Preoperative weight loss for open abdominal wall reconstruction: study protocol for a randomized controlled trial.

BACKGROUND: Obesity is widely recognized as a risk factor for poor outcomes following ventral hernia repair. This belief has led many surgeons to implement arbitrary body mass index (BMI) cutoffs, typically 35-40 kg/m2, before offering elective hernia repair. These practices are based on low-quality evidence and create significant challenges for patients with symptomatic hernias who lack access to metabolic and bariatric surgery or obesity medications. We aim to compare medical weight loss followed by surgery versus upfront surgery in patients with severe obesity undergoing ventral hernia repa

Ventral Hernia and Obesity: A Contemporary Surgical Challenge.

Ventral hernias are abdominal wall defects classified as primary or incisional. Obesity is a significant risk factor, contributing to wound healing impairment and abdominal wall weakness. We present a 37-year-old female patient with obesity (body mass index (BMI) 48.9) who developed incisional hernia (IH) one year after laparoscopic sleeve gastrectomy. Imaging revealed infraumbilical and paraumbilical IHs, leading to surgical repair with anterior component separation, unilateral transverse muscle release, and retromuscular mesh placement. Abdominoplasty was performed for functional and estheti

Outcomes of open versus robotic preperitoneal ventral hernia repair: a propensity-matched analysis.

PURPOSE: Minimally invasive abdominal wall reconstruction has become increasingly accepted and performed in our complex population. Despite certain post-operative advantages, including possible decreased length-of-stay and wound complications, robotic ventral hernia repair (RVHR) remains controversial. The aim of this study was to evaluate outcomes, cost, and operative time following preperitoneal open (OVHR) versus RVHR. METHODS: Prospectively maintained data from a tertiary hernia center, including 1112 OVHR and 94 RVHR, was reviewed from 2016 to 2023. Patients were matched based on factors

How can we predict or prevent late mesh infection in ventral hernia repair? : Description of the type of study: a retrospective comparative study.

BACKGROUND: Mesh infection (CMI) after ventral hernia repair (VHR) is uncommon, with an incidence of 1-7%. The aim of this observational retrospective study was (1) to analyse the incidence and aetiologies of late CMI (one or more years after prosthetic repair) in a series of VHR patients, (2) to identify pre-operative and perioperative variables related to late CMI and (3) to consider possible prevention measures to reduce its occurrence. MATERIALS AND METHODS: A multivariate analysis of a prospective database of patients with a diagnosis of CMI who underwent open VHR between January 2014 an

The role of negative incisional pressure in the prevention of surgical site complications in patients with median incisional hernia.

BACKGROUND: A single-center retrospective study was conducted to investigate whether the prophylactic application of a single-use negative pressure wound therapy (sNPWT) dressing on closed surgical incisions following incisional hernia (IH) repair of the abdominal wall with meshes reduces the risk of surgical site occurrence (SSO) and the necessity for surgical reoperation. METHODS: A retrospective study was conducted on 55 patients with incisional hernias classified as W2 (> 4-10 cm) or W3 (> 10 cm) according to the European Hernia Society classification, treated between 2013 and 2023. A Prospective Observational Study Comparing Retrorectus and Anterectus Mesh Placement in Incisional Hernia Repair.

Background Incisional hernias are a frequent complication following abdominal surgeries, significantly contributing to morbidity. Surgical repair using mesh placement has become the standard of care, with ongoing debates regarding the optimal anatomical plane for mesh placement. Even though the retrorectus plane advocated by Rives and Stoppa has become the choice of plane for most surgeons, it is not without recurrence. This prospective observational study compares the outcomes of incisional hernia repair using anterectus versus retrorectus mesh placement techniques. Patients and methods A to

Does prior mesh infection matter? Clinical outcomes of patients undergoing complex abdominal wall reconstruction after infected mesh explantation.

BACKGROUND: While mesh infection after ventral hernia repair often requires explantation and subsequent repair, the association between prior mesh infection with outcomes following complex abdominal wall reconstruction remains poorly understood. METHODS: A two-stage propensity score-matched analysis was performed using the Abdominal Core Health Quality Collaborative database. Patients undergoing clean, elective, open ventral hernia repair with prior mesh infections were compared with those with no prior wound morbidity and non-mesh-related surgical site infections (SSIs). All patients underwe

Prophylactic mesh reinforcement in elective abdominal surgeries: a systematic review, meta-analysis, and GRADE evidence assessment.

BACKGROUND: Elective laparotomies account for a larger fraction of laparotomy procedures performed worldwide. Although surgical techniques continue to advance, the incidence of incisional hernia (IH) and other post-operative complications remain challenging to surgeons. This study aimed to evaluate the significance of using prophylactic mesh reinforcement during elective laparotomy. METHODS: A comprehensive search was conducted in PubMed, Scopus, and Web of Science to identify studies that included adults undergoing elective abdominal surgery and compared prophylactic mesh reinforcement of th

Are outcomes of giant ventral hernia repair inferior? A propensity-matched analysis.

