Trained immunity and related memory responses in lung health and disease.

Immune memory has long been attributed to adaptive immunity, but trained immunity has broadened this view by showing that innate immune cells can retain altered function after prior stimulation and mount an enhanced response upon rechallenge. In the lung, prior local exposures can alter resident alveolar macrophages, whereas systemic immune stimulation can reprogramme bone-marrow progenitors and alter their myeloid output. Epithelial signals and alveolar-capillary barrier integrity help determine whether these changes protect tissue or promote injury. Reported metabolic pathways and chromatin

Endoplasmic reticulum stress and maladaptive repair in fibrotic interstitial lung diseases: A human-evidence-based narrative review.

Fibrotic interstitial lung diseases (ILDs) are characterized by progressive extracellular-matrix accumulation but arise from biologically diverse injuries. This narrative review examines endoplasmic reticulum (ER) stress as a context-dependent regulator of maladaptive repair, prioritizing evidence from human genetics, tissue, single-cell and spatial studies, patient-derived systems, biomarkers, and clinical trials. In selected familial surfactant disorders, pathogenic variants provide direct evidence that impaired proteostasis can cause fibrotic lung disease. In sporadic idiopathic pulmonary f

The Clinical Impact of Early Steroid Withdrawal on Diabetes Mellitus After Liver Transplantation: A Population-Based Cohort Study.

Despite the metabolic risks associated with corticosteroids after liver transplantation (LT), the optimal timing for their withdrawal remains uncertain due to limited and inconsistent evidence. To evaluate the impact of corticosteroid withdrawal timing on the development of de-novo post-transplant diabetes mellitus (PTDM), we performed a retrospective cohort study of 6,295 adult recipients who underwent LT between 2009 and 2021 in South Korea, utilizing a national health insurance claims database. A landmark analysis with time-varying propensity score matching was conducted at one-, three-, an

Optimal Timing for Surgical Intervention in Orbital Fracture Management: A Systematic Review and Meta-Analysis.

BACKGROUND: The optimal timing for surgical repair of orbital floor and combined orbital wall fractures remains a subject of clinical debate. While early surgery may prevent fibrosis, muscle entrapment, and enophthalmos, delayed intervention after edema resolution can sometimes yield comparable outcomes. This systematic review and meta-analysis aimed to evaluate the influence of surgical timing on functional and esthetic outcomes following orbital fracture repair. METHODS: The present systematic review followed PRISMA 2020 guidelines and was prospectively registered on PROSPERO (CRD4202450279

Quantitative Changes in Iris Vessel Area Density During Valsalva Maneuver in Healthy Eyes.

IMPORTANCE: Acute kidney injury (AKI) complicates 20% to 25% of hospital admissions and is associated with long-term mortality, especially from cardiovascular disease. Lower systolic blood pressure (SBP) following AKI may be associated with lower mortality, but potentially at the cost of higher short-term complications. OBJECTIVE: To determine associations of SBP with mortality and hospital readmissions following AKI, and to determine whether time from discharge affects these associations. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study of adults with AKI during a hospital

Integrating ketone bodies in multi-marker risk prediction of incident heart failure in the multi-ethnic study of atherosclerosis.

AIMS: Circulating ketone bodies (KB) have emerged as a potential adjunctive biomarker for incident heart failure (HF) risk and might provide incremental information beyond established biomarkers. A multi-marker risk score may improve risk stratification of incident HF in the community. The authors aim to develop a risk score using N-terminal proB-type natriuretic peptide (NT-proBNP) and high-sensitivity troponin (hs-cTnT) and a unique systemic biomarker of KB to predict HF among participants without cardiovascular disease. METHODS AND RESULTS: A multi-marker score was developed incorporating

Simulated Aeromedical Evacuation Causes Hippocampal Neuronal Damage in Rats With Acute Lung Injury.

INTRODUCTION: Aeromedical rescue plays an important role in the extensive evacuation of war casualties, sudden natural disasters (e.g., earthquakes, tsunamis), major public health events, and special needs across borders and regions. The most prominent environmental consideration in aeromedical evacuation (AE) is the impact of changes in barometric pressure on oxygen delivery and gas expansion. However, the hypobaric and hypoxic environment sustained by critically ill patients in flight can cause lung injury, leading to hypoxemia, which remains one of the few limiting factors for AE. Hyaluronan Ameliorates Viral Pneumonia in Mice and Humans by Inhibiting Transcription Factor E2F1.

Viral lung infections are a major cause of morbidity and mortality worldwide. Despite significant advances in vaccines and antivirals, there remains a tremendous need for broadly applicable treatments that can be utilized across viral infections. Before infecting epithelial cells, viruses interact with the epithelial glycocalyx, which contains high-molecular weight hyaluronan (HMWHA), a glycosaminoglycan that has beneficial effects in lung injury. In this study, we sought to determine the role of HMWHA in viral pneumonia. We infected mice with influenza or severe acute respiratory syndrome cor

Hyperoxia and surfactant dysfunction in critical illness: insights and future therapeutic prospects.

