The vertebro-subclavian artery angle modulates hemodynamic mechanisms of atherosclerosis in vertebral artery origin: a combined clinical and computational fluid dynamics study.

OBJECTIVE: By dictating local hemodynamics, vascular geometry played a critical role in atherosclerosis (AS). The objective of this research was to clarify how the vertebro-subclavian artery (VA-SCA) angle affected local blood flow patterns in the pathophysiology of AS at the vertebral artery origin (VAO). METHODS: This research integrated clinical retrospective analysis with computational fluid dynamics (CFD) simulation. The clinical component examined 364 VAs and classified them into Plaque/Stenosis-free and Plaque/Stenosis groups based on DSA images. Baseline data and morphological paramet

Thoracic endovascular aortic repair for high-risk Stanford type B aortic dissection in a 15-year-old patient with genetic mutations: a case report with follow-up of four years.

This case report presents mid- to long-term outcomes of thoracic endovascular aortic repair for high-risk type B aortic dissection in a 15-year-old patient with myosin heavy chain protein 11 genetic mutations. A CT scan showed the primary entry tear located on the lesser curvature near the left subclavian artery, and the dissection originated from the descending aorta to the abdominal aorta, accompanied by 80% narrowing of the true lumen. Following successful endovascular therapy using stent-grafts, the patient recovered well. A four-year follow-up CT scan showed significant thoracic aorta rem

Hemodynamic factors of aortic dilatation after thoracic endovascular aortic repair for type-B aortic dissection.

OBJECTIVE: We aimed to investigate the hemodynamic factors associated with aortic dilatation following thoracic endovascular aortic repair (TEVAR) for type-B aortic dissection (TBAD). METHODS: This retrospective study enrolled patients who developed aortic dilatation following TEVAR for TBAD, patients who did not develop aortic dilatation following TEVAR for TBAD, and healthy control subjects. Computed tomography angiography (CTA) images for the dilatation group were acquired at three time points: pre-TEVAR (Group A), 1-week post-TEVAR (Group B), and the most recent follow-up (Group C). For t

Exploratory comparison of diffusion-weighted whole-body imaging with background body signal suppression and 18F-FDG PET/CT for assessing disease activity in large-vessel vasculitis.

OBJECTIVES: To compare diffusion-weighted whole-body imaging with background body signal suppression (DWIBS) with ^18F-fluorodeoxyglucose positron emission tomography/computed tomography (FDG-PET/CT) for assessing disease activity in large-vessel vasculitis (LVV), and to evaluate whether segment-level analysis provides complementary information beyond global grading. METHODS: We enrolled 25 patients with LVV (Takayasu arteritis [TAK], n = 11; giant cell arteritis [GCA], n = 14). Two blinded readers graded 11 arterial territories (18 segments) on a four-point visual scale for DWIBS and FDG-PET

Endovascular management of asymptomatic large descending thoracic aortic aneurysm long after coarctation of aorta repair: a case report.

BACKGROUND: Coarctation of the aorta represents 6-8% of congenital heart defects. Surgical repair is often done in infancy, but late complications such as aneurysm formation may be present decades later. Late aneurysm after childhood coarctation repair remains a recognized complication, but surveillance lapses and complex arch branch involvement can delay detection and complicate repair planning. We present a case of a large descending thoracic aortic aneurysm found incidentally over 60 years after initial coarctation repair, a latency period exceeding many reported cases [5]. CASE PRESENTATI

Thoracic Branch Endoprosthesis as Safe Alternative Therapy for Blunt Traumatic Aortic Injury Requiring Zone 2 Coverage.

BACKGROUND: Blunt traumatic aortic injury (BTAI) involving the left subclavian artery (zone 2) poses a challenging problem. Thoracic branch endoprosthesis (TBE) with a left subclavian artery side branch has emerged as a novel tool for thoracic endovascular aortic repair (TEVAR) for injuries requiring a zone 2 seal. We hypothesized that TBE is a noninferior method for zone 2 BTAI repair. DESIGN: Retrospective analysis of a prospectively collected data from National Trauma Data Bank. METHODS: The 2017-2022 American College of Surgeons Trauma Quality Programs Participant Use File data were abst

Clinical characteristics and vascular involvement of Takayasu arteritis versus Kawasaki disease in infants and young children.

