Fully quantitative CMR rest perfusion reveals myocardial perfusion abnormality in Kawasaki disease: Association with left ventricular Remodeling.

BACKGROUND: Kawasaki disease (KD) is associated with various myocardial injuries, including myocardial perfusion abnormality. This study aims to assess the distribution of myocardial perfusion abnormality in patients with KD and explore their relationships with left ventricular (LV) remodeling. METHODS: This prospective single-center study enrolled children with KD who underwent fully quantitative CMR rest perfusion. Myocardial blood flow (MBF) was measured and corrected based on the heart rate-blood pressure product (MBFcor). Statistical analyses included ANOVA, Pearson correlation and multi

Diffuse Perfusion Impairment Versus Global Myocardial Flow Reserve and Focal Perfusion Impairments in Patients Undergoing 82Rb-PET Imaging.

BACKGROUND: Although global myocardial flow reserve (MFR) is a powerful prognostic factor, it may overlook segmental abnormalities and does not differentiate focal from diffuse perfusion impairments. Therefore, we sought to assess whether integrated MFR provides incremental prognostic value beyond global MFR. METHODS: Consecutive patients undergoing positron emission tomography myocardial perfusion imaging between 2019 and 2025 were included. For each patient, global MFR and the proportion of the myocardium with diffuse and focal integrated MFR impairment were quantified. The primary outcome

Association between job insecurity and cardiovascular diseases in workers with type 2 diabetes mellitus.

In organs, the correct architecture of vascular and ductal structures is indispensable for proper physiological function, and the formation and maintenance of these structures is a highly regulated process. The analysis of these complex, 3-dimensional structures has greatly depended on either 2-dimensional examination in section or on dye injection studies. These techniques, however, are not able to provide a complete and quantifiable representation of the ductal or vascular structures they are intended to elucidate. Alternatively, the nature of 3-dimensional plastic resin casts generates a pe

Coronary microvascular dysfunction in systemic sclerosis: insights from PET-derived myocardial flow reserve.

OBJECTIVES: Cardiovascular death is the second leading cause in systemic sclerosis (SSc), with coronary microvascular dysfunction (CMVD) playing a key role. PET-derived myocardial blood flow (MBF) and flow reserve (MFR) offer a validated, non-invasive way to assess CMVD. We studied the prevalence of impaired MFR in SSc and its association with vasodilator or immunomodulatory therapy. METHODS: SSc patients who underwent dynamic 82-Rubidium PET myocardial perfusion imaging (MPI) at Yale New Haven Hospital (July 2016 to April 2025) were studied. Patients were matched 3:1 with controls without au

Incremental Prognostic Value of Subendocardial Myocardial Flow Reserve in Patients With Normal Perfusion.

BACKGROUND: Although the prognostic utility of positron emission tomography (PET) myocardial flow reserve (MFR) is well established, emerging data suggest that reduced subendocardial flows also predict adverse outcomes. However, the incremental value of subendocardial MFR (MFRSE) beyond transmural MFR (MFRTM) remains unclear. METHODS: We studied patients in a multicenter PET registry with normal perfusion on stress/rest Rb-82 PET, excluding those with a previous history of coronary artery bypass surgery, heart transplantation, or left ventricular ejection fraction <40%. The optimal MFRSE cuto

Non-invasive and invasive diagnostic modalities for the assessment of coronary vasomotor dysfunction in ANOCA patients.

In patients who have angina with non-obstructive coronary artery disease (ANOCA), invasive coronary function testing (ICFT) is the gold standard to comprehensively diagnose coronary dysfunction. Coronary dysfunction is divided into the endotypes coronary vasospasm, coronary endothelial dysfunction, and coronary microvascular dysfunction (i.e., an abnormal reduced coronary flow reserve [CFR] and/or enhanced microvascular resistance [MR]). However, because of the inherently invasive nature of ICFT, it is important to investigate non-invasive approaches for the diagnosis of coronary dysfunction.

Combined Anatomic and Functional Testing Identifies Patients With Obstructive Coronary Artery Disease Who Benefit From Revascularization.

BACKGROUND: In patients with obstructive coronary artery disease, early revascularization does not improve outcomes but may reduce angina symptoms. The objective of this study was to examine whether changes in health status outcomes following revascularization are explained by the extent of myocardial perfusion defects and improvement in myocardial perfusion. METHODS: Two trials enrolling stable patients with new-onset chest pain suggestive of obstructive coronary artery disease, the ISCHEMIA (International Study of Comparative Health Effectiveness With Medical and Invasive Approaches) and th

Incremental Prognostic Power of Coronary Flow Reserve and Artificial Intelligence-Enabled ECG-Derived Filling Pressure in Angina With Nonobstructive Coronary Arteries.

