Iatrogenic Nerve Injuries in Head and Neck Surgeries: A Systematic Review of Mechanisms, Outcomes, and Prevention Strategies.

Iatrogenic nerve injury in head and neck surgery remains a substantial yet potentially preventable source of morbidity across endocrine, otolaryngologic, and related procedures. This systematic review synthesized evidence to identify the nerves most frequently affected, delineate operative mechanisms of injury, describe the clinical course, and evaluate preventive strategies. Traction on visually intact nerves emerged as the predominant mechanism, with additional contributions from thermal injury, compression, ischemia, and entrapment by suture or clip. The recurrent laryngeal, facial, trigemi

Mitochondria transplantation preserves retinal ganglion cells and promotes CNS axonal regeneration.

Mitochondrial dysfunction is a central driver of retinal ganglion cell (RGC) loss in glaucoma and other forms of optic neuropathies, leading to irreversible blindness. Here, we demonstrate that replenishing the mitochondrial pool through exogenous mitochondrial transplantation ("mitotherapy") in adult mice not only preserves neuronal survival but also promotes regenerative competence in the central nervous system (CNS). In aging or injured RGCs, we identified profound deficits in mitochondrial biogenesis, fission-fusion balance, and mitophagy. Transplantation of functional mitochondria in in v

GPX4 promotes optic nerve regeneration and retinal ganglion cell neuroprotection.

Glaucoma is an optic neuropathy characterized as progressive degeneration of retinal ganglion cells (RGCs) and their axons in the optic nerve (ON). Recently, lipid peroxidation and ferroptosis have been linked to experimental glaucoma, suggesting a new therapeutic strategy. Here, we report that glutathione peroxidase 4 (GPX4), a key regulator of lipid peroxidation, is upregulated in surviving and regenerating RGCs in two mouse optic neuropathy models, traumatic ON crush and glaucoma. To explore the potential neuroprotective role of GPX4, we test adeno-associated virus-mediated RGC-specific ove

Outcomes of outpatient thyroidectomy for high-risk patients: A single-site retrospective cohort study.

BACKGROUND: While outpatient thyroidectomy is well-studied in healthy patients, data are limited in patients with previously defined relative contraindications including obstructive sleep apnea, anticoagulant or antiplatelet use, substernal goiter, and Graves' disease. This study evaluates safety of outpatient thyroidectomy in high-risk patients compared to low-risk patients at a high-volume endocrine surgery center. STUDY DESIGN: Retrospective cohort. RESULTS: From 2021 to 2025, 513 patients underwent outpatient total thyroidectomy (271 low-risk; 242 high-risk). Complication rates were low

Neurapraxia in Time and Space.

Since its introduction into medical terminology by Lord Henry Cohen in 1941, the term "neurapraxia" has become established in the neurological and neurosurgical lexicon, although it is often misused or misspelled. It denotes a transient dysfunction of a peripheral nerve, as distinct from axonotmesis and neurotmesis, which describe more significant structural injuries to a peripheral nerve. To expand our understanding of neurapraxia, it is necessary to examine the phenomenon in both temporal and spatial contexts. This study examines neurapraxia in: (A) Time-(1) the origins of the term neuraprax

Otopathology of Facial Nerve and Chorda Tympani Traumatic Neuromas.

HYPOTHESIS: Traumatic neuromas of the facial nerve are rare lesions that can mimic other tumors of the temporal bone and might be consequential to injuries of common otologic surgeries. Literature is scarce in describing these lesions in the temporal bone. The aim of this study is to expand the histologic understanding of these lesions. BACKGROUND: Traumatic neuromas are benign proliferations of neural structures after total or partial sectioning, representing what seems to be a reparative attempt. In otologic surgeries, the facial nerve and its branch chorda tympani are at risk of iatrogenic

Management of facial nerve during parotid recurrent pleomorphic adenoma revision surgery.

Revision surgery remains the standard treatment for parotid recurrent pleomorphic adenoma (RPA). Despite recent advancements in surgical techniques, the management of the facial nerve (FN) during parotid RPA surgery still poses a challenge. This retrospective study collected demographic data, clinical characteristics, and details of the surgical procedure, with a particular focus on FN management of parotid RPA patients between 2020 and 2024. Sixty-six patients who underwent parotid RPA surgeries were included; the FN was successfully preserved in 39 (59%). A significant difference in recurren

The mitochondria-targeted peptide HDAP2 reduces mitochondrial loss and retinal ganglion cell degeneration after optic nerve injury.

Mitochondrial dysfunction is a critical early driver of retinal ganglion cell (RGC) loss in optic nerve injury. We evaluated whether HDAP2, a mitochondria-targeted aromatic peptide designed to support mitochondrial membrane integrity, could preserve neuronal structure after optic nerve crush (ONC) in C57BL/6 mice (both sexes, n = 31). Systemically administered HDAP2 penetrated the blood-retinal barrier and localized to RGCs and mitochondrial-rich retinal layers. Daily treatment significantly improved RGC survival compared to saline-treated ONC animals. RGC densities increased across central, m

Root of Helix Inter Tragus Notch Incision (RHITNI) for open temporomandibular joint surgery: A five-year retrospective cohort study on safety and complications.

