Laparoscopic Cholecystectomy In Situs Inversus Totalis (SIT): A Case Series - Modified French Technique Provides Optimum Outcomes.

BACKGROUND: Situs inversus totalis (SIT) is a rare congenital condition characterized by the mirror-image reversal of thoracic and abdominal organs. Its incidence is between 0.04% and 0.30%. Patients with SIT presenting with cholelithiasis pose diagnostic and technical challenges, particularly during laparoscopic cholecystectomy (LC), due to reversed anatomic landmarks and difficulties in orientation. METHODS: We conducted a retrospective review of 12 patients with SIT who underwent LC between September 2006 and April 2023 at our tertiary center. Preoperative evaluation included hematology, l

iNKT cell activation exacerbates isoproterenol-induced cardiac injury through macrophage IFN-γ-STAT1 signaling.

AIMS: With the extensive application of immunotherapy in treating cancer, the immunotherapy-related cardiovascular toxicity (ITR-CVT) has gotten a rapid recognition due to its high mortality. Previously, we have found that potential cancer immunotherapies based on promoting iNKT cell activation exacerbate ISO-induced cardiac injury, but the underlying mechanism is unknown. The current study is to determine which specific cell type/s and the corresponding molecular pathways are responsible for such a cardiotoxicity. MATERIALS AND METHODS: Transcriptome sequencing and bioinformatic analysis wer

Penetrating thoracic stab wounds and the cardiac box: a single-center experience of in-hospital treatment and outcome in Germany.

BACKGROUND: Thoracic stab injuries (TSI) are rare but potentially life-threatening emergencies. In Germany, their incidence in emergency departments remains low. The cardiac box (CB) concept has been proposed to identify cardiac involvement in penetrating thoracic trauma, although its clinical relevance remains uncertain. This study aimed to evaluate the in-hospital management of TSI and to assess the predictive value of the cardiac box for major intrathoracic injuries. METHODS: A retrospective and exploratory analysis was conducted of all patients with TSI resulting from assault or self-harm

Myocardial Injury and Incidence of Early Recurrence of Atrial Fibrillation after Catheter Ablation between Two Types of Cryoballoon Systems.

Two types of cryoballoon (CB) systems are currently available for catheter ablation of atrial fibrillation (AF). It is not clear how the difference between POLARx (Boston Scientific) and AFA-Pro (Medtronic) relates to myocardial injury and the incidence of early recurrence of fibrillation (ERAF).Patients (n = 137) who underwent catheter ablation for paroxysmal AF by 2 CB devices were included (AFA-Pro in 87; POLARx in 50). We assessed creatine kinase (CK)-MB pre and post-procedure, ERAF, and the number of atrial premature contractions (APCs) at Holter monitoring 1 month and 3 months after the

Defining Wound Care Learning Objectives for Medical Students: A Multidisciplinary Comparison of Expert and Practitioner Perspectives.

Assessing and treating a patient's wound is something that every medical student may be faced with from day one of their postgraduate clinical career, regardless of their chosen field of specialisation. This includes acute or post-surgical wounds as well as chronic wounds. In contrast, learning content concerning wound care is currently underrepresented in medical school curricula, although there are many possible topics and options to incorporate this subject into medical teaching. As a first step in curriculum development, this study provides a comprehensive yet clinically relevant catalogue

The Interventional "Backstop": The Stent-Mid-Perforation Coiling Technique for Large Distal Coronary Perforations.

Coronary artery perforations (CAP) presentations following percutaneous intervention (PCI) may vary depending on the size and location of the injured vessel. Distal coronary perforations (DCP) can rapidly progress, making treatment notoriously challenging. Endovascular coils have frequently been utilized for DCP. Unfortunately, anatomical restrictions, the size of perforation, or equipment deficiencies can complicate deployment. We present a case of large DCP complicated by acute cardiac tamponade that was unamenable to conventional methods. This was successfully treated with a drug-eluting st

Perfusion-Decellularized Goat Spinal Cord Scaffold Promotes Neuroregeneration and Functional Recovery in a Rat Model of Spinal Cord Injury.

Spinal cord injury (SCI) causes permanent loss of motor and sensory functions, which is primarily caused by restricted regenerative capacity and inhibitory post-injury environment. Maintenance of extracellular matrix (ECM) architecture is essential to sustain endogenous repair processes. This study aims to prepare perfusion-decellularized goat spinal cord scaffold (dGSC) and assess its therapeutic potential, biocompatibility, and structural integrity in fostering neuroregeneration and functional recovery in complete SCI rat model. This study employed agitation and perfusion method to goat spin

Evaluation of inflammation-mediated cancer progression during FLASH and conventional irradiation-induced lung injury.

Radiation-induced inflammatory responses are known to contribute to lung injury, but how these responses occur under ultra-high dose rate FLASH irradiation (FLASH) remains poorly characterized. The present study aimed to compare inflammation-mediated cancer progression in FLASH versus conventional (CONV) irradiation-induced lung injury in mice. Using a modified Varian 23CX clinical accelerator to deliver electron beam FLASH, we performed whole thoracic irradiation in healthy C57BL/6J mice with both FLASH and CONV modalities. While both triggered similar acute inflammatory responses, FLASH resu

Impact of inflammatory biomarkers and surgical interventions on one-month recovery after rib fractures: a propensity-matched cohort study.

