Cost-benefit analysis of rib fixation for multiple rib fractures in US trauma patients.

BACKGROUND: The surgical stabilization of rib fractures has seen increased adoption as indications expand beyond flail chest. However, little literature addresses the cost implications of this broader use. This study compares hospitalization costs and outcomes in US trauma patients undergoing rib fixation. METHODS: The Nationwide Readmissions Database (2016-2020) was queried for trauma patients with multiple rib fractures. Those undergoing rib fixation were identified and matched 1:1 to nonoperative patients using propensity scores based on injury severity, mechanism, flail chest, age, sex, l

Impact of Surgical Stabilization of Flail Chest Injuries on Postoperative Computed Tomography Lung Volumes.

Background: Flail chest (FC) injuries are multiple adjacent segmental rib fractures, commonly associated with a high complication and mortality risk. Recent evidence suggests that the early surgical stabilization of FC injuries is beneficial for restoring breathing mechanics. However, little is known about the effects on lung volumes when invasive ventilation is performed after surgery. Methods: This retrospective study included multiple trauma (MT) patients operatively treated for an FC injury between 2011 and 2024. The indication for surgery was based on a computed tomography (CT) proof of a

Early surgical stabilization of multiple rib fractures and flail chest is associated with better outcomes compared with nonoperative management.

BACKGROUND: Surgical stabilization of rib fractures (SSRF) is increasingly performed. Nationwide data comparing its outcomes with nonoperative management (NOM) and defining the best timing for SSRF are scarce. METHODS: We analyzed data from the American College of Surgeons Trauma Quality Improvement Program, 2017-2021. Adults with three or more blunt rib fractures and no major extrathoracic injury were included. Surgical fixation was compared with risk-weighted NOM using inverse probability of treatment weighting. Primary outcome was in-hospital mortality. Secondary outcomes were hospital and

Impact of frailty on outcomes after multiple rib fractures and flail chest undergoing surgical stabilization of rib fractures: a propensity score-matched analysis of the Nationwide Inpatient Sample 2005-2020.

BACKGROUND: This study examined the impact of frailty on in-hospital outcomes in patients undergoing surgical stabilization of multiple rib fractures and flail chest (SSRF). METHODS: This retrospective study used U.S. Nationwide Inpatient Sample data (2005-2020) to analyze patients ≥20 years old who underwent SSRF for multiple rib fractures. In-hospital outcomes (mortality, discharge status, complications) were compared between frail and non-frail groups using 1:4 propensity score matching (PSM). RESULTS: After PSM, 2690 patients were included in the analyses. Frail patients had a higher lik

Real-world time-trend impact of rib fracture fixation on patients with multiple trauma using propensity score and ARIMA approaches.

BACKGROUND/PURPOSE: Thoracic injuries, especially rib fractures, carry high morbidity and mortality. Surgical stabilization of rib fractures (SSRF) has emerged as a promising approach to reduce pulmonary complications. We aimed to analyze the benefits of SSRF beyond flail chest and make the study applicable to clinical practice. METHODS: This study compared SSRF and conservative treatments at a tertiary trauma center. A time series analysis and propensity score matching were applied to evaluate the outcomes. Data from a trauma registry (January 2017 to December 2020) were used. Exclusion crit

Surgical stabilization of flail chest: a retrospective evaluation of clinical outcomes and injury score predictive validity.

Flail chest is a severe thoracic injury associated with high morbidity, prolonged hospitalization, and an increased risk of chronic pain. Surgical stabilization of rib fractures (SSRF) has become an increasingly accepted treatment modality in selected patients. However, the optimal timing of intervention and the prognostic value of radiologic injury scoring systems remain areas of investigation. Our findings confirm prior evidence supporting the early use of SSRF in selected patients and further explore the predictive value of radiologic injury scores in postoperative outcomes. This retrospect

Safety Evaluation of an Alternative Chest Compression Landmark for Cardiopulmonary Resuscitation: A Cadaveric Randomized Controlled Trial.

