Reflected-wire technique for internal carotid artery catheterisation via the left radial approach in cerebral angiography.

BACKGROUND: The aim of this study was to evaluate factors causing difficulty in internal carotid artery (ICA) access using the left transradial approach for diagnostic cerebral angiography. METHODS: This was a prospective, single-centre, analytical study (n = 32 enrolled, n = 31 analysed) with an indication for diagnostic cerebral angiography. Diagnostic angiography via left transradial access was performed in all cases. Factors influencing the bilateral ICA catheterisation were evaluated. RESULTS: Diagnostic angiography was successfully completed in all 31 analysed patients (100%). Successf

[New applications of the coronary flow meter. Acute dissection of the ascending aorta with involvement of the supra-aortic trunks: a case report].

Coronary flow meter is commonly used in myocardial revascularization surgery since it allows an intraoperative assessment of blood flow through the different bypasses performed, ensuring an optimal result of the surgery. However, in cardiovascular surgery, the measurement of intraoperative arterial flows could be applied in other types of surgery other than coronary surgery. We present the case of a 68 years-old female patient diagnosed with Standford type A dissection who underwent urgent surgery, replacing the ascending aorta and aortic arch with an Evita Open Neo® prosthesis with direct rei

Do Early Aortic Remodelling Patterns at 6 Months Predict Mid-Term Outcomes After Frozen Elephant Trunk for Chronic Aortic Dissection?

OBJECTIVES: To determine whether CT within 6 months after total arch replacement (TAR) with a frozen elephant trunk (FET) for chronic aortic dissection predicts mid-term outcomes and informs the timing of distal treatment. METHODS: We analysed 56 consecutive patients who underwent TAR with FET at 2 centres (2009-2022) and had evaluable 6-month postoperative CT. Early remodelling was defined as the change from baseline to 6 months in the maximal outer-to-outer diameter of the proximal descending thoracic aorta at Level A (Ishimaru zone 3, 20 mm distal to the left subclavian artery, measured on

Delayed presentation of aortic coarctation as an aortic dissection: a case report.

INTRODUCTION: Coarctation of the aorta is a congenital heart defect characterized by a localized narrowing, typically situated just distal to the left subclavian artery. While it is often diagnosed and treated early in life, some cases may remain asymptomatic until adulthood, potentially leading to severe complications such as hypertension, stroke, heart failure, and, in rare instances, aortic dissection. CASE PRESENTATION: We present the case of a 30-year-old female who experienced progressive chest pain accompanied by refractory hypertension. Upon conducting a CT angiography, we discovered

Surgical Repair of a Giant Aberrant Artery Aneurysm in an Adult with Type B Interrupted Aortic Arch.

Interrupted aortic arch (IAA) is a rare congenital anomaly usually diagnosed in infancy and associated with intracardiac defects, making survival into adulthood without repair exceptional. We report the case of a 62-year-old woman with longstanding hypertension who presented with progressive dyspnea and dysphagia. Computed tomography angiography identified a type B IAA associated with a giant 96-mm aneurysm arising from an aberrant retroesophageal artery connecting the right subclavian artery to the descending thoracic aorta, without associated cardiac abnormalities. Surgical management consis

Transradial Subclavian Artery Stenting With Proximal Vertebral Artery Protection Using a Sheathless Balloon Guiding Catheter: A Case Report.

Subclavian artery (SCA) stenting is an established treatment for symptomatic stenosis; however, the optimal strategy for embolic protection of the vertebral artery (VA) remains controversial, particularly in cases with preserved antegrade VA flow. We report a case of symptomatic left SCA stenosis treated using proximal VA protection with a sheathless balloon-guiding catheter (BGC) via a transradial approach. A 77-year-old man with a history of cerebral infarction and coronary artery disease presented with dizziness. Preoperative imaging demonstrated severe stenosis of the left SCA adjacent to

Left thoracotomy removal of a fenestrated stent graft and arch/descending aortic replacement for chronic aortic dissection.

A 58-year-old woman with chronic aortic dissection developed rapid aneurysmal enlargement of the distal aortic arch and descending aorta following fenestrated thoracic endovascular aortic repair with left subclavian artery stenting. Computed tomography revealed a proximal type I endoleak associated with progressive aneurysmal dilatation. The patient underwent urgent open surgical repair via a left thoracotomy, including complete removal of the fenestrated stent graft and replacement of the distal aortic arch and descending aorta. The post-operative course was uneventful, and follow-up imaging

Left Subclavian Artery Revascularization in Aortic Arch Replacement: Comparative Patency of In Situ Reconstruction and Extra-Anatomic Bypass.

