Anatomical variations and surgical implications of axillary artery branches: an anatomical study of the coracoid process region.

The complex of neurovascular structures surrounding the coracoid process, particularly the axillary artery, the thoracoacromial artery and theirs branches, plays a critical role in shoulder function. Detailed dissection was performed in 36 shoulders from 18 embalmed cadavers. The focus was on axillary artery branches in relation to the coracoid process and the documentation of anatomical variations in this area. Significant findings include the categorization of thoracoacromial artery variations and the identification of acromial and clavicular branches as variable. A key measurement was mean

Rare occurrence of upper-limb ischaemia necessitating transhumeral amputation following axillary arterial line placement.

Arterial cannulation, commonly performed in the radial artery, is a widely used method for continuous blood pressure monitoring. Occasionally, the axillary artery is used as an alternate site of cannulation. However, complications like occlusion can lead to adverse events and severe outcomes. We present a case of axillary arterial line placement resulting in ischaemic limb injury and amputation, highlighting the need to recognise risk factors.A woman in her mid-60s underwent a lobectomy for adenocarcinoma of the lung and suffered postoperative complications resulting from arterial line placeme

Direct Axillary Artery Cannulation as Standard Perfusion Strategy in Minimally Invasive Coronary Artery Bypass Grafting.

OBJECTIVE: Cardiopulmonary bypass (CPB) via the right axillary artery (RAA) has become an alternative perfusion strategy, especially in complex aortic procedures. This study delineates our technique and outcome with direct axillary cannulation utilizing the Seldinger technique, which we adopted as the standard perfusion strategy in the sternum-sparing minimally invasive total coronary revascularization via left anterior thoracotomy (TCRAT) using CPB. METHODS: From November 2019 to December 2023, a total of 413 consecutive patients underwent nonemergent isolated coronary artery bypass grafting

Sequential injury: Delayed-onset axillary artery thrombosis following brachial plexus paralysis from a traumatic shoulder injury.

The association of brachial plexus nerve injury with arterial involvement in the context of shoulder trauma is well documented in the literature. However, the clinical presentation can vary significantly from case to case. In severe trauma cases, there may be a simultaneous occurrence of both types of injuries. An arterial injury can lead to the formation of a hematoma, causing compression of the nerves that innervate the upper limb. Additionally, avulsion or stretching of the roots of the brachial plexus may also generate a peri‑arterial hematoma, which can result in arterial thrombosis. Thes

TDAP flap in reconstruction of a severe postburn contracture of the axillary region-a case report.

Herein we report a case of a male patient who was burned with hot oil and as a consequence developed a severe axillary contracture with limited abduction and elevation of his left arm. Due to the restriction in the range of movements, our patient reported limitations in everyday activities with decreased quality of life. For this patient we proposed a reconstructive plan that required a combination of multiple techniques of scar release and reconstruction of the defect with thoracodorsal artery perforator (TDAP)-based flap. One year postoperatively, we achieved a significant improvement in ran

Iatrogenic axillary aneurysm related to coronary angiography.

A man in his late 60s presented to the emergency assessment unit with chest pain. An incidental finding of a right axillary mass was noted, initially thought to represent lymphadenopathy, and investigations for underlying malignancy were initiated. Contrast CT imaging ultimately demonstrated a large axillary artery aneurysm. Further interrogation of the patient's medical history revealed previous coronary angiograms via right radial artery access. The patient was referred to vascular surgery and underwent open repair. This case highlights the need to include axillary artery aneurysm as a diffe

[Resection of false posttraumatic aneurysm of the axillary artery within a separate medical airmobile detachment in special military operation zone].

The authors describe surgical treatment of soldier with false posttraumatic aneurysm of the left axillary artery in special military operation zone. As a result of mine-explosive wound, the left axillary artery was injured by metal fragment. This led to aneurysm with neuropathy and venous insufficiency of the left upper limb. Axillary artery replacement with saphenous vein was performed, and foreign body was removed. Postoperative period was uneventful. In 15 days after surgery, swelling of the left upper limb regressed. Hand sensitivity and motions recovered.

Iatrogenic Axillary Artery Branch Injury Following Peripherally Inserted Central Catheter (PICC) Line Insertion Managed With Coil Embolization: A Case Report.

Iatrogenic vascular injuries during peripherally inserted central catheter (PICC) placements are uncommon but can lead to significant complications, such as pseudoaneurysms and hematomas. We present the case of a 71-year-old woman with multiple comorbidities who developed a right subpectoral hematoma with active arterial bleeding following PICC line insertion. This injury was successfully managed using minimally invasive coil embolization by the interventional radiology team. This case underscores the importance of early recognition and management of such injuries, highlighting the efficacy of

Axillary Artery Approach for Ductal Stenting in Neonates and Infants.

