The use of somatosensory evoked potentials for detection of neuropraxia during shoulder arthroscopy.

With the increase in the use of shoulder arthroscopy in the past decade, there has been an increased awareness of complications. Reports of the occurrence of transient neuropraxia indicate an incidence of 10%-30%. The recording of somatosensory evoked potentials (SEP) for the study and functional monitoring of the sensory pathway is well accepted as a reproducible method of monitoring peripheral nerve and spinal cord function during surgery. SEPs were recorded during shoulder arthroscopy in 20 patients to monitor the musculocutaneous nerve, ulnar nerve, and either the median or radial nerve. I

Neurotization via the spinal accessory nerve in complete paralysis due to multiple avulsion injuries of the brachial plexus.

The authors report their experience with 21 cases of neurotization via the spinal accessory nerve for multiple nerve root avulsion injuries of the brachial plexus associated with total paralysis of the upper limb. They performed microneuroanastomoses with interposed cable nerve grafts between the spinal accessory nerve taken in the supraclavicular fossa and the musculocutaneous nerve at its entrance into the biceps muscle. Surgical indications depend on the accurate diagnosis of spinal nerve root avulsion, especially C5. The anatomicosurgical basis of this technique is as precise as are the in

Nerve repairs for traumatic brachial plexus palsy with root avulsion.

Thirty-six patients with traumatic brachial plexus lesions and root avulsions were treated surgically between 1972 and 1986 and were followed for more than 24 months (average, 42.6 months). Neurotization of the musculocutaneous nerve with intercostal nerves or the spinal accessory nerve resulted in satisfactory elbow flexion in 21 of the 33 cases (64%). Combined nerve repairs (i.e., intercostal and spinal accessory neurotization of the terminal branch of the brachial plexus in combination with nerve grafts from the upper spinal nerves of the brachial plexus) created a useful function in at lea

Management of the spastic upper extremity in the neurologically impaired adult.

Spasticity that interferes with upper extremity function is common in adults following stroke, brain injury, or anoxia. During the period of neurologic recovery definitive surgical procedures are avoided. Techniques to temporarily reduce spasticity include the implantation of a MicroPort reservoir and catheter for repeated branchial plexus blocks and phenol nerve blocks, which provide longer lasting relief of noxious muscle tone. Percutaneous blocks of the musculocutaneous and recurrent median nerves and motor point blocks of the pectoralis major, the brachioradialis, and forearm flexor muscle

Traumatic brachial plexus palsy in the adult. Retro- and infraclavicular lesions.

The author reviewed 420 adults treated with surgery for traumatic palsy of the brachial plexus. The results of 105 patients (25%) who suffered plexus injury behind or beyond the clavicula are presented. In 69 patients, lesions affected secondary trunks and in the other 36 affected terminal branches. Simultaneous fractures or vascular injuries were not uncommon and often influenced overall prognosis. Decision criteria for nerve grafting or neurolysis are presented. Surgery is generally performed three weeks to six months after injury. Most patients present extensive damage due to traction lesio

Musculocutaneous nerve injury after coracoid process transfer for clavicle instability. Report of three cases.

During the last 10 years, we have treated 54 cases of fracture or dislocation of the lateral clavicle by transposing the coracoid process onto the clavicle. Recently, a postoperative musculocutaneous nerve injury was observed in 3 cases, all of which recovered spontaneously within 5 months. Our technique seems to entail a certain risk of causing stretch injury to the musculocutaneous nerve. Apart from resting the affected arm for a month or two, the condition does not require specific treatment.

Satisfactory elbow flexion in complete (preganglionic) brachial plexus injuries: produced by suture of third and fourth intercostal nerves to musculocutaneous nerve.

The third and fourth intercostal nerves were sutured to the musculocutaneous nerve to restore flexion of the elbow joint in complete (preganglionic) brachial plexus injuries. Seventeen patients were followed on the average for 5 years and 7 months after surgery. The results were evaluated by means of manual muscle tests and electrical diagnostic tests. Good (grade IV), or better, flexion of the elbow joint occurred in 12 patients over 3 years after the operation.

[Injuries of the brachial plexus and neighboring peripheral nerves in vertebral fractures and other trauma of the cervical spine].

On the one hand, out of 115 patients admitted to hospital with 162 various fractures of the cervical spine without injury to the spinal cord, only 3 (2.6%) had an associated lesion to the brachial plexus or nerves in the vicinity. On the other hand, among 500 consecutive patients with injuries to the brachial plexus, 55 (11%) presented fractures of the cervical spine (including T1 and the 1st rib), whiplash injuries, severe distortions and dislocations, and contusions of that vertebral segment. Five (1% resp. 9%) had spinal cord injuries, including four patients with partial Brown-Sequard's sy

Injury to the brachial plexus during Putti-Platt and Bristow procedures. A report of eight cases.