Supplemental oxygen is an essential therapy during critical illness. However, patients with severe hypoxemic respiratory failure and/or acute respiratory distress syndrome often require high oxygen concentrations, exposing lungs to alveolar hyperoxia despite systemic hypoxemia, with consequent pulmonary oxygen toxicity. Pulmonary oxygen toxicity causes disruption of surfactant, which is essential for maintenance of alveolar functional anatomy, as well as efficient and effective gas exchange and immune regulation. This surfactant dysregulation can increase alveolar surface tension, causing alve

Specialized proresolving mediator-loaded extracellular vesicles mitigate pulmonary inflammation.

Extracellular vesicles (EVs) have emerged as versatile carriers of therapeutic cargo, including nucleic acids, proteins, and small molecules. However, their potential to deliver bioactive lipid mediators remains largely unexplored. Here, we present a novel synthetic biology-based strategy to selectively load EVs with proresolving lipid mediators of the resolvin D- and E-series by coexpressing the resolvin biosynthetic enzymes cyclooxygenase 2, 5-lipoxygenase, and 15-lipoxygenase using a custom-designed multigene expression vector. Spleen Transcriptome of Nonhuman Primates Exposed to High Doses of Total- or Partial-Body Radiation: Comparisons with Jejunum and Lung.

Total-body irradiation (TBI) and partial-body irradiation (PBI) after a radiological or nuclear event result in acute radiation syndrome. Such radiation-induced injuries require immediate diagnosis and treatment. New strategies are required for radiation biodosimetry, along with the advancement of mitigation measures. Understanding gene expression alterations in irradiated cells pretreated with medical countermeasure (MCM) reveals the complex cellular responses to radiation and the radioprotective efficacy of MCM. In this study, we analyzed transcriptomic responses in irradiated nonhuman prima

Clonal Hematopoiesis and Incident Heart Failure.

IMPORTANCE: Clonal hematopoiesis of indeterminate potential (CHIP), the age-related clonal expansion of hematopoietic cells with acquired preleukemic variants, has been associated with cardiometabolic diseases, including heart failure (HF). However, prior studies have lacked power to examine less common CHIP driver variants and have not investigated potential mediators of the CHIP-HF association. OBJECTIVE: To test whether specific CHIP subtypes are associated with incident HF and determine the extent to which CHIP-associated comorbidities mediate this association. DESIGN, SETTING, AND PARTI

Tristetraprolin protects against ozone-induced acute lung injury and inflammation in mice.

Tristetraprolin (TTP) is an anti-inflammatory protein that mediates messenger RNA (mRNA) decay of certain transcripts, especially those encoding proinflammatory cytokines. TTP modulates various pathological outcomes in diverse inflammatory diseases; however, its role in ozone (O3)-induced acute lung injury (ALI) has never been tested. Here, we hypothesized that the loss of TTP would exacerbate O3-induced ALI and that the systemic overexpression of TTP would mitigate O3-induced ALI. Accordingly, TTP-knockout (TTPKO), airway epithelial cell-specific TTP-deficient (EpiKO), myeloid cell-specific T

A physiologic Comparison of Continuous Neurally Adjusted Ventilation (NeuroPAP) Versus Neurally-Adjusted Ventilatory Assist (NAVA) in Infants With Respiratory Failure.

OBJECTIVES AND HYPOTHESIS: Tonic diaphragmatic activity is common during non-invasive ventilation (NIV), suggesting efforts to increase end-expiratory lung volume. We assessed the feasibility and physiological impact of NeuroPAP, a novel NIV mode continuously adjusting the delivered positive pressure proportionally to diaphragm electric activity (Edi) during both inspiration and expiration, in infants with respiratory failure. We hypothesized NeuroPAP would enable dynamic control of end-expiratory pressures (PEEP). METHODOLOGY: This prospective crossover study enrolled premature neonates (25-

Donor-Derived Cell-Free DNA in Acute and Chronic Rejection of Solid Allograft Transplantation.

Today, there have been spectacular advances in surgical techniques, organ preservation for transplantation, optimal and efficient donor and recipient selection, more efficient diagnosis of transplant complications, and major progress in pharmacological immunosuppression procedures. In this regard, survival rates after transplantation of various organs have been gaining ground, particularly in the case of lung transplants, whose average survival rate is lower than other types of transplants. In this regard, it is important to detect acute and subclinical clinical rejection, as well as chronic a

Lung injury associated with electronic cigarette use or vaping: A narrative review.

EVALI is a rapidly developing toxic inhalation syndrome that has been linked to electronic cigarettes. It is characterized by a wide variety of radiological and histological patterns, with diffuse alveolar damage and acute respiratory distress syndrome being the most severe forms. This condition mainly affects young people and typically has a nonspecific clinical presentation. The diagnosis is one of exclusion, making it essential to thoroughly investigate into the medical history in order to perform a broad differential diagnosis that allows other causes to be ruled out. Treatment is based on
