NEC-Associated Bronchopulmonary Dysplasia and the Gut-Lung Axis in Preterm Infants.

Prevailing evidence underscores the critical influence of infant gut microbiota on systemic immune responses and intestinal health. The role of functional programming of effector immune cells at extra-intestinal mucosal sites is increasing in interest. Common connections between development of gut and lung microbiomes and reciprocal signaling between the two organ systems has reinforced the concept of a "gut-lung axis." Narrative review of existing literature evaluating mechanistic evidence linking microbial dysbiosis and necrotizing enterocolitis (NEC) to development of preterm acute lung inj

Dual Inhibition of PB2 and JAK2 for Influenza: A Strategy Combining Antiviral and Host-Directed Immune Modulation.

Influenza virus infection remains a major global health burden, with severe disease outcomes driven not only by viral replication but also by excessive host inflammatory responses. Current antiviral therapies predominantly target viral components and fail to adequately control virus-induced hyperinflammation. In this study, we report a dual-target therapeutic strategy integrating direct antiviral activity with host-directed immunomodulation. Using a molecular hybridization approach, we designed and synthesized several dual-target inhibitors simultaneously targeting the influenza virus PB2 cap-

Microbial metabolite oxindole curbs acute lung injury by suppressing CXCL13.

The gut-lung axis is involved in acute lung injury (ALI) and its fatal sequela, acute respiratory distress syndrome (ARDS), yet the molecular mechanisms governing this crosstalk remain poorly defined. Untargeted metabolomics of plasma revealed significant dysregulation of tryptophan metabolism in ARDS patients compared to healthy controls. Murine dietary interventions demonstrated that high tryptophan intake alleviated ALI severity, whereas deficiency exacerbated injury, with protection being gut microbiota dependent. 16S ribosomal RNA (16S rRNA) gene sequencing revealed marked depletion of a

Protective effects of harringtonine on lipopolysaccharide-induced acute lung injury via a novel intratracheal instillation method.

Acute lung injury (ALI), an inflammatory disease with high mortality, urgently requires novel therapeutic interventions. Harringtonine (HT) is a natural alkaloid with anti-inflammatory and antiviral properties. However, its efficacy in mitigating LPS-induced ALI remains unclear. This study systematically optimized the administration strategies of HT, to enhance therapeutic outcomes. Using high performance liquid chromatography-fluorescence detection (HPLC-FLD), we observed rapid HT degradation in SD rat plasma compared to other species. By exploring the tissue distribution of HT in SD rats fol

Different States of Lung Allograft Injury Assessed by Plasma Donor-Derived and Total Cell-Free DNA.

Lung allografts are susceptible to myriad injury types, including acute rejection (AR), infectious disease (ID), baseline lung allograft dysfunction (BLAD), and chronic lung allograft dysfunction (CLAD), that affect outcomes. Donor-derived cell-free DNA (dd-cfDNA) is validated for detecting AR after lung transplantation (LT). However, data are limited regarding the ability of dd-cfDNA or total cell-free DNA (TcfDNA) to detect or differentiate other clinical conditions. This study stratified patients into Stable, AR, CLAD, and ID cohorts. Stable double LT recipients were further stratified into

The modified fighting hypothesis of handedness: Evidence from sharp force injuries and further considerations.

The modified fighting hypothesis (MFH) proposes that most humans are right-handed because it conveyed an advantage during intraspecific fights with sharp weapons, due to the leftward location of the heart and aorta. An examination of the literature on sharp force injury showed that the thoracic region is penetrated more than any other region, and the left thorax is penetrated approximately 2.4 times more often than the right thorax. Handedness influenced the side of the thorax targeted, with most right-handers penetrating the left thorax in front of their right hand. As two thirds of the heart

Successful Management of Iatrogenic Aortocoronary Dissection Induced by a Guide Extension Catheter Using Intravascular Ultrasound Guidance.

