESIGN, SETTING, AND SAMPLE: This was a retrospectiv

Navigation-Assisted Pure Endoscopic Transoral Styloidectomy for Recurrent Eagle Syndrome: A Pictorial Essay.

BACKGROUND: Complete resection of the styloid process at its skull-base origin is challenging in recurrent Eagle syndrome because of postoperative fibrosis, distorted anatomy, and proximity to the internal carotid artery (ICA). METHODS: Bilateral dissections of two silicone-injected cadaveric heads defined a pure endoscopic transoral trans-medial pterygomandibular corridor to the styloid base. The technique was applied in a patient with recurrent Eagle syndrome after previous transoral styloid shortening. RESULTS: Cadaveric dissections demonstrated a reproducible route to the styloid base th

Clinical and MRI morphometric characterization of symptomatic cerebellar tonsillar ectopia.

BACKGROUND: Cerebellar tonsils normally extend up to 5 mm below the foramen magnum (FM). Some individuals with tonsillar ectopia (TE) measuring under 5 mm may present with symptoms of Chiari type 1 malformation (CM1). We analyzed MRI data of the posterior cranial fossa (PCF) in affected patients to assess TE's impact on clinical manifestations. METHODS: The retrospective study examined 83 adults with 2-4 mm symptomatic cerebellar TE without syringomyelia (SM), 16 with syndromic CM1 without SM, and 19 controls. Participants with TE were divided into three groups: syndromic (TEs, n = 15), incom

Emerging biomedical imaging applications of tumor-derived extracellular vesicles for premetastatic niche detection: Biological rationale, engineering strategies, and translational challenges.

BACKGROUND: Accurate fetal imaging is essential to the practice of maternal-fetal medicine. While ultrasonography has been the traditional mainstay of fetal imaging, its ability to resolve critical features of central nervous system (CNS) anatomy remains limited. As interest in intrauterine therapy for myelomeningocele has increased, so has the need for more accurate, noninvasive imaging of the CNS. Fetal magnetic resonance imaging (MRI) promises to fill the gap left by ultrasound. METHODS: Thirty-seven MRI scans of fetuses previously diagnosed with myelomeningocele were reviewed by 2 neurora

Supra Occipital Flattening: A Key Sign for Antenatal Diagnosis of Isolated Sagittal Craniosynostosis.

Craniosynostosis is an isolated finding in most cases, involving either the metopic or the sagittal suture. It is detected only sporadically during prenatal ultrasound, in the third trimester for non-isolated cases. When synostosis of the sagittal suture occurs, head shape growth proceeds in an anteroposterior direction, leading to scaphocephaly (long, narrow, boat-shaped skull). Moreover, the reduced vertical growth of the skull vault causes a supra-occipital flattening, detectable at the profile view. Fetuses' head shape diagnosed as dolicocephalic should be carefully studied for the presenc

Evaluating the utility of a novel 3D mobile imaging application in the surgical management of craniosynostosis.

PURPOSE: Craniosynostosis is one of the most common craniofacial defects. It is typically treated with surgical intervention to correct abnormal head shape. Currently, there is no standard method for evaluating treatment effectiveness. We aim to assess the utility of MirrorMe3D, a novel three-dimensional mobile imaging application, in monitoring the head shape changes of two patients with craniosynostosis. METHODS: We implemented the application in the surgical management of a 22-month-old with mild metopic craniosynostosis and a 19-month-old with severe trigonocephaly. Using the application,

Craniofacial measurements using zero echo time magnetic resonance imaging.

OBJECTIVES: Radiation exposure remains a major concern in dentomaxillofacial imaging, especially for children and young adults who require repeated examinations. A precise, non-ionizing alternative for craniofacial measurement would have significant implications for clinical practice. Computed tomography (CT) and cephalometry are widely used but expose patients to ionizing radiation. Zero Echo Time Magnetic Resonance Imaging (ZTE-MRI) enables visualization of short-T2 tissues, including cortical bone, without radiation by minimizing TE towards zero; however, clinical head-and-neck use has been

Natural history of craniocervical alignment in Chiari patients and the impact of posterior fossa decompression.

OBJECTIVE: Chiari malformation (CM) involves a broad disease spectrum, where rare complex CM cases can be associated with craniocervical junction (CVJ) instability and require occipitocervical fusion. However, the natural progression of CVJ alignment in the general CM type I and 1.5 populations treated with posterior fossa decompression (PFD) remains insufficiently characterized. The authors aimed to compare CVJ alignment changes in patients who underwent PFD versus patients with CM who did not undergo surgery. METHODS: The authors conducted a retrospective cohort study at their institution o

Wormian bones: expanded differential diagnosis and implications for abnormal head shape in infancy.