BACKGROUND: Childhood-onset Takayasu arteritis (c-TAK) in infants and young children often presents with systemic inflammation rather than ischemic symptoms, and its clinical features overlap with intravenous immunoglobulin (IVIG)-resistant Kawasaki disease (KD). To aid in early differentiation, this study compared clinical characteristics and vascular involvement between c-TAK and IVIG-resistant KD in infants and young children. METHODS: This single-center retrospective cohort study included patients under three years old diagnosed with c-TAK or IVIG-resistant KD between June, 2016, and Dece

Pediatric vascular compression of the esophagus: Endoluminal functional lumen imaging probe as a complement to imaging and endoscopy.

OBJECTIVES: Vascular anomalies can cause extrinsic esophageal compression, leading to dysphagia or feeding difficulties in children. Diagnosis typically relies on imaging and endoscopy, which may under- or overestimate functional narrowing. Endoluminal functional lumen imaging probe (EndoFLIP) provides luminal parameters in real-time, but its role in vascular compression is not well-defined. METHODS: We retrospectively reviewed children with vascular anomalies who underwent upper endoscopy with EndoFLIP (July 2021-April 2025). Demographics, imaging, endoscopic/EndoFLIP findings, and outcomes

Dysphagia Lusoria in Childhood: An Uncommon Cause of Swallowing Difficulty.

Dysphagia lusoria is a rare cause of esophageal compression, typically resulting from an aberrant right subclavian artery (ARSA). ARSA is the most frequently occurring anomaly of the aortic arch. Although usually asymptomatic, most patients complain of dysphagia or respiratory symptoms. This case is of a nine-year-old boy with asthma and allergic rhinitis who presented with chronic epigastric pain, vomiting, and progressive dysphagia to solids. His growth parameters were normal but on the lower end of the body mass index (BMI) range. Esophagogastroduodenoscopy showed a pulsatile external compr

Case Report: A newly identified cause of ST-segment elevation in lead aVR.

BACKGROUND: ST-segment elevation in lead aVR typically signifies left main or triple-vessel disease. In post-coronary artery bypass grafting (CABG) patients, this pattern, alongside patent grafts, necessitates investigating alternative causes of myocardial ischemia. CASE PRESENTATION: A 74-year-old male post-CABG presented with angina and ECG showing ST elevation in aVR with diffuse depression. Angiography revealed severe native coronary disease but patent grafts, including left internal mammary artery (LIMA) to left anterior descending artery. Persistent symptoms prompted further evaluation,

Brachial artery cannulation for right upper limb ischemia secondary to acute type A aortic dissection.

Upper limb ischemia is rare complication of acute type A aortic dissection (ATAAD) with high morbidity and mortality. A 61-year-old male presented following 8-hour onset of severe chest pain and progressive right arm pain with motor-sensory deficits. Computed tomography angiography revealed ATAAD with brachiocephalic and right subclavian artery occlusion. He underwent successful ascending aortic and arch replacement with intraoperative right brachial perfusion and forearm fasciotomy. Proximal limb function was preserved, with gradual distal motor recovery at 5 months under intensive physiother

Reducing descending aortic tortuosity improves thoracic endovascular aortic repair outcomes in type B aortic dissection: from a hemodynamic perspective.

For patients with tortuous descending aortas undergoing thoracic endovascular aortic repair (TEVAR), reducing tortuosity may optimize stent-wall conformity and improve surgical outcomes, but its necessity remains unclear. This study aims to verify the necessity of reducing descending aortic tortuosity while sealing the tears during TEVAR for type B aortic dissection (TBAD) patients from a hemodynamic perspective. Radiological imaging data (pre- and post-TEVAR) of nine TBAD patients with tortuous descending aortas were collected, and subject-specific aortic geometries were reconstructed. Meanwh