Iatrogenic aortocoronary dissection is an uncommon but potentially life‑threatening complication of percutaneous coronary intervention. We describe a case caused by a guide extension catheter, in which intravascular ultrasound (IVUS) enabled precise assessment and successful percutaneous management. A 79-year-old female with exertional chest pain underwent percutaneous coronary intervention after persistent symptoms despite optimal medical therapy. Severe, heavily calcified stenosis in the distal right coronary artery required balloon dilatation with a guide extension catheter. Following dilat

Hyaluronic acid engineered melanin-MOF nanoreactor synergistically remodeling redox and immune homeostasis for targeted acute lung injury therapy.

Reactive oxygen species (ROS)-triggered oxidative stress, inflammatory responses, and overactivation of immune cells are critical factors exacerbating acute lung injury (ALI). Conventional clinical therapies are often limited by non-specificity, single-target limitations, and significant side effects to precisely modulate the complex pathological microenvironment. Here, we designed a hyaluronic acid-engineered nanoreactor (HA@MZ) through in situ encapsulation of melanin (MA) within ZIF-67, allowing multi-target synergistic therapy of ALI. The HA coating facilitates mucus penetration and target

Airway molecular signatures in antibody-mediated lung transplant rejection.

Severe antibody-mediated rejection (AMR) is associated with chronic lung allograft dysfunction (CLAD) and death in lung transplant recipients. However, AMR diagnostics are imprecise, and new biomarkers are needed. We assessed whether airway inflammation gene signatures could distinguish AMR cases from controls. We analyzed small airway brush RNA sequencing from 16 AMR cases and 39 controls across 2 centers. We compared gene signatures for complement activation, molecular target of rapamycin (mTOR) signaling, and natural killer cell-mediated injury. Differential gene expression and pathway anal

Cardiac radiosensitivity in the era of thoracic chemoradiotherapy and immunotherapy: a scoping review.

Concurrent chemoradiotherapy followed by immune checkpoint inhibitor (ICI) consolidation is now the standard of care for unresectable stage III non-small-cell lung cancer (NSCLC) and is increasingly applied to other thoracic malignancies. Although survival outcomes have improved, concerns about cardiac toxicity have emerged, as both chemoradiotherapy and ICIs are independently cardiotoxic and their combined effects remain unclear. This scoping review first examines chemoradiotherapy-associated and ICI-associated cardiac toxicity separately, and then evaluates their convergence in combined chem

Perfluorocarbon ameliorates high-altitude pulmonary edema and blood-gas barrier dysfunction in a canine model and a lung-on-chip model by suppressing the HIF-1α/HO-1 pathway.

High-altitude pulmonary edema (HAPE) is an increasing problem with high morbidity and mortality due to its rapid onset and progression. Perfluorocarbon (PFC) is reported to have promising effects on acute lung injury (ALI) by offering oxygenation and anti-inflammatory effects. However, PFC's effect on HAPE is still unknown. In our study, we investigated the effect of PFC, first in a HAPE canine model and then in a lung-on-chip model. We exposed beagle dogs to a simulated altitude of 6000 m for 48 h to establish the HAPE model and treated them with vaporized PFC. Next, we created a lung-on-chip

The invasive and non-invasive monitoring of inspiratory effort and drive during assisted mechanical ventilation.

BACKGROUND: In patients with respiratory failure, assisted mechanical ventilation is often used after the initial acute phase to support patient's respiratory activity, enhance oxygen exchange, and improve lung ventilation and perfusion. In this phase, monitoring inspiratory effort and respiratory drive might reduce the risk of patient self-inflicted lung injury (P-SILI), thus potentially improving the outcome. P-SILI depends on the negative pressure generated by inspiratory muscles and the positive pressure delivered by the ventilator machine. Measuring the amount of inspiratory pressure gene

Decoding lung-kidney interactions in sepsis: an integrated view of molecular mechanisms, pathophysiology, and therapeutic interventions.