BACKGROUND: Ventral hernia repair (VHR) of large defects poses significant technical challenges due to the size of the hernia and complexity of operative techniques required for fascial closure. This study examined clinical outcomes in "giant" open VHR (GVHR), with hernia defect size (HDS) ≥ 200 cm2, versus "non-giant" open VHR (NGVHR) with HDS < 200 cm2 using a propensity-matched approach. METHODS: A prospectively-maintained database from a tertiary hernia center was reviewed for patients undergoing open VHR. 1:1 propensity-score matching was performed for GVHR versus NGVHR based on age, BMI

Reconstruction of the right side of the abdominal wall due to post-traumatic muscle avulsion.

Introduction: Traumatic detachment of the lateral abdominal muscle group from the iliac crest occurs as a result of the acceleration mechanism when the body impacts a seatbelt during a head-on collision of motor vehicles. Due to the rare and variable nature of such defects, a standard treatment protocol has not yet been developed. As a result, the surgical technique and timing of the procedure are always planned individually. The optimal surgical method and type of implant for such cases remain undetermined. Given the anatomical location of the defect, surgical intervention

Tapering-pressure VAC therapy for wound exudation in POPF after pancreatoduodenectomy: a single-center experience.

BACKGROUND: Pancreaticoduodenectomy(PD) is the only effective treatment for the peri-ampullar carcinoma. However, postoperative pancreatic fistula(POPF) is the most intractable complication causing relevant mortality. Moreover, pancreatic juice may exude from the wound that would lead to more serious complications. Tapering pressure of wall vacuum-assisted closure (VAC) therapy is considered one of the best treatment to wound exudation. Here, we report on a single center series of 5 POPF cases accompanying wound exudation following open or Laparoscopic-assisted pancreatoduodenectomy, successfu

Serratia marcescens as an Uncommon Cause of Infection Following Craniectomy: A Case Report and Literature Review.

Serratia marcescens (S. marcescens) is a common bacterial species isolated from patients' specimens. Medical equipment like catheters, cannulas, tubing, dressing materials, and surgical instruments can become colonized by S. marcescens. Critically sick patients are the main victims, especially those in intensive care units or surgical settings where extended hospital stays and the use of broad-spectrum antibiotics are typical. Although S. marcescens is commonly linked to bloodstream infections, respiratory tract infections, and urinary tract infections, it is comparatively uncommon to be invol

The impact of preoperative optimization for abdominal wall reconstruction on long-term glucose control and smoking cessation.

BACKGROUND: Poorly controlled diabetes mellitus (DM) and tobacco use are known risk factors for surgical complications, particularly poor wound healing and infection. Thus, preoperative optimization prior to elective abdominal wall reconstruction (AWR) to lower patients' glycated hemoglobin (HbA1c) and achieve smoking cessation improves surgical outcomes and patients' overall health. The impact of surgeons and surgery on long-term maintenance of preoptimization is not adequately described, so this study evaluated the sustainability of preoptimized HbA1c and smoking status after AWR. METHODS:

Traumatic Intraperitoneal Bladder Rupture Presenting With Massive Ascites and Acute Kidney Injury Following a Ground-Level Fall.

A previously healthy 39-year-old man presented with progressive abdominal distension and discomfort one week after a high-velocity ground-level fall. He initially sought care elsewhere but was discharged without a definitive diagnosis. On presentation to our institution, he exhibited signs of acute kidney injury, abdominal compartment syndrome, and elevated intra-abdominal pressure. Point-of-care paracentesis revealed 7.9 L of fluid with markedly elevated creatinine, consistent with urinary ascites. A CT urogram confirmed bladder dome rupture. The patient underwent exploratory laparotomy with

A Rare Bullet Trajectory: A Case Report of an Abnormal Gunshot Wound.

Gunshot wounds (GSWs) to the thoracic region are often associated with high mortality due to the injury of vital organs such as the heart, lungs, and aorta. Abnormal bullet trajectories complicate diagnosis and treatment, particularly when the bullet remains lodged within the body and no exit wound is present. This case report describes a rare instance of a gunshot wound entering through the anterior chest wall and ultimately lodging in the rectal region without an exit wound. Initial chest and abdominal imaging showed no acute pathology; however, advanced imaging, including contrast-enhanced

Delayed Enterocutaneous Fistula Following Incisional Hernia Repair Using the Plug-and-Patch Technique: A Report of a Surgically Managed Case.

INTRODUCTION: For inguinal hernia repair, the plug-and-patch technique is commonly employed. Although abdominal wall hernias are occasionally treated with plugs, their safety remains uncertain. Herein, we report a surgical case of enterocutaneous fistula occurring 18 years after incisional hernia repair using the plug-and-patch technique. CASE PRESENTATION: An 89-year-old woman presented with right lower abdominal discomfort and a skin ulcer and was admitted to our hospital. She had undergone an open appendectomy 64 years prior, followed by incisional hernia incarceration (leading to small in

Transanal Intrusion Avoided: A Rare Case of Pediatric Accidental Penetrating Gluteal Trauma Induced by a Pencil Without Internal Perforation.

Penetrating injuries to the gluteal region are rare in children and often pose a diagnostic and therapeutic challenge due to the proximity of vital pelvic structures such as the rectum, bladder, and major vessels. Injuries caused by common classroom supplies like pencils are uncommon, but their ability to penetrate deeply into the pelvis warrants high suspicion for visceral injury. Here, we report a case of an 11-year-old previously healthy boy who suffered a left-sided gluteal injury after unintentionally sitting on a pencil placed upright on a chair as a prank during school hours.
