Emphysematous Osteomyelitis: Three Rare Cases.

Emphysematous osteomyelitis is a rare condition caused by gas-producing bacteria in the bone and may be visualized by the presence of intraosseous gas on imaging. We report three cases of emphysematous osteomyelitis. The first case is an 80-year-old with acute myeloid leukemia with emphysematous osteomyelitis of the sacrum, right iliac bone, and multiple vertebrae and ribs. Blood cultures grew Enterococcus faecium and Clostridium spp. The second case is a 45-year-old man with diabetes mellitus with emphysematous osteomyelitis of the proximal left tibia with blood cultures positive for Proteus

Global, regional, and national burden of HIV/AIDS, 1990-2023, with the estimated burden attributable to intimate partner violence and forecasted impacts of funding cuts through 2030: results from the Global Burden of Disease Study 2023.

BACKGROUND: Despite a general improvement in the HIV burden over the past two decades, gendered social determinants including intimate partner violence (IPV) continue to affect women's vulnerability to HIV, which could be further exacerbated as resources dwindle over the coming years. Our study aimed to quantify recent trends in the HIV burden, the magnitude of the HIV burden associated with IPV, and the potential impact of declining financial support globally. METHODS: Using the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2023 framework, we estimated annual HIV incidenc

Global, regional, and national burden of meningitis, its risk factors, and aetiologies, 1990-2023: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Meningitis remains the leading infectious cause of neurological disabilities globally, disproportionately affecting children younger than 5 years and populations in the African meningitis belt. Whereas previous global estimates focused on ten pathogen categories, this study presents the most comprehensive analysis to date, assessing the meningitis burden attributable to 17 causative pathogens based on the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2023 framework. METHODS: GBD is a systematic, scientific effort aimed at quantifying the comparative magnitude o

Global burden of enteric infectious diseases, diarrhoeal diseases, and corresponding aetiologies, 1990-2023: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Enteric infectious diseases claim more than 1 million lives annually and are among the top ten causes of death in children younger than 5 years. Remarkable global investment has been dedicated to enteric infectious disease prevention and control; however, the shifting global health landscape is testing the continuance of progress. To evaluate the current status and guide future interventions, we present the latest epidemiological estimates of enteric infectious diseases from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2023 and assess progress towards the G

Advanced limb salvage reconstruction following complicated myxoid liposarcoma resection using hybrid gastrocnemius flap and sub-flap irrigating vacuum assisted closure therapy: a case report.

Myxoid liposarcoma is a malignant soft-tissue sarcoma with a high risk of local recurrence. Repeated surgery and adjuvant treatment may compromise soft tissues and therefore predispose patients to chronic infection, creating challenges for limb salvage. We report a case of recurrent myxoid liposarcoma complicated by chronic surgical-site infection that was successfully managed using gastrocnemius flap reconstruction combined with sub-flap irrigating vacuum-assisted closure therapy. A 33-year-old man presented with recurrent myxoid liposarcoma of the posterior left knee following multiple previ

Global, regional, and national prevalence of second-hand smoke and attributable disease burden in 204 countries and territories, 1990-2023: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Second-hand smoke (SHS) exposure remains a major source of morbidity and mortality among non-smokers. This study presents the first dedicated Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) publication to systematically quantify SHS prevalence and evaluate its effect as a global risk factor across a broader range of outcomes. METHODS: Using the GBD 2023 comparative risk assessment framework, we estimated SHS exposure prevalence and attributable disease burden across 204 countries and territories from 1990 to 2023, stratified by age and sex. Exposure estimates wer

Irreducible shoulder dislocation in a patient with Alzheimer's disease: A case report.

INTRODUCTION: Anterior shoulder dislocations are common injuries usually managed by closed reduction. However, irreducible cases, especially in elderly patients with Alzheimer's disease, complicate both diagnosis and management. CASE PRESENTATION: A 78-year-old female with Alzheimer's disease presented with severe right shoulder pain and reduced function following physiotherapy. Cognitive decline complicated neurovascular assessment and communication. Radiographs identified a right anterior shoulder dislocation with a displaced greater tuberosity fracture. Multiple closed reductions under sed

Prospective observational study to assess the performance accuracy of clinical decision rules in children presenting to emergency departments with possible cervical spine injuries: the Study of Neck Injuries in Children (SONIC).

