Directed pterygomaxillary disjunction versus direct downfracture in Le Fort I osteotomy: application of a fracture quality framework using bilateral ten-point cone-beam computed tomography in a retrospective cohort of 205 patients.

BACKGROUND: Control of the pterygomaxillary junction (PMJ) fracture is critical in Le Fort I osteotomy. We tested whether a directed PMJ disjunction (osteotome-assisted intermediate release) improves fracture quality, symmetry, and alignment versus direct downfracture. MATERIAL AND METHODS: Single-center retrospective comparative cohort (January 2019-April 2025). Two hundred five consecutive patients underwent Le Fort I: directed disjunction (n=127) or direct downfracture (n=78). Postoperative cone-beam computed tomography at day 90-110 was scored with a bilateral ten-point map. Primary outco

Development of a Modified Weight-Drop Apparatus for Closed-Skull, Repetitive Mild Traumatic Brain Injuries in a Mouse Model.

Repetitive mild traumatic brain injury (rmTBI) is a major contributor to long-term neurological dysfunction, yet many preclinical models lack precise control and quantification of biomechanical forces across impacts. We developed a reproducible, closed-skull mouse model of rmTBI using a custom-built weight-drop apparatus featuring a solenoid-based rebound arrest system, integrated high-speed videography, and accelerometry to track head kinematics during impact. Adult male and female mice received either a single impact or nine daily impacts. Linear and angular acceleration data were analyzed a

Temporal bone fractures after falls.

OBJECTIVE: This study describes the management and outcomes of temporal bone fractures resulting from falls. METHODS: We retrospectively reviewed patients with traumatic temporal bone fractures from 2018 through 2022. RESULTS: We analysed 171 patients with temporal bone fractures, 62 (36.3 per cent) of which occurred secondary to falls. Fall patients were significantly older than non-fall patients (mean age 46 vs. 38 years; p = 0.0079) and had higher Modified Frailty Index-5 scores (0.63 vs. 0.20; p = 0.0003). Fall patients had shorter hospital stays (10.1 vs. 15.8 days; p = 0.015), were mor

Can construction helmets save lives? Evidence from a biomechanical reconstruction of a work-related head trauma.

Each year, 320 000 people die from occupational injuries. The construction sector is one of the most hazardous sectors, showing a high incidence of workplace fatalities, of which many are caused by traumatic head injuries. In this study, the efficiency of construction helmets has been investigated through an in-depth accident reconstruction of a real-world workplace head trauma, aiming to investigate causation, prevention and liabilities in an ongoing police investigation. The accident was reconstructed with a state-of-the-art subject-specific head model, used to predict the skull fracture and

Appropriateness of imaging requests for pediatric traumatic brain injury in the emergency department: a retrospective cross-sectional study.

BACKGROUND: Pediatric traumatic brain injury (TBI) accounts for a significant proportion of emergency department (ED) visits, with an estimated global incidence of up to 200 cases per 100,000 children each year. Despite clinical guidelines, imaging is often overused, particularly in mild cases, exposing children to unnecessary radiation. In Latin America, evidence regarding the appropriateness of imaging requests (AIR) and their diagnostic value remains scarce. OBJECTIVE: To assess the association between the AIR and final radiologic findings in traumatic brain injury, and to explore predicto

Timing of Craniotomy and Associated Outcomes in Severe Blunt Traumatic Brain Injury.

INTRODUCTION/RATIONALE: Traumatic brain injuries (TBIs) are a leading cause of death among adults, resulting in approximately 214,110 TBI-related hospitalizations and 69,473 TBI-related deaths in the United States. There remains some debate regarding the optimal timing of craniotomy particularly as it relates to the use of intracranial pressure (ICP) monitoring. OBJECTIVE: Our study aims to determine the association of time to craniotomy intervention (≤6 or >6 hr), ICP monitoring, and trauma center level on patient outcomes in blunt, severe, non-subarachnoid TBI with skull fracture. METHODS:

Submental intubation as an alternative to tracheostomy in complex panfacial fracture: a case report.

Submental intubation, first described in 1986, is an effective alternative to tracheostomy in patients with complex facial fractures in whom nasotracheal intubation is contraindicated and orotracheal intubation would interfere with the surgical field. We report the case of a 38-year-old man with a history of morbid obesity who suffered a panfacial fracture with involvement of the frontal sinuses and intracranial communication after falling from a second floor. Nasotracheal intubation was contraindicated, and orotracheal intubation would have interfered with the planned intermaxillary fixation.

The impact of previous head trauma and history of cranial surgery on clinical findings in acute pediatric head injury.

BACKGROUND: This study aimed to identify independent predictors of intracranial pathology in children presenting with acute blunt head trauma, with a particular focus on patients who had a history of previous head trauma or cranial surgery and did not meet the PECARN (Pediatric Emergency Care Applied Research Network) criteria for cranial computed tomography (CCT). The study further sought to evaluate the impact of these historical factors on acute clinical findings and to inform more objective decision-making for CCT use in this specific subgroup. METHODS: In this prospective cohort study, i

Efficacy of Polyvinylpyrrolidone Soaked Synthetic Polyurethane Foam on Postoperative Pain of Nasal Bone Fracture.

BACKGROUND: Nasal bone fractures are the most common type of facial bone fracture. Closed reduction is the standard treatment and is typically followed by intranasal packing. Conventional materials such as Vaseline or Furacin roll gauze and Merocel are widely used, but biodegradable synthetic polyurethane foam (SPF, Nasopore) has recently gained popularity as it does not require removal. This study focused on postoperative pain primarily caused by mucosal injury, and investigated whether polyvinylpyrrolidone (PVP), a mucosal protective agent, could reduce pain when applied to SPF packing. CT Brain Utilization After Low-Risk Head Injury in Adults: A Retrospective Cohort Study and Literature Review.

Introduction Falls in adults aged ≥65 years contribute substantially to emergency department presentations and mild traumatic brain injury, with variable risk of intracranial hemorrhage. Existing CT decision tools often exclude older adults, resulting in potentially unnecessary imaging. This study aimed to identify predictors of intracranial pathology in all adults and compare older versus younger patients to inform a future prospective study. Materials and methods In preparation for a prospective study, we performed a retrospective imaging-selected cohort study analyzing all CT brain studies