Sepsis, a great health concern globally, is characterized by multi-organ dysfunction and high mortality rate. As highly vascularized organs, the kidney and lung are the most susceptible in sepsis. Moreover, the loss of normal function in either of them may increase the risk of injury to the other, and the combined dysfunction of the respiratory system and the renal system may significantly increase the mortality rate of sepsis patients. As a part of systematic response, soluble cytokines and inflammatory mediators released from lung and kidney would exacerbate overall immune disorder, while bi

Angiography-Derived Fractional Flow Reserve During Percutaneous Coronary Intervention.

BACKGROUND: Angiography-derived fractional flow reserve (Angio-FFR) is an emerging tool for guiding percutaneous coronary intervention (PCI). Its uptake and outcomes compared with pressure wire (PW)-based assessment in the United States are unknown. METHODS: We conducted a cohort study using US Medicare beneficiary data from January 1, 2019 to December 31, 2024. Propensity score matching (1:3) of Angio-FFR to PW was performed in patients who underwent PCI during the same procedure, and separately among those who did not undergo PCI during the same procedure. The primary outcome was the cumula

Intravenous lidocaine reduces systemic inflammation but not myocardial injury following thoracic surgery for lung cancer: a randomized controlled trial.

BACKGROUND: Elevated high-sensitivity troponin T levels shortly after noncardiac surgery are closely linked to myocardial injury, a key factor in 30-day postoperative mortality. Intravenous lidocaine, known for its potent anti-inflammatory and membrane-stabilizing properties, has shown cardioprotective potential in other surgical settings, but its efficacy in noncardiac thoracic surgery remains unclear. OBJECTIVES: This study was a double-blind, placebo-controlled randomized trial. Participants were randomly allocated to the lidocaine or placebo group with a 1:1 ratio. DESIGN: Single-centre, d

Glucagon-like peptide-1 receptor agonist use and clinical outcomes after lung transplantation: A single-center cohort study.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1 RA) are increasingly used for diabetes and obesity, but their safety and clinical impact after lung transplantation (LTx) are unclear. METHODS: We performed a single-center retrospective cohort study of adults undergoing primary LTx from 2018 to 2024. GLP-1 RA exposure was defined as either active treatment at the time of transplantation and/or GLP-1 RA use within the first 90 postoperative days. We compared early postoperative complications (including primary graft dysfunction grade 3 (PGD3), acute kidney injury, and respiratory in

Cardiotoxic Effects of Osimertinib Compared to Other EGFR Inhibitors: A Systematic Review and Meta-Analysis.

Osimertinib, a third-generation epidermal growth factor receptor (EGFR) inhibitor, was developed to overcome resistance from EGFR-mutant non-small-cell lung cancer (NSCLC). While it offers significant therapeutic benefits, reports have linked Osimertinib to cardiotoxic effects. This study aims to clarify the direct cardiotoxicity of Osimertinib by reviewing clinical trials and cohort studies involving Osimertinib monotherapy compared to other EGFR inhibitors. A search was conducted in online databases. Measured outcomes included risk of heart failure (HF), myocardial infarction (MI), decline i

Near infrared enhanced palladium loaded siraitia grosvenorii carbon dots amplify mitophagy for acute lung injury immunotherapy.

Mitophagy is a self-protection mechanism for cells to eliminate dysfunctional mitochondria, and maintain mitochondrial homeostasis. Thus, precisely inducing mitophagy represents a promising strategy for acute lung injury (ALI) immunotherapy. Here, the mitochondrial targeted palladium loaded siraitia grosvenorii derived carbon dots (CPs@SS31) were engineered designed to integrate PTT, mitophagy induction, and immunoregulation for synergistic enhanced ALI therapy. CPs@SS31 combining with near infrared (NIR) irradiation not only directly scavenged reactive oxygen species to achieve antioxidant an
