Predicting the outcome of specialist rehabilitation after non-traumatic subarachnoid haemorrhage: a multicentre cohort analysis from the UK Rehabilitation Outcomes Collaborative (UKROC) database.

BACKGROUND AND METHODS: Non-traumatic subarachnoid haemorrhage (ntSAH) is associated with high mortality and disability. This cohort analysis aims to assess the impact of inpatient rehabilitation after ntSAH using the UK Rehabilitation Outcomes Collaborative database 2012-2023. Patients who gained a Minimal Clinically Important Difference (MCID) of 26.2 on functional independence (UK Functional Independence Measure+Functional Assessment Measure (UK FIM+FAM)) were compared with those who did not, using binary logistic regression to assess for predictors of positive outcomes. RESULTS: Among 292

Comparison of two prognostic calculators predicting functional independence of patients with severe traumatic brain injury at discharge from rehabilitation services.

AIM: This study evaluated functional outcomes months after severe traumatic brain injury (sTBI), and it compared the Corticosteroid Randomisation after Significant Head injury (CRASH) and International Mission for Prognosis and Analysis of Clinical Trials in TBI (IMPACT) models. METHODS: This was a retrospective observational cohort study using the Te Manawa Taki (TMT) trauma registry from 1 June 2012 to 31 December 2022. Seventy-nine sTBI patients who had CRASH and IMPACT parameters and Functional Independence Measure (FIM) scores were analysed. A Bland-Altman plot was used to compare the tw

Solving a cold case through scientific evidence.

What role can a rigorous methodological approach play in establishing scientific evidence? This study examines the death of a 36-year-old homeless man in Northern Italy in the late 1990 s. Found unconscious with signs of assault, he was hospitalised in a coma with facial trauma. A brain CT scan showed multiple contusions and a subarachnoid haemorrhage. Due to his condition, he was transferred to another hospital, where he died shortly after. The forensic autopsy, based on the presence of purulent material and leukocytic infiltration in the brain, identified cardiorespiratory failure due to men

Development of chronic subdural haematoma from mild head injury: A case report and review of current Malaysian guidelines on traumatic brain injury.

Delayed chronic subdural haematoma (cSDH) is a common but potentially serious complication following traumatic brain injury (TBI). Mild TBIs are commonly managed by primary care providers (PCPs), particularly in large, resource-limited settings such as Malaysia, where access to tertiary neurosurgical services may be delayed. Early identification of red-flag signs and symptoms and timely referrals are crucial to prevent clinical deterioration. We describe the case of a 66-year-old man who sustained mild head injury following a vasovagal syncope. His initial brain CT revealed evidence of a small

Isolated tricuspid Libman-Sacks endocarditis in a patient with systemic lupus erythematosus and antiphospholipid syndrome: case report.

BACKGROUND: Libman-Sacks endocarditis (LSE) is an immune-mediated, culture-negative valvulopathy complicating systemic lupus erythematosus (SLE) and often amplified by antiphospholipid syndrome (APS). Although classically left-sided, isolated tricuspid involvement is rare. Advanced three-dimensional (3-D) quantification refines tricuspid regurgitation (TR) grading and informs surgical timing. CASE SUMMARY: A 41-year-old woman with 2-year SLE and triple-positive secondary APS presented with 2 weeks of fever, migratory arthralgia, and a small non-traumatic subarachnoid haemorrhage (SAH). Serial

CRYSTALLBrain: crystalloid fluid choice and neurological outcome in patients with non-traumatic subarachnoid haemorrhage-a study protocol for a multi-centre randomised double-blind clinical trial.

BACKGROUND: Vasospasms are common in patients presenting with non-traumatic subarachnoid haemorrhage (SAH) and are the main contributor to long-term disability or death in these patients. The key immediate management of vasospasms is the improvement of brain perfusion by the administration of intravenous fluid and vasopressors if needed. Yet, there is no clear recommendation regarding the choice of fluid in this particular patient population. Data suggests a survival benefit using normal saline in patients with TBI; however, its impact on outcomes in patients with SAH is lacking. Thus, the aim

Acute Computer Tomography Findings in Pediatric Accidental Head Trauma-Review.

