PURPOSE: This study evaluated electroacupuncture (EA) as adjunctive therapy after peripheral nerve repair in a rat model.
METHODS: Male Wistar rats underwent right sciatic nerve transection followed by neurorrhaphy and were randomized into EA or control groups. EA was performed twice weekly for 12 weeks. Sensory and motor recovery were assessed by nociceptive, mechanical, thermal, and sciatic functional index tests. Histological analyses evaluated Schwann cell and vascular densities, and gastrocnemius muscle weight was measured at euthanasia.
RESULTS: Sensory recovery was significantly great

The gold standard for repair of peripheral nerve injuries is a nerve autograft. Donor nerve harvest creates a second surgical site, invariable numbness and donor site chronic neuropathic pain in 20% of cases. We developed a poly-caprolactone (PCL) multichannel, biomimetic scaffold with surface features promoting cell and vascular attachment and rapid, linear axonal regeneration. The cross-sectional area available for axonal regeneration exceeds that of autografts. When compared to nerve autografts and FDA-approved nerve conduits in 1cm-long rat and 3cm-long rabbit sciatic gap injury, biomimeti

The neutralization of interleukin-16 (IL-16) or of its main target, CD4 receptors, as well as the reduction of its synthesis by inhibiting caspase-3 have been recently considered as possible strategies to inhibit pain. Here we study the involvement of peripheral mechanisms related to the IL-16/CD4 axis in the nociceptive responses evoked after ligation of sciatic nerve in mice. Systemic administration of anti-IL-16 or anti-CD4 antibodies inhibited hyperalgesia and allodynia measured 2 days after ligation in male and female mice. In addition, both antibodies were also analgesic 9 days after ner

ObjectiveThe effect of Gelsemine on neuropathic pain was investigated using a sciatic nerve (SN) chronic constriction injury (CCI) rat model.MethodsNetwork pharmacology analysis was performed to identify potential molecular targets of Gelsemine associated with neuropathic pain, followed by molecular docking analysis of the identified targets. Neuropathic pain was induced in male rats by SN-CCI. The rats were divided into four groups: sham, CCI, Gelsemine 2 mg/kg, and Gelsemine 4 mg/kg. Gelsemine (2 or 4 mg/kg, i.p.) was administered to the treatment groups for 30 consecutive days. The effect o

Diabetic peripheral neuropathy (DPN) is a common and disabling complication of diabetes and a major cause of sensory dysfunction. In established disease, nerve injury may continue despite partial metabolic improvement, suggesting that local nerve microenvironmental mechanisms sustain damage. The mediators linking macrophage activation to Schwann-cell dysfunction and sensory abnormalities remain unclear. We studied matrix metalloproteinase-12 (MMP12) in 20-week-old male C57BLKS/J db/db mice and normoglycemic m/m controls. Sciatic nerves were assessed by histology, Luxol fast blue staining, and

Functional neurological disorder (FND) is a diagnosis grounded in positive clinical signs, yet it remains vulnerable to underdocumentation and incomplete communication across care transitions, particularly when it is suspected but not formally recorded. In this case, a 24-year-old woman with a history of major trauma (three gunshot wounds sustained four years earlier) presented with sudden-onset left lower extremity (LLE) weakness, pain, and tingling coinciding with the anniversary month of her injury. Extensive neuroimaging and serologic workup were unrevealing aside from an incidental disc p

BACKGROUND: Sural nerve injury is an uncommon complication following foot and ankle surgery, including procedures performed for Haglund deformity. Postoperative sural nerve entrapment remains rarely reported and may be challenging to diagnose.
CASE PRESENTATION: A 56-year-old woman presented with persistent lateral ankle pain and paraesthesia 12 months after open lateral heel surgery for Haglund deformity. Clinical examination revealed a trigger point at the distal portion of the surgical scar associated with a positive Tinel's sign. A diagnostic local anaesthetic injection provided temporary

Nerve guidance conduits (NGCs) represent a promising alternative to autologous nerve transplantation; however, their clinical efficacy remains limited by insufficient structural integrity and suboptimal fascicular guidance. Here, we report a novel extrusion-stretched strategy for fabricating multigrooved NGCs (MNGCs) reinforced with oriented multiwalled carbon nanotubes (MWCNTs). This scalable and controllable method effectively prevents conduit collapse during fabrication and enables precise regulation of conduit geometry, overcoming key limitations of conventional forming approaches. BACKGROUND AND OBJECTIVES: Macrophage-associated responses at sites of nerve injury are involved in the initiation and persistence of neuropathic pain (NP). Immune semaphorins (SEMAs), including SEMA3A, SEMA3E, SEMA4A, SEMA4D, and SEMA7A, regulate macrophage migration and activation, but their roles in NP remain unclear. This study aimed to identify immune SEMAs associated with NP by analyzing their serum levels and expression in sensory nerve tissues of patients with NP, and to evaluate the potential effects of SEMA-targeted intervention in a mouse model.
METHODS: Serum SEMA levels were meas

