Reversal of chronic arterial stenosis with biologic therapy including tocilizumab in Takayasu arteritis.

Biologic agents have demonstrated efficacy in the treatment of refractory Takayasu arteritis (TAK). Although vascular stenosis is a common manifestation of the chronic phase of TAK, evidence on the effects of biologic therapy on established vascular lesions remains limited. Here, we report a case series of TAK in which chronic arterial stenosis showed marked improvement following treatment with biologic agents. The first case was a 27-year-old woman with stenosis of the left common carotid artery. Initial treatment with prednisolone (PSL) led to clinical improvement; however, the vascular lesi

Extracorporeal Membrane Oxygenation Related Axillary Artery Cannulation and Aortic Hemodynamics: Insights From Computational Fluid Dynamics.

Axillary and subclavian artery cannulation for veno-arterial extracorporeal membrane oxygenation (V-A ECMO) offers clinical advantages but introduces complex flow dynamics within the aortic arch. This study employed patient-specific computational fluid dynamics (CFD) models derived from 10 computed tomography (CT)-based geometries to simulate ECMO flow via either the right axillary artery or left axillary artery access under varying levels of cardiac dysfunction. Three distinct flow behaviors were observed-descending-directed, arch-split, and retrograde aortic valve (AV)-directed patterns-depe

Dynamic vascular changes on long-term ultrasound monitoring predict relapse in large-vessel giant cell arteritis.

BACKGROUND: Color duplex ultrasound (CDUS) is increasingly used to assess disease activity in large-vessel giant cell arteritis (LV-GCA), but its role in long-term monitoring and relapse prediction remains unclear. We aimed to evaluate dynamic changes in ultrasound findings during follow-up and their prognostic value for relapse. METHODS: In this retrospective longitudinal study, 43 newly diagnosed LV-GCA patients from a fast-track clinic underwent serial color duplex ultrasound (CDUS) of temporal, axillary, subclavian, and carotid arteries during follow-up. Several semiquantitative ultrasound

3D printing-assisted triple-vessel in situ fenestration combined with a diameter-restricting technique for a complex giant aortic arch aneurysm in an octogenarian: a case report and technical innovation.

BACKGROUND: Aortic arch aneurysms involving branch vessels traditionally require open surgery with cardiopulmonary bypass, which poses prohibitive risks for octogenarians with complex comorbidities. This case demonstrates the successful application of total endovascular aortic repair (TEVAR) with three dimensional (3D) printing-assisted triple-vessel in situ fenestration and a diameter-restricting technique in an 85-year-old patient with a giant (9.0 cm) aortic arch aneurysm involving the left subclavian artery. CASE PRESENTATION: The involvement of the three arch branches (brachiocephalic tr

A rare complication of transaxillary TAVI: Management of a bioprosthetic valve entrapment in the subclavian artery.

The transaxillary (TAx) approach represents the most commonly preferred alternative access route for transcatheter aortic valve implantation (TAVI) when transfemoral (TF) access is not feasible. Therefore, vascular complications and their management remain of particular importance. In this case report, we present a 75-year-old male patient with severe iliofemoral calcification who underwent TAx-TAVI and developed entrapment of the transcatheter aortic valve in the subclavian artery- a complication reported here for the first time in the literature. The entrapped valve was implanted in the subc

ICE-Guided Percutaneous Thrombectomy of Thrombus-in-Transit With Impending Paradoxical Embolism Through PFO and Pulmonary Embolism.

OBJECTIVE: Impending paradoxical embolism (IPE), defined as thrombus entrapped across a patent foramen ovale, is rare in acute pulmonary embolism (PE) and is associated with high mortality. We present to our knowledge the first successful intracardiac echocardiography (ICE)-guided mechanical thrombectomy for simultaneous treatment of right ventricular thrombus-in-transit (TIT), IPE, and high-risk PE. KEY STEPS: ICE was used to locate the mobile TIT straddling the patent foramen ovale. For embolic protection, a Sentinel cerebral protection system was deployed via the right radial artery, while

The anatomic feasibility of thoracic branched endoprosthesis in the treatment of blunt thoracic aortic injury.

INTRODUCTION: Blunt thoracic aortic injury (BTAI) is one of the leading causes of death among trauma patients who sustain high impact thoracic trauma with rapid deceleration. Thoracic endovascular aortic repair (TEVAR) is indicated in high grade injuries and requires a management strategy for the left subclavian artery (LSA). Gore TAG thoracic branch endoprosthesis (TBE) is a newly approved TEVAR device for to maintain LSA patency utilizing a side branch with reported use in acute indications. The anatomic suitability of this device for a population of BTAI patients and optimal inventory for o

Development of a scoring system for the involvement of supra-aortic vessel in patients with type A aortic dissection.