OBJECTIVES: Revascularization of the left subclavian artery (LSCA) is routinely required during total aortic arch replacement. In situ reconstruction may be technically difficult in cases with large aneurysms or posterior vessel displacement. Extra-anatomic aorto-axillary bypass has been proposed as an alternative, but its long-term patency relative to in situ repair remains uncertain. This study compared LSCA patency between the 2 techniques. METHODS: We retrospectively analysed 436 consecutive patients (median age 69.0 years [interquartile range (IQR) 62.0-75.0]; 347 men) who underwent tota

Origin of the right vertebral artery from the right common carotid artery in the setting of an aberrant right subclavian artery: Case and retrospective review to determine frequency.

Anomalies of the vertebral arteries are rare. The second most common vertebral artery anomaly is the right vertebral artery arising from the right common carotid artery in the setting of an aberrant right subclavian artery (ARSA). We present a case of this rare anomaly and determine the relative frequency with which the right vertebral artery arises from the right subclavian artery vs the right common carotid artery in the setting of an ARSA by retrospectively reviewing over 500 patients with an ARSA and a contrast-enhanced CT. We found that in the setting of an ARSA the right vertebral artery

Anatomy and possible variants of left subclavian artery and vertebral arteries.

INTRODUCTION: The left subclavian artery (LSA) and vertebral arteries (VAs) undergo complex embryological development, resulting in a wide spectrum of anatomical variants. These variations are of significant clinical relevance due to their potential impact on cerebral, spinal, and upper limb perfusion, as well as their influence on surgical and endovascular decision-making. EVIDENCE ACQUISITION: A comprehensive review of the available literature was performed focusing on the embryology, anatomical variants, and clinical implications of LSA and VA anomalies. Relevant anatomical, radiological,

Physician-modified endografts for left subclavian artery revascularization during aortic arch and descending thoracic endovascular repair: a literature review and center experience.

BACKGROUND: Management of the left subclavian artery (LSA) during thoracic endovascular aortic repair (TEVAR) in zone 2 remains debated, as intentional coverage without revascularization increases the risk of cerebrovascular accident and spinal cord ischemia. Among available strategies, physician-modified endografts (PMEGs) have emerged as a practical, fully endovascular option for LSA preservation. METHODS: The study reports the single-center experience at San Raffaele University Hospital, Milan, with PMEGs for TEVAR involving the distal aortic arch and the proximal descending thoracic aorta

Supernumerary transitional cervicothoracic vertebra and interrupted left subclavian artery reconstituted by a left vertebral arteria lusoria crossing a foramen transversarium.

We report an observation of a right aortic arch with an isolated left subclavian artery (Edwards III-C) occurring in association with a supernumerary transitional cervicothoracic vertebra. As a unique feature, reconstitution of the left subclavian artery involved a left vertebral arteria lusoria, resulting in the formation of a composite anomalous vessel that mimicked an aberrant subclavian artery, followed a retroesophageal course, and passed through the foramen transversarium of the supernumerary vertebra.

Treatment of a Type II Endoleak from an Atypical Mediastinal Collateral Branch Following Total Arch Replacement with Frozen Elephant Trunk.

We present a rare case of type II endoleak (T2EL) from an atypical mediastinal artery following total arch replacement with a frozen elephant trunk (FET) for chronic aortic dissection. A 63-year-old male with a history of multiple thoracic aortic surgeries, including FET, developed enlargement of an aortic arch aneurysm on follow-up. Computed tomography and diagnostic angiography revealed a T2EL due to a newly developed mediastinal collateral artery arising from the left subclavian artery feeding the sac. Selective transcatheter embolization with N-butyl-2-cyanoacrylate successfully eliminated

Saving Subclavian Arteries with Percutaneous Approach: a Solution for Catheter Misplacement.

INTRODUCTION: Inadvertent placement of a central venous catheter (CVC) into a major artery can lead to potentially devastating complications. In critically ill patients, the use of vascular closure devices is an attractive therapeutic option. We report two cases of catheter misplacement into cervical arteries managed with the Perclose ProStyle® system. METHODS: A retrospective analysis was conducted on adult patients admitted to the Intensive Care Unit (ICU) between July and December 2023 who underwent removal of a misplaced catheter using an endovascular approach. RESULTS: Two patients were

Successful salvage of late-presenting isolated type B arch interruption.

The catastrophic presentation, intriguing pathophysiology, and tailored management of a baby with late-presenting isolated interrupted aortic arch type B are presented herein. A 4-month-old baby presented with shock, arrested, and was resuscitated. She had severe left ventricular dysfunction, the ductus arteriosus (DA) was closed, and the ventricular septum was intact. Multiorgan dysfunction precluded emergency surgery. Inotropes were escalated, accepting upper limb hypertension, thereby facilitating collateral-dependent end-organ perfusion at higher perfusion pressures. Gradually, organ funct
