Clinicopathological Evolution of Pyrrolizidine Alkaloid-induced Liver Injury: Transition From SOS to PSVD?

BACKGROUND AND AIMS: Hepatic sinusoidal obstruction syndrome (SOS) is characterized by hepatic sinusoidal endothelial cell injury and detachment, hepatic sinusoidal congestion, and hepatic cell necrosis. Currently, limited data exist concerning changes during the recovery period, especially histopathological changes. The purpose of this study was to investigate the evolution of pathology in patients with pyrrolizidine alkaloid (PA)-induced SOS and in a monocrotaline-induced SOS rat model. METHODS: Patients diagnosed with PA-induced SOS who underwent liver biopsy after achieving clinical remis

An Anatomical Variant of the Accessory Right Hepatic Artery Arising From the Superior Mesenteric Artery: A Cadaveric Case Report.

The right hepatic artery (RHA) provides blood flow to the right lobe of the liver. In typical anatomy, the RHA branches from the proper hepatic artery, which branches from the common hepatic artery originating from the celiac trunk of the abdominal aorta. During a standard cadaveric dissection, a large accessory right hepatic artery (aRHA) was noted arising directly from the superior mesenteric artery. The lumen of the identified aRHA measured approximately 1 cm in diameter. The artery also appeared darker in color than the surrounding vasculature, with no evidence of prior surgery or procedur

Association of deep vein thrombosis with poststroke epilepsy after acute ischemic stroke: a multicenter retrospective cohort study.

BACKGROUND: Deep vein thrombosis (DVT) and poststroke epilepsy (PSE) are clinically important complications after acute ischemic stroke. DVT commonly occurs in patients with greater neurological impairment, immobility, inflammation, and hypercoagulability, factors that may also be associated with PSE. Whether DVT is independently associated with PSE remains uncertain. METHODS: We conducted a secondary analysis of a multicenter cohort of 21,459 patients with acute ischemic stroke followed for 1 year. The association between recorded DVT and PSE was examined using sequential logistic regression

Duodenal obstruction from superior mesenteric artery syndrome with nutcracker phenomenon in an adolescent: a case report.

INTRODUCTION AND IMPORTANCE: Superior mesenteric artery (SMA) syndrome is a rare cause of duodenal obstruction. Its diagnosis is often delayed due to nonspecific symptoms that mimic common functional gastrointestinal disorders. This case highlights the diagnostic challenge and the critical role of imaging, further complicated by an incidental, anatomically related finding. PRESENTATION OF CASE: A 13-year-old male presented with a 2-month history of post-prandial vomiting, epigastric pain relieved by a prone position, and unintentional weight loss of 9.3% of total body weight, having previousl

Cardiovascular Complications of Chronic Liver Disease.

Chronic liver disease is an increasingly prevalent and heterogeneous syndrome with important consequences for cardiovascular function. As the global burdens of chronic liver disease and cirrhosis continue to rise, cardiovascular disease has emerged as a major driver of overall morbidity and mortality in affected patients. Despite the growing recognition of the intersectionality between hepatology and cardiology care clinically, cardiovascular disease risk among patients with liver disease is underrecognized. This state-of-the-art review summarizes the current understanding of the bidirectional

Blockade of gastrin-releasing peptide receptor in the PVN alleviates maternal vascular and placental dysfunction in preeclamptic rat.

Preeclampsia (PE) is characterized by placental ischemia-induced systemic inflammation, oxidative stress, and maternal vascular dysfunction. We previously reported that upregulated gastrin-releasing peptide (GRP) and its receptor (GRPR) within the hypothalamic paraventricular nucleus (PVN) of PE rats contribute to the progression of PE. However, whether such central upregulation contributes to the peripheral organs damage in PE remains unknown. This study investigated the protective effects of the GRPR antagonist ICI-216140, administered via PVN microinjection, on mesenteric arteries and place

Native Spleen T1 Mapping for the Assessment of Hepatic Congestion in Children With Congenital Heart Disease.