Ductal stenting in patients with unusual arterial duct anatomy presents a challenge. The axillary artery (AA) approach provides a safe and effective alternative for access, particularly when conventional femoral routes fail. A retrospective analysis was conducted on patients with ductal-dependent pulmonary blood flow who underwent ductus arteriosus (DA) stenting via the AA route between November 2018 and December 2022 at two tertiary cardiac centers. Demographic and procedural data were reviewed. The study included 39 patients, mostly neonates and infants with complex DA anatomy. The median ag

Clinical management of the Impella 5.5 pump.

The Impella 5.5 (Abiomed, USA) is a catheter-based micro-axial flow pump that has emerged as a vital tool in managing patients with cardiogenic shock (CS). Delivering up to 5.5 L/min of flow, it enables full left ventricular (LV) support with beneficial hemodynamic and metabolic effects. Its unique advantages include high-flow, antegrade circulatory support with the potential for prolonged usage, making it suitable for bridging to recovery or heart replacement therapies. This manuscript provides a comprehensive, structured guide for the clinical management of patients supported with the Impell

Hidden Bone Shard Causes Life-Threatening Axillary Artery Transection During Elective Total Shoulder Arthroplasty: A Case Report.

We describe an unpredictable clinical scenario involving an intraoperative vascular injury during an elective total shoulder arthroplasty. Shortly into the procedure, the patient became acutely hypotensive due to frank hemorrhage. The patient experienced a complete transection of the third segment of the axillary artery caused by an unrecognized bone fragment, suspected to have dislodged into the joint space following a traumatic ground-level fall 12 months prior. Emergency vascular surgical intervention was required for hemostasis.

Intertwining Coil and Catheter During Brain Arterial-Venous Malformation Embolization: A Case Report of a Rare Complication.

Cerebral arteriovenous malformations (AVMs) are rare vascular anomalies associated with a risk of devastating intracerebral hemorrhage. They are often diagnosed following the appearance of seizures, focal neurological signs, or bleeding. In such cases endovascular embolization, which aims to occlude the AVM nidus and reduce risk of hemorrhage, has become a crucial therapeutic approach. Herein, we describe the case of a 62-year-old patient with a history of bleeding caused by cerebral AVM. During urgent endovascular embolization of the patient's AVM, a combination of large-caliber platinum coil

Anthropometric Cadaveric Analysis of the Quadrangular Space and Deltoid Region: Anatomical Perspectives for Clinical Applications in the Indian Population.

BACKGROUND: Neurovascular structures beneath the deltoid, particularly the axillary nerve and posterior circumflex humeral artery, are vulnerable to injury from intramuscular injections or compression in the quadrangular space, especially in overhead athletes. Studies by Kakati et al (2013) and Desai et al (2019) report intramuscular injection-related nerve injury rates of 1.5% to 15% and 82.5%, respectively. However, there is a lack of cadaveric studies in the Indian population, prompting this anthropometric study. AIM: Aims and objectives: This study aims to assess the dimensions of the qua

Individualized Management of Traumatic Limb Pseudoaneurysms: A Case-Based Comparison of Endovascular and Open Repair.

Traumatic limb pseudoaneurysms are rare vascular complications that can cause acute limb ischemia and require urgent treatment. Traditionally addressed through open surgical repair, advances in endovascular techniques have introduced less invasive options, particularly beneficial for high-risk patients. This article presents two cases illustrating individualized decision-making in choosing between endovascular and open surgical repair. A comparative case-based approach was employed. The first case involved a 94-year-old male with multiple comorbidities and a femoral pseudoaneurysm following bl

Rare combined variations of the axillary artery: superficial ulnar artery and circumflex humeral arteries.

The variations in the branching pattern of the axillary artery are clinically significant. We report an extremely rare combination of arterial variations discovered during the routine anatomical dissection of the left upper limb in a 73-year-old male cadaver. The most significant findings were a superficial ulnar artery originating directly from the axillary artery, proximal to the ansa pectoralis. The superficial ulnar artery coursed superficially in the forearm and terminated as the primary contribution to the superficial palmar arch. Additionally, a common trunk for the circumflex scapular

Cannulation Strategies for Aortic Arch Surgery.

Aortic arch surgery remains associated with significant mortality and morbidity especially in the setting of acute type A aortic dissection. Adequate cerebral protection is essential, and several methods have been proposed to avoid neurological injury during aortic arch surgery. The most common techniques include selective antegrade perfusion of brachiocephalic arteries or an interval of deep hypothermic circulatory arrest. A range of cannulation strategies have been employed safely to provide adequate cerebral protection. Optimal cannulation selection is based on the consideration of air or p

Network Meta-Analysis of Axillary, Femoral, and Central Arterial Cannulation in Minimally Invasive Cardiac Surgery.

The optimal arterial cannulation site in minimally invasive cardiac surgery (MICS) remains debated. While axillary, femoral, and central approaches each offer distinct advantages, no prior network meta-analysis has compared all 3. We conducted a Bayesian network meta-analysis of 11 retrospective studies including 11,353 patients to evaluate their impact on postoperative outcomes. The analysis examined mortality, stroke, acute kidney injury (AKI), aortic dissection, atrial fibrillation, reoperation for bleeding, and length of stay, reporting odds ratios (ORs) with 95% credible intervals (CrIs)
