Management of postoperative hypotony with fibrin glue following scleral tunnel-based explantation of pIOL and simultaneous ICL implantation: A case report.

RATIONALE: Long-term implantation of anterior phakic intraocular lenses (pIOLs) can lead to progressive corneal endothelial cell loss, ultimately necessitating explantation. Although scleral tunnel approaches reduce corneal trauma during lens exchange, postoperative hypotony due to wound microleakage is an uncommon but vision-threatening complication. This report aims to present the 1st documented clinical case in which fibrin glue was successfully used to seal microleakage-induced hypotony following pIOL explantation and simultaneous toric implantable collamer lens (ICL) implantation. PATIEN

Vascular regeneration and blood flow recovery in glaucoma.

The retina and optic nerve rely on a tightly regulated neurovascular unit that sustains the highly dynamic and metabolically demanding neural tissues required for vision. Adequate oxygen and nutrient delivery are essential for maintaining tissue function and cellular survival. Over the past decades, extensive research within and beyond the field of ophthalmology has sought to elucidate the mechanisms that govern neurovascular regulation in health and disease. Growing evidence indicates that neurovascular dysfunction plays an important role in both the initiation and progression of glaucoma, a

Neuroinflammation-Associated Optic Structural-Functional Degeneration in Early Diabetic Optic Neuropathy.

BACKGROUND: Diabetic optic neuropathy (DON) is characterized by optic nerve degenerative changes, which may progress to optic atrophy and even blindness. Although DON is relatively common, its early diagnosis remains challenging. This study employed a noninvasive in vivo imaging combined histopathological assessment system to investigate changes in optic structural-functional degeneration in DON models. METHODS: Animals were divided into experimental group (8 db/db mice) and control group (8 db/m mice). Blood glucose, body weight, intraocular pressure, peripapillary retinal nerve fiber layer

The Impact of Vitreoretinal Surgery in Patients with Uveitis: Current Strategies and Emerging Perspectives.

Uveitis constitutes a heterogeneous group of intraocular inflammatory pathologies, including both infectious and non-infectious aetiologies, often leading to substantial morbidity and permanent loss of vision in up to 20% of the affected cases. Visual impairment is most prominent in intermediate, posterior, or panuveitis and is commonly associated with cystoid macular oedema, epiretinal membranes, macular holes, and retinal detachment. In the context of uveitis, these complications arise as a result of recurrent flare-ups or chronic inflammation, contributing to cumulative ocular damage. Pars

[Regulation of SARM1 on SNPH expression and its participation in glaucomatous optic neuropathy].

Objective: To investigate the molecular mechanism of toll-like receptor adaptor protein SARM1 in glaucomatous optic neuropathy. Methods: The experimental study was conducted from February 2024 to October 2025. A chronic ocular hypertension glaucoma model was established by injecting micro-magnetic beads into the anterior chamber of 8- to 10-week-old male Wistar rats. At 3 days, 1 week, and 2 weeks post-modeling, retinal and optic nerve tissues from 6 eyes of 6 rats were collected as the chronic ocular hypertension glaucoma model group, and 6 eyes from 6 wild-type rats that received an equal vo

Proteomic Profile Analysis of Plasma and Aqueous Humor from Glaucoma and Nonglaucomatous Patients.

PURPOSE: Glaucoma, a multifactorial ocular neuropathy, can lead to irreversible vision loss. Diagnosis involves assessing optic cupping (increased cup-to-disc ratios) and structural changes (like retinal nerve fiber layer thinning) through clinical imaging. Elevated intraocular pressure (IOP) is commonly associated with glaucoma but is not universally present. However, understanding disease progression is hindered by limited access to donor ocular tissue and consistent clinical data. We hypothesized that the proteomes of aqueous humor (AH) and plasma may be altered in disease and correlate wit

Melatonin and glaucoma: Current insights and clinical prospectives.

This review aims to synthesize current evidence on the role of melatonin in glaucoma pathophysiology, its relationship with sleep disturbances and circadian dysregulation, and its potential as a therapeutic agent beyond intraocular pressure (IOP) control. Glaucoma is a chronic neurodegenerative optic neuropathy characterized by progressive retinal ganglion cells (RGCs) loss. While IOP remains the primary modifiable risk factor, disease progression often persists despite adequate pressure control, highlighting the need for adjunctive neuroprotective strategies. Sleep disturbances are highly pre

Longitudinal analysis of retinal cell state transitions in RB1-deficient retinal organoids reveals the nascent cone precursors are the earliest cell-origin of human retinoblastoma.

