Recurrent hamstring injury is a very common problem in rugby union, but has been largely ignored in the literature. It is concluded that a multifactorial aetiology may be present, and that these symptoms may be part of a continuum of symptoms that may lead to more serious injuries. Effective management needs to focus not just locally, but include proximal issues such as lumbo-pelvic stability and correcting lumbar spine dysfunction. The factors addressed in a prevention programme are likely to contribute to performance gains.

In this study, the incidence of football injuries and complaints as related to different age groups and skill levels was studied over the period of 1 year. All injuries and complaints as well as the amount of time players spent in training and games were recorded. All injured players were examined weekly by physicians, and all injuries were assessed according to the International Classification of Diseases (ICD-10), which describes them in terms of injury type and location, the treatment required, and the duration of subsequent performance limitations. A total of 264 players of different age g

Axial back pain is commonly encountered by the sports medicine physician and has a variety of potential pain generators. Internal disc disruption is an important diagnosis to consider, particularly if there is a history of spinal trauma. The pathogenesis of IDD is not definitively known, although related theories exist. The process may begin with a vertebral endplate fracture followed by an inflammatory degradation of the disc matrix. Annular tears extend from the nucleus pulposus to the periphery and nociceptive nerve endings grow into the inner annulus and become sensitized by the biochemica

Previous research agrees that the majority of injuries that affect male golfers are located in the lower back and that they are related to improper swing mechanics and/or the repetitive nature of the swing. This study describes the trunk motion and paraspinal muscle activity during the swing of a golfer with related low back pain (LBP) and assesses the effect of a 3-month period of muscle conditioning and coaching on these variables. Motion of the trunk was measured using three-dimensional video analysis and electromyograms (EMGs) were recorded from the same six sites of the erector spinae at

A major disaster was encountered at the 1997 Maccabiah Games in Israel. As the Australian team was about to enter the main stadium for the opening ceremony, a pedestrian bridge they were crossing collapsed, killing 4 athletes and injuring many others. The aim of this paper is to establish the rates, types and anatomical locations of musculoskeletal injuries incurred by members of the Australian Maccabiah Games team, with particular reference to the impact of the bridge collapse. In total, the 410 members of the team (360 athletes and coaches and 50 team officials) reported 166 injuries from th

Here, I review research that has investigated the aetiology of injuries experienced by adolescent and adult fast bowlers. Mechanical factors play an important role in the aetiology of degenerative processes and injuries to the lumbar spine. This is particularly so in fast bowling, where a player must absorb vertical and horizontal components of the ground reaction force that are approximately five and two times body weight at front-foot and rear-foot impact, respectively. Attenuated forces are transmitted to the spine through the lower limb, while additional forces at the lumbo-sacral junction

The aim of the study was to evaluate ultrasound technique in preemployment medical assessment of the risk for low-back pain. Volunteers for the study were recruited among agricultural workers employed in the "Agraria Department" of the University of Bologna, Italy. The group consisted of 90 subjects, 52 male and 38 female, aged 25 to 58 years. The subjects filled in a questionnaire on medical history of low-back pain and were examined using an ultrasonograph equipped with a high linear frequency probe (3.5 MHz). The oblique parasagittal diameter of the lumbar spinal canal was measured by trans

STUDY DESIGN: The recording of electromyographic responses from the in vivo lumbar multifidus of the cat, obtained while cyclic loading was applied as in occupational bending/lifting motion over time.
OBJECTIVES: To determine whether the effectiveness of stabilizing reflexive muscular activity diminishes during prolonged cyclic activity; the recovery of lost muscle activity by a 10-minute rest; and whether such diminished muscular activity is caused by fatigue, neurologic habituation, or desensitization of mechanoreceptors in spinal viscoelastic tissues resulting from its laxity.

Because of the inability of strength tests to accurately discriminate between low back pain patients and healthy subjects, a multifactorial evaluation of low back pain patients is warranted. It is postulated that measurements of endurance, kinematics, postural stability, and coordination, in addition to strength, are necessary to fully document the patients' functional capabilities. This research study was conducted in order to understand the effects of fatigue on the above factors. Twelve healthy male subjects performed a repetitive lifting test in which a submaximal load was lifted at a maxi

A description of dance injuries in a Broadway show using ballet technique is reported for the first time. Presentation of this material is used as a vehicle to discuss issues in dance epidemiology and etiology. As interest and research in dance medicine increases, standardization of reporting methods and definitions becomes critical in discussions of epidemiology and etiology. Borrowing from sports medicine classifications, which define sports injury as "time lost from play," we suggest dance injury be defined as "time lost from performing". The overall injury rate was 40.0%, which was low com

Occupational problems of the foot and ankle have received less attention than those of other body regions, namely the hand, neck, and lumbar spine. Functional requirements of the foot and ankle differ from other body regions, necessitating a different clinical approach to injuries than those indicated in the upper extremity or back. This article reviews the clinical characteristics of the foot and ankle pertinent to the worker and highlights the unique features of the management of foot and ankle problems encountered in occupational orthopedic practice.

The extensive claim and medical bill payment databases of the Washington state workers' compensation system have been used to conduct epidemiologic and outcome studies of work-related conditions. Computerized administrative data must be supplemented with medical record review and structured interview of workers in outcome studies in order to adequately adjust for baseline severity and to address functional and patient satisfaction outcomes, respectively. Three examples of surgical outcome studies are described (carpal tunnel, lumbar fusion, thoracic outlet). Duration of disability prior to sur

Low back injuries occur commonly in tennis, but the pathophysiologic, biomechanical, and clinical characteristics are not well defined. Tennis players may be at an increased risk of lumbar disc pathology from rotational and hyperextension shearing effects. Treatment of low back injury at present is empiric, but sport-specific lumbar stabilization and unloading of the lumbar disc should be developed. Prospective longitudinal research protocols are needed to study the lumbar spine in tennis players.

A continuing challenge for ergonomists has been to determine quantitatively the types of trunk motion and how much trunk motion contributes to the risk of occupationally-related low back disorder (LBD). It has been difficult to include this motion information in workplace assessments since the speed at which trunk motion becomes dangerous has not been determined. An in vivo study was performed to assess the contribution of three-dimensional dynamic trunk motions to the risk of LBD during occupational lifting in industry. Over 400 industrial lifting jobs were studied in 48 varied industries. Th

We have reviewed 37 patients under the age of 18 years with lesions of the lumbar posterior end plate. All but one were active in sport, and most were seen because of low back pain. An abnormality was commonly found at the inferior rim of the body of L4 and at the superior rim of the sacrum. All adjacent intervertebral discs showed a decrease of signal intensity on the T2-weighted MRI. In 12 patients there was no interposed tissue at the posterior end-plate lesions. When disc material had migrated posteriorly none protruded beyond the posterior margin of the end plate, the dissociated portion

This study investigated the stress distribution in the L1 vertebra under a vertical axial load. Five normal spinal units (T12-L2) were obtained from embalmed human cadavers, to measure the surface strain in the L1 vertebra. Stress screening using a brittle-coat was prepared beforehand, then fourteen sites on the lateral half of the surface on the L1 vertebra were carefully chosen to install rosette strain gauges. A vertical axial load was applied up to 1471 N in two degrees of flexion, in the neutral position, and in two degrees of extension of the intact spine, and in the neutral facetectomiz