BACKGROUND: Congestive hepatopathy in congenital heart disease (CHD) can lead to portal hypertension. Liver native T1 relaxation time (LT1) was reported to be associated with elevated central venous pressure and liver injury in CHD. This study investigated the utility of spleen native T1 relaxation time (ST1) in children with CHD, particularly those with Fontan circulation, and its association with hemodynamic parameters and biomarkers of liver injury. METHODS AND RESULTS: This study included 120 patients with CHD (biventricular repair, 39; bidirectional Glenn shunt, 31; and Fontan circulatio

Hemostatic dysfunction and thrombotic risk in metabolic dysfunction-associated steatotic liver disease.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is increasingly recognized as a systemic disorder with clinically relevant vascular and thrombotic consequences. Beyond hepatic steatosis, inflammation, and fibrosis, MASLD is associated with disturbances in hemostatic regulation that may shift the overall balance toward thrombosis. The prothrombotic phenotype of MASLD arises from the interaction of metabolic dysfunction, chronic inflammation, endothelial injury, platelet hyperreactivity, increased procoagulant activity, reduced natural anticoagulant capacity, and impaired fibrin

Deceased donor interposition graft repair of superior mesenteric artery injury during cytoreductive nephrectomy for metastatic renal cell carcinoma.

Iatrogenic injury to the superior mesenteric artery (SMA) represents a rare but serious complication of nephrectomy, associated with significant morbidity and mortality. Repair can be technically challenging. We describe the case of a 71-year-old patient who had an SMA injury during an open nephrectomy for a large carcinoma. Her SMA was reconstructed successfully using a deceased-donor iliac artery interposition graft. Due to her high-risk pathology and progressive disease early in the post-operative period, she was commenced on lenvatinib and pembrolizumab. Despite the vascular allograft, imm

Performance and incremental value of preoperative multimodal imaging and perioperative clinical characteristics in predicting postoperative major adverse events in type A aortic dissection.

BACKGROUND: Postoperative major adverse events (MAEs) can occur after surgical repair of acute Stanford type A aortic dissection (ATAAD), and conventional clinical assessment may not fully capture the prognostic information provided by preoperative multimodal imaging. This study aimed to develop and externally validate a staged risk prediction model for postoperative MAEs and to evaluate the incremental value of preoperative multimodal imaging relative to conventional clinical and laboratory assessment in the prediction of MAEs. METHODS: In this retrospective, two-center cohort study, patient

Median Arcuate Ligament Syndrome in an Elderly Patient: A Symptomatic Post-stenotic Aneurysm and Systemic Atherothrombotic Disease.

Median arcuate ligament syndrome (MALS) is atypical in elderly patients, in whom it frequently coexists with severe atherosclerotic disease. We report the case of a 77-year-old male with marked mitral regurgitation who presented with intestinal claudication and malnutrition. CT angiography demonstrated MALS associated with a fusiform aneurysm of the celiac trunk with dissection, along with 60% atherosclerotic stenosis of the superior mesenteric artery. Given the limiting ischemic presentation, the patient underwent laparoscopic release of the ligament. The procedure was complicated by avulsion

Duodenal atresia associated with apple-peel configuration of distal small bowel in a late preterm neonate: a rare case report and literature review.

INTRODUCTION: The association of duodenal atresia with apple-peel (Christmas tree) configuration of the distal small bowel is exceptionally rare, with only a few cases reported in the literature. This unusual combination is of particular interest because it may provide evidence supporting a vascular etiology for selected cases of duodenal atresia. We report a neonate with type III duodenal atresia associated with apple-peel bowel and absent proximal superior mesenteric artery anatomy. CASE PRESENTATION: A late-preterm (35 + 5 weeks) female neonate with antenatal suspicion of jejunal atresia p

Large esophageal varices predict hepatocellular carcinoma in newly diagnosed cirrhosis: insights from machine learning and propensity score analysis.

