Facial nerve injury following superficial temporal artery biopsy.

BACKGROUND: With proper training, superficial temporal artery biopsy is a safe office procedure with few complications. Surgeons from different disciplines, including ophthalmologists, dermatologists, general surgeons, and plastic surgeons may be called upon to perform this common procedure to confirm giant cell arteritis. OBJECTIVE: To emphasize the surgical anatomy of the scalp as it pertains to superficial temporal artery biopsy and to raise awareness that significant complications can occur after superficial temporal artery biopsy. METHOD: Case report. RESULTS: A 75-year-old woman with

Laser-assisted hair transplantation: histologic comparison between CO2 and Ho:YAG lasers.

BACKGROUND: Various laser wavelengths and devices have been advocated for use in the creation of recipient channels during hair transplant surgery, including flash-scanned CO2, Ho:YAG (lambda = 2.12 microm), and Er:YAG (lambda = 2.94 microm). OBJECTIVE: To determine the tissue injury caused by flash-scanned CO2 and pulsed Ho:YAG lasers during the creation of hair transplant recipient channels and to assess the efficacy of the Ho:YAG laser. METHODS: Recipient channels were created in vivo in human scalp tissue using both lasers, and were excised and prepared for histologic examination. Optica

The preputium: an overlooked skin graft donor site.

Plastic surgeons often encounter tissue defects consisting of the superficial layer of the skin. Fortunately, many of these defects are reconstructed easily with skin grafts. The authors have used the preputium as a skin graft for the reconstruction of skin defects of various sizes and locations in children. The defect size ranged from 2.3 x 4.0 cm to 4.8 x 6.5 cm. The average defect size was 3.5 x 5.5 cm. The defects were caused from trauma, acute burn, and release of burn contractures, and were located in the lower extremity, upper extremity, and scalp. All patients were treated successfully

The sandwich temporoparietal free fascial flap for tendon gliding.

Microsurgical transfer of the superficial and deep temporal fascia based on the superficial temporal vessels has been documented. This article analyzes the functional recovery when each layer of this facial flap is placed on either side of reconstructed or repaired tendons, to recreate a gliding environment. This fascial flap also provided a thin, pliable vascular cover in selected defects of the extremities. Six patients (four male and two female) with tendon loss and skin scarring of the hand (three dorsum, one palmar, and one distal forearm) and posttraumatic scarring of the ankle with tend

[Reconstruction of both eyebrows with scalp flaps with the anastomotic branch in occipital region of superficial temporal artery as the pedicle].

OBJECTIVE: To explore the effects of the reconstruction of both eyebrows with scalp flaps with pedicles supplied by superficial temporal arteries. METHODS: The parietal branch of the superficial temporal and its anastomotic branch of occipital artery at temporoparietal region on the same side were designed as the pedicles, with which two scalp flaps on temporoparietal and temporo-occipital regions with reverse directions were employed to restore the eyebrows on both sides. RESULTS: Four patients were treated with this method with satisfactory appearance and survival rate (100%). The reconstr

Replantation of avulsed scalps and secondary aesthetic correction.

Five patients with avulsed scalps were treated with replantation between 1992 and 1998. All patients were women age 20 to 36 years. The percentage of the avulsed scalp ranged from 50% to 100% of the whole scalp. The vessels chosen for anastomosis were the superficial temporal artery, occipital artery, and superficial temporal vein. A vein graft harvested from the cephalic vein of the forearm was performed on the venous and arterial sides in 1 patient. Two patients experienced complete survival of the replanted scalp. Three patients showed 40%, 50%, and 80% survival areas, with the remaining de

Syringe needle located deep in the brain: image-guided removal.

BACKGROUND: Removal of a thin but long foreign body lodged in the deep brain tissue may need special technique to remove it without causing major injury. We report a special technique for the safe, elective removal of such objects. CASE DESCRIPTION: A 5-year-old boy presented following mild head trauma; he was asymptomatic and physical examination revealed an intact scalp and no neurological deficit. The incidental presence of a 3.4 cm-long syringe needle in the right frontal region was noted on the skull radiographs. Cranial computed tomography (CT) scan showed the deeper end of the needle i

Prevention and treatment of wide scar and alopecia in the scalp: wedge excision and double relaxation suture.

The visible linear scar of the scalp is a cosmetically serious complication of a scalp incision in scalp surgery, forehead lift, and craniofacial surgery, especially on the temporal scalp. Its causes are cicatrical alopecia and scar widening. To solve this problem, we performed the wedge excision of the scalp and the double relaxation suture of the galea in 2 patients undergoing facial surgery through the coronal approach and in 15 patients with scalp alopecia ranging from 0.5 to 3.0 cm in width. The wedge excision using the beveling incision at an angle of 30 degrees to the hair follicles pre

Pediatric scalp laceration repair complicated by skin staple migration.

Skin staples are ideal for pediatric scalp laceration closure because of their rapidity of placement and economy and ease of use. We report two cases of rotatory staple migration necessitating improvised removal techniques. Clinicians should be alert for this complication, which may result from a combination of staple design, local anatomic factors, superficial placement, and prolonged delay prior to removal.

