Unmasking Parasitic Colitis Mimicking Inflammatory Bowel Disease.

Parasitic infections may clinically mimic inflammatory bowel disease (IBD), particularly in patients with underlying liver cirrhosis, creating substantial diagnostic uncertainty. We describe a 27-year-old Egyptian male with presumed autoimmune hepatitis (AIH)-related cirrhosis who presented with abdominal pain and bloody diarrhea. Endoscopic evaluation suggested IBD; however, histopathology demonstrated eosinophilic colitis. Subsequent stool and serologic testing confirmed co-infection with Schistosoma mansoni and Strongyloides stercoralis. Treatment with antiparasitic therapy resulted in mark

Diversity of Strongyloides stercoralis Isolates from Humans in Iran.

Strongyloides stercoralis (S. stercoralis), the etiological agent of strongyloidiasis, is a medically important intestinal nematode that can cause life-threatening disseminated infections, particularly in immunocompromised individuals. Despite its clinical significance, limited information is available on the genetic diversity, population structure, and phylogenetic relationships of this parasite, especially in endemic regions. Therefore, generating molecular data from different geographic areas is essential for better understanding transmission patterns and epidemiology. In the present study,

Beyond antibody titers: Corticosteroid effects on IgG avidity dynamics in experimental strongyloidiasis.

Strongyloidiasis is a neglected tropical disease caused by Strongyloides spp., capable of lifelong persistence and fatal hyperinfection or dissemination in immunosuppressed individuals. Animal models play a key role in understanding host immune responses and improving diagnostics. This study evaluated the effects of dexamethasone and prednisolone, administered orally or subcutaneously, on the humoral immune response in a Strongyloides venezuelensis rat model. IgG kinetics and antibody avidity were analyzed by ELISA. Infected control animals seroconverted 8 days post infection (dpi), with all p

Strongyloides Genetic Diversity among Humans, Dogs, and Nonhuman Primates, Central African Republic, 2016-2022.

Strongyloides stercoralis nematode infection occurs in ≈600 million persons worldwide and is listed by the World Health Organization as a neglected tropical disease. Understanding zoonotic potential is critical, especially in areas where humans, domestic animals, and wildlife interact. We explored cross-species sharing of Strongyloides roundworms by analyzing fecal samples from humans, dogs, and nonhuman primates in the Dzanga-Sangha Protected Areas, Central African Republic. We detected positive samples by quantitative PCR and assessed genetic diversity through amplification of the 18S rRNA H

Dynamics of Interleukin-9 Producing Lymphocytes in Strongyloides ratti-Infected Mice.

Helminths infect a quarter of the human population and are controlled in the frame of a canonical type-2 immune response. Interleukin-9 is a cytokine with pleiotropic functions during type-2 immunity that can be produced by many different cells. Accumulating evidence suggest that IL-9 is of particular relevance in controlling intestinal helminth infections. Using mice infected with the parasitic nematode Strongyloides ratti, we showed previously that ejection from the intestine depends on IL-9 and IL-9-mediated activation of mucosal mast cells. Here we use IL-9 reporter mice to identify the re

Tuft cells promote reactivation of memory Th2 cells and are required for protective immunity to intestinal helminth re-infection.

Small intestinal (SI) tuft cells are critical sentinels for innate type 2 immunity against helminths, but their role in adaptive responses is unclear. We show that despite the presence of protective memory CD4+ T helper 2 (Th2) cells, tuft cells remain essential for worm clearance during a secondary infection with Heligmosomoides polygyrus bakeri (H. polygyrus bakeri). Th2 cells still develop in the absence of tuft cells or group 2 innate lymphoid cells but upregulate receptors for tuft cell effectors interleukin-25 (IL-25) and leukotriene C4 (LTC4) when they enter the SI. IL-25 and LTC4 are s

Disseminated Strongyloides stercoralis infection diagnosed by metagenomic next-generation sequencing of a cell-free DNA blood sample in a patient with hematologic malignancy in Germany: A case report.

BACKGROUND: Rare infections that are atypical for Central Europe are increasingly relevant due to global migration, climate change, and the widespread use of immunosuppressive therapies. Diagnosing such infections is often delayed or missed entirely because conventional testing relies on prior clinical suspicion and region-specific test panels. Hypothesis-free metagenomic next-generation sequencing (mNGS) offers a promising diagnostic strategy in these cases. CASE PRESENTATION: We report a case of disseminated Strongyloides stercoralis (S. stercoralis) infection with hyperinfection syndrome i

An unusual presentation of strongyloidiasis in a patient on chronic dialysis causing clinical deterioration.

