A Rectus Sheath Block in Open Hysterectomy.

Introduction: Open gynecological surgery results in a large wound and severe postoperative pain, and adequate postoperative analgesia is necessary. Rectus sheath block (RSB) is used to block the sensory nerves of the anterior abdominal wall, thereby contributing to pain relief after lower abdominal surgeries. RSB provides effective perioperative analgesia and is related to lower perioperative opioid consumption and decreased opioid-related adverse effects. The prospective randomized study explores the effect of RSB on the evaluation of the postoperative pain following transabdominal open hyste

Use of pedicled thigh flaps for complex abdominal wall reconstruction: A case series.

Complex abdominal wall defects that result from oncologic resection, infection, or trauma often require advanced reconstructive techniques beyond primary fascial closure or mesh reinforcement alone. In cases where primary closure is not feasible due to extensive tissue loss, pedicled thigh-based flaps offer a robust, vascularized solution to restore abdominal wall integrity. In this case series, we present three consecutive patients who underwent abdominal wall reconstruction using variations of the pedicled thigh flap based on the lateral femoral circumflex vascular pedicle, performed in conj

Comparing Laparoscopic Versus Open Repair of Recurrent Incisional Hernia: An Updated Systematic Review and Meta-Analysis of Randomized Controlled Studies on Recurrence and Complications.

BACKGROUND: Incisional hernia is the most common postoperative complication of abdominal wall surgery that significantly increases morbidity. We aimed to evaluate recurrence rates and perioperative outcomes associated with Laparoscopic and open repair for Incisional hernia recurrence. METHODS: A comprehensive and systematic literature search was conducted across PubMed, Embase, CINAHL Ultimate, Medline, Scopus, and the Cochrane Controlled Trials Register. Studies published between July 2013, and November 2024 were screened for inclusion. The data extracted included recurrence rates, surgical

Enterotomy Outcomes in Abdominal Wall Reconstruction.

INTRODUCTION: Reconstruction of complex incisional hernias can be technically challenging. Enterotomy (ENT) is infrequent in most abdominal wall reconstruction (AWR) cases, yet in complex, reoperative fields, bowel injury can be difficult to avoid. Data describing the incidence and outcomes of ENT in these operations remain limited. METHODS: A prospective, tertiary hernia center database was queried for elective AWR. Outcomes following an operative ENT (small bowel) were compared with no ENT (non-ENT). Standard statistical analyses were performed. Kaplan-Meier analysis compared recurrence-fre

Laparoscopic Suitcase Technique for lateral incisional hernias enables fully laparoscopic fascial closure: a retrospective comparative study.

BACKGROUND: Lateral incisional hernias represent a distinct subset of abdominal wall defects and pose specific challenges in minimally invasive surgery due to unfavorable force vectors and the absence of a central anchoring structure. Hybrid approaches combining laparoscopic dissection with open fascial closure can facilitate repair but partially negate the benefits of minimally invasive surgery. This study evaluated the safety and effectiveness of a fully laparoscopic method for fascial closure—the Laparoscopic Suitcase Technique (LST). While central traction techniques have been previously d

A traction suture for abdominal wall closure in large incisional hernias: a prospective analysis of the "suitcase technique" - a simple horizontal offloading z-suture for gradual myofascial approximation.

PURPOSE: Primary myofascial closure in large incisional hernia repair remains technically challenging due to excessive tension, loss of domain, and limited accessibility of costly traction devices. This study aimed to evaluate the feasibility, biomechanical effect, and short-term outcomes of a novel horizontal traction suture method—the suitcase technique—designed to facilitate gradual fascial approximation. METHODS: This prospective, single-center study included 69 patients undergoing elective open repair of large midline incisional hernias (defect width 8–20 cm) with retromuscular mesh place

Long-Term Outcomes of Preoperative Botulinum Toxin Use in Abdominal Wall Reconstruction: A Propensity-Matched Analysis of More Than 5000 Patients Over 5 Years.

BACKGROUND: Preoperative botulinum toxin (BTX) has been proposed to facilitate myofascial medialization and tension reduction in abdominal wall reconstruction (AWR), but long-term comparative outcomes are unclear. OBJECTIVES: To compare short- and long-term postoperative outcomes after AWR in patients receiving preoperative BTX vs no BTX. METHODS: We performed a retrospective cohort study using the TriNetX National Health Research Network. Adults undergoing AWR were identified by ICD-10/CPT codes and assigned to BTX or no-BTX cohorts. The Importance of Plastic Surgery in Multidisciplinary Teams: A Systematic Review and Meta-analysis of Comparative Studies.

BACKGROUND: Multidisciplinary team (MDT) care facilitates coordinated expertise that leads to improved patient safety and outcomes. Yet, no prior study has quantified reported benefits from plastic surgery-inclusive MDTs across several procedures. We thereby hypothesized that plastic surgery-inclusive MDTs would lead to better surgical outcomes following cranioplasty, spine surgery, abdominoperineal resection, abdominal wall reconstruction, and extremity reconstruction, compared with the primary specialties acting alone. METHODS: Following Preferred Reporting Items for Systematic Reviews and

Subcutaneous Wound Management in Complex, Contaminated Abdominal Wall Reconstruction (AWR) - The "French Fry" Technique.

