Differences in Reaction Time Variability on the Senaptec Sensory Station by Concussion Status in Special Operations Forces Service Members.

INTRODUCTION: Computerized cognitive tests are widely used in post-concussion assessments. The psychometric properties and clinical utility of employing central tendency measures have been questioned. This is true for many outcomes, including reaction time (RT). Cognitive dispersion metrics, such as reaction time variability (RTV) measured by intraindividual variability (IIV), may offer increased sensitivity to concussions. Evaluating IIV across different computerized cognitive platforms is key to determining the utility of central dispersion metrics. This study aimed to extract IIV from the S

Neuropathological and Behavioral Effects of Mild Traumatic Brain Injury at High Altitude.

OBJECTIVE: In lowlanders, ascent to and prolonged stay at high altitude (HA) can trigger maladaptive changes in neurovascularization, immune function, and hippocampal-dependent cognitive impairment. Mild traumatic brain injury (mTBI) induces similar neuropathological and behavioral alterations. The primary objective of this study was to characterize the impact of mTBI at HA on behavioral and neuropathological outcomes in controlled murine models. METHODS: Baseline magnetic resonance imaging (MRI), diffusion tensor imaging (DTI), and [18F]fluorodeoxyglucose positron emission tomography (PET) w

Systematic search and review of racial and ethnic differences in traumatic brain injury prevalence and incidence.

BACKGROUND: Previous research suggests that some racial/ethnic groups are at increased risk for poor health outcomes following a traumatic brain injury (TBI). Less is known about the extent to which TBI prevalence and incidence vary by race/ethnicity. This paper presents results of a systematic search and review of TBI prevalence and incidence among US racial and ethnic groups. METHODS: Studies were included if they were peer-reviewed, included any prevalence or incidence measure of head injury (TBI and concussion) in a US population, stratified by race and/or ethnicity and included data coll

Rumination, but Not Reflection, Predicts the Reporting of Post-concussive Symptoms in a Non-clinical Sample.

OBJECTIVE: Although traditionally associated with mild head trauma, post-concussive symptoms are commonly reported across both healthy and other clinical populations. Existing research indicates that individuals with depression report high levels of post-concussive symptoms, though the underlying causes of this association remain unknown. The current study aimed to explore potential factors underlying this relationship: specifically, how maladaptive and adaptive self-focused cognitive coping styles, namely, rumination and reflection, respectively, differentially contribute to post-concussive s

Utility of Heart Rate Variability and Exertional Task Completion During Recovery of Mild Traumatic Brain Injury in Active Duty Service Members.

INTRODUCTION: Primary care exertional testing is recommended for mild traumatic brain injury (mTBI) management in active duty service members (ADSM) before return-to-duty (RTD), yet there are no validated standardized tasks with objective physiological measures. Impairments to the automatic nervous system (ANS) after mTBI may last beyond symptom resolution and impact readiness for duty. Assessment of ANS functioning (heart rate variability, HRV) could provide an objective measure of mTBI recovery. MATERIALS AND METHODS: Eighteen ADSM with mTBI (2 weeks or less post-injury) and 18 age-matched

Midlife plasma proteomic profiles indicate altered amyloid and tau processing in former elite rugby players.

BACKGROUND: Contact sports, including rugby union, are associated with higher rates of neurodegenerative dementia, due to various underlying pathologies such as Alzheimer's disease (AD) and chronic traumatic encephalopathy (CTE). New ultrasensitive multiplexed immunoassays may clarify disease mechanisms after repetitive head impacts (RHI) and traumatic brain injury, potentially aiding risk-stratification, early diagnosis and dementia treatment. METHODS: Midlife participants in the ABHC cohort underwent plasma biomarker quantification (NULISA - NUcleic acid Linked Immuno-Sandwich Assay; n=124

The Social Determinants of Health in Pediatric Concussion: A Scoping Review.

INTRODUCTION: In North America, concussions are a common injury in the pediatric population. Much of the research to date has focused on biological aspects of concussion. Consideration of psychological, social and ecological factors, specifically the social determinants of health (SDH), relevant to concussion is essential to advancing the field of pediatric concussion. OBJECTIVES: Using the WHO Conceptual Framework for Action on the Social Determinants of Health, we performed a scoping review to (1) identify and describe the research areas studied in the literature on SDH and pediatric concus

Retired contact sports athletes with cognitive concerns: promoting lifelong brain health.

There is widespread concern among former athletes about the link between head injury and dementia. Neurologists are increasingly assessing ex-contact sports athletes with cognitive and behavioural issues following repetitive head impacts and traumatic brain injury. Their assessment and management can be challenging due to the broad differential diagnosis, including psychiatric issues, trauma-related impairment and, in some cases, neurodegeneration. There may be a range of pathologies present after trauma exposure, including Alzheimer's disease and chronic traumatic encephalopathy. Currently, w

A Shape and Size-Scaled Deep Learning Brain Injury Model for Near Real-Time Dynamic Impact Simulation.

Efficient deep learning brain injury models enable large-scale, strain-based investigations of traumatic brain injury (TBI). Here, we extend a previous model based on a generic 50th percentile adult male brain to a more individualized surrogate (applicable to male, female, and youth) for rapid estimation of brain deformation over impact duration. We utilize a previous training dataset (N = 1363) based on the randomly scaled anisotropic Worcester Head Injury Model (WHIM) V1.0 simulating random head impacts generated from real-world datasets. Brain-skull relative displacement was voxelized accor

Linking Symptom Phenotypes to Patterns of White Matter Injury in Mild Traumatic Brain Injury: A Latent Class Analysis.

BACKGROUND AND PURPOSE: Mild traumatic brain injury (MTBI) is a common public health concern with potential long-term consequences, yet its underlying pathophysiology remains poorly understood. Clinical heterogeneity of individuals having diverse extent and array of symptoms has impeded the identification of reliable imaging biomarkers. Traditional group-level analyses may obscure biologically meaningful subtypes. This study uses latent class analysis (LCA) to classify MTBI subjects into symptom-defined subgroups and examines corresponding WM microstructural alterations using advanced diffusio

Interpreting Scores from Adolescents with Attention-Deficit/Hyperactivity Disorder on Sway Medical System Balance and Cognitive Modules.

PURPOSE: Many high schools in the United States conduct preseason "baseline" cognitive and balance testing as part of their concussion management procedures. Prior research has identified differences in cognitive and balance test scores between youth with and without attention-deficit/hyperactivity disorder (ADHD). The present study examined whether preseason baseline scores on the Sway Medical System Balance and Cognitive Modules differed between adolescent athletes with self-reported ADHD and those without ADHD. METHOD: The Sway Medical System is a smartphone or tablet-based assessment that
