Wallerian degeneration (WD) is a conserved axonal self-destruction program implicated in a spectrum of neurological conditions, including peripheral neuropathies, traumatic brain injury, and possibly neurodegenerative diseases. Wallerian degeneration is triggered by depletion of the NAD+-synthesizing enzyme NMNAT2, subsequent NMN accumulation, and activation of the NAD+-hydrolyzing SARM1, ultimately driving axonal fragmentation. While pharmacological inhibition of SARM1 is an emerging therapeutic strategy, current challenges due to paradoxical toxicity with orthosteric base-exchange SARM1 inhi

Immune checkpoint inhibitors (ICIs) have revolutionised the management of advanced malignancies, but can cause immune-related adverse events (irAEs), including vision-threatening complications such as optic neuropathy. Ocular irAEs have been described with several programmed death-1/programmed death-ligand 1 and cytotoxic T lymphocyte-associated antigen-4 inhibitors, but ocular toxicities of tislelizumab have so far been limited to Vogt-Koyanagi-Harada (VKH)-like uveitis, panuveitis and retinal vasculitis. We describe a woman with metastatic lung adenocarcinoma who was treated with second-line

Optic nerve injury often causes irreversible vision loss due to limited self-regeneration capacity, persistent neuroinflammation, and failed remyelination. Current therapies partially promote axonal regeneration but rarely achieve meaningful visual recovery, limited mainly by unstable myelin reconstruction and inadequate injury microenvironment modulation. Here, we developed a photo-crosslinkable bilayer hydrogel system (GO/GI@MPDA-Cle). Its outer layer (GM-Odex) provides strong wet tissue adhesion for optic nerve sheath sealing, while the inner layer (GM-imid) enables sustained local release

After optic nerve injury, several retinal ganglion cell (RGC) intrinsic signaling pathways have been shown to enhance RGC survival and RGC axonal growth. In contrast, few extrinsic cues have been identified that guide regenerating RGC axons toward and across the optic chiasm. Here, we use live-cell imaging in larval zebrafish to show that regrowing RGC axons initiate growth toward the midline and extend along a trajectory similar to their original projection. From a candidate genetic screen, we identify the glycosyltransferase Lh3 (also referred to as Plod3) to be required to direct regrowing

BACKGROUND: Nicotinamide riboside (NR) is an NAD+ precursor and is reported to upregulate NAD+ levels in several tissues. We previously demonstrated that NR exhibited protection against axon loss induced by tumor necrosis factor (TNF) with upregulation of SIRT1 and enhancement of autophagy. We recently found that NR upregulated phosphorylated adenosine monophosphate-activated protein kinase (p-AMPK) protein in the optic nerve. The present study aimed to examine whether a SIRT1 inhibitor prevents axonal protection by NR and alters p-AMPK expression. METHODS AND RESULTS: Rats were given intravit

Stress signaling is important for determining the fates of neurons following axonal insults. Previously, we showed that the stress-responsive kinase PERK contributes to injury-induced neurodegeneration (Larhammar et al., 2017). Here, we show that PERK acts primarily through activating transcription factor-4 (ATF4) to stimulate not only pro-apoptotic but also pro-regenerative responses following optic nerve damage. Using conditional knockout mice, we find an extensive PERK/ATF4-dependent transcriptional response that includes canonical ATF4 target genes and modest contributions by C/EBP Homolog

Axon regeneration and visual function recovery after optic nerve injury remain a major challenge in ophthalmology. Current research on optic nerve regeneration has evolved from merely promoting axon regeneration to a new stage focused on long-distance regrowth, targeted reinnervation, and function restoration. Centering on these core goals, this article discusses strategies for achieving accurate circuit repair by reactivating developmental guidance mechanisms, modulating neural activity, and utilizing neurotrophic factors. It also explores the opportunities brought by high-throughput multi-om

BACKGROUND: Traumatic optic neuropathy (TON) is a devastating cause of irreversible vision loss for which no effective treatment currently exists. Its poor prognosis stems from two major challenges: the limited regenerative capacity of retinal ganglion cells (RGCs) and the hostile, inflammation-driven environment that follows injury.
RESULTS: In this work, using transcriptomic bioinformatic and histopathological analysis, we discovered that mechanical trauma and subsequent neuroinflammation trigger microglial pyroptosis through the NLRP3/CASP1/GSDMD pathway. This process amplifies inflammator