BACKGROUND: Rib fractures are common, but the value of surgical stabilization and inflammatory biomarkers for early recovery remains uncertain. We assessed the impact of pretreatment inflammatory indices and surgical stabilization on 1-month outcomes after rib fractures. METHODS: We retrospectively analyzed adults with radiologically confirmed rib fractures treated between 2020 and 2023. Primary outcomes were thoracic complications and ongoing oral analgesic use at 1 month; the secondary outcome was total hospital cost. Propensity score matching (1:2) was used to compare surgical with non-surg

Bio-Inspired In Situ Injectable Hemostatic Agent with Covalent-Ionic Hybrid Network for Non-Compressible Hemorrhage.

Non-compressible hemorrhage is an arduous clinical challenge that primarily occurs at thoracic/abdominal solid organ injuries, deep arterial ruptures, and major vascular trauma, and cannot be stopped by compressing the hemorrhage site. Currently available commercial hemostatic materials for non-compressible hemorrhage are plagued by issues such as poor sealing ability and insufficient hemostatic performance. In the current work, tetra-PEG-succinimidyl glutarate (TPEG-SG), carboxymethyl chitosan (CMCS), and the coagulation factor Ca2 + were used as the primary ingredients to create a covalent-i

Frailty Index Predicts Withdrawal of Life-Sustaining Therapy in Geriatric Patients With Rib Fractures.

INTRODUCTION: Geriatric patients with rib fractures have a high morbidity and mortality. The 5-item-modified frailty index (mFI-5) is associated with increased complications and mortality in trauma patients. We aim to determine predictors of withdrawal of life-sustaining therapy (WDLST) in geriatric patients with rib fractures. MATERIALS AND METHODS: Patients were stratified by WDLST and compared. We conducted a multivariable logistic regression (MLR) to determine predictors of WDLST. RESULTS: A total of 4707 patients were included, 92 (2%) patients underwent WDLST. MLR found history of adva

Hexavalent chromium inhalation exposure induces metabolic reprogramming underlying lung injury and partial endogenous repair in mice.

Hexavalent chromium [Cr(VI)] is a pervasive toxicant in occupational and environmental settings, posing significant respiratory risks, yet the metabolic mechanisms underpinning its pulmonary toxicity and repair remain unclear. In this study, C57BL/6 J mice underwent 28 days of whole‑body inhalation exposure to aerosolized 150 μg/m³ Cr(VI), followed by a two‑week unassisted recovery. Blood chromium (Cr) levels were quantified using inductively coupled plasma mass spectrometry. Lung function, inflammatory cells of bronchoalveolar lavage fluid (BALF), and histopathology were assessed alongside un

Analysis of Bicorticality in a Cohort of Post-RibXcar Patients: A Clinical and Imaging Follow-up Study.

BACKGROUND: RibXcar is a minimally invasive technique for waistline contouring that uses ultrasound-guided monocortical rib fractures to achieve controlled angulation while preserving thoracic stability. Although its safety profile is favorable, bicorticalization fracture extension through both cortices remains a potential complication requiring systematic evaluation. OBJECTIVES: This study aimed to assess the incidence, clinical evolution, and radiologic characteristics of bicorticality in a prospective cohort of RibXcar patients. METHODS: A prospective cohort of 328 female patients (mean a

Combined polyphenols in Psidium guajava-citrus limon leaf extract attenuate fructose-induced cardiac injury by modulating metabolic and oxidative stress pathways in rats.

BACKGROUND: High fructose consumption is associated with metabolic and cardiovascular dysfunction. Polyphenol-rich lemon and guava leaves may protect the heart. OBJECTIVE: This study investigated the protective effects of Citrus limon leaf extracts, individually and in combination with Psidium guajava, against high fructose-induced cardiac injury in rats. We hypothesized that the polyphenol-rich extracts would attenuate cardiac damage through metabolic and oxidative stress modulation, with synergistic enhancement in the combined treatment. METHODS: The chemical composition of lemon and guava

Fecal microbiota transplantation ameliorates radiation-induced lung injury by reshaping gut metabolic homeostasis to activate FAM134B-mediated ER-phagy.

Radiation-induced lung injury (RILI) is a serious complication of thoracic radiotherapy, with limited effective treatment options. This study demonstrates that fecal microbiota transplantation (FMT) confers protection against RILI through modulation of the gut-lung axis. In a total lung irradiation (TLI) mouse model, FMT significantly alleviated pulmonary histopathological injury, inflammatory responses, oxidative stress, and collagen deposition during fibrogenesis. Concurrently, FMT improved intestinal motility, enhanced mucosal barrier integrity, and restored TLI-induced dysbiosis in gut mic

Incidence of pulmonary complications in rib fracture patients after surgical stabilization of rib fractures compared to RibScore prognostication.

BACKGROUND: Surgical stabilization of rib fractures (SSRF) has been associated with improved pain scores, fewer ventilator days, lower rates of ventilator-associated pneumonia and tracheostomy, shorter hospitalization, and reduced mortality. RibScore is a 6-point scoring system using chest wall injury radiographic data to predict adverse pulmonary outcomes (APOs). This study examines the incidence of APOs using RibScore criteria, hypothesizing the incidence of APOs decreases after SSRF. METHODS: A single-institution retrospective review was performed for adult SSRF patients at a Level I traum

PRMT5 Mediates Sepsis-Associated Lung Injury by Modulating JAK1 Arginine Methylation: A Mechanism Study.

Lung injury is a common complication in critical sepsis. PRMT5 is implicated in endothelial inflammation and lung diseases, but its role in sepsis-associated lung injury remains unclear. This study collected clinical sepsis samples and detected the mRNA expression of PRMT5. Subsequently, a murine sepsis model (CLP) was constructed to assess disease severity (survival, sepsis score, temperature, weight). Then, lung histopathology was evaluated with HE staining. ELISA evaluated the expression of inflammatory cytokines in mice blood, and immunohistochemistry detected PRMT5 expression. In vitro, a
