α-ketoglutarate ameliorates spinal motor neuron ferroptosis after sacral plexus transection via enhancing PINK1 O‑GlcNAcylation and mitophagy.

Sacral plexus transection (SPT), mostly caused by high‑energy trauma, induces secondary death of spinal motor neurons in the lumbosacral segments and contributes to poor clinical outcomes after nerve repair. This study aimed to investigate the protective effect of α‑ketoglutarate (AKG) against ferroptosis in spinal motor neurons following SPT and the underlying molecular mechanism. Our results showed that endogenous AKG levels were significantly decreased in the spinal cord of SPT rats and in oxidative stress‑injured motor neurons, accompanied by abnormal expression of core ferroptosis‑related

Delayed repair of singly transected and segmental-loss peripheral nerve injuries in rat sciatic nerves using optimized PEG-fusion protocols and stored viable peripheral nerve allografts.

BACKGROUND: Singly transected and segmental-loss peripheral nerve injuries (ST- and SL-PNIs) produce immediate loss of sensorimotor functions. Polyethylene glycol (PEG)-fusion repairs performed immediately following ST- and SL-PNIs improve long-term outcomes in animal models, with SL-PNIs repaired using fresh or 1-to-3 day stored viable peripheral nerve allografts (VPNAs). Here, we propose several optimizations for the existing PEG-fusion protocols to repair ST- and SL-PNIs with a delay of 24-48 h post-injury, using longer stored VPNAs for SL-PNIs, both conditions critical for translating PEG-

Melatonin attenuates diabetic peripheral neuropathy through modulation of the COX2-IRE1α-mediated endoplasmic reticulum stress axis.

BACKGROUND: Diabetic peripheral neuropathy (DPN) involves Schwann cell injury closely linked to endoplasmic reticulum (ER) stress. Melatonin shows neuroprotective potential, but the specific regulatory mechanisms involving the COX2-IRE1α axis remain to be clarified. OBJECTIVES: To investigate the potential regulatory role of melatonin in attenuating DPN through the modulation of the COX2-IRE1α ER-stress signaling axis. METHODS: In-vitro, RSC96 Schwann cells were exposed to high glucose (HG) and treated with melatonin (MT), COX2 overexpression, or IRE1α knockdown. Cell viability, apoptosis, a

Ultrasound-Guided Percutaneous Cryoneurolysis of Multiple Lower Extremity Nerves for Treatment of Pain From Complex Regional Pain Syndrome: A Case Report.

Complex regional pain syndrome (CRPS) is a multifactorial neuropathic disorder characterized by pain disproportionate to injury. We report a 58-year-old man with CRPS type II of the right lower extremity after traumatic nerve injury, diagnosed via Budapest criteria. After limited response to medical and rehabilitative therapy, ultrasound-guided percutaneous cryoneurolysis was performed on saphenous, deep peroneal, sural, and superficial peroneal nerves. The procedure yielded >80% pain reduction within 1 week and sustained analgesia with improved function at 9 months. This case suggests cryoneu

Targeted muscle reinnervation surgery modulates apoptosis and synaptic plasticity to improve motor function following tibial nerve injury in rats.

BACKGROUND: Targeted muscle reinnervation (TMR) is represents an advanced neural-machine interface that enhances prosthetic control and facilitates motor recovery in amputees. Although TMR is known to connect residual nerve fibers and supply neurotrophic factors, its impact on spinal cord motor neurons remains understudied. This study investigated the effects and possible mechanisms of TMR on spinal motor neurons in a rat model of tibial nerve transection (TNT). MATERIALS AND METHODS: There were 30 Sprague Dawley rats grouped into control, TNT, and TMR groups. TMR was grafted proximal tibial

Insights into the Effects of Carbamylated Erythropoietin on Schwann Cells in Peripheral Nerve Injury.