Neurological emergencies such as stroke and traumatic brain injury are major contributors to morbidity and mortality in critically ill patients. These conditions frequently result in alterations in cerebral haemodynamics, including raised intracranial pressure, which require timely recognition and management to optimise outcomes. Neuro point-of-care ultrasound (NeuroPOCUS), incorporating transcranial Doppler (TCD), transcranial colour-coded duplex (TCCD) ultrasound, and optic nerve sheath diameter (ONSD) measurement, offers a non-invasive, bedside means of assessing cerebral physiology and is

BACKGROUND: Growing evidence implicates early metabolic dysfunctions in retinal ganglion cells (RGCs) as a contributor to both high- and normal-tension glaucoma, yet no approved therapy directly protects RGCs to preserve vision. We aimed at identifying a safe, druggable neuroprotective strategy that restores RGC metabolic homeostasis for glaucoma therapy.
METHODS: Using a live-cell mitochondrial screen in human embryonic stem cell-derived retinal ganglion cells (H7; female donor), we identified the clinically tested 5-HT1A antagonist WAY-100635 (WAY) as a neuroprotective agent. Mechanisms are

Retinal ischemia–reperfusion (I/R) injury is a key pathological process in retinal-associated diseases such as glaucoma. However, the spatiotemporal dynamics and cell interactions during this injury are not fully understood. This study integrated single-cell RNA sequencing and spatial transcriptomics to map retinal cell responses in a rat I/R injury model at acute, subacute, and chronic stages. We identified 14 cell populations and tracked gene expression changes over time. Retinal ganglion cells (RGCs) showed activation of apoptotic, autophagic, and oxidative stress pathways within 24 h. Optic neuropathy represents a leading cause of irreversible vision loss, in which oxidative stress, chronic inflammation, dysregulated lipid metabolism, and mitochondrial dysfunction contribute to the progressive degeneration of retinal ganglion cells (RGCs). In recent years, a number of nutraceuticals have been investigated as potential neuroprotective agents; however, the molecular mechanisms through which they exert their effects remain incompletely understood and are often considered in isolation. In the present in silico study, an integrative network-based approach was applied for a syste

BACKGROUND: Neuroinflammation induces astrogliosis, a process marked by astrocyte hypertrophy, increased migration, and altered protein expression. While these changes involve both up- and downregulation of specific genes, the mechanisms governing protein turnover-particularly degradation-remain poorly understood. Here, we investigated how astrocyte endo-lysosomal function is altered during inflammation and explored the role of key regulators such as RAB7 in this response.
METHODS: We analyzed gene expression datasets from an optic nerve injury model to identify pathways associated with react

Secondary degeneration following optic nerve crush (ONC) is driven in part by mitochondrial dysfunction and microglial activation. Inspired by hibernation, where reduced succinate oxidation limits reactive oxygen species (ROS) production, we tested whether pharmacological inhibition of this pathway confers neuroprotection. Using in vivo ONC models and in vitro microglial assays, we evaluated the effects of dimethyl malonate (DMM), an inhibitor of succinate dehydrogenase, and a cell-permeable succinate analog (succinate-NV). Succinate-NV increased pro-inflammatory cytokine expression (IL-1β) an

Glaucoma is a group of eye diseases that affects 4 million people in the US and is one of the leading causes of vision loss due to damage to the eye's optic nerve (ON) which is composed of axons from retinal ganglion cells (RGCs) that transmit visual information to the brain. Injury to the ON often triggers RGC death and subsequent loss of visual function. Despite its increasing prevalence worldwide, effective therapies for glaucoma remain elusive. Notably, the thirteen-lined ground squirrel (TLGS) exhibits intrinsic neuroprotection during hibernation; however, reproducing this protective stat

Traumatic brain injury is a widespread health concern that annually affects millions of people worldwide, with blast-mediated TBI (bTBI) being of particular relevance to military personnel and civilians in war zones. Current treatments primarily target symptoms, rather than the underlying neuropathology, and are ultimately limited by the need for an improved etiological understanding of TBI. Here, we utilized a pre-clinical mouse model of bTBI, previously shown to evoke a high susceptibility to retinal ganglion cell (RGC) death, to investigate whether neuroinflammatory pathways associated with

BACKGROUND: Optic pathway cavernous malformations (CMs) are rare lesions that may cause progressive or fluctuating visual deterioration through recurrent hemorrhage and chronic compression. Optimal management and surgical timing remain controversial.
OBSERVATIONS: A 30-year-old male presented with progressive right-sided visual decline after recurrent episodes of visual loss since 2018. Initial near-complete visual loss was followed by spontaneous recovery, supporting conservative follow-up. Sustained deterioration later prompted reevaluation. Neuro-ophthalmological examination demonstrated o