BACKGROUND: Strongyloidiasis is a parasitic infection caused by the helminth Strongyloides stercoralis. Strongyloidiasis is endemic in tropical regions, including Mali. Diffuse forms of the disease occur in seriously immunocompromised patients and those undergoing immunosuppressant therapies, such as corticosteroids. We report an unusual case of disseminated strongyloidiasis in a patient undergoing chronic hemodialysis in Mali. CASE PRESENTATION: A 62-year-old woman from Mali with a past medical history of diabetic nephropathy was followed in our clinic. She has been undergoing chronic hemodia

CD160-competent ILC2 are crucial for the ejection of intestinal helminths by the innate immune system.

1.6 billion people are currently infected with parasitic worms. Group 2 innate lymphoid cells (ILC2) play a central role in promoting the protective type 2 immunity against these parasites. Here we show that a subpopulation of intestinal ILC2 express the immune checkpoint molecule CD160 in mice infected with the parasitic nematode Strongyloides ratti. CD160+ ILC2 represented a distinct ST2-IL-17RB+Ki-67+ subset that expanded in vivo during S. ratti infection. By contrast, CD160- ILC2 were ST2+IL-17RB-Ki-67- and represented the dominant producers of type 2 cytokines. Upon in vitro stimulation,

In-silico design of a multi-epitope vaccine candidate against Strongyloides stercoralis.

BACKGROUND: Strongyloides stercoralis is a soil-transmitted helminth causing strongyloidiasis, mainly affecting impoverished populations in tropical and subtropical regions. Despite its high global prevalence, affecting an estimated 613.9 million people, it remains neglected in public health initiatives. Current treatments, including albendazole and ivermectin, show variable efficacy and face potential resistance, making vaccine development critical for long-term control and achieving WHO targets. METHODS: We employed an immunoinformatics approach to design a multi-epitope vaccine candidate by

Predictors of severe strongyloidiasis and mortality in hospitalized patients from Southern Thailand.

BACKGROUND: Strongyloidiasis is a neglected tropical infection that can progress to life-threatening disease in immunocompromised hosts. However, predictors of severe disease and mortality remain incompletely defined, particularly in endemic regions. We aimed to identify clinical predictors of severe strongyloidiasis and 30-day mortality among hospitalized patients in southern Thailand. METHODS: We conducted a 16-year retrospective cohort study of hospitalized patients aged ≥13 years with laboratory-confirmed Strongyloides stercoralis infection at a tertiary-care hospital between 2009 and 202

Pre- and post-kidney transplant gnathostomiasis: Clinical manifestations, treatment, and outcomes.

Gnathostomiasis (GNT) is a foodborne parasitic infection endemic to Southeast and East Asia. Its clinical relevance in transplant populations remains poorly defined. We assessed the clinical presentation, management, and outcomes of GNT in kidney transplant candidates and recipients in Thailand (2017-2023). Eighteen patients were identified (median age 51.5 years; 72.2% male), with 61.1% diagnosed at a median of 8.4 months posttransplant. Diagnosis was based on compatible clinical features and a positive Gnathostoma immunoblot test. Eosinophilia was more common in kidney transplant candidates

Development of Recombinase Polymerase Amplification and CRISPR-Cas12a-Enhanced Isothermal Amplification Assays for Strongyloides stercoralis DNA Detection: A Pilot Study.

Soil-transmitted helminth (STH) infections are prevalent worldwide, but the true burden of strongyloidiasis is unclear due to lack of sensitive and field-friendly diagnostic tools. Diagnosis is often based on serological assays that are typically not point-of-care (POC). Although polymerase chain reaction (PCR) tests are sensitive and specific, the need for expensive equipment and highly skilled personnel limits their use in resource limited areas. Isothermal amplification assays are largely instrument-free, making them simpler to implement without loss of either sensitivity or specificity. We

Words and Worms: The Nosology of Strongyloidiasis.

Strongyloidiasis impacts millions of people on several continents. Although chronic infection is often subclinical, select forms of immune dysregulation predispose patients to severe clinical manifestations resulting from acceleration of the nematode's autoinfective cycle. The terms "hyperinfection syndrome" and "disseminated strongyloidiasis" commonly refer to these disease states; however, contemporary usage is not uniform. We herein review the origins of terms related to the nosology of strongyloidiasis and highlight important mechanisms of disease pathogenesis that make untreated strongylo

Pleural effusion associated with Strongyloides stercoralis hyperinfection in a splenectomised dog.

Strongyloides stercoralis is a soil-transmitted helminth that is increasingly reported in dogs across Europe. While most infections are subclinical, hyperinfection syndrome, marked by excessive larval proliferation and systemic spread, can occur in immunocompromised hosts. An 11-year-old male Boston Terrier dog, originating from southern France with a history of chronic diarrhoea and recent splenectomy was presented with severe regenerative anaemia, thrombocytopenia, and a degenerative left shift. Haemotropic Mycoplasma haemocanis infection was diagnosed, and doxycycline therapy was initiated.