Secondary expansion of a replanted scalp salvaged by an intrinsic arteriovenous shunt.

1. Although scalping injuries reported in most large series rarely involve young children, partial scalp replantation, even in a 2-year-old boy after a dog-bite avulsion, should also be warranted. However, technical difficulties because of the diminutive vasculature can be expected. 2. Unconventional methods for reestablishing inflow or outflow for the replanted scalp may become the only practical alternative and must not be overlooked. In this case, an arteriovenous shunt between the frontal branch of the superficial temporal artery within the amputated part itself to a more distal scalp vein

Sequential use of instruments at operative vaginal delivery: is it safe?

OBJECTIVE: Our purpose was to examine maternal and neonatal outcomes in a cohort of women who underwent delivery with the sequential use of instruments. STUDY DESIGN: This retrospective case-control study included deliveries from May 1996 through March 1998. Charts of women who underwent delivery with the sequential use of instruments (vacuum first, then forceps, or vice versa) were identified. Two control groups (1 forceps group, 1 vacuum group) were randomly selected and matched for each case. Maternal and neonatal outcomes were abstracted and compared. RESULTS: There were 34 patients in e

Traumatic aneurysm of the superficial temporal artery as a complication of pin-type head-holder device. Case report.

BACKGROUND: Despite the widespread use of pin head-holder devices in neurosurgical procedures, associated complications are relatively infrequent and usually minor. Inadvertent puncture of a major scalp vessel is one of these complications. Usually it is not problematic but the injured vessel may develop a traumatic aneurysm with subsequent rupture. CASE DESCRIPTION: We report the case of a 51-year-old man who underwent a left pterional craniotomy for intracranial aneurysm surgery. The head was fixed with the Sugita pin head-holder. Three weeks after discharge, the patient returned to the hos

Eyelid laceration in a neonate by fetal monitoring spiral electrode.

PURPOSE: To report an unusual case of birth injury caused by an internal fetal monitoring spiral electrode. METHOD: Case report. RESULTS: A neonate with eyelid lacerations was examined within the first hour of life after a fetal scalp electrode was inadvertently affixed to the left upper eyelid during labor. The superficial lacerations healed without sequelae, and there was no globe injury. CONCLUSIONS: Ocular adnexal birth injury by a fetal monitoring scalp electrode has been reported rarely. Because fetal brow malpresentation is uncommon, the occurrence of this injury is unusual. Although

Brain temperature and limits on transcranial cooling in humans: quantitative modeling results.

Selective brain cooling (SBC) of varying strengths has been demonstrated in a number of mammals and appears to play a role in systemic thermoregulation. Although primates lack obvious specialization for SBC, the possibility of brain cooling in humans has been debated for many years. This paper reports on the use of mathematical modeling to explore whether surface cooling can control effectively the temperature of the human cerebrum. The brain was modeled as a hemisphere with a volume of 1.33 1 and overlying layers of cerebrospinal fluid, skull, and scalp. Each component was assigned appropriat

Vacuum extraction: does duration predict scalp injury?

OBJECTIVE: To identify variables that increase the chance of neonatal scalp injury during vacuum extraction. METHODS: We conducted a prospective observational study of 134 vacuum extraction-assisted deliveries at Olive View-UCLA Medical Center in 1995. Data collected included parity, gestational age, duration of first and second stages of labor, indication for operative delivery, station and position of fetal head, duration of vacuum application, number of "pop-offs," neonatal weight, and descriptions of scalp marks or injury. Cranial imaging studies were obtained if clinically indicated. RE

[The problems in the use of superficial cervical artery skin flap and it's relevant solution].

Superficial cervical artery skin flap is widely used in clinical practice. In order to inprove the outcome of the flap in clinic, eleven cases of skin defect of scalp who were treated with the flap was discussed. After operation, the donor area healed but there was no hair growth on recipient area. Among them, six cases occurred partial necrosis of skin flaps. In order to avoid these problen, the relevant solution discussed as follows: 1. Handle well the pedicle of the skin flap to prevent the interference with venous returm. 2. Adhere strictly to indications. 3. Apply skin expander to obtain

Endoscopic-assisted midface lift utilizing retrograde dissection.

Endoscopic facial surgery is gaining wide acceptance, as results are providing comparable if not superior to the standard, superficial musculoaponeurotic system rhytidectomy, with minimal scarring. Frontal branch facial nerve injury remains a troublesome complication. The purpose of our study was to determine if a subciliary retrograde dissection would decrease facial nerve injury and allow for more accurate midfacial suspension. Fresh cadaver dissections were performed using endoscopic equipment. Anterograde temporal incisions in the scalp and retrograde subciliary incisions were studied. The

Pseudoaneurysm of the superficial temporal artery.

Ours is the first report of pseudoaneurysm of the superficial temporal in a 90-year-old woman. Although falls with associated head and scalp injuries in the geriatric population are common, pseudoaneurysm of the superficial temporal artery is rarely reported, probably due to the generally protected course of the superficial temporal artery across the skull. Clinicians' familiarity with this entity is important for early diagnosis and intervention.