BACKGROUND: Stroke is one of the most severe postoperative complications in patients with acute type A aortic dissection (ATAAD). This study aimed to develop a novel scoring system to supplement existing classification systems and evaluate its association with postoperative stroke and other complications. METHODS: A total of 257 patients were included in the study. The degree of involvement of the innominate artery (IA), left common carotid artery (LCCA), left subclavian artery (LSA), and right common carotid artery (RCCA) was assigned a score. These scores were then combined, and a grid sear

[New applications of the coronary flow meter. Acute dissection of the ascending aorta with involvement of the supra-aortic trunks: a case report].

Coronary flow meter is commonly used in myocardial revascularization surgery since it allows an intraoperative assessment of blood flow through the different bypasses performed, ensuring an optimal result of the surgery. However, in cardiovascular surgery, the measurement of intraoperative arterial flows could be applied in other types of surgery other than coronary surgery. We present the case of a 68 years-old female patient diagnosed with Standford type A dissection who underwent urgent surgery, replacing the ascending aorta and aortic arch with an Evita Open Neo® prosthesis with direct rei

Do Early Aortic Remodelling Patterns at 6 Months Predict Mid-Term Outcomes After Frozen Elephant Trunk for Chronic Aortic Dissection?

OBJECTIVES: To determine whether CT within 6 months after total arch replacement (TAR) with a frozen elephant trunk (FET) for chronic aortic dissection predicts mid-term outcomes and informs the timing of distal treatment. METHODS: We analysed 56 consecutive patients who underwent TAR with FET at 2 centres (2009-2022) and had evaluable 6-month postoperative CT. Early remodelling was defined as the change from baseline to 6 months in the maximal outer-to-outer diameter of the proximal descending thoracic aorta at Level A (Ishimaru zone 3, 20 mm distal to the left subclavian artery, measured on

Delayed presentation of aortic coarctation as an aortic dissection: a case report.

INTRODUCTION: Coarctation of the aorta is a congenital heart defect characterized by a localized narrowing, typically situated just distal to the left subclavian artery. While it is often diagnosed and treated early in life, some cases may remain asymptomatic until adulthood, potentially leading to severe complications such as hypertension, stroke, heart failure, and, in rare instances, aortic dissection. CASE PRESENTATION: We present the case of a 30-year-old female who experienced progressive chest pain accompanied by refractory hypertension. Upon conducting a CT angiography, we discovered

Surgical Repair of a Giant Aberrant Artery Aneurysm in an Adult with Type B Interrupted Aortic Arch.

Interrupted aortic arch (IAA) is a rare congenital anomaly usually diagnosed in infancy and associated with intracardiac defects, making survival into adulthood without repair exceptional. We report the case of a 62-year-old woman with longstanding hypertension who presented with progressive dyspnea and dysphagia. Computed tomography angiography identified a type B IAA associated with a giant 96-mm aneurysm arising from an aberrant retroesophageal artery connecting the right subclavian artery to the descending thoracic aorta, without associated cardiac abnormalities. Surgical management consis

Transradial Subclavian Artery Stenting With Proximal Vertebral Artery Protection Using a Sheathless Balloon Guiding Catheter: A Case Report.

Subclavian artery (SCA) stenting is an established treatment for symptomatic stenosis; however, the optimal strategy for embolic protection of the vertebral artery (VA) remains controversial, particularly in cases with preserved antegrade VA flow. We report a case of symptomatic left SCA stenosis treated using proximal VA protection with a sheathless balloon-guiding catheter (BGC) via a transradial approach. A 77-year-old man with a history of cerebral infarction and coronary artery disease presented with dizziness. Preoperative imaging demonstrated severe stenosis of the left SCA adjacent to

Left thoracotomy removal of a fenestrated stent graft and arch/descending aortic replacement for chronic aortic dissection.

A 58-year-old woman with chronic aortic dissection developed rapid aneurysmal enlargement of the distal aortic arch and descending aorta following fenestrated thoracic endovascular aortic repair with left subclavian artery stenting. Computed tomography revealed a proximal type I endoleak associated with progressive aneurysmal dilatation. The patient underwent urgent open surgical repair via a left thoracotomy, including complete removal of the fenestrated stent graft and replacement of the distal aortic arch and descending aorta. The post-operative course was uneventful, and follow-up imaging

Left Subclavian Artery Revascularization in Aortic Arch Replacement: Comparative Patency of In Situ Reconstruction and Extra-Anatomic Bypass.

OBJECTIVES: Revascularization of the left subclavian artery (LSCA) is routinely required during total aortic arch replacement. In situ reconstruction may be technically difficult in cases with large aneurysms or posterior vessel displacement. Extra-anatomic aorto-axillary bypass has been proposed as an alternative, but its long-term patency relative to in situ repair remains uncertain. This study compared LSCA patency between the 2 techniques. METHODS: We retrospectively analysed 436 consecutive patients (median age 69.0 years [interquartile range (IQR) 62.0-75.0]; 347 men) who underwent tota