Open temporomandibular joint (TMJ) surgery must balance exposure, facial nerve preservation, and cosmesis. The Root of Helix Inter Tragus Notch Incision (RHITNI), an endaural modification with promising aesthetics and low morbidity, lacks systematic outcome data. A single-center retrospective cohort study of consecutive primary open TMJ procedures using RHITNI over five years was performed. Primary endpoint: branch-specific facial nerve dysfunction (temporary or permanent). Secondary endpoints: auriculotemporal sensory change, salivary complications, hematoma, infection, wound problems, and sc

Effects of human embryonic stem cell-derived Schwann cells for nerve regeneration after facial nerve injury.

Facial nerve transection frequently results in incomplete functional recovery despite microsurgical repair due to axonal misalignment and limited intrinsic regenerative capacity. Schwann cells (SCs) are essential for peripheral nerve repair, supporting axon elongation, remyelination, and neurotrophic signaling. Human embryonic stem cell-derived Schwann cells (hESC-SCs), including Schwann cell precursors (SCPs) and immature SCs, constitute a scalable source of cells for tissue-engineered nerve grafts. This study investigated whether silicone conduits loaded with hESC-SCs could enhance facial ne

Anatomical validation of safe zones for facial nerve preservation in supraorbital keyhole approach: a cadaveric study.

PURPOSE: The supraorbital keyhole approach is a minimally invasive technique for treating lesions in the anterior and middle cranial fossa. A major complication of this procedure is injury to the temporal branch of the facial nerve. This study aimed to define safe surgical zones for the lateral borders of the incision to minimize facial nerve injuries. METHODS: This study examined 15 formalin-fixed adult cadaveric heads (30 sides). The temporal branches of the facial nerve were dissected using a surgical microscope. Three distances were measured using anatomical landmarks: (M1) horizontal dis

Facial recess size estimation using sagittal slices on cone-beam CT for cochlear implantation.

OBJECTIVE: To evaluate sagittal-plane cone-beam computed tomography (CBCT) as a reliable method for estimating facial recess (FR) width, enabling preoperative assessment of posterior tympanotomy (PT) size in cochlear implantation without complex image reconstruction. BACKGROUND: Posterior tympanotomy provides the surgical corridor for cochlear implantation but carries risk of facial/chorda tympani nerve injury. Accurate preoperative imaging is essential for anticipating surgical difficulty. Although multiplanar and 3D reconstructions can estimate PT dimensions, these require additional techno

Continuous Intraoperative Recurrent Laryngeal Nerve Monitoring in Transcervical Inflatable Mediastinoscopic Esophagectomy.

Transcervical inflatable mediastinoscopic esophagectomy (TIME), with fewer incisions and without going through the thoracic cavity, may achieve the same results and lymph node dissection compared with Thoracoscopic assisted minimally invasive esophagectomy (TAMIE). However, its small visual field increases recurrent laryngeal nerve (RLN) injury risk. Intraoperative nerve monitoring (IONM) reduces RLN paralysis in TAMIE, but few reports have used it in TIME. This paper introduces continuous IONM in TIME to protect RLN. A patient underwent TIME with continuous left RLN monitoring using an EMG en

Extracellular vesicles conjugated with c(RGDyk) peptide targeting integrin αVβ3 repair optic nerve injury through YAP/TAZ and Smad2/3 signaling.

Extracellular vesicles (EVs) derived from mesenchymal stem cells have therapeutic potential for optic nerve injury. However, further investigations are needed to increase their efficacy. In this study, we tried to enhance targeting and recovery function of EVs using conjugation with integrin αVβ3 antagonist-c(RGDyk) peptide. The molecular mechanism of neuronal repair was investigated as a potential treatment for optic nerve injury. EVs were shown to restore effectively the abnormal regulations of neuronal markers in optic nerve injury models. Notably, the functionally optimized c(RGDyK)_EVs ex

Pathologically Confirmed Symptomatic Carotid Webs: Histopathological Features and Long-Term Surgical Outcomes.

BACKGROUND AND OBJECTIVES: Carotid webs are an underrecognized cause of ischemic stroke in young adults and are associated with a high risk of recurrence when managed with medical therapy alone. Although carotid endarterectomy (CEA) is increasingly performed, histopathological confirmation remains infrequently reported, and the underlying pathological substrate of symptomatic carotid webs is not well defined. In this study, we present the largest pathologically confirmed series of symptomatic carotid webs treated with CEA, providing a comprehensive clinicopathological characterization and eval

Engineered small extracellular vesicles provide low-dose salidroside delivery to attenuate retinal ganglion cell degeneration.

Optic neuropathy is characterized by impaired optic nerve function resulting from various pathological processes, often leading to retinal ganglion cell (RGC) degeneration and irreversible vision loss. Several studies have demonstrated the neuroprotective effects of salidroside (Sal). However, its clinical application has been limited by the high dosage required and the short half-life of Sal. To enhance drug efficacy and prolong therapeutic effects, we developed engineered small extracellular vesicles (sEVs) loaded with Sal (sEVs-Sal) for intravitreal administration in a mouse model of optic
