An examination of skeletal fracture patterns in car occupants following fatal motor vehicle incidents.

Hundreds of thousands of car drivers and passengers die each year in motor vehicle incidents globally. In addition to determining cause of death, forensic practitioners may be asked to infer the circumstances of the incident from associated injury patterns. At present however, evidence of the skeletal fracture patterns of deceased car occupants and associated mechanisms is sparse. Therefore, this research aimed to examine whole-body fracture patterns in occupants of four-wheeled passenger vehicles (i.e., cars) who died after motor vehicle incidents in the context of the intrinsic and extrinsic

High Risk Predictors for Thoracic Aortic Injury in Pediatric Blunt Chest Trauma.

INTRODUCTION: Incidence of Blunt Thoracic Aortic Injuries (BTAIs) in children is low (0.03%) but has 40%-66% mortality [1, 2]. The gold standard for BTAI diagnosis is chest computed tomography angiogram. However, significant radiation exposure makes routine chest computed tomography angiogram for BTAI screening in children inappropriate. The purpose of this study is to identify predictors for children at high risk for BTAI based on simple history, clinical exam, and x-ray findings. METHODS: In this retrospective cohort study, the Trauma Quality Improvement Program database from 2017 to 2023 w

Orthogeriatric co-management of fragility fractures: a retrospective cohort study on one-year mortality and functional outcomes across fracture types.

BACKGROUND: Fragility fractures in older adults are associated with high morbidity and mortality, yet evidence beyond hip fractures remains limited. This study analysed one‑year mortality and functional outcomes across multiple fracture types managed under structured orthogeriatric co‑management in a certified Geriatric Trauma Centre. METHODS: We retrospectively reviewed 486 patients aged ≥ 70 years (mean 83.8 ± 6.6; 70.2% female) with fragility fractures (proximal and periprosthetic femur, cervical and thoracolumbar spine, pelvic ring, proximal humerus) treated between 2019 and 2021. When One Fracture Hides Another: A Case Highlighting Cognitive Bias in Trauma Reporting.

Fractures of the occipital condyle and the C1 lateral mass are rare and often difficult to detect on imaging, particularly when other, more obvious spinal injuries are present. We describe a 48-year-old male who sustained an unwitnessed fall, resulting in severe neck pain. Initial CT scan, follow-up MRI, and X-rays revealed only a fracture of the C7 spinous process. The patient was managed and discharged with cervical immobilization in a Miami J collar, but ongoing symptoms persisted. Only upon retrospective review by a different radiologist, after a repeated presentation to the emergency depa

Traumatic isolated spinous process fractures.

BACKGROUND: Isolated traumatic spinous process fractures account for a small proportion of diagnosed spinal fractures. Among spinal fractures, the cervical vertebra is the most common site of involvement, often referred to as a Clay-Shoveler's fracture. This study included patients with isolated spinous process fractures through radiologic examinations over the past five years. The study aimed to investigate the cause of trauma, the presence of spinal ligament injury, and the relationship between these fractures with age and sex. METHODS: Magnetic resonance imaging (MRI) and computed tomograp

Odontoid base hypodensity and its role in type II fracture risk and nonunion: a CT study.

BACKGROUND: Type II odontoid fractures show high incidence and a notable risk of nonunion. Whether regional variation in trabecular density at the odontoid base contributes to this vulnerability remains unclear. METHODS: We retrospectively analyzed cervical CT scans from 136 adults. Standardized oval regions of interest (ROIs) were placed within cancellous bone while avoiding cortex and sclerosis. Mean Hounsfield unit (HU) values were compared across three predefined regions-the odontoid tip, odontoid base, and C2 vertebral body-with age- and sex-stratified analyses using analysis of variance

Prolonged glucocorticoid use and rheumatoid arthritis-associated cervical spine deformity.

OBJECTIVES: RA patients are often prescribed glucocorticoids, although it is known that their long-term use increases the risk of osteoporosis and fractures. The association between glucocorticoid use and RA-associated cervical spine deformity is yet to be determined. METHOD: Duration and dose of glucocorticoid use were evaluated in patients with new onset RA (BeSt Trial). Missing values on the exposure were imputed using the last observation carried forward. Lateral X-rays at 5- and 10-year follow-ups were assessed for atlantoaxial subluxation (AAS) and subaxial subluxation (SAS). To estimat

Ski and Snowboard-Related Spinal Trauma and Spinal Cord Injury: A Northeastern Level I Trauma Experience.