IntroductionPrimary closure of contaminated abdominal wall reconstruction (AWR) wounds carries significant wound complication risks. Vacuum-assisted, delayed primary closure (VADPC) improves outcomes but requires a second anesthetic. The "French fry" technique (FFT) utilizes VA benefits without reoperation. The aim of this study was to evaluate the outcomes of FFT in complex, contaminated AWR.MethodsA single center's open AWRs (CDC class 2-4) managed with FFT were reviewed. Technique: incision is closed except for small gaps to place subcutaneous 1 × 1 × 8cm NPWT foam "French fries" at 5-6 cm

[Comparison of different sampling procedures in negative pressure wound therapy used in wound infections following reconstruction of abdominal wall hernias with mesh on homogeneous patient material].

INTRODUCTION: The repair of postoperative abdominal hernias remains a challenge to this day, even though the surgery is sterile. Wound infection is a problem, which is exacerbated by the comorbidity of the patient population. The incorporation of foreign material always carries a risk of infection. In the event of wound infection, the possibility of hernia recurrence is 0.2-8%, while if the mesh also needs to be removed, the probability of recurrence is over 30%. Compared to open wound treatment, negative pressure therapy is more effective in promoting wound healing and preserving the implante

Bioorthogonally Active Silk Fibroin Patch Facilitate Immunomodulatory Prodrug Activation for Tissue Repair.

Current silk fibroin (SF)-based tissue repair patches often exhibit limited immunomodulatory efficacy, exacerbating inflammatory responses and compromising functional tissue integration. To address this limitation, we developed a bioorthogonal prodrug activation strategy within an SF patch via tetrazine functionalization (SF-Tz). This platform enables the spatiotemporally controlled release of trans-cyclooctene (TCO)-caged dexamethasone (TCO-Dex) via inverse electron-demand Diels-Alder reactions. The SF-Tz platform was engineered through optimized carboxylation and tetrazine conjugation protoc

Long-term wound morbidity and hernia recurrence in diabetic patients with HbA1c ≥ 8.5% undergoing open transversus abdominis release: a descriptive longitudinal follow-up of a single-center cohort.

BACKGROUND: Preoperative glycemic control, as measured by HbA1c, is widely used in risk stratification for diabetic patients undergoing abdominal wall reconstruction (AWR). In a prior analysis, our group demonstrated that among diabetic patients undergoing open ventral hernia repair (VHR) with transversus abdominis release (TAR), preoperative HbA1c levels did not correlate with short-term wound morbidity. This study evaluates long-term outcomes in patients with poor preoperative glycemic control (HbA1c ≥ 8.5%) undergoing open AWR with TAR. METHODS: Adult diabetic patients with HbA1c ≥ 8.5% wh

Subxiphoid hernias: does surgical approach matter? An analysis of the ACHQC database.

BACKGROUND: Subxiphoid incisional hernias (SIH) are challenging to repair with recurrence rates ranging from 10 to 43%. There is a paucity of data regarding the optimal repair method and a small amount of early data on robotic SIH repair. The purpose of this study was to compare how surgical approach-open, laparoscopic, and robotic-affects clinical outcomes. METHODS: A retrospective analysis of the Abdominal Core Health Quality Collaborative was performed from 2014 to 2024. Patients undergoing incisional hernia repair involving only midline 1 (M1) or midline 1 and 2 (M1-2) defects were includ

Comparison of Mochi Needle and Conventional Hand Suturing for Laparoscopic Port Closure: A Prospective Comparative Study.

Background Secure laparoscopic port-site closure is essential to prevent postoperative complications such as wound infection, wound dehiscence, and trocar-site hernia, particularly at port sites measuring 10 mm or more. Conventional hand suturing remains the standard technique for fascial closure; however, it may be technically demanding and time-consuming, especially in patients with deep port sites or increased abdominal wall thickness. The Mochi needle is a reusable fascial closure instrument designed to facilitate controlled fascial approximation during laparoscopic port-site closure. This

Robot-assisted kidney transplantation in obese recipients: wound-risk reduction, graft safety, and access implications.

Obese kidney transplant recipients are at increased risk of surgical site infection, impaired wound healing, lymphocele, hematoma, and incisional hernia, and these wound-related risks have led some transplant programs to apply restrictive body mass index (BMI) thresholds. Robot-assisted kidney transplantation (RAKT) provides a minimally invasive approach by combining small-incision graft insertion with three-dimensional visualization, wristed instrumentation, and intracorporeal vascular and ureteral reconstruction. In obese recipients, this approach may reduce abdominal wall trauma while prese

Approaches to Ventral Hernia Repair: A Narrative Review.

Ventral hernia repair is a significant and evolving challenge in general surgery, encompassing a wide spectrum of primary and incisional abdominal wall defects of varying complexity. The burden of ventral hernia continues to rise globally due to increasing rates of abdominal surgeries, aging populations, and the growing prevalence of risk factors such as obesity and metabolic disorders. Despite advances in surgical techniques, recurrence and postoperative complications remain important concerns, necessitating continuous refinement of management strategies. This narrative review synthesizes con
