Complex ventral hernias pose a significant surgical challenge, particularly those with large defects or loss of domain. Traditional techniques may be associated with high complication rates or insufficient defect coverage. The modified Carbonell-Bonafé anatomical component separation (ACS) technique offers an innovative approach to achieve tension-free closure and functional abdominal wall reconstruction. We report the case of a 44-year-old woman with a giant recurrent ventral hernia (European Hernia Society (EHS) classification M2-4 W3 R6), a history of 11 previous abdominal surgeries, and a

INTRODUCTION: Abdominal compartment syndrome (ACS) is a serious complication that can occur after endovascular aneurysm repair (EVAR) for ruptured abdominal aortic aneurysm (rAAA). Prompt recognition and appropriate management are crucial to improve patient outcomes.
CASE PRESENTATION: An octogenarian with an 11-cm rAAA underwent emergent EVAR due to cardiovascular instability. Postoperatively, the patient developed ACS, necessitating open abdominal management (OAM) due to intestinal edema and retroperitoneal hematoma. Despite multiple surgical interventions, including aneurysmorrhaphy and re

As life expectancy increases, the prevalence of ruptured abdominal aortic aneurysms (rAAA) poses a significant challenge for our healthcare system. Aging induces biochemical changes, including degradation of the extracellular matrix and loss of vascular smooth muscle cells, which increase the propensity for the development of aneurysms and subsequent rupture due to compromised integrity of the aortic wall. The mortality rate for elderly patients presenting with rAAA is high, ranging from 80 to 90%. Both open and endovascular repair come with substantial risk for elderly patients. While EVAR ha

Incisional hernia is a frequent postoperative complication following laparotomy, often associated with significant morbidity and recurrence. We present the case of a 47-year-old male with a history of blunt abdominal trauma and prior exploratory laparotomy, who presented with multiple anterior abdominal wall defects. Clinical examination and imaging confirmed a "Swiss cheese" configuration of incisional hernias. Elective open sublay mesh repair using the Rives-Stoppa technique was performed. The postoperative period was uneventful. This case highlights the importance of meticulous imaging and

Intraoperative adhesions, particularly between the bladder and cesarean scar, as well as between the uterus and anterior abdominal wall, are commonly encountered in patients with a history of cesarean sections. These adhesions substantially elevate the risk of severe intraoperative complications, including bowel or bladder injuries, during hysterectomy. The incidence of bladder complications is significantly lower in patients with one or two prior cesarean sections compared to those with three or more. Moreover, the likelihood of inadvertent cystotomy is several times higher in patients with t

BACKGROUND: The introduction of evidence-based medicine has challenged many concepts. In analyzing comparative study results, it is common to find narratives highlighting favorable outcomes based on a p-value of < 0.05, without understanding the clinical impact of the observed difference. Given the prevalence of this issue in hernia surgery research, we aimed to evaluate the prevalence of studies reporting a cutoff for clinical relevance in published comparisons of open, laparoscopic, and robotic inguinal hernia repair (IHR).
METHODS: We searched Hernia, Surgical Endoscopy, Annals of Surgery,

INTRODUCTION: Large incisional hernia is a complex pathology, and its treatment yields better outcomes when performed by a surgical team specialized in abdominal wall management. Waiting lists are long, and many patients fail to meet the minimum criteria for surgery due to morbid obesity, poor control of comorbidities, or ongoing smoking. While waiting, patients often experience pain crises, increased hernia volume, loss of function, financial deterioration, and emergency procedures. In such contexts, surgery has high rates of mortality, infection, and hernia recurrence. Expanding the number o

Abdominal wall reconstruction following severe abdominal pathology presents a significant surgical challenge. The management of an open abdomen, coupled with the need for delayed closure, often requires advanced techniques to restore abdominal wall integrity, minimize complications, and optimize patient outcomes. This case report describes the successful management of a complex abdominal wall defect following severe pancreatitis and abdominal compartment syndrome in a 48-year-old male patient with a history of alcohol use disorder. After initial decompressive laparotomy and lack of progress wi

We report a rare case of recurrent evisceration following radical cystoprostatectomy with Bricker urinary diversion in a 71-year-old patient with type 2 diabetes mellitus, chronic obstructive pulmonary disease, hypertension, and peripheral arterial disease. Despite an initially uneventful recovery, the patient presented with full-thickness wound dehiscence and evisceration just eight days after discharge. His recovery was further complicated by comorbidities and poor adherence to postoperative activity restrictions. After several abdominal wall re-explorations and debridements, combined with n

