Site-Specific Associations Between CBCT-Derived Gray Value and Fractal Dimension and DEXA T-Scores in Postmenopausal Women: A Cross-Sectional Analytical Study.

OBJECTIVE: Osteoporosis is a silent systemic skeletal disease characterized by microarchitectural deterioration and increased fracture risk; thus, early case-finding is essential. Although dual-energy X-ray absorptiometry (DEXA) is the reference standard for bone mineral density (BMD) assessment, limited accessibility and cost restrict its use for large-scale screening. Given that cone-beam computed tomography (CBCT) is routinely obtained in dental practice, image-derived parameters-gray value (GV) and fractal dimension (FD)-may provide insight into their association with systemic bone mineral

Growing skull fracture: Delayed presentation, surgical management, and review of the literature.

BACKGROUND: Growing skull fracture (GSF), also known as post-traumatic leptomeningeal cyst, is a rare late complication of a linear skull fracture in young children. It requires an underlying dural tear with progressive herniation of arachnoid membrane and/or brain tissue. CASE DESCRIPTION: We report a 15-month-old child who sustained a right frontoparietal linear skull fracture at 1 month of age and returned 14 months later with a pulsatile scalp swelling and left hemiparesis. Computed tomography showed a wide right frontoparietal diastasis and adjacent ventricular dilatation. At surgery, th

Definitive reconstruction of frontal asymmetry in an adolescent with anterior plagiocephaly due to unilateral coronal craniosynostosis using a hybrid osteo-titanium technology based on an individualized approach: A case report.

BACKGROUND: Unilateral coronal craniosynostosis causes three-dimensional craniofacial asymmetry. Although correction is usually performed during infancy, some patients present in adolescence with an untreated or persistent deformity. We describe an individualized hybrid reconstruction combining a patient-specific titanium implant with autologous bone. CASE DESCRIPTION: A 15-year-old girl with untreated left unilateral coronal craniosynostosis presented with marked frontal and fronto-orbital asymmetry. Three-dimensional computed tomography demonstrated left frontal retrusion, orbital asymmetry

Conservative management of a cervically fractured zygomatic implant using a novel titanium device: a technical report with biomechanical validation and 24-month follow-up.

Fracture of a zygomatic implant is an uncommon but clinically challenging complication. Conventional management generally involves implant removal or alternative anchorage strategies, both of which may require extensive surgery, increase patient morbidity, and compromise an existing implant-supported rehabilitation. This technical report describes the development, biomechanical validation, and first clinical application of a novel prosthetic rescue device for the conservative management of cervical fractures of zygomatic implants while preserving the osseointegrated apical segment. A customize

Exploring risk groups for concomitant injuries in facial fracture patients: a data-driven analysis.

PURPOSE: Patients with facial fractures are a diverse group with varying injury mechanisms and extent of injury. These patients often suffer from concomitant injuries (CIs) of different severity outside the facial region, some of which are life-threatening and may require urgent care. Therefore, early identification of patients at risk for CI is important. This study aimed to investigate exploratory risk groups for concomitant injuries in patients with facial fractures, focusing on the impact of injury energy, sex and age on the prevalence and type of concomitant injury. METHODS: In this retr

Predictive models for the occurrence of expansive intracranial hematomas and outcomes after surgical evacuation in patients with traumatic brain injury in Uganda: a prospective cohort study.

BACKGROUND: Hematoma expansion is a frequent and clinically important complication of traumatic intracranial hemorrhage and is associated with poor functional outcomes. Early identification of patients at risk of expansive intracranial hematomas (EIH) remains challenging, particularly in low-resource settings. OBJECTIVE: To determine the prevalence of EIH and develop exploratory predictive models for EIH occurrence and neurosurgical outcomes among patients with traumatic brain injury (TBI) in Uganda. METHODS: We conducted a prospective cohort study among adult patients with TBI and radiologi

Carotid Cavernous Fistula: A Case Report of a Rare Diagnosis and Presentation.

Periorbital swelling can differ from simple allergic conjunctivitis, orbital cellulitis, and cavernous sinus thrombosis, as well as rapidly expanding retrobulbar hemorrhage. An uncommon cause is the carotid-cavernous fistula (CCF), an abnormal arteriovenous connection between the carotid artery and the cavernous sinus. CCFs can cause periorbital swelling, diplopia, cranial nerve palsies, and ultimately vision loss secondary to retinal detachment, vitreous detachment, or intracranial hemorrhage. CCFs are strongly associated with previous head trauma, especially basilar skull fractures, and with

Relating Real-World Falls to Laboratory Test Conditions.