Head trauma in paediatric patients is a worldwide and constant issue. It is the number one cause for childhood mortality and morbidity. Children of all ages are susceptible to sustaining head trauma and the anatomical characteristics of the region put them in a high-risk category for developing severe traumatic brain injuries. Boys are more frequently victims of accidental head traumas, and their injuries are more severe than those encountered in girls. The mechanisms of the trauma are a determining factor for the types of lesions we find. The traumatic injuries fall into two categories, prima

Hyperoxia and unfavourable outcome in patients with non-traumatic subarachnoid haemorrhage: A systematic review and meta-analysis.

BACKGROUND: It is common practice to administer oxygen to neurocritical patients in the Intensive Care Unit (ICU). Consequent hyperoxia has been associated with unfavourable outcomes including in patients with brain injury, after cardiac arrest, sepsis, and traumatic brain injury. The aim of this systematic review was to explore the association between hyperoxia exposure and unfavourable outcome in patients following a non-traumatic subarachnoid haemorrhage (SAH). METHODS: Systematic searches of Medline, Embase, Emcare, CINAHL and PubMed were performed in February 2024 using key words for SAH

Aphasia associated with non-traumatic subarachnoid haemorrhage: a systematic review.

PURPOSE: Individuals with non-traumatic subarachnoid haemorrhage (SAH) are often excluded from studies of stroke populations due to differing pathophysiology and treatment pathways. Thus, aphasia presentation in the non-traumatic SAH population is potentially under-represented within existing research evidence. MATERIALS AND METHODS: Five databases were systematically searched with terms related to "aphasia" and "subarachnoid haemorrhage." Studies were included if aphasia was attributed to non-traumatic SAH or its associated complications, and where at least one language assessment measure us

Brain microdialysate tau dynamics predict functional and neurocognitive recovery after poor-grade subarachnoid haemorrhage.

Subarachnoid haemorrhage is a devastating disease that results in neurocognitive deficits and a poor functional outcome in a considerable proportion of patients. In this study, we investigated the prognostic value of microtubule-associated tau protein measured in the cerebral microdialysate for long-term functional and neuropsychological outcomes in poor-grade subarachnoid haemorrhage patients. We recruited 55 consecutive non-traumatic subarachnoid haemorrhage patients who underwent multimodal neuromonitoring, including cerebral microdialysis. Mitochondrial dysfunction was defined as lactate-t

Paediatric subarachnoid haemorrhage and severe vasospasm secondary to traumatic pseudoaneurysm of a fenestrated vertebral artery: a case report and review of the literature.

Paediatric intracranial aneurysms are rare entities accounting for less than 5% of all age intracranial aneurysms. Traumatic aneurysms are more common in children and have an association with anatomical variations such as arterial fenestrations. Here, we present a case of a child initially presenting with traumatic subarachnoid haemorrhage who returned to baseline and was discharged home only to return within 2 weeks with diffuse subarachnoid and intraventricular re-haemorrhage. A dissecting aneurysm of a duplicated (fenestrated) V4 vertebral artery segment was identified as a rare cause of re

Traumatic cavernous sinus syndrome - A peculiar presentation of multiple cranial nerve neuropathies following a minor head injury: Case report and literature review.

BACKGROUND: In patients with traumatic head injuries, the percentage of cranial nerve injuries (CNI) range from4.3 to 17.6% in which majority are isolated CNI[1-5].In present literature, moderate to severe types of head injuries are often studied which may result in a lack of representation and description of CNI associated with minor head injuries (MHI). Alongside this peculiar case of a traumatic cavernous sinus syndrome (CSS) that is non-thrombotic and non-fistulous in nature, this paper aims to analyse traumatic CNI in non-severe head injuries and the surrounding literature. CASE REPORT:

Reversible cerebral vasospasm and cerebral infarction secondary to meningitis following penetrating head trauma with bamboo.

