BACKGROUND: Systemic venous congestion and right-ventricular (RV) dysfunction are key drivers of postoperative hemodynamic instability, prolonged ventilation, and acute kidney injury (AKI) after mitral valve surgery. Comprehensive echocardiography is informative but may be impractical in high-throughput pre-anesthesia workflows. We evaluated a pragmatic three-point bedside congestion score combining internal jugular vein (IJV) collapsibility, inferior vena cava (IVC) collapsibility, and invasively measured central venous pressure (CVP) for preoperative risk stratification.
METHODS: In this pr

INTRODUCTION: Horner syndrome (HS) is a rare but recognized complication of internal jugular vein (IJV) catheterization in children, resulting from injury to the cervical sympathetic pathway. Neonates and infants are particularly vulnerable due to the compact anatomy of the neck.
PRESENTATION OF THE CASE: We report a 38-day-old female infant who developed ipsilateral HS following open surgical placement of a Broviac catheter via the right IJV. The catheter was inserted under local anesthesia with sedation for prolonged intravenous therapy. Within 24 hours postoperatively, the patient presente

IntroductionThe benefits of veno-venous extracorporeal membrane oxygenation (VV-ECMO) for patients with acute respiratory distress syndrome (ARDS) are well established. However, cannulation-related vascular complications, while rare, can be life-threatening.Case ReportAn 82-year-old man with ARDS caused by pleural empyema and lung abscess required VV-ECMO for worsening acidemia and hypoxemia. During internal jugular vein cannulation, a purulent effusion abruptly emerged from the outflow catheter. CT revealed catheter perforation of the brachiocephalic vein with fistulization into the pleural e

Continuous renal replacement therapy (CRRT) is a widely used treatment modality for critically ill patients with acute kidney injury. The choice of vascular access route is a critical factor in determining the effectiveness of CRRT and its effects on hemodynamic stability. This study aimed to compare the effectiveness and hemodynamic effects of CRRT via the internal jugular vein (IJV) and femoral vein (FV). A total of 102 patients admitted to the intensive care unit who required CRRT were included in this prospective cohort study. Patients were divided into the IJV group and FV group based on

Necrotizing pneumonia and empyema caused by Fusobacterium necrophorum are uncommon. The classical presentation of Lemierre's syndrome, characterized by pharyngotonsillitis and internal jugular vein thrombosis, is not always observed, and incomplete and atypical variants can cause diagnostic uncertainty and delay in treatment. We describe the case of a previously healthy 22-year-old male athlete who developed severe necrotizing community-acquired pneumonia, bilateral empyema, and acute hypoxemic respiratory failure due to F. necrophorum infection, without the typical vascular thrombosis associa

BACKGROUND: Central venous catheters (CVC) in the internal jugular vein are established in perioperative care; however, the incidence of catheter-associated thrombosis (CAT) and catheter-related thromboembolic complications (CTEC) remains insufficiently studied.
OBJECTIVE: This study aimed to assess the incidence and characteristics of CAT with a focus on differences between geriatric and nongeriatric patients. Among the guiding questions were do the risk and incidence of CAT fundamentally differ between geriatric and nongeriatric patients and is the catheter dwell time associated with increa

Migration of a haemodialysis catheter is an uncommon complication that may compromise dialysis efficacy and patient safety. This report describes a case of haemodialysis catheter tip migration into the azygos vein. A man in his 40s presented with progressive dyspnoea and reduced extracorporeal blood flow during haemodialysis. Chest radiography revealed that both limbs of the left internal jugular tunnelled haemodialysis catheter had migrated to the right brachiocephalic vein and azygos vein, respectively, causing inefficient dialysis. The catheter was successfully repositioned using the modifi

Central venous catheter (CVC) rupture is an uncommon but clinically significant complication. We report a case involving a 59-year-old woman with multiple myeloma who experienced CVC rupture following prolonged infusion of undiluted etoposide (VP‑16). The patient received continuous VP‑16 infusion through a polyurethane double-lumen catheter and subsequently developed puncture-site pain, local oozing, and catheter malfunction. Radiography did not initially demonstrate catheter displacement or breakage. After multidisciplinary evaluation, the catheter was removed, revealing a longitudinal intra

Vertebrojugular arteriovenous fistulas (AVFs) are rare vascular anomalies characterized by an abnormal communication between the extracranial vertebral artery and the internal jugular vein. They may arise from trauma, iatrogenic injury, or, rarely, occur spontaneously. We present a 50-year-old male who sustained an iatrogenic injury and developed progressive pulsatile swelling on the right side of the neck, which raised suspicion of an underlying vascular lesion. Subsequent carotid Doppler, MRI with non-contrast angiography, and digital subtraction angiography revealed a vertebrojugular AVF. T