OBJECTIVES: Out-of-hospital cardiac arrest (OHCA) remains a significant global health challenge. Cardiopulmonary resuscitation (CPR) plays a pivotal role in patient survival; the International Liaison Committee on Resuscitation (ILCOR) recommends compressions on the lower half of the sternum. However, emerging evidence suggests that performing compressions below this point, directly targeting the maximal diameter of the left ventricle, may improve cardiac output and clinical outcomes. This study assessed the safety of the new compression landmark by comparing complications with the standard ap

Contemporary and Future Perspectives on Thoracic Trauma Care: Surgical Stabilization, Multidisciplinary Approaches, and the Role of Artificial Intelligence.

BACKGROUND/OBJECTIVES: Thoracic trauma remains a leading cause of trauma-related illness and death. Despite advances in imaging, ventilation strategies, and surgical fixation, its management remains a topic of debate, with varying practices across hospitals. Current Gaps: Although surgical stabilization of rib fractures (SSRF) has shown a mortality benefit in cases of flail chest and in elderly patients, its indications for non-flail cases remain uncertain. Analgesia strategies are evolving, and epidural remains the gold standard; however, it is limited by contraindications. In contrast, regio

Mechanics Meet Perfusion: A Retrospective Cohort Study on Optimizing Ventilatory Parameters in Traumatic Flail Chest.

Background A traumatic flail chest impairs chest wall mechanics and gas exchange. Bedside indices that integrate lung mechanics (driving pressure and dynamic compliance) with perfusion efficiency (dead-space surrogates) may guide ventilator titration more effectively than oxygenation alone. Objective To determine whether dead-space burden, driving pressure, and dynamic compliance are associated with intensive care unit (ICU) mortality and other clinically relevant outcomes (ventilator-free days to day 28 (VFD-28), ICU length of stay (LOS), barotrauma, and ICU-acquired pneumonia) in invasively

Surgical stabilization of rib fractures in conjunction with veno-venous ECMO for traumatic chest wall injury: a multi-institutional case series from the Chest Wall Injury Society (CWIS).

INTRODUCTION: Rib fractures are prevalent in 10% of blunt injury admissions and contribute significantly to trauma-related mortality. Severe cases, such as flail segments or multiple displaced fractures, frequently necessitate surgical stabilization of rib fractures (SSRF) to mitigate pain and improve outcomes. Pulmonary contusions, present in about 25% of chest wall injuries, can be exacerbated by severe rib fractures, leading to acute respiratory distress syndrome. These cases can lead to a combination of therapies-SSRF and veno-venous extracorporeal membrane oxygenation (VV-ECMO)-that is ra

Surgical strategy for chest wall reconstruction secondary to cardiopulmonary resuscitation versus post-traumatic.

PURPOSE: In mechanically cardiopulmonary resuscitated (CPR) patients, chest compressions at the level of the 3rd to 5th rib on the sternum result in reproducibly similar injury patterns: parasternal osteochondral dissociation (OCS) on both sides in combination with a sternal fracture with or without an additional serial rib fracture in the anterolateral column (ALS). This injury biomechanically impairs physiological breathing, resulting in an inverse breathing pattern. Trauma patients, on the other hand, often show a mixed pattern depending on the location of the main energy. The aim of the st

A new technique for intramedullary screw fixation of sternal fractures.

INTRODUCTION: Sternal fractures are uncommon but may result in significant morbidity when associated with respiratory compromise or severe pain. Conventional methods such as plate fixation are often invasive and technically challenging. METHODS: We retrospectively reviewed eight cases of transverse sternal fractures treated using an intramedullary fixation technique with cannulated cancellous screws (CCS). Preoperative computed tomography with 3D reconstruction was used for surgical planning. Reduction was achieved percutaneously or through a limited incision, followed by guidewire insertion