The infrapatellar branch of the saphenous nerve (IPBSN) is implicated in nerve injury from different knee surgeries because of its intimate course relative to the knee joint capsule. Pain physicians encounter patients in their practice for the management of neuralgia of this nerve or in the context of advanced management of knee osteoarthritis. This article aims to provide a comprehensive review of the anatomy, sonoanatomy, and the intervention of the IPBSN in pain management of infrapatellar neuralgia and chronic knee pain.

BACKGROUND: Anterior cruciate ligament reconstruction (ACLR) can cause severe postoperative pain. However, consensus regarding the most effective regional analgesia is lacking. We hypothesized that, compared with femoral triangle block (FTB) and local infiltration analgesia, adding an iPACK block would decrease postoperative morphine consumption.
METHODS: Patients scheduled for ACLR under general anesthesia were randomly allocated to the FTB (n=45) or the FTB+iPACK group (n=45). The primary outcome was the cumulative oral morphine equivalent (OME) consumption during the first two postoperativ

Diabetic peripheral neuropathy (DPN) is a common complication of diabetes characterized by demyelination and axonal degeneration, yet effective therapies remain limited. This study investigates the role of Zinc finger and BTB domain-containing 16 (ZBTB16) in myelin damage during DPN. We identified that ZBTB16 was significantly elevated in sciatic nerves of streptozotocin (STZ)-induced diabetic mice via mRNA sequencing. In vivo, adeno-associated virus (AAV9)-mediated overexpression of ZBTB16 under the myelin protein zero (MPZ) promoter improved motor nerve conduction velocity and sensory nerve

Peripheral nerve myelin is the most cholesterol-rich structure in the body, with the majority of cholesterol being synthesized by Schwann Cells (SCs). Following peripheral nerve injury, myelin disintegration leads to substantial cholesterol release and accumulation, which has been suggested to aggravate neuroinflammation and hinder nerve repair in the central nervous system. However, whether cholesterol synthesis by SCs is detrimental or beneficial for peripheral nerve regeneration remains a critical and unresolved question. Present findings reveal that FDFT1, a key cholesterol biosynthesis en

Neuropathic pain (NP) is a refractory chronic pain disorder with a complex pathogenesis and limited effective treatment options. In recent years, exosomes derived from mesenchymal stem cells (MSC-Exos) have attracted attention as promising therapeutic agents due to their anti-inflammatory, neuroregenerative, and immunomodulatory properties. This study aimed to investigate the therapeutic effects and underlying mechanisms of human umbilical cord mesenchymal stem cell-derived exosomes (hUC-MSC-Exos) in a rat model of neuropathic pain induced by chronic constriction injury (CCI) of the sciatic ne

Neuropathic pain (NPP) remains therapeutically challenging, with oxidative/nitrosative stress and neuroinflammation-amplified by nitric oxide (NO)-as key drivers. This study investigated geraniin (GRN), a naturally occurring hydrolyzable ellagitannin widely distributed in various plant species, including Phyllanthus spp. and Nephelium lappaceum (rambutan), in a rat model of sciatic nerve chronic constriction injury (CCI), focusing on NO-pathway involvement. Male Wistar rats (n = 8/group) received intraperitoneal GRN (3, 10, 30, or 100 mg/kg) or vehicle (1% DMSO in saline) daily for 21 days. Be

OBJECTIVE: Perineural invasion - a hallmark of cancers such as prostate and pancreatic - is strongly associated with severe, treatment-resistant pain. To dissect the neuron-specific mechanisms underlying this pain, we established a rat model of prostate cancer-associated perineural invasion to get insights into neuroinflammatory processes arising from direct tumor-nerve interactions and to provide a platform for evaluating targeted therapeutic strategies.
METHODS: GFP-expressing AT-1 prostate cancer cells were directly microinjected into the perineurium of the sciatic nerve in syngeneic Copen

Peripheral nerve injury (PNI) poses significant challenges due to the complex structure and regenerative microenvironment of peripheral nerves, which limit self-repair capabilities. Artificial nerve conduits have been widely used for nerve repair. Here, a conductive-piezoelectric integrated microstructured conduit is designed, using poly(lactic glycolic acid) (PLGA) and poly(vinylidene fluoride) (PVDF) via electrostatic spinning to obtain an implantable, biodegradable piezoelectric nanofibrous membrane. This membrane is further enhanced with a reduced graphene oxide/methacrylated gelatin (rGO/