Persistent left superior vena cava (PLSVC) is an uncommon thoracic venous anomaly that is often identified incidentally during central venous access or thoracic imaging. We describe a case in which transthoracic echocardiography (TTE) was used to safely and effectively reposition a hemodialysis catheter within a PLSVC to enable continuous hemodiafiltration (CHDF). A 71-year-old man developed septic shock complicated by acute kidney injury requiring CHDF. Because vascular access options were limited, a dialysis catheter was inserted via the left internal jugular vein. Imaging revealed that the

BACKGROUND Internal jugular vein catheterization is a primary vascular access method for hemodialysis; however, it carries a risk of complications. Common adverse events include arterial puncture, hematoma, and pneumothorax; rarer but more serious events involve catheter malposition into major arteries. Despite real-time ultrasound guidance, anatomic factors, technical considerations, and patient-specific characteristics may contribute to these adverse outcomes. CASE REPORT A 64-year-old man with end-stage renal disease and decompensated heart failure required urgent dialysis. During ultrasoun

This case report describes the successful management of a critically ill elderly patient with severe SARS-CoV-2-associated acute respiratory distress syndrome (ARDS) using an integrated strategy of awake veno-venous extracorporeal membrane oxygenation (VV-ECMO) via a single-site, dual-lumen cannula (SDLC) combined with a protocolized early rehabilitation programme. Despite high predicted mortality based on validated prognostic scores, the patient achieved a favorable outcome. The SDLC, inserted via the right internal jugular vein, was instrumental as a technological enabler. By providing unobs

Accurate knowledge of cervical vascular anatomy is essential during neck dissection (ND) to prevent serious intraoperative complications. While several variations in the cervical course of the internal carotid artery (ICA) have been documented, an ICA coursing posterior to the internal jugular vein (IJV) remains extremely rare and may increase the risk of vascular injury by altering expected anatomic relationships. Therefore, a thorough preoperative imaging evaluation is critical for a safe surgical procedure. This report presents two oral cavity cancer patients who exhibited an uncommon poste

BACKGROUND: Guidewire misplacement into the ipsilateral internal jugular vein is a recognized complication of subclavian vein catheterization. Current methods can identify the misplacement but typically provide only post-procedural verification, precluding intra-procedural correction. We aimed to describe and evaluate a novel ultrasound-guided technique for real-time detection and redirection of misplaced guidewires during subclavian vein catheterization.
METHODS: This prospective observational study enrolled patients undergoing elective gastrointestinal tumor surgery who required ultrasound-

BACKGROUND: Transvenous lead extraction (TLE) is the gold-standard treatment for cardiac implantable electronic device removal but carries a small risk of major complications, including superior vena cava (SVC) injury. The prompt use of endovascular occlusion devices can provide temporary haemostasis and haemodynamic stability in the rare event of SVC laceration. The presence of congenital heart disease and venous anomalies introduces additional technical complexity and necessitates careful procedural planning.

BACKGROUND: Neonates with congenital kidney failure or acute kidney injury often require continuous kidney replacement therapy (CKRT). Historically, venous access for CKRT in this population has relied on non-approved adult catheters, commonly via the internal jugular (IJ) vein. Limited international reports describe using the umbilical vein for CKRT, but this practice is not widespread in the USA. The Nuwellis® dual lumen extended length catheter (dELC), originally created for adult ultrafiltration, may support alternative access for neonatal CKRT.
METHODS: This case series describes the cli

The right internal jugular vein (IJV) is the preferred site for central venous access due to its predictable anatomy and direct course to the superior vena cava. Congenital agenesis of the IJV is exceedingly rare and may have significant implications during central venous cannulation. We present a 67-year-old male patient undergoing hepatic resection who required central venous access after failure of peripheral intravenous lines. Pre-procedural ultrasound of the right neck demonstrated the absence of the right IJV despite appropriate positioning. Further examination confirmed right IJV agenes

Veno-venous extracorporeal membrane oxygenation (V-V ECMO) serves as an advanced extracorporeal life support modality capable of totally or partially substituting pulmonary gas exchange function. It is indicated for severe acute respiratory distress syndrome, chronic obstructive pulmonary disease, and bridging therapy prior to lung transplantation. Currently, the fixation of the internal jugular vein catheter for ECMO mainly relies on transparent dressings supplemented by secondary securing with ties. However, this method has drawbacks such as insecure fixation, susceptibility to catheter disp