A laparostomy procedure, a critical intervention frequently employed in both trauma and non-trauma patients, is a key component of damage control resuscitation. The use of a vertical traction device (VTD), specifically Fasciotens® Abdomen (FTA), is a relatively novel technology that prevents fascial retraction and facilitates primary fascial closure. This report describes the case of a male in his 20s with severe pancreatitis, complicated by multi-organ failure and abdominal compartment syndrome (ACS), necessitating decompressive laparotomy. FTA was applied alongside botulinum toxin (Botox) an

Background Inguinal hernia repair is one of the most commonly performed operations in general surgery. The laparoscopic transabdominal preperitoneal approach gives a better view of the inguinal anatomy, and the procedure also has a short learning curve. We aim to evaluate the safety and outcomes of laparoscopic transabdominal preperitoneal (TAPP) repair using the synthetic mesh in a single consultant series and compare the outcomes with the available literature. Methods Retrospective evaluation of the outcomes was carried out on all patients who underwent laparoscopic TAPP repair from Septembe

BACKGROUND: To critically appraise the evidence supporting the European and American Hernia Society (EHS/AHS) guidelines on abdominal wall closure in colorectal and emergency surgery patients, we conducted a granular analysis focused on the representation and quality of available data.
METHODS: References that addressed key questions (KQ) and recommendations in the original guidelines were screened and included if colorectal/emergency surgery was reported. Data extraction was performed with a standardised form and authors were contacted for missing data. Quality and risk of bias were assessed

Injuries to the inferior vena cava (IVC) from penetrating trauma are uncommon but carry a high risk of fatality due to the potential for rapid, massive haemorrhage. We present the case of a 56-year-old man who sustained a stab wound to the right lower chest and presented in profound haemorrhagic shock. A bedside extended Focused Assessment with Sonography for Trauma (eFAST) not only identified free abdominal fluid but also provided a direct, real-time visualisation of a tear in the IVC wall with an adjacent haematoma. Despite immediate surgical intervention, the patient died from his injuries.

BACKGROUND: Traumatic abdominal wall hernias (TAWHs) are uncommon but serious injuries resulting from blunt abdominal trauma. Despite their low incidence, the associated morbidity and mortality rates necessitate effective management strategies. This study aims to assess the incidence, injury patterns, and optimal management strategies for TAWHs.
METHOD: This retrospective cohort study was conducted at a level-1 trauma center in Melbourne, Australia, spanning 20 years. Patients diagnosed with TAWHs between October 2002 and October 2023 were identified through trauma and radiological databases.

AIM: This study evaluates the efficacy and safety of Dynatract®, a novel device designed to facilitate early primary closure and prevent aponeurotic retraction in patients with an open abdomen (OA).
METHOD: A preclinical trial was conducted using a porcine model, comparing two groups: one treated solely with AbThera™ Negative Pressure Wound Therapy (NPWT), and another combining AbThera™ NPWT with Dynatract®. The primary endpoint was to evaluate the distance between fascial edges, with secondary measures including the force required to achieve abdominal closure and overall closure success.

INTRODUCTION: Parastomal hernia is a common and challenging complication after stoma formation, often requiring complex surgical management. To address limitations of conventional techniques, we developed the Hybrid Parastomal Endoscopic Repair (HyPER) technique, which combines laparoscopic and open approaches. This case series aimed to evaluate the long-term safety, efficacy, and technical considerations of the HyPER method in a large, consecutive cohort of patients.
METHODS: This retrospective, single-center case series included 200 consecutive patients treated between 2014 and 2024. Adult

INTRODUCTION: Blunt traumatic lateral abdominal-wall hernias are complex injuries often resulting from high-energy mechanisms. These injuries commonly involve disruption of the transversus abdominis muscle, obliques, as well as potential injury avulsion of the diaphragm in thoracoabdominal hernias. Because of the variety of injury patterns, blunt traumatic lateral abdominal-wall hernias present unique reconstructive challenges, particularly when involving the iliac crest or costal margin. Despite increasing use of robotic-assisted techniques for abdominal wall reconstruction, limited data exis

INTRODUCTION: The evolution of abdominal wall reconstruction has produced multiple effective techniques for hernia repair. Transversus abdominis release and preperitoneal repair allow for the placement of large mesh constructs. The outcomes of these techniques have not been compared, and this was the aim of this study.
METHODS: Prospective data from 3,783 open abdominal wall reconstructions underwent a 1:1 propensity-score match for elective transversus abdominis release and preperitoneal repair using comorbidities, wound class, and defects. Standard descriptive and comparative statistics wer