STUDY DESIGN: Retrospective review. OBJECTIVE: To review the traumatic spinal injuries in alpine athletes treated at a single level I trauma center. SUMMARY OF BACKGROUND DATA: Recreational and competitive skiers/snowboarders are prone to spinal injuries, and recent changes in the sport may have led to increases in the incidence and severity of spinal injuries. Currently, there is a paucity of data on the epidemiology of spinal injuries resulting from skiing and snowboarding. METHODS: A review of patients admitted with traumatic spinal injuries from skiing/snowboarding, between January 2015

[Intraspinal subdural hematoma after polytrauma].

A severely injured patient was intubated at the accident site. The polytrauma computed tomography (CT) revealed complex injuries to the trunk and open fractures of the extremities. Stable fractures of the C2 and L4 vertebrae were identified in the spine. During the course of treatment a sensorimotor deficit in the lower extremities persisted on extubation. Magnetic resonance imaging (MRI) revealed an intraspinal subdural hemorrhage with absolute stenosis of the spinal canal. Immediate decompression did not result in any improvement of symptoms. A spinal subdural hematoma is a rare condition wi

Esophageal Injury in Patients with Ankylosing Spondylitis After Cervical Spine Trauma: Our Case Series and Narrative Review.

Introduction: Ankylosing spondylitis (AS) is a chronic inflammatory disorder that causes progressive ossification and fusion of the spine, particularly in the cervical region. This results in a rigid spinal column that is highly susceptible to unstable fractures, even after low-energy trauma. Cervical fractures in AS are often complex, extending through multiple spinal segments, and are associated with a high risk of neurological compromise. Esophageal injury associated with such fractures is rare but clinically significant, as the anatomical vicinity of the esophagus makes it vulnerable to di

Sequential Cranial and Overlooked Cervical Spine Injuries Due to Head Trauma: A Billiard-Like Mechanism.

A 55-year-old male construction worker presented with multiple fractures after falling downstairs and striking his forehead on a concrete wall, an incident preceded by consuming approximately 50 g of alcohol. Initial evaluation revealed a contaminated 7 cm forehead laceration and a depressed frontal bone fracture, with no neurological deficits or intracranial bleeding on computed tomography (CT), leading to discharge with follow-up instructions despite mild intoxication. The next day, neck pain prompted further imaging, uncovering a complex injury pattern: frontal bone fracture, occipital base

Concomitant body-wide trauma patterns in patients with head and neck injuries: a comparison based on the trauma register DGU® by the German trauma society and the dortmund maxillofacial trauma registry.

INTRODUCTION: Interdisciplinary trauma patients with multiple injuries require special attention from the first minute of care. This study aimed to determine the overall injury distribution in patients with head and neck injury (HNI) with respect to the head and neck injury location. METHODS: Data on patients with HNI were collected in the Dortmund Maxillofacial Trauma Register 2007-2017 based on a review of patient radiographs and files. All patients with concomitant injuries to central body parts were selected and analysed further by an expert traumatologist collecting data on GCS, ISS, inj

Multiple pathological bone fractures in a dog with adrenal-dependent Cushing's syndrome.

Pathological bone fractures may occur secondarily to Cushing's syndrome in people. Although low bone mineral density due to cortisol excess has been described in dogs, there is no evidence that dogs with Cushing's disease can develop osteoporosis or secondary bone fractures. Here, we report a case of a dog with Cushing's disease and multiple pathological bone fractures. A 14-year-old female spayed mixed-breed dog was attended due to progressive lethargy, weakness, difficulty moving, trembling and clinical signs of Cushing's syndrome (polyuria, polydipsia, alopecia and abdominal distension). The first case report of osteochondroma in a tetraplegic patient after spinal injury.

RATIONALE: Osteochondroma (OC) is a benign bone tumor commonly found near the ends of long bones. While spinal and hip localizations are rare, bilateral hip involvement is often associated with hereditary multiple exostoses, occurring in 30% to 90% of such cases. This report describes a unique case of bilateral hip OC developing after a spinal cord injury, in the absence of family history or hereditary multiple exostoses. PATIENT CONCERNS: A 35-year-old male presented with painful bilateral hip swelling and restricted mobility following a rural accident that caused a C6-C7 vertebral fracture
