Improved survival after non-traumatic subarachnoid haemorrhage with structured care pathways and modern intensive care.

OBJECTIVE: Patients with subarachnoid haemorrhage (SAH) often require multidisciplinary management and their treatment is difficult to standardize. The aim was to describe baseline characteristics, care pathways and discharge status in an unselected group of patients with first ever non-traumatic SAH, and to examine whether their care pathways and outcomes vary. METHODS: Patients admitted with first ever non-traumatic SAH to a neurosurgical unit (NSU) in Sweden during a period of 18 months in 2009-2010 were included. The data was retrospectively collected from patient charts. RESULTS: A tota

[Cortical spreading depolarization: an underestimated phenomenon after human brain injury?].

BACKGROUND AND PURPOSE: Cortical spreading depolarization waves (CSD) are massive temporary neuronal depolarizations that slowly propagate through cerebral cortex from brain injured tissue. CSD waves cause temporary brain electrical silence, local tissue hemodynamic responses and metabolic increases required for cellular repolarization. Due to this metabolic imbalance in compromised tissue, CSD could participate in the extension of secondary insults after brain injury. From the analysis of the human literature, we aimed at determine the CSD incidences in brain injured patients. METHODS: Medli

Cost-effectiveness of CTA, MRA and DSA in patients with non-traumatic subarachnoid haemorrhage.

OBJECTIVES: Intra-arterial digital subtraction angiography (DSA), magnetic resonance angiography (MRA) and computed tomographic angiography (CTA) are imaging modalities used for diagnostic work-up of non-traumatic subarachnoid haemorrhage. The aim of our study was to compare the cost-effectiveness of MRA, DSA and CTA in the first year after the bleed. METHODS: A decision model was used to calculate costs and benefits (in quality-adjusted life-years [QALYs]) that accrued to cohorts of 1,000 patients. Costs and characteristics of diagnostic tests, therapy, patients' quality of life and associat

A meta-analysis of treating acute traumatic brain injury with calcium channel blockers.

The purpose of this systematic review was to evaluate and meta-analyse the current evidence for the use of calcium channel blockers (CCBs) in the treatment of acute traumatic brain injury (TBI) and traumatic subarachnoid haemorrhage (tSAH). A systematic search of clinical trials.gov, Cochrane library databases, EMBASE, MEDLINE, Web of science search and WHO trial registry, plus hand-searching of grey literature, was undertaken in March 2013. Two reviewers independently extracted the data using a pre-defined data extraction form. RevMan 5 software was used to synthesise data and calculate the r

Idiopathic pulmonary calcification and ossification in an elderly woman with a missed diagnosis of subarachnoid haemorrhage.

This is a case of idiopathic pulmonary calcification and ossification in a 70 year old with long-standing diabetes and hypertension. Thirteen years prior to her demise, she was first noticed to have multiple calcific deposits in her lungs on a chest X-ray film. She had no risk factors for soft tissue calcification and ossification. Histology of tissue from autopsy showed intraparenchymal pulmonary calcification and ossification with marrow elements. Idiopathic pulmonary calcification and ossification is rare. At autopsy, she was also found to have had bilateral subarachnoid haemorrhage (SAH),

Subarachnoid haemorrhage as the initial manifestation of cortical venous thrombosis.

Aneurysmal rupture is the commonest cause of non-traumatic subarachnoid haemorrhage (SAH). SAH can rarely be the manifestation of cortical venous thrombosis (CVT). CVT is potentially lethal but treatable disorder with positive outcome if timely treatment is instituted. The site of bleeding is mainly on convexities and sulcus with sparing of basal cisterns in SAH related to CVT. Anticoagulation is the mainstay of treatment, in spite of SAH complicated by CVT. In this submission, the author highlighted a case of SAH presented as initial manifestation of CVT in an elderly woman. Early therapy wit

Traumatic brain injury in older adults: characteristics, causes and consequences.