INTRODUCTION AND OBJECTIVES: Hepatocellular carcinoma (HCC) is a major complication of chronic liver disease (CLD) requiring improved risk stratification. Esophageal varices (EVs), as manifestations of severe portal hypertension, may promote HCC development through vascular remodeling and hypoxia-related pathways. We aimed to evaluate whether variceal size independently predicts HCC risk and whether specific biomarker profiles can be identified. PATIENTS AND METHODS: In this 12-year prospective cohort study, 545 patients with newly diagnosed compensated advanced chronic liver disease (cACLD)

Influence of exogenous glucagon-like peptide 2 on mechanisms of enteric neurotransmitter-mediated vasorelaxation in the isolated bovine mesenteric artery and vein.

Exogenous glucagon-like peptide 2 (GLP-2) administration increases small intestinal growth and mesenteric blood flow, although the vascular mechanisms involved remain unclear. The objective of this study was to characterize the effects of exogenous GLP-2 administration on glucagon-like peptide 2 (GLP-2)-, vasoactive intestinal peptide (VIP)-, serotonin (5-HT)-, and acetylcholine (ACh)-mediated vascular relaxation in isolated bovine mesenteric arteries (MA) and mesenteric veins (MV). Ten steers received repeated subcutaneous injections of bovine serum albumin or GLP-2 for 48 h before vessel col

Resection of a Large Intra-Abdominal Desmoid Tumor with Preoperative Superior Mesenteric Artery Balloon Catheter Placement: A Case Report.

INTRODUCTION: Large tumors occupying the abdominal cavity can hinder surgical exposure, particularly of the posterior aspect, increasing the risk of arterial bleeding. Because arterial hemostasis is difficult, hemodynamic instability may occur. This report describes the safe use of preoperative balloon catheter placement in the superior mesenteric artery (SMA), the tumor's primary feeding vessel, to control arterial bleeding through on-demand intraoperative occlusion. CASE PRESENTATION: A 40-year-old woman with a history of 2 cesarean sections presented with abdominal distension. An intra-abd

Ultra-short bowel syndrome after near-total small bowel resection for gangrenous gut due to superior mesenteric artery thrombosis: a case report.

Ultra-short bowel syndrome (USBS) is a rare and life-threatening condition defined by the retention of ˂30 cm of small intestine following resection. We report the case of a 53-year-old male who presented with gangrenous gut secondary to superior mesenteric artery thrombosis, necessitating near-total small bowel resection. The patient was left with ~15 cm of proximal jejunum and 10 cm of distal ileum, with a jejuno-ileal end-to-end anastomosis. He developed severe postoperative watery diarrhoea and required Total Parenteral Nutrition (TPN) via a peripherally inserted central catheter line. Wit

Case Report: Ischemic bowel necrosis following intra-aortic balloon pump support in a patient with cardiogenic shock.

BACKGROUND: The intra-aortic balloon pump (IABP) is a widely used mechanical circulatory support device for patients with acute myocardial infarction complicated by cardiogenic shock. Although generally safe, IABP use may impair visceral organ perfusion and lead to rare but severe complications, including ischemic bowel necrosis. CASE PRESENTATION: A 59-year-old man was admitted after experiencing a cardiac arrest caused by an extensive acute anterior myocardial infarction. Following successful cardiopulmonary resuscitation, emergency coronary angiography revealed total occlusion of the mid-s

Megamonas hypermegale with indole-3-lactic acid-producing capacity ameliorates hepatic fibrosis.

Hepatic fibrosis lacks effective direct antifibrotic therapy. Human V3-V4 16S rRNA profiling identified a sequence feature provisionally assigned to M. hypermegale that was depleted in hepatic fibrosis; subsequent functional experiments used a cultured M. hypermegale strain. Administered from the onset of fibrosis induction, oral M. hypermegale attenuated CCl₄- and DDC-induced fibrosis in preventive/early-intervention models, reduced liver injury and collagen accumulation, and partially preserved intestinal epithelial integrity. M. hypermegale-conditioned medium suppressed transforming growth