All cancers arise from the malignant transformation of normal cells, yet their cells-of-origin remain challenging to identify due to the inability to directly observe dynamic changes in human tumors. Retinoblastoma (Rb), a malignant intraocular cancer, serves as a well-established model for investigating the molecular and cellular mechanisms underlying tumorigenesis. While the maturing cone precursors (CPs) have been proposed as the cellular origin of human Rb, it is unclear whether other retinal cell types are similarly sensitive to RB1 inactivation. In this study, we developed RB1-deficient

AI to Identify Strain-Sensitive Regions of the Optic Nerve Head Linked to Functional Loss in Glaucoma.

PURPOSE: The purposes of this study were to assess whether optic nerve head (ONH) biomechanics, quantified by tissue strain, improves classification of progressive visual field (VF) loss patterns in glaucoma beyond morphology, and to use saliency maps to identify ONH regions associated with the predictions. METHODS: We recruited 249 patients with glaucoma (mean age 69 ± 5 years, 54% female patients). One eye per subject was imaged under (1) primary gaze and (2) primary gaze with IOP elevated to approximately 35 millimeters of mercury (mm Hg) via ophthalmo-dynamometry. Twelve subjects were exc

Misdiagnosis of uveal melanoma and other intraocular malignancies in enucleated eyes: a 28-year national review.

BACKGROUND: Intraocular tumors are often diagnosed clinically rather than by biopsy, so a benign lesion can be mistaken for malignancy and lead to enucleation. We determined how often uveal melanoma (UM) and other intraocular tumors are clinically misdiagnosed among enucleated eyes at the Swedish national ophthalmic pathology laboratory over 28 years, and the lesions responsible. METHODS: In this retrospective, registry-based case series, all enucleation specimens accessioned at the St. Erik Ophthalmic Pathology Laboratory, Stockholm, between 1995 and 2022 under a clinical diagnosis of malign

A Systematic and Narrative Review of Safety and Complications in Minimally Invasive Glaucoma Surgery (MIGS) Between 2014-2024.

INTRODUCTION: Earlier intervention in glaucoma has been suggested to slow disease progression and preserve visual function and quality of life. Consequently, minimally invasive glaucoma surgery (MIGS) is increasingly used in mild-to-moderate glaucoma. Although numerous techniques and devices are available, their comparative efficacy remains debated: most procedures lower intraocular pressure to the mid-teens in primary open-angle glaucoma, but without clear evidence of superiority. As these surgeries are now offered to younger, otherwise healthy patients, safety has therefore become a central

Gene Therapy for Glaucoma: Overcoming Limitations of Traditional Therapeutics.

Glaucoma is a chronic neurodegenerative disease characterized by the deterioration of the optic nerve and the progressive loss of retinal ganglion cells (RGCs), making it a leading cause of irreversible blindness worldwide. This neurodegenerative process is characterized by the apoptotic death of neurons whose axons constitute the optic nerve, leading to permanent visual field deficits. Although current standard-of-care therapies-including pharmacological, laser, and surgical interventions-mainly target reducing intraocular pressure (IOP), their clinical efficacy remains limited due to side ef

Neuroinflammation in glaucoma: a myriad of cellular pathways and players.

Glaucoma is a complex neurodegenerative disease with multiple subtypes, yet all are characterized by the progressive dysfunction and loss of retinal ganglion cells (RGCs), which ultimately results in vision impairment and blindness. Elevated intraocular pressure (IOP) is a major risk factor for glaucoma; however, it is neither necessary nor sufficient for glaucomatous neurodegeneration, as patients can exhibit high IOP without developing glaucoma and patients can develop glaucoma with normal IOP. Yet FDA-approved treatment options are largely limited to approaches to minimize risk and reduce I

Tau abnormalities and microglial remodeling in glaucoma: potential links to phagocytosis-related injury.

BACKGROUND: Glaucoma is a progressive optic neuropathy characterized by retinal ganglion cell degeneration and optic nerve damage. Increasing evidence indicates that glaucomatous injury is not limited to the retina, but may also involve downstream visual structures and neuroimmune responses. Tau abnormalities and microglial remodeling have each been reported in glaucoma models and human glaucomatous tissues, yet they are usually discussed as separate pathological findings. Whether these processes are connected, and how they might contribute to synaptic, dendritic, and axonal injury, remains in

Systemic Inflammatory and Microcirculatory Determinants of Long-Term Outcomes in Neovascular Glaucoma: A Multicenter Cohort Study.

BACKGROUND: Neovascular glaucoma (NVG) represents one of the most severe forms of secondary glaucoma and is characterized by an aggressive clinical course, a high risk of irreversible vision loss, and substantial variability in treatment outcomes. At the same time, the roles of systemic inflammation, ocular microcirculation, and neovascularization of the iridocorneal angle (according to the Weiss D. classification, 1978) in determining the long-term prognosis remain insufficiently investigated. PURPOSE: To identify clinical, microcirculatory, and systemic inflammatory predictors of long-term
