Self-reported prescription medication use and polypharmacy following physical combat injury in the UK: the ADVANCE cohort study.

INTRODUCTION: The consequences of serious physical traumatic injury may require pharmacological intervention, however long-term use and polypharmacy can increase risk of morbidity/mortality. This study aims to describe prescription medication use in a cohort of UK Armed Forces personnel who sustained physical combat injuries during the Afghanistan conflict (injured group) compared to demographically similar personnel without such injuries (uninjured group) a median of 11 years post injury. METHODS: One thousand and fifty one participants of the ADVANCE cohort (injured group n = 526 uninjured

Concussion Safety Beliefs and Participation Decisions Among US Adults With a History of Concussion.

INTRODUCTION: Concussion safety education must address specific participation decisions as well as general beliefs. METHOD: An anonymous, opt-in online survey of 100 US adults with self-reported concussion history was analyzed descriptively. RESULTS: Ninety-one percent endorsed avoiding same-day return after suspected concussion, and 81% would not allow a child to continue immediately after a "mild" concussion. Among the 91 endorsing no same-day return, 9 would allow immediate continuation, 7 selected sport-dependent participation, and 2 were unsure. Medical clearance was supported by 88% on

Detection of the RNA-Editing Enzyme ADARB1 in Primary Auditory Neurons and Anti-ADARB1 Immunofluorescence as a Means for Automated Counting.

Spiral ganglion neurons (SGNs), the primary auditory afferents in the inner ear, are vulnerable to degeneration from neuropathies, concussion, and aging, among other causes. Accurate quantification of SGNs is critical for assessing degeneration and therapeutic interventions, but manual counting is arduous due to dense cellular crowding and unreliable nuclear markers. Because SGNs receive input from GluA2-containing AMPA receptors, the transcripts for which must be RNA-edited for calcium impermeability, we hypothesized that SGNs express high levels of the RNA-editing enzyme ADARB1. Here, we con

Emergency Department Diagnosis of Concussion in Very Young Children with Head Injury.

Although very young children ages 1-4 years accounted for the largest proportion of head injury encounters across a pediatric emergency department network, they had the lowest proportion of concussion diagnoses compared with older groups. These findings highlight the need for age-appropriate concussion assessment and may suggest underrecognition in young patients.

Cognitive and Mental Health in Recently Retired Collegiate Football Players: Findings from the CARE Consortium.

BACKGROUND: While long-term psychosocial health in football players has been extensively studied, little is known about cognitive and mental health during early sport retirement. This study examined the association between football participation factors and self-reported cognitive and mental health among recently retired collegiate players. METHODS: Participants in the Concussion Assessment, Research, and Education (CARE) Consortium (n = 627; mean age = 25.0 ± 1.7) completed surveys across 26 United States universities (2014-2021) within 5 years of collegiate exit. Measures included Quality o

Symptoms, Sensorimotor Function, and Recovery Time Following Law Enforcement Combatives.

INTRODUCTION: Law enforcement cadets (LECs) complete training that prepares them for the hardest parts of their job. Subsequently, this training can involve possible head impacts (i.e., head acceleration events [HAEs]). There is limited information regarding the effect of these HAEs on acute sensorimotor function or methods that can attenuate the negative consequences of HAEs sustained in training. To fill these gaps, we conducted 2 sequential experiments. Experiment 1 examined the effects of HAEs on symptoms and sensorimotor function in LECs undergoing defensive tactics training. Experiment 2

Behavioral hyperactivity and hippocampal mitochondrial dysfunction following recurrent concussions in young mice.

Although traumatic brain injury (TBI), particularly recurrent concussions during adolescence, is strongly associated with post-traumatic hyperactivity, the role of mitochondrial dysfunction in driving this behavioral alteration remains poorly understood and insufficiently characterized. In this context, we investigated the effects of varying frequencies of recurrent concussions on exploratory behavior, hippocampal mitochondrial bioenergetics, and nitrosative stress in adolescent male Swiss mice (32 days old) subjected to a weight-drop model over 10 days and allocated to Control, 4, 7, and 10-c

Reattach-Informed Integrated Occupational Therapy In Persistent Post-Concussion Syndrome: A Pilot Case Series.

OBJECTIVE: Persistent post-concussion syndrome (PCS) affects a substantial proportion of individuals after mild traumatic brain injury (mTBI). Persistent symptoms may reflect functional dysregulation - including altered autonomic and sensory processing - rather than irreversible structural damage. This pilot case series explored the feasibility and preliminary outcomes of a ReAttach-informed integrated occupational therapy protocol on symptom severity, perceived stress, and functional participation. METHOD: Ten adults with post-concussion symptoms persisting more than six weeks post-injury re

Non-invasive neuromodulation after repetitive head impacts: current evidence and a precision framework for circuit-based rehabilitation.

INTRODUCTION: Repetitive head impacts (RHIs) are associated with heterogeneous cognitive, behavioral, psychiatric, sleep-related, pain, and neurological symptoms. Chronic traumatic encephalopathy (CTE) remains a postmortem neuropathological diagnosis, whereas traumatic encephalopathy syndrome (TES) is a clinical research syndrome assessed during life. No intervention has been shown to modify CTE pathology, but neuromodulation may support the surviving neural circuits. AREAS COVERED: PubMed/MEDLINE, Web of Science, Scopus and Google Scholar were searched from database inception to August 2026

The effects of serial assessment using the trail making test in elite rugby league players.

OBJECTIVE: The aim of the study was to map trajectories of inter- and intra-individual cognitive performance in non-injured elite rugby players to further understand the effects of serial cognitive assessment in athletes at risk of concussion. METHOD: The Trail Making Test (TMT) was administered to male elite rugby league players (N = 38) at three time-points pre-season and on average 6.8 times per month during a playing season. Three players were excluded from the analyses due to concussion or suspected concussion. The data were analyzed using multilevel models including fixed and random eff

Traumatic Brain Injury in First Responders: From Hidden Burden to Occupational Health Action.

Traumatic brain injury (TBI), including concussion, is an important but often underrecognized occupational injury among first responders. Despite frequent exposure to circumstances that increase the risk of TBI, law enforcement officers, firefighters, and emergency medical services personnel often lack consistent pathways for injury recognition, reporting, clinical assessment, rehabilitation, and safe return to duty. This topical issue of the Journal of Head Trauma Rehabilitation brings together emerging evidence on TBI prevalence, underrecognition, mental health, functional recovery, and occu