INTRODUCTION: Traumatic brain injury is of particular concern in the older population. We aimed to examine the trends in hospitalisations, causes and consequences of TBI in older adults in New South Wales, Australia. METHODS: TBI cases from 1 July 1998 to 30 June 2011 were identified from hospitalisation data for all public and private hospitals in NSW. Direct aged standardised admission rates were calculated. Negative binomial regression modelling was used to examine the statistical significance of changes in trend over time. RESULTS: There were 12,564 hospitalisations for TBI over the 13 y

Incidence, treatment, and case-fatality of non-traumatic subarachnoid haemorrhage in the Netherlands.

BACKGROUND: Non-traumatic subarachnoid haemorrhage (SAH) is a devastating disorder and in the majority of cases it is caused by rupture of an intracranial aneurysm. No actual data are available on the incidence of non-traumatic SAH and aneursymal SAH (aSAH) in the Netherlands and little is known about treatment patterns of aSAH. Our purpose was therefore to assess the incidence, treatment patterns, and case-fatality of non-traumatic (a)SAH within the Dutch general population. METHODS: Two population based data sources were used for this retrospective cohort study. One was the nationwide hospi

Maximum thickness of subarachnoid blood is associated with mortality in patients with traumatic subarachnoid haemorrhage.

This study was designed to evaluate whether the maximum thickness of subarachnoid blood is an independent prognostic marker of mortality after traumatic subarachnoid haemorrhage. Multivariate analysis showed the maximum thickness of subarachnoid blood was an independent predictor of death versus survival 1 month after injury and was inversely associated with Glasgow Coma Scale (GCS) score. Receiver operating characteristic curve analysis showed that maximum thickness of subarachnoid blood > 6.7 mm immediately after non-surgical resuscitation predicted 1-month mortality with 83.9% sensitivity a

Traumatic epidural haematoma of the posterior cranial fossa.

A posterior fossa epidural haematoma (EDH) is uncommon and the diagnosis is difficult because the clinical symptoms are non-specific. Therefore, a computed tomography scan is important for the early diagnosis and management. Thirty-four patients with a posterior fossa EDH were admitted between 2001 and 2008. A retrospective analysis of the clinical and radiographic findings with regard to outcome and prognostic factors was carried out. The Glasgow Coma Scale (GCS) score on admission was recorded to be: one in 3-5, five in 6-8, six in 9-12 and 22 patients in 13-15. The admission GCS score was t

[Hyperammonaemic encephalopathy due to valproic acid].

INTRODUCTION: One of the less frequent idiosyncratic side effects of valproic acid (VPA) is encephalopathy. Here we report one case. CASE REPORT: An 83-year-old female with no relevant past history, who received treatment with VPA following a post-traumatic subarachnoid haemorrhage and two convulsive seizures. A few days later, she was admitted to the Emergency Department because of a progressive clinical picture of mental slowness, nauseas and apathy. The systemic examination was normal. Neurologically, the most striking features were inattention and disorientation, despite her having a good

Vasospasm in traumatic brain injury.

Given the large societal burden from morbidity and mortality associated with traumatic brain injury (TBI), this disease entity has been the focus of extensive research over the past decades. Since primary injury in TBI is preventable whereas secondary injury is treatable, most of the research effort has been targeted at identifying factors that contribute to secondary injury and ways to minimize their deleterious effects. Whether post-traumatic vasospasm is one such factor is open for debate. Although radiological or anatomical vasospasm following head injury has been repeatedly demonstrated u

Rehabilitation implications of non-traumatic subarachnoid haemorrhage.

OBJECTIVE: Subarachnoid haemorrhage (SAH) remains an important cause of stroke in the rehabilitation population, whose incidence has not been changed by pre-morbid medical treatment. The understanding of the pathophysiological changes that occur after SAH has been more clearly defined, therefore the treatment and outcomes of these patients have undergone drastic changes over the past few years. The purpose of this review is to update and familiarize the rehabilitation professional on the state of the art treatment and common complications associated with this disease and how this may affect th
