Bone scintigraphy in the assessment of spondylolysis in patients attending a sports injury clinic.

Thirty-three patients attending the Nottingham Sports Injury Clinic in whom a clinical diagnosis of a stress fracture of the pars interarticularis was made were evaluated with both radiography and bone scintigraphy. This was done in an effort to distinguish a recent stress fracture which would produce a positive scintigram from an established 'incidental' spondylolysis giving a normal scintigram. Nine patients demonstrated abnormal radiography with a normal scintigram. A further nine patients had a positive scintigram. Six were associated with a spondylolysis indicating a recent stress fractur

Mechanical behavior of the human lumbar spine. II. Fatigue strength during dynamic compressive loading.

Seventeen cadaveric human lumbar motion segments from eight spines were cyclically loaded in vitro under axial compression. Loading frequency and magnitude were chosen to simulate rigorous activity within an in vivo physiological level. The load magnitude was determined as a percentage of the ultimate compressive load, the latter estimated from the bone mineral content (BMC) of lumbar vertebrae determined by dual-photon absorptiometry. Following testing, the degree of macroscopic disc degeneration was assessed and the type of fracture in each specimen was determined from serial sagittal sectio

Low back pain.

Low back pain is a frequent source of disability during the productive years of life. Repeated trauma is a major causative factor. Maintaining ideal body weight and good muscle tone, especially of the abdominal musculature, can minimize low back pain. Lumbar disc protrusion and extrusion may be treated by medical therapy, by neurosurgical intervention or possibly by chemonucleolysis. Other important causes of back pain are arthritis, facet disease and spinal stenosis. Evaluation of low back pain should include tests to uncover a psychogenic source.

The relationship of disability to compensation status in railroad workers.

The role that the secondary gain of work-related back pain plays in the length of disability is an issue that has been debated for years without absolute resolution. This article compares the disability period of lumbar sprain/strain and operated back cases in employees injured on duty and off duty. The patients injured on duty had a statistically significantly longer period of disability than those injured off duty, even when the cases were matched for gender and type of injury.

Neck and low back injuries in wrestling.

Drs. Wroble and Albright review the neck and low back injuries of the University of Iowa wrestling teams over the last 8 years with respect to diagnosis, mechanism of injury, time lost from wrestling, and recurrence. They describe examination of the low back and treatment of low back injuries. Examination of the neck is reviewed in detail and common neck injuries are discussed. Emergency management of acute neck injuries is outlined. Various aspects of prevention of neck injuries are presented.

Prediction of overexertion injuries using biomechanical and psychophysical models.

This paper summarizes a three year epidemiological study conducted in five large industrial plants in order to evaluate the validity of two alternative modeling approaches to overexertion injury prediction. Detailed biomechanical and psychophysical job evaluations were performed for 55 industrial jobs comprised of 2934 potentially stressful manual materials handling tasks. The medical experiences of 6912 incumbent workers were monitored retrospectively for two years and prospectively for one year to establish a data base for comparison of the different models. The results show that each of the

[Thermographic studies with fluid crystals in peripheral nerve damage].

Common peripheral neurogenic lesion in various locations and of different origins were examined thermographically and the results were compared with those obtained by clinical and electromyographic methods. Lesions in the regions of the N. ulnaris and N. medianus can, except in the case of carpal tunnel syndrome, be identified without difficulty by thermography. The results obtained in the case of paresis of the radialis are non-specific, and thermographic examination of radicular lesions are also sometimes ambiguous. Damage in the plexus-brachialis and root region yield impressive results whi

The authors describe a systematic approach to the radiographic evaluation of spinal trauma, discussing cervical spine anatomy, the mechanism of injury, and classification of injuries, and then discuss several specific injuries of the cervical and thoracolumbar spine and their radiographic evaluation.

Rehabilitation of neck and low back injuries.

Sports-related mechanisms of cervical and lumbar spine injury and their clinical presentations have been discussed. The keys to treatment of these injuries in the athlete include rehabilitation of the injured area, recognition of the mechanism of injury, and correction of any technical faults that may have contributed thereto. Criteria for return to sport are a critical part of the rehabilitation of the injured athlete. Most importantly, the practitioner can anticipate potential areas of injury and can recommend prehabilitation exercises to avert many of these injuries.

Spinal joint blocking.

The paper describes the history, symptoms and clinical findings of a typical spinal segmental strain with joint blocking. Also provided are an overview and discussion of joint blocking theories. A mechanism is hypothesized whereby joint blocking may be viewed as an actively maintained, reversible, biomechanical phenomenon. The theory proposes that unisegmental multifid and rotator spasm physiologically locks the motion segment. The axes of motion shift towards one facet joint and mobility becomes blocked by the inability of the segment to articulate about its new axis. Manipulative cavitation

Biomechanical aspects of lumbar spine injuries in athletes: a review.

One of the areas of the body which is very often injured by athletes is the lower lack, or the lumbar area of the spine. This problem is of some concern to physical educators, athletic therapists, coaches, athletes, and physicians. The type of injury which occurs in the lumbar spine is dependent on the direction, magnitude, and the point of application of the forces to the spine. This part of the body is susceptible to injury due to the large forces which must be supported, which include the body weight and any external weights, as well as the forces due to very high accelerations of the body

Mechanical back pain in the athlete.

Effective management of mechanical back pain in the athlete requires an understanding of the significance of the nature and the location of the pain and which injuries are related to the age or sport of the athlete. Muscle strains and interspinous ligament sprains occur at any age and in all sports. However, the adolescent may have an iliac crest apophysitis rather than a muscle strain, and the gymnast or diver may have interspinous process bursitis or a stress fracture of the pars interarticularis rather than a ligament sprain. Similarly, the swimmer with upper thoracic pain, training in the

Back injuries in the athlete.

In summary, the approach to the athlete with low back pain must include an emphasis on aggressive nonoperative intervention, education, and rehabilitation. A diligent attempt must be made to establish a correct diagnosis, though this may be difficult at first. A firm diagnosis allows individualized treatment that meets the strenuous needs of the athlete. Work-up should be standardized to avoid missing what will appear obvious in retrospect. Unlike most of the general patient population, athletes are unlikely to tolerate a long period of "wait and see" therapy. It is necessary to have qualified

Low back pain in the athlete. From spondylogenic injury during recreation or competition.

Low back pain is a common complaint of athletes, and accurate diagnosis is essential, since many causes of back problems can lead to disability. If the obvious mechanical causes, such as muscle strain, "kissing spines," and leg-length inequality, have been ruled out, the more serious sources of low back pain, such as disk herniation, Scheuermann's disease (swimmer's back), compression fractures of the vertebral body, and stress fractures of the posterior elements, should be considered. Treatment goals for all these disorders include pain relief and prevention of further injury.