BACKGROUND: Coronary flow reserve reflects microvascular function, whereas filling pressure indicates myocardial hemodynamic burden. In angina with nonobstructive coronary arteries, abnormalities in either could contribute to a supply-demand mismatch; however, their combined prognostic significance remains unclear. METHODS: Patients with angina with nonobstructive coronary arteries who underwent invasive coronary reactivity tests were studied retrospectively. Coronary microvascular dysfunction (CMD) was coronary flow reserve <2.5. An artificial intelligence-enabled ECG marker of elevated fill

Microvascular vasomotor dysfunction is more prevalent in female versus epicardial vasomotor dysfunction in male ANOCA patients.

BACKGROUND: Coronary vasomotor dysfunction, diagnosed by coronary function testing (CFT), has only recently been recognized as important contributor to chronic coronary syndromes, exposing knowledge gaps on sex and age differences. Therefore, these differences are evaluated in the Dutch national CFT registry (NL-CFT). METHODS: Included were 1008 patients (83% women, 60 ± 9.6 years old) with angina and non-obstructive coronary arteries (ANOCA) undergoing CFT between 2020 and 2024. CFT consisted of coronary vasospasm provocation testing and reduced microvascular vasodilatory capacity assessment

Hemodynamic analysis of the aortic root based on differences in outflow graft angle in patients with implantable left ventricular assist devices.

Aortic insufficiency (AI) that develops during long-term support with continuous-flow left ventricular assist devices (LVADs) remains a significant clinical problem. Abnormal flow patterns and altered wall shear stress (WSS) on the aortic valve are believed to contribute to leaflet remodeling, yet the hemodynamic influence of outflow graft orientation in patient-specific anatomies is not fully understood. This study investigated how different outflow graft angles affect aortic valve WSS and coronary perfusion using computational fluid dynamics (CFD). Three LVAD recipients who underwent preoper

Perforation of the right ventricle and inferior vena cava by bone cement after percutaneous vertebroplasty: A case report.

RATIONALE: Percutaneous vertebroplasty (PVP) is widely used to treat vertebral compression fractures. Although it is generally considered a minimally invasive procedure, leakage and systemic migration of polymethylmethacrylate bone cement may rarely occur and can cause potentially fatal cardiovascular complications. Intracardiac bone cement embolism accompanied by perforation of the right ventricle and inferior vena cava (IVC) is particularly uncommon. PATIENT CONCERNS: A 70-year-old woman presented with palpitations and perioral numbness 2 months after undergoing PVP for an L4 vertebral comp

ALCAPA syndrome: a rare cause of sudden cardiac arrest in adulthood?-a case report.

BACKGROUND: Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare coronary artery anomaly occurring in approximately 1 in 300,000 live births. Typically diagnosed in infancy, ALCAPA can lead to significant cardiovascular complications, including myocardial ischemia, heart failure, and sudden cardiac death if untreated. The adult form of ALCAPA is less common and often identified incidentally through modern cardiac imaging techniques. CASE PRESENTATION: We describe the case of a 30-year-old white German woman who suffered an out-of-hospital cardiac arrest. I

Incremental prognostic value of AI-based coronary CT angiography combined with PET/CT dynamic myocardial perfusion imaging in patients with suspected or confirmed coronary artery disease.

BACKGROUND: Coronary computed tomography angiography (CCTA) and positron emission tomography/computed tomography (PET/CT) myocardial perfusion imaging (MPI) provide complementary anatomical and functional information for coronary artery disease (CAD). However, the prognostic value of integrating CCTA-derived coronary imaging characteristics with PET-MPI parameters remains to be established. Therefore, this retrospective single-center study aimed to evaluate the prognostic and incremental value of integrating artificial intelligence (AI)-based CCTA with PET/CT MPI for predicting major adverse c

Microvascular resistance reserve: impact on health status and myocardial perfusion after revascularization in chronic coronary syndrome.

BACKGROUND AND AIMS: The microvascular resistance reserve (MRR) is a novel invasive index of the microcirculation, which is independent of epicardial stenoses, and has both diagnostic and prognostic implications. This study investigates whether MRR is associated with health status outcomes by revascularization in patients with moderate coronary stenoses. METHODS: Consecutive patients with stable chest pain and moderate (30%-90% diameter) stenoses on invasive coronary angiography (n = 222) underwent invasive physiology assessment.
