BACKGROUND: Loiasis is a chronic filarial infection endemic to Central and West Africa. Although long considered benign, increasing evidence links loiasis to substantial morbidity and mortality. The infection is associated with immune modulation, including Th2-skewed responses and elevated regulatory cytokines. Clinically, loiasis is classified as microfilaremic (presence of circulating microfilariae) or amicrofilaremic ("occult") disease, the latter defined by a history of eyeworm migration without detectable microfilaremia. This study investigated how chronic L. loa infection influences anti

Loiasis exists in regions where malaria is highly endemic, yet few studies have investigated their association as concomitant infectious diseases. Secondary data analysis from a cross-sectional survey conducted in Gabon (2015-2016) was performed to assess the association between malaria and loiasis. A total of 947 participants of all ages were enrolled in the original study. In crude analyses, malaria showed a seemingly protective association with loiasis, manifesting in an odds ratio (OR) of 0.67 (95% CI: 0.45 to 1.01; p = 0.0521). This borderline association disappeared completely after adju

Loiasis is a parasitic infection caused by the filarial nematode Loa loa, endemic to Central and West Africa. Its occurrence in non-endemic regions such as Morocco is rare and may pose a diagnostic challenge. We report the case of a Moroccan patient presenting with an intermittent sensation of a mobile foreign body in the eye, associated with redness and photophobia. The patient reported no recent travel to endemic regions but had close contact with individuals originating from Cameroon, a loiasis-endemic area, raising questions regarding possible indirect or remote exposure. Following several

Filariasis is a parasitic infection that mostly involves the skin and ocular surface, although all layers of the eye and orbit can be affected. We report a case of a 42-year-old woman with Loa loa infection manifesting as a sequential bilateral forehead Calabar swelling, severe orbital cellulitis and thereafter mimicking a conjunctival tumor. Anterior segment optical coherence tomography helped to differentiate between a neoplasm and the filarial worm and show typical features such as longitudinal and cross-sections of the parasite. The worm was surgically removed and microbiologists confirmed

BACKGROUND: Loa (L.) loa hypermicrofilaraemia (≥ 8,000 mf/mL) increases the risk of severe adverse events during mass ivermectin administration for onchocerciasis control. Albendazole has been proposed as a potential alternative for reducing microfilaraemia prior to ivermectin administration.
METHODOLOGY AND PRINCIPAL FINDINGS: This prospective study was conducted in northern Gabon from November 2021 to April 2022. Individuals infected with L. loa were screened and allocated to three groups: two treatment arms receiving 400 mg or 800 mg of albendazole daily for 30 days among hypermicrofilarae

The Death to Onchocerciasis and Lymphatic Filariasis (DOLF) project conducts clinical and translational research to accelerate the global elimination of lymphatic filariasis (LF) and onchocerciasis (river blindness). Research needs were identified during several meetings of experts and helped study design. Over the last 15 years the project has conducted mass drug administration (MDA) studies and clinical trials with outstanding research teams in 11 countries that have led to many important discoveries. For example, DOLF studies showed that annual MDA was as effective as semiannual MDA for LF

BACKGROUND: Loiasis is a parasitic disease caused by the filarial nematode Loa loa. CASE PRESENTATION: This report describes loiasis in a 6-year-old child presented to a public hospital in Yaoundé, Cameroon, with an immobile worm located beneath the conjunctiva of the left eye. The calibrated thick blood smear for microfilariae was negative and there are no other typical signs of the disease, although the patient has not taken any antifilarial medication. CONCLUSION: This case highlights the importance of continued vigilance for loiasis, raising awareness of the disease, and promoting early di

BACKGROUND: The design and analysis of randomized clinical trials (RCTs) in filarial diseases such as onchocerciasis, loiasis, and mansonellosis pose unique statistical challenges, including skewed endpoints and limited sample sizes. This systematic review summarizes design and analysis approaches of RCTs conducted in these diseases with a focus on the statistical methodology.
METHODS AND FINDINGS: A systematic search was conducted in PubMed and four trial registries to identify RCTs investigating treatments for onchocerciasis, loiasis, and mansonellosis published or registered between 2000 a

Loiasis is a vector-borne filarial infection endemic to parts of sub-Saharan Africa. It disproportionally affects economically disadvantaged communities in rural, forested regions. To better understand the economic burden of loiasis, we conducted a comprehensive cost-of-illness study in an endemic region of Gabon, with the aim of quantifying the financial costs incurred by individuals infected with the disease from a societal perspective. We conducted a cross-sectional survey in 2023 in rural Gabon. Study participants took part in diagnostic testing for loiasis and were interviewed based on a

OBJECTIVE: Although loiasis affects more than 20 million people and that its associated clinical complications can cause multiple organ failures, this disease remains understudied. Consequently, the elimination of loiasis is not on the agenda of public health services. Defining an effective strategy to tackle this disease is challenging and requires a better understanding of the parasite's biology, including its genetic aspects. Loa loa adult worms provide large amounts of biological tissues for genetic sequencing, but are very difficult to collect. Therefore, limited genomic data are availabl