Recent advancements in biology and medicine have facilitated the progress of nerve regeneration that markedly improves the treatment of peripheral nerve injuries, enhancing outcomes and recovery rates. It has been reported that erythropoietin (EPO) is currently being studied as a potential agent for neural repair. However, much evidence has confirmed that EPO treatment can induce systemic adverse effects in the clinical fields, including coronary stent thrombosis and deep vein thrombosis. Herein, a derivative of EPO without any hematopoietic activities, which is named carbamylated erythropoiet

Conservative Safety Assessment of a Nitinol Microhook Coaptation Device on Intact Nerve.

INTRODUCTION/AIMS: Nerve Tape is a novel nerve coaptation device that eliminates the technical challenges of microsuturing. The device incorporates tissue-engaging Nitinol microhooks within a flexible backing of decellularized porcine small intestine submucosa (SIS). Preclinical nerve injury models have demonstrated device safety and effective promotion of regeneration and recovery. Here, we further assess the impact of the device's components on nerve tissue and function, using a highly conservative model in which the device is applied to intact nerves. METHODS: Nerve Tape, sutures, or SIS w

Source-Dependent Mitochondrial Transplantation Drives Schwann Cell Bioenergetic Reprogramming and Peripheral Nerve Regeneration.

Following peripheral nerve injury (PNI), metabolic reprogramming of Schwann cells (SC) plays a critical role in axonal regeneration and functional recovery. Although mitochondrial transplantation (MT) has been proposed as a potential therapeutic strategy, it remains unclear whether its effect is limited to transient energy supplementation. We systematically compared the differential effects of mitochondria derived from human induced pluripotent stem cells (iPSCs, iP-MT) and induced mesenchymal stem cells (iMSCs, iM-MT) on regulating SC metabolism and promoting peripheral nerve regeneration. Nerve conduits with multiple physical and biochemical guiding factors for peripheral nerve defects.

Peripheral nerve injury is a common complication in patients with trauma, and severe cases can result in sensory and motor function loss, affecting the quality of life. Although the peripheral nervous system has spontaneous regenerative ability, bridging long-distance nerve gaps remains a clinical challenge. Therefore, we innovatively constructed a multi-guided nerve conduit to address this issue. Using gelatin methacryloyl as the raw material, directional micropores were created through casting, demolding, and directional freezing to achieve topographical guidance. A concentration gradient of

Temporary Percutaneous Lateral Femoral Cutaneous Nerve Stimulation for Surgically Refractory Meralgia Paresthetica: A Case Report.

BACKGROUND: Meralgia paresthetica (MP) is a neuropathic pain syndrome resulting from lateral femoral cutaneous nerve irritation or injury. Patients who fail to respond to conservative therapies can find relief after interventions, including nerve blocks and decompression surgery, but some continue to experience pain. Temporary percutaneous peripheral nerve stimulation (PNS) is a minimally invasive option for targeted neuropathic pain management, though there is limited evidence of its efficacy in the management of MP. CASE REPORT: A 63-year-old man with MP unresponsive to conservative and sur

Surgical Approaches for Peripheral Nerve Injury Models in Yucatan Pigs.

INTRODUCTION: Peripheral nerve injury (PNI) is a common component of combat extremity trauma, frequently associated with residual disability. Large animal models are critical for the study of PNI because of their anatomical and physiological similarities to human nerves. This study describes reproducible surgical techniques for creating PNI models in Yucatan pigs using the median and sciatic nerves. MATERIALS AND METHODS: For median and ulnar nerve exposure, pigs were positioned in the right lateral recumbent with the left forelimb retracted off the field. A transverse incision was made at th

A Near-Infrared Fluorogenic Spray Probe for Rapid and Wash-Free Intraoperative Nerve Imaging.

Intraoperative nerve injury remains a major surgical challenge. Rapid, wash-free nerve visualization is highly desirable for fluorescence image-guided surgery (FIGS), yet existing probes lack spray compatibility and sufficient fluorogenicity. Here we introduce N4, a near-infrared fluorogenic spray probe that enables ∼10 s, wash-free visualization of peripheral nerves. N4 is engineered to remain nonfluorescent in its spirocyclic form under physiological conditions but undergoes MBP-triggered spiro-opening to a bright zwitterionic state, yielding a 6.3-fold turn-on. Topical spraying of N4 produc