Eight patients with documented recurrent anterior dislocation of the shoulder sustained iatrogenic brachial plexus injuries during either Putti-Platt or Bristow procedures. Two patients also sustained axillary artery injuries. There were six males and two females. Postoperatively, complete paralysis of the musculocutaneous nerve was noted in six cases and incomplete paralysis in one case. Two patients had complete axillary nerve palsies. There were two cases of partial paralysis of the radial, median, and ulnar nerves, respectively. Seven of the patients underwent brachial plexus exploration a

Short latency somatosensory evoked potentials in patients with painful dysaesthesias in peripheral nerve lesions.

Abnormally delayed and attenuated short latency somatosensory evoked potentials after electrical stimulation in the distribution of painful dysaesthesias after peripheral nerve lesions are reported in 4 selected patients with lesions in very proximal parts of the peripheral nerves. These included painful spondylopathic cervical radiculopathy, meralgia paraesthetica, pyriformis syndrome and allodynia after injury of the femoral nerve. Locally developed techniques for testing musculocutaneous, lateral femoral cutaneous and saphenous nerves were used in 3 patients. Possible pathophysiological mec

Sensory reinnervation in reconstruction of the foot.

Intact sensation constitutes an important protective mechanism for the plantar surface of the foot. For optimal functional results, sensory reinnervation in reconstruction of the heel of the plantar foot would appear to be helpful. Various techniques are presented in which both intact local tissue and distant tissue are used. The design of neurosensory fasciocutaneous flaps and musculocutaneous flaps from the foot, sensory-innervated full thickness skin grafts, and microsurgically transferred skin flaps, muscle flaps, and musculocutaneous flaps are presented. Free tissue transfer techniques ca

Forearm neuropathy and pruritus.

Five adult patients (four of them men) had episodic brachioradial pruritus associated with forearm paresthesia and hypalgesia. No cervical, shoulder, or forearm trauma was known. Onset was variable, but two had had the condition for more than ten years. In each, sensory alteration was detectable by pinprick and temperature in the distribution of the posterior cutaneous nerve of the forearm supplying the skin over the proximal portion of the brachioradial muscle. This seems to be a benign neuropathy.

Compressive neuropathy in childhood: a report of 14 cases.

During a 6-year period, 14 of 898 patients examined at a pediatric electromyographic laboratory had a compressive mononeuropathy. These included five injuries to the peroneal nerve, four to the sciatic nerve, two to the distal radial nerve, and one each to the deep palmar branch of the ulnar nerve, the musculocutaneous nerve, and the long thoracic nerve. A potentially preventable mechanism was identified in 9 of the 14 patients, including prolonged posture in three of the four sciatic and two of the peroneal neuropathies. Four other mononeuropathies were secondary to pressure from an orthopedi

[Free flap-plasty in a larger soft tissue defect of the hip and gluteal region].

The purpose of this paper is to show a case in which we felt at the end, that the chosen procedure did not fulfill our expectations and in which an alternative procedure would have been preferable. In a 47 year old woman a deep, healed soft tissue defect between the left trochanter and the iliac crest was built up with a free latissimus dorsi musculocutaneous flap. The original plan was to suture the vessels to the gluteal vessels and the nerve to the gluteal nerve. The muscle would have been placed in such a way that the muscle fibres would run from the iliac crest to the trochanter to replac

Repair of avulsion of insertion of biceps brachii tendon.

An end-result study was made of 16 cases of avulsion of the insertion of the biceps brachii tendon. The follow-up period spanned one to 25 years post-treatment (average, eight years). Fourteen of the 16 cases included in the study were acute cases with primary repair and reinsertion of the biceps tendon into the area of the radial tuberosity. One case, with the loss of function of the biceps muscle secondary to temporary paralysis of the musculocutaneous nerve, was explored. The tendon was found intact and did not require repair. The sixteenth case was included for comparison of a patient who

Microsurgery in brachial plexus lesions.

Twenty-two patients with brachial plexus lesions were reviewed 3-6 years after microsurgical repair. Four patients underwent nerve transfer between intercostal nerves and the musculocutaneous nerve. None of these achieved useful elbow flexion. Nerve grafting mainly in the upper part of the plexus was performed in six patients of whom five recovered a useful motor function in at least one important area. Twelve patients underwent neurolysis; useful motor function in at least one important area was achieved in eight. We conclude that microsurgical treatment of brachial plexus lesions is useful i

Patterns of injury to the terminal branches of the brachial plexus. The place for early exploration.

Non-operative management has frequently been adopted for closed injuries of the infraclavicular brachial plexus and its branches in the belief that spontaneous recovery is likely to occur, and surgical exploration is performed only if recovery has not occurred in the expected time. This paper correlates the clinical and electrophysiological features with the operative findings in six patients with such injuries. The axillary nerve was ruptured in all six patients, the musculocutaneous nerve in two and the radial nerve in two. When the muscles supplied by a branch of the plexus were denervated,