PURPOSE: Wormian bones (WB) are accessory ossicles that develop within cranial sutures. While typically benign, their presence in large numbers has been associated with various genetic and developmental disorders. This study aims to characterize the prevalence, anatomical distribution, and clinical associations of WB in a pediatric population undergoing cranial CT imaging. METHODS: A retrospective review was conducted at Monroe Carell Jr. Children's Hospital at Vanderbilt. Pediatric patients aged 0 to 18 years who underwent cranial CT imaging for any clinical indication were included. WB were

Age-related patterns and strong correlation between cranial and ear asymmetry in healthy children: a CT-based cross-sectional study.

PURPOSE: Severe cranial deformity has been thought to cause secondary ear misalignment; however, the developmental patterns of cranial and ear asymmetry have not been systematically characterized. We aimed to clarify age-related variations and the interrelationship of cranial and ear asymmetry in healthy Japanese children using head computed tomography (CT). METHODS: We retrospectively analyzed CT data from children imaged for minor head trauma between 2006 and 2023. Cranial asymmetry was assessed using the cranial vault asymmetry index (CVAI), and ear asymmetry was quantified by the absolute

The effect of jugular foramen volume on the development of idiopathic intracranial hypertension.

IntroductionIdiopathic intracranial hypertension is a condition characterized by elevated intracranial pressure in the absence of an intracranial mass lesion and is commonly associated with neuro-ophthalmic symptoms. Although venous outflow abnormalities, particularly transverse sinus stenosis, have been extensively investigated in idiopathic intracranial hypertension, the potential role of jugular foramen volume has not been previously studied. Considering that major venous structures pass through the jugular foramen, this study aimed to evaluate whether jugular foramen volume plays a role in

Uniportal full-endoscopic decompression of the posterior cranial fossa: a comparative anatomical study of defect assessment using ultrasonography versus radiography and computed tomography.

STUDY AIMS: The surgical treatment of symptomatic Arnold-Chiari malformation type I (CM-I) typically involves decompression of the posterior cranial fossa. A full-endoscopic approach is emerging as an alternative to traditional microsurgical techniques. However, this method presents challenges in intraoperative assessment of the decompression extent, potentially resulting in inadequate decompression. In neurosurgery, ultrasound is gaining increasing importance, as it avoids ionizing radiation and enables real-time examination. The aim of this anatomical study is to evaluate the suitability of

Narrower jugular bulbs and their tributaries are associated with skull base venous hyperintensity on arterial spin-labeling MRI.

Incidental arterial spin-labeling MRI hyperintensity (ASL+) at the jugular bulb (JB) can mimic dural arteriovenous fistula but its anatomical basis remains incompletely understood. We investigated whether JB and tributary sizes are associated with ASL+. We retrospectively analyzed MRIs from 25 ASL+ patients and 25 controls, using post-contrast SPGR images to measure JB and tributary diameters. Group comparisons, effect sizes, false discovery rate (FDR) correction, and receiver operating characteristic (ROC) analyses were performed, with leave-one-out cross-validation (LOOCV) to assess model st

Bony venous malformations in facial bones: Diagnosis and treatment of improperly called cavernous hemangioma.

This case series and literature review introduces Bony Vascular Malformations (BVM) of facial bones as a well-defined entity to provide a more accurate reference to these often mislabeled vascular anomalies, aiming to standardize terminology, elucidate their clinical-radiological features and guide their management. This retrospective review analyzes 20 patients treated between 2017 and 2024, all affected by venous malformations involving facial bones. Facial BVMs are apparently rarer than spine or skull bony venous malformations, but they are often misdiagnosed as hemangiomas and, as such sta

Deep learning for discriminating cochlear malformations on temporal bone CT.

OBJECTIVES: Diagnosis of cochlear malformation on temporal bone CT images is often difficult because the imaging findings are frequently subtle. Our aim was to assess the utility of deep learning analysis in diagnosing cochlear malformation on temporal bone CT images. METHODS: A total of 373 temporal bone CTs (187 with normal and 186 with malformed cochlea) were included in this study. The network Swin UNETR was used for cochlea automatic segmentation, then we divided it into left and right sides. three classification networks, ResNet50, EfficientNet-B0 and DenseNet121, were used for training