BACKGROUND: Human filariases remain a major group of neglected tropical diseases (NTDs) in Africa. Despite large-scale control strategies and availability of effective drugs, persistent and re-emerging infections indicate gaps in surveillance and diagnostic capacity. Over the past two decades, diagnostic technologies and elimination programs have evolved, yet no synthesis has assessed how these changes affected research output and prevalence estimates across the continent. The objective of this study was to systematically review and synthesize population-based studies on human filariases in Af

BACKGROUND: Ivermectin is widely used in mass drug administration programs for the control of onchocerciasis and lymphatic filariasis. In areas where Loa loa is co-endemic, ivermectin administration may precipitate severe neurological adverse events among individuals with high microfilarial densities. We report a case of probable Loa loa-associated encephalopathy following ivermectin administration in a patient with malaria infection.

Loiasis is known for two main clinical manifestations: eyeworm and Calabar swellings. Calabar swellings are non-pitting edemas, often on the upper limbs or presenting as unilateral orbital swelling. They are described as itchy or painful, limit mobility, and cause notable morbidity. Although adult Loa loa filariae (macrofilariae) are known to migrate widely through host tissues, their exact pathways and tissue preference (tropism) remain unclear. Whilst the exact mechanisms remain unclear, Calabar swellings are considered to be localized allergic reactions to worm antigens, resulting in angioe

INTRODUCTION: Loiasis has been associated with increased morbidity and mortality, particularly in cases of high microfilarial density. Treatment depends on initial microfilaraemia (Mf) and relies on ivermectin (IVM), diethylcarbamazine (DEC) or albendazole (ALB), all associated with serious adverse events. Our objective was to describe the clinical characteristics, management and outcomes of patients diagnosed with loiasis and Mf in France.
METHODS: We conducted a multicenter retrospective study including patients with loiasis and Mf diagnosed and treated in France between 2000 and 2022, with

OBJECTIVES: Onchocerciasis remains a major public health problem in the Democratic Republic of Congo (DRC), one of the countries with the highest burden globally. Despite more than two decades of community-directed treatment with ivermectin (CDTI), progress toward elimination has been slower than expected. This review examines the main epidemiological, operational, and governance-related barriers that continue to hinder elimination efforts.
METHODS: We conducted a structured narrative review informed by World Health Organization (WHO) frameworks for evaluating neglected tropical disease progr

BACKGROUND: Onchocerciasis and loiasis are co-endemic filarial neglected tropical diseases in Central and West Africa. While ivermectin-based mass drug administration has reduced onchocerciasis burden, it can trigger severe neurological adverse events in individuals with high Loa loa microfilaraemia. This limitation highlights the urgent need for safe macrofilaricidal therapies and appropriate preclinical models.
METHODOLOGY/PRINCIPAL FINDINGS: We conducted a systematic review following PRISMA guidelines to evaluate animal models used for preclinical drug screening in onchocerciasis and loias

In regions where Loa loa is coendemic with lymphatic filariasis and onchocerciasis, mass drug administration of ivermectin has posed a risk for serious adverse reactions in individuals with high microfilarial loads. To overcome this problem, easy-to-use point-of-care diagnostic tools are crucial for identifying individuals at risk in population-based control programs. The NTDscope is a device that uses smartphone components for video microscopy and may support decentralized L. loa microfilariae screening under field conditions. This study evaluated the usability and operational feasibility of

This study aimed to achieve consensus on the case definitions and clinical aspects of loiasis, a filarial parasite infection caused by Loa loa, in order to support the development of guidelines for diagnosis and clinical management and to provide harmonised definitions for future research and scientific communication. We applied the Delphi method. Statements on the definition of eyeworm, Calabar swelling, possible/probable/confirmed loiasis, occult loiasis, and the classification of loiasis clinical manifestations were submitted individually to a panel of 18 experts participating in the guidel

BACKGROUND: Loiasis is a filarial disease caused by Loa loa, endemic to Central and West Africa. Cases are observed in migrants and occasionally travellers. Its diagnosis may be difficult due to the unspecific clinical picture and the high percentage of people who do not have microscopically detectable circulating microfilariae. We performed a landscape analysis of the laboratory-based diagnostic techniques available for loiasis and of their estimated sensitivity and specificity.
METHODS: We performed a systematic review of cross-sectional, cohort, case-control, diagnostic accuracy, and clini

BACKGROUND: Previously, antifilarial antibody testing in the evaluation of returned travelers and immigrants to North America has relied on IgG- and IgG4-specific responses to crude filarial extracts (BmA), tests that suffer from significant cross-reactivity with Strongyloides and cannot distinguish among the infecting filarial species. Nevertheless, between 1992 and 2016 we have tested >11000 samples from providers across North America in whom a filarial infection was being considered; close to 20% were able to be definitively diagnosed with a filarial infection (based on IgG4 anti-BmA reacti
