Transcatheter removal of a briefly penetrated inferior vena cava filter in an open abdomen: a case report.

BACKGROUND: The likelihood of inferior vena cava filter penetration increases with prolonged implantation. Despite the generally low risk associated with an ALN inferior vena cava filter (ALN IMPLANTS CHIRURGICAUX, Ghisonaccia, France), we present a case in which penetration occurred 43 days after implantation. At present, no consensus is available on the standardized approach for filter removal in such cases. In this report, we describe a secure and reliable method involving surgical access to the abdomen and transcatheter filter removal while directly observing the inferior vena cava. CASE

Physician-modified endograft for repairing iatrogenic innominate artery injury: A case report and literature review.

BackgroundInnominate artery (IA) injury resulting from right subclavian vein (RSV) puncture is a relatively rare and life-threatening iatrogenic vascular injury.MethodsIn a 72-year-old female patient, in whom iatrogenic IA injury occurred during the RSV puncture, a 6-Fr sheath had entered into ascending aorta through IA. Due to the concern about the difficulty in hemostasis after removing the sheath, the patient was immediately transferred to our hospital. In emergency situations, when there were no covered stents of appropriate sizes available, we ultimately opted to physician-modified endogr

Intraoperative Evidence of Early Neovascularization in a SCIP Flap Following Postoperative Trauma: A Case Report.

Microvascular free-flap transfer is a widely used technique for reconstructing complex tissue defects. While early postoperative flap survival depends on intact vascular anastomoses, neovascularization may contribute to long-term viability. However, direct intraoperative human evidence of early neovascularization in free flaps remains limited. This case report aims to provide direct intraoperative evidence of early neovascularization in a free SCIP flap. We present a case of an 88-year-old female who underwent wide resection of a soft tissue sarcoma and reconstruction with a free superficial c

Stent-Graft Malposition Into a False Lumen Causing Occlusion Following Blunt External Iliac Artery Injury: Case Report of a Novel Technique of Endovascular Therapy for Recanalization.

IntroductionBlunt trauma to the external iliac artery (EIA) is rare but potentially fatal. Endovascular stent-graft placement is used to control hemorrhage and restore limb perfusion. However, the safety profile and potential complications associated with stent-graft treatment are not well documented. We report a case of EIA injury following blunt trauma complicated by stent-graft deployment into a false lumen, successfully managed with an endovascular rescue technique.Case ReportAn 88-year-old man sustained blunt pelvic trauma with active extravasation from the left EIA. Initially, a covered

Iliac blood vessel injuries in traumatic pelvic fractures: a National Trauma Data Bank analysis.

BACKGROUND: Pelvic fractures and iliac vessel injuries share an anatomical relationship because of the proximity of the vessels to the bony pelvis. This study aimed to evaluate the impact of concomitant pelvic fractures on clinical outcomes in patients with iliac vessel injuries. METHODS: We performed a retrospective cohort study using secondary analysis of the 2017 National Trauma Data Bank Research Dataset. Adult patients (≥ 18 years old) with iliac vessel injuries were identified using ICD codes and stratified by the presence of pelvic fractures. Demographics, injury patterns, complication

Fatal, self-inflicted injury caused by a pepper spray gun.

Pepper spray guns have gained popularity as self-defense weapons in recent years. Marketed as "less-lethal," these devices are designed to be easy to use, accurate, and capable of deterring assailants without causing permanent harm. We present a case of a 53-year-old man who sustained a fatal injury caused by a JPX6 Jet Protector pepper spray gun. The device was found in close proximity to the body at the scene. External examination revealed a small entry wound and a triangular, burned stretch zone in the right inguinal area. The cause of death was exsanguination due to laceration of the right

Vein 2 Worlds Collide: May-Thurner Syndrome and Oral Contraceptive Pill Use Precipitating Deep Vein Thrombosis in an Otherwise Healthy Patient.

May-Thurner syndrome (MTS) is a developmental anatomical variant in which the right common iliac artery compresses the left common iliac vein against the underlying lumbar vertebral body. This compression leads to endothelial injury, venous stasis, and progressive luminal narrowing, significantly increasing the risk of left-sided iliofemoral deep vein thrombosis (DVT). Combined oral contraceptives containing ethinyl estradiol are known to increase the risk of DVT, particularly during the initial months of use or during dose changes, with thrombotic risk persisting for up to 10 to 12 weeks afte

Prolonged partial aortic occlusion worsens neurologic outcomes without affecting brain lesion size in a swine model of traumatic brain injury and hemorrhage.

BACKGROUND: Traumatic brain injury and hemorrhagic shock are the leading causes of death in trauma. The partially occluding resuscitative endovascular balloon occlusion of the aorta (p-REBOA) device has emerged as a tool for hemorrhage control with reduced ischemic consequences, allowing for prolonged use. Prior studies in swine show no increase in brain lesion size with prolonged p-REBOA use, but long-term neurologic outcomes remain unknown. We hypothesized that prolonged p-REBOA deployment would worsen neurological outcomes. METHODS: Female Yorkshire swine (n = 5/group; 37-42 kg) were subje

CT angiography of the lower abdomen should be considered with vaginal trauma and excessive bleeding, even though there are no pelvic fractures present.

A healthy woman in her 20s presented to the emergency department (ED) after she endured a high-energy trauma during a dirt bike race. Physical examination showed no signs of thoracic or abdominal injury. Her pubic bone was painful at palpation, with a large amount of vaginal bleeding and a rupture of the vaginal back wall. Additional imaging with X-rays and extended focused assessment with sonography in trauma showed no fractures or free fluid in the abdomen, and the patient was then taken to the operating room for closure of the rupture. This patient became haemodynamically unstable during th

Dual Endovascular Approach for Post-traumatic Rupture of Left Iliac Vein in Emergent Setting.

Traumatic venous injuries are associated with high mortality rates. When they involve the inferior vena cava and iliac veins, prompt treatment is necessary in cases of patient instability. Endovascular treatment of a traumatic injury at the iliac confluence extending to the proximal external iliac vein was performed in a 50-year-old patient following a fall. A GORE Excluder PLC141400 endograft was placed at the bleeding site via percutaneous transfemoral access, combined with simultaneous embolization of the internal iliac vein through right percutaneous jugular access. Effective haemostasis w

Iliopubic rami morphology and its vascular relationships in percutaneous retrograde fixation.

PURPOSE: Pelvic ring fractures involving the iliopubic rami can cause functional impairment. Percutaneous retrograde fixation is a less invasive procedure when compared to traditional open approaches, however precise anatomical knowledge is crucial for safe screw placement. This study aims to describe the morphology of the iliopubic rami, define a safety corridor for percutaneous screw fixation, specially focusing on the relationships between the iliopubic rami and neurovascular structures. METHODS: A retrospective cross-sectional study was conducted on 29 patients using high-resolution compu