A man in the 70s fell on a bamboo and punctured his left upper eyelid. CT of the head showed fractures of the medial and superior walls of the left orbit, intracranial traumatic subarachnoid haemorrhage, intraventricular haematoma and left frontal cerebral contusion. He was treated conservatively. Despite prophylactic antibiotic therapy, he had prolonged loss of consciousness. A cerebrospinal fluid examination revealed bacterial meningitis. Imaging studies on day 15 showed extensive subacute cerebral infarction in the bilateral parieto-occipital lobes and main trunk artery stenosis. On day 31,

Type I atlanto-occipital dislocation complicated by non-communicating hydrocephalus - A case report.

Hydrocephalus, an extremely rare complication of craniocervical junction injuries, is postulated to result from compression of the fourth ventricular cerebrospinal fluid (CSF) outlets by fractured and displaced bone fragments, a swollen upper spinal cord or adhesions formed after a traumatic subarachnoid haemorrhage. We present the case of a 21-year-old woman for whom an injury to the cervical spine complicated by a type I atlanto-occipital dislocation contributed to the development of non-communicating hydrocephalus. The hydrocephalus was probably a consequence of impaired CSF circulation at

Neuro-leptospirosis - A batty diagnostic enigma.

Leptospirosis is an important, worldwide zoonotic, with a high incidence in warm-climate and tropical countries. Leptospirosis has a broad spectrum of manifestations, from a non-specific febrile illness to severe disease with multi-organ involvement. Transmission typically occurs following exposure to urine from infected animals, in particular domesticated animals such as rodents and cattle. Leptospira species have been identified in bats, however bat-human transmission is uncertain, and bat-associated leptospirosis in humans is seldom reported. We report a unique case of severe neuro-leptospi

BACKGROUND: Aphasia is common following stroke, with associated negative impacts on quality of life and psychosocial functioning. However, the language function of individuals who develop non-traumatic subarachnoid haemorrhage (SAH) has been minimally investigated from a speech-language pathology (SLP) perspective. AIMS: To examine and characterise the language function of individuals following non-traumatic SAH in the acute phase and describe the clinical profile and outcomes of individuals in this cohort with language impairment. METHODS & PROCEDURES: Prospective cohort study of adults adm

Respiratory failure due to diaphragm paralysis after brachial plexus injury diagnosed by point-of-care ultrasound.

A man in his fifties was injured in a traffic accident and diagnosed with traumatic subarachnoid haemorrhage, liver injury, and fractures of the rib, right clavicle, right scapula and right femur. He also presented with motor and sensory disturbances of the right upper extremity and was suspected of having a brachial plexus injury. After undergoing mechanical ventilation due to multiple traumas, he was extubated. However, he developed acute respiratory failure and required reintubation. Respiratory symptoms were not clear until just before reintubation. The diagnosis of right diaphragm paralys

Convulsive Status Epilepticus Induced by Electroconvulsive Therapy in a Patient with Major Depression.

Electroconvulsive therapy (ECT) is a well-known, safe, and efficient treatment for a variety of psychiatric diseases. We present here an unusual case of a 34-year-old patient with major depression, who developed convulsive status epilepticus persistent for eight days in connection to her first ECT-a very uncommon but serious complication. The patient was, prior to ECT treatment, treated with lithium carbonate and clomipramine for her depression. Six years prior to the ECT, the patient had experienced a convulsive syncope resulting in traumatic subarachnoid haemorrhage. This case emphasizes the

Paediatric Intracranial Aneurysms: Long-term Angiographic and Clinical Outcomes in a Contemporary Series.

OBJECTIVE: Paediatric aneurysms are rare and difficult to treat. Studies on the long-term angiographic and clinical outcomes conducted within the past decade are lacking. We aimed to investigate the clinical and radiographic outcomes of paediatric aneurysms treated with different strategies in a contemporary series. METHODS: We performed a retrospective medical record review of paediatric patients examined at our institution between 2011 and 2018. Patient charts were retrospectively reviewed for age, presentation, type and location of the aneurysm, modalities of treatment, complications, and