PURPOSE: Falls account for over half of fatal work-related traumatic brain injuries, yet current safety helmet standards do not test energy levels representative of falls. Additionally, little is known about how fall impact mitigating mechanisms (FIMMs), such as fall arrest movements and/or object contact, and reduce head impact velocity. This study used a multi-step analytical approach to estimate head impact speeds from various fall heights while factoring in energy reduction due to FIMMs. METHODS: Laboratory drop tests established peak linear accelerations (PLAs) and head injury criterion

Immediate post-traumatic pseudoaneurysm of the external carotid artery following condylar fracture: case report.

INTRODUCTION: Pseudoaneurysms following mandibular condylar fractures are extremely rare and usually present weeks after trauma. Acute life-threatening presentations are exceptional. MATERIALS AND METHODS: A 19-year-old man was admitted in hemorrhagic shock after high-energy facial trauma. Imaging revealed bilateral comminuted condylar fractures associated with an actively bleeding pseudoaneurysm of the distal external carotid artery. Emergency endovascular coil embolization achieved rapid hemorrhage control, followed by surgical fixation of the mandibular symphysis. Postoperatively, the pati

3D superimposition of cone-beam computed tomography and intraoral scan models for forensic identification: Development and validation of a quantitative analysis method.

In the absence of dental treatment, dental morphology analysis is a promising method for human identification. This study aimed to develop and validate a method that superimposes 3D models derived from CBCT (3DCT) and intraoral scans (3DIOS), and to establish reference values for this approach. The evaluation sample comprised sixty corresponding 3DCT-3DIOS model pairs and sixty non-corresponding pairs. The validation sample comprised thirty non-corresponding 3DCT-3DIOS pairs. For each superimposition, the root mean square (RMS), absolute average (Abs Avg), half the interdecile range ((90-10)/2

Injuries from falling out of bed in infants under 1 year of age: a systematic review.

CONTEXT: Falls from beds are a leading cause of unintentional injuries in infants under 1 year of age. While most cases result in mild outcomes, the potential for severe injuries, such as skull fractures or intracranial haemorrhages, exists. Despite the prevalence of these incidents, no systematic review has focused specifically on injuries from bed falls in this specific age. OBJECTIVE: To evaluate the risk and severity of injuries caused by falls from beds in infants under 1 year of age through a systematic review. DATA SOURCES: We searched in PubMed, Embase, Cochrane Library and Lilacs Sc

Directed pterygomaxillary disjunction versus direct downfracture in Le Fort I osteotomy: application of a fracture quality framework using bilateral ten-point cone-beam computed tomography in a retrospective cohort of 205 patients.

BACKGROUND: Control of the pterygomaxillary junction (PMJ) fracture is critical in Le Fort I osteotomy. We tested whether a directed PMJ disjunction (osteotome-assisted intermediate release) improves fracture quality, symmetry, and alignment versus direct downfracture. MATERIAL AND METHODS: Single-center retrospective comparative cohort (January 2019-April 2025). Two hundred five consecutive patients underwent Le Fort I: directed disjunction (n=127) or direct downfracture (n=78). Postoperative cone-beam computed tomography at day 90-110 was scored with a bilateral ten-point map. Primary outco

Development of a Modified Weight-Drop Apparatus for Closed-Skull, Repetitive Mild Traumatic Brain Injuries in a Mouse Model.

Repetitive mild traumatic brain injury (rmTBI) is a major contributor to long-term neurological dysfunction, yet many preclinical models lack precise control and quantification of biomechanical forces across impacts. We developed a reproducible, closed-skull mouse model of rmTBI using a custom-built weight-drop apparatus featuring a solenoid-based rebound arrest system, integrated high-speed videography, and accelerometry to track head kinematics during impact. Adult male and female mice received either a single impact or nine daily impacts. Linear and angular acceleration data were analyzed a

Appropriateness of imaging requests for pediatric traumatic brain injury in the emergency department: a retrospective cross-sectional study.

BACKGROUND: Pediatric traumatic brain injury (TBI) accounts for a significant proportion of emergency department (ED) visits, with an estimated global incidence of up to 200 cases per 100,000 children each year. Despite clinical guidelines, imaging is often overused, particularly in mild cases, exposing children to unnecessary radiation. In Latin America, evidence regarding the appropriateness of imaging requests (AIR) and their diagnostic value remains scarce. OBJECTIVE: To assess the association between the AIR and final radiologic findings in traumatic brain injury, and to explore predicto