INTRODUCTION: Paediatric cervical spine injury (CSI) is uncommon but can have devastating consequences. Many children, however, present to emergency departments (EDs) for the assessment of possible CSI. While imaging can be used to determine the presence of injuries, these tests are not without risks and costs, including exposure to radiation and associated life-time cancer risks. Clinical decision rules (CDRs) to guide imaging decisions exist, although two of the existing rules, the National Emergency X-Radiography Low Risk Criteria and the Canadian C-Spine Rule (CCR), focus on adults and a n

In vitro, genomic characterization and pre-clinical evaluation of a new thermostable lytic Obolenskvirus phage formulated as a hydrogel against carbapenem-resistant Acinetobacter baumannii.

The urgent threat of carbapenem-resistant Acinetobacter baumannii (CRAB) necessitates the development of new antimicrobial strategies. Bacteriophage (phage) therapy is one of the most promising alternative strategies that can be implemented to combat multidrug-resistant (MDR) bacterial infections. Herein, an A. baumannii phage VB_AB_Acb75 that exhibited lytic activity against 6 CRAB isolates (21.43%) with stability at up to 70 °C, pH 2-12, and high concentrations of organic solvents was isolated and characterized. The transmission electron microscope (TEM) detected a tailed phage with an icosa

Penetrating Laryngeal Trauma: Management of Severe Injuries Without Open Reduction Internal Fixation.

OBJECTIVE: Thyroid and cricoid cartilage fractures that are displaced or have multiple fracture lines are traditionally managed with open reduction and internal fixation (ORIF). In practice, patients with penetrating laryngeal trauma may have small fragments of cartilage that are difficult to reduce and fixate. This study examines outcomes in patients managed with and without ORIF. METHODS: A retrospective review was conducted of patients aged 18 years and older with penetrating laryngeal trauma who were treated at a single Level I trauma center affiliated with a tertiary-level teaching hospi

Development of Computed Echo Tomography-An Imaging Breakthrough Addressing the Limitations of Conventional Ultrasound: A Baseline Imaging Analysis for Traumatic Injuries.

OBJECTIVES: The diagnosis and triage of trauma in austere environments using ultrasound can be severely limited by bone and other obstructions, particularly when dealing with intracranial, spinal, thoracic, and long bone injuries. A novel form of ultrasound, computed echo tomography (CET), may provide for more complete "whole body" imaging capability, thereby significantly improving patient management. METHODS: To document and assess the imaging capabilities of the recently Food and Drug Administration-cleared CET system (MAUI Imaging K3900), we conducted 3 whole-body imaging sessions using 6

Repeated Exposures to Air Travel-Relevant Hypobaria Induce Anxiety-Like Behavior and Alter Functional Connectivity and White Matter Integrity in a Ferret Model of Traumatic Brain Injury.

Under-vehicle blast (UVB) caused by landmine detonation induces a distinct traumatic brain injury (TBI) and can be accompanied by head impact. Injured soldiers often undergo multiple flights after injury to access medical care and return to duty. Previous work has shown that low air pressure during air travel (hypobaria [HB]) exacerbates neurological injury, but the effects of one or more HB exposures on chronic brain injury are unknown. We hypothesized that multiple HB exposures after TBI would result in worse outcomes than 0-1 exposures. Sedated male ferrets underwent UVB and controlled cort

Non-accidental Trauma as an Example of an Underestimated Problem in Pediatrics: Maltreated Child Syndrome and Shaken Baby Syndrome.

Injuries, including both accidental and non-accidental trauma, represent a common reason for hospitalization in the pediatric population. The impact of violence in the caregiver-child relationship is an important and often underestimated problem in pediatrics. Child abuse can present itself in various forms, including both psychological and physical manifestations. This study details the maltreated child syndrome and the shaken baby syndrome, also known as abusive head trauma. Non-accidental trauma presents with a broad spectrum of neurological symptoms, along with multiple injuries at differe
