BACKGROUND AND OBJECTIVES: Transforaminal endoscopic diskectomy is an effective approach for treatment of thoracic and lumbar disk pathologies. Classically, intraoperative neuromonitoring is used in these approaches given the risk to the exiting nerve root during transforaminal targeting. The trans-superior articular process (trans-SAP) approach is a targeting technique that docks on the ventrolateral SAP, allowing access to lateral recess pathology. This approach theoretically minimizes risk to the exiting nerve root which could occur with targeting or aberrant reaming. We investigate the saf

Spinal root magnetic stimulation can be used to assess diaphragm voluntary activation (VA), but its validity during inspiratory efforts remains unclear. In addition, its use to evaluate abdominal muscle VA during expiratory efforts has not been explored. This study investigates spinal root magnetic stimulation as a method to assess VA of inspiratory and expiratory muscles. In 13 healthy participants (8 female, 18-40 yr), magnetic stimulation was delivered over the cervical spine (C6) and lower thoracic spine (T10) to activate the diaphragm (n = 11) during inspiratory efforts and the abdominal

Spinal extradural arachnoid cysts (SEACs) are rare, benign, non-neoplastic lesions arising from focal dural defects, most frequently reported in the thoracic spine. Large multilocular with intraforaminal extension SEACs represent a challenging surgical case, as attempts for complete resection may result in dural defect and cerebrospinal fluid (CSF) leakage. The optimal surgical approach in the setting of adherent foraminal involvement remains an area of debate. We report a 32-year-old woman who presented with a three-month history of progressive low back pain, bilateral lower limb pain and wea

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

BACKGROUND: Posterior laminectomy is a standard treatment for thoracic ossification of the ligamentum flavum (TOLF), but it often leads to neurological deterioration during surgery. This study aimed to reduce iatrogenic neurological deterioration by using an S8 navigation system combined with an ultrasonic osteotome for three-dimensional real-time dynamic visualization decompression.
METHODS: A retrospective analysis was conducted on patients who underwent laminectomy and internal fixation for TOLF in our centre from January 2016 to January 2023. Patients were divided into a visualization gro

Evidence of left-right asymmetry in the spinal cord has been accumulating for some time. This report adds an anatomical example. For thoracic segments in the cat, the dorsal root entries of a given segment, at least for T5 to T9, were shown to have a tendency to be located more rostrally on the right than on the left. Measurements of the root positions have been brought together from notes in experimental protocols over nine years (36 animals). The rostral-most root of a segment on the right was found to lie up to 4.5 mm more rostrally than that on the left (mean 0.93 ± 1.05 mm, n = 86), while

Study DesignMulticenter retrospective cohort study.ObjectivesCement-augmented pedicle screws (CAPS) enhance fixation in osteoporotic patients, but carry a risk of cement leakage (CL), potentially leading to pulmonary embolism and neurological deficits. This study aimed to investigate the incidence and classification of CL and identify risk factor.MethodsWe included 140 patients undergoing thoracic or lumbar spinal fusion with CAPS at 7 institutions. CL was assessed by postoperative CT and classified as type S (segmental vein), type B (basivertebral vein), or type I (malpositioned screws). Mult

Spinal extradural arachnoid cyst is a rare condition, commonly located in the thoracic spine. It can be asymptomatic or present with symptoms of myelopathy due to spinal cord or nerve root compression. MRI is the initial imaging modality to diagnose this condition. CT myelography is the imaging modality of choice for detecting the actual site of communication between the subarachnoid space and arachnoid cysts; hence, it plays a pivotal role in surgical planning. We describe the imaging features of a symptomatic large spinal extradural arachnoid cyst and cord changes in a 12-year-old boy, who p

PURPOSE: In congenital scoliosis, the surgical strategy approach of hemivertebra excision, with or without instrumentation and fusion, is a common approach to correction of scoliosis. However, hemivertebra excisions are technically challenging, with potential complications including spinal cord injury, nerve root injury and cerebrospinal fluid leak. The purpose of this study was to determine whether correction of congenital scoliosis can be achieved using a posterior instrumentation/fusion-only approach without the need for hemivertebra excision.
METHODS: 35 patients with congenital scoliosis

Extradural spinal tumors represent a large proportion of all spinal tumors and commonly present with spinal cord or nerve root compression. Although metastatic lesions are the most frequent, benign and less common entities may also occur. Prompt recognition is essential to prevent irreversible neurological compromise. We report the case of a 55-year-old male patient with diabetes mellitus who presented with paraplegia following three months of progressive dorsal pain and lower limb paresthesias. Neurological examination revealed flaccid paralysis of the lower limbs, sensory loss below the T5 d

Symptomatic thoracic discogenic syndrome poses a diagnostic challenge due to its rarity. The relative immobility of the thoracic spine due to its particular orientation, structure, and function in the vertebral column contributes to the rarity of thoracic discogenic syndrome. At birth, the thoracic spine and sacrum exhibit a kyphotic curvature, whereas the cervical and lumbar spine develop full lordosis around puberty. The lordotic nature of the cervical and lumbar spine enables them to bear most of the axial skeleton's weight compared to the thoracic and sacral spine. As a result, the latter

On demand and localized treatment for excessive muscle tone after spinal cord injury (SCI) is currently not available. Here, we examine the reduction in leg hypertonus in a person with mid-thoracic, motor complete SCI using a commercial transcutaneous electrical stimulator (TES) applied at 50 or 150 Hz to the lower back and the possible mechanisms producing this bilateral reduction in leg tone. Hypertonus of knee extensors without and during TES, with both cathode (T11-L2) and anode (L3-L5) placed over the spinal column (midline, MID) or 10 cm to the left of midline (lateral, LAT) to only acti
