Remnant preservation improves early prognosis in patients with rotator cuff tears: a retrospective study.

BACKGROUND: There is still debate regarding whether the remnant tendon on the greater tuberosity should be removed during rotator cuff repair (RCR). This study aimed to evaluate whether remnant preservation (RP) in RCR leads to better clinical outcomes. METHODS: Medical records of 833 consecutive patients with rotator cuff tears (RCTs) between December 2019 and April 2022 were retrospectively reviewed. Patients were divided into a RP group and a conventional repair (CR) group, and 1:1 propensity score matching was performed based on age, sex, tear size, tear type, repair method, and fatty inf

Clinical and radiological outcomes of callus preservation versus excision in gunshot-related distal humerus fractures.

BACKGROUND: High-energy gunshot injuries to the distal humerus frequently result in extensive comminution, severe soft-tissue damage, and contamination, making definitive fixation both technically demanding and biologically challenging. During staged management with temporary stabilization, extra-articular callus formation may occur prior to definitive fixation, potentially influencing the surgical decision to preserve or excise this tissue. This study aimed to evaluate the clinical and radiological outcomes of a fixation strategy that preserves extra-articular callus in high-energy distal hum

Initial static, pseudo-static, dynamic, and cognitive fit evaluation of three passive shoulder exoskeletons while performing simulated manufacturing tasks.

Shoulder disorders have been associated with repetitive overhead work and manual lifting. Passive exoskeletons may reduce exposure to physical risk factors associated with shoulder injuries. This repeated-measures study evaluated the initial static, pseudo-static, cognitive, and dynamic fit of three passive shoulder exoskeletons compared to a no-exoskeleton condition. Twenty-four participants (sex-balanced) performed range of motion tasks, simulated manufacturing activities, and a cognitive performance test. Results indicated that sex did not significantly affect any fit outcomes. Cognitive pe

Long-term functional and radiographic outcomes after removal of radial head arthroplasty.

PURPOSE: If reconstruction of the radial head using osteosynthesis is not possible, radial head arthroplasty (RHA) is performed. Despite the increasing use of RHA, little reliable data is available on the functional and clinical outcomes following removal of RHA in a mid- to long-term follow-up. METHODS: This study included 27 patients who underwent removal of RHA between 2010 and 2021. The mean follow-up period was 8.8 years (2.0 to 14.2 years). The time to removal, as well as functional elbow scores (Mayo Elbow Performance Score (MEPS), Oxford Elbow Score (OES) and Disabilities of arm, shou

Trabecular titanium metaglene is associated with improved clinical outcomes in reverse shoulder arthroplasty for proximal humeral fractures in elderly patients.

INTRODUCTION/OBJECTIVES: There is currently conflicting evidence regarding the effectiveness of reverse shoulder arthroplasty (RSA) in the treatment of proximal humeral fractures. Trabecular titanium (TT) is a highly porous biomaterial increasingly adopted in joint arthroplasty, particularly in cases with poor bone stock and compromised bone quality. This study aims to evaluate the effectiveness of TT and the clinical and radiological outcomes in the treatment of complex proximal humeral fractures using RSA as the first choice in elderly patients. We evaluated the differences and advantages of

Minimum 15-Year Outcomes and Return to Activity After Linked Tape Bridging Knotless Double-Row Rotator Cuff Repair.

BACKGROUND: Rotator cuff tears are common in older adults, and double-row repair is favored for its biomechanical strength and clinical outcomes. Although short-term and midterm results of linked tape bridging (LTB) knotless double-row repair are well established, data on long-term single-technique outcomes remain limited. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate the long-term clinical and functional outcomes of knotless double-row rotator cuff repair. It was hypothesized that LTB knotless double-row rotator cuff repair would provide sustained functional outcomes but woul

Management of Rope Friction Injuries of the Hand.

BACKGROUND: Rope friction injuries result from rapid, repetitive contact with coarse ropes or chains, causing combined mechanical and thermal damage. These injuries commonly affect young individuals during activities such as tug-of-war, climbing, air acrobatics, and rope handling. Although superficial and deep burns are most frequent, injuries may also involve vessels, nerves, tendons, and bone. This study presents our experience managing rope friction burns and reviews functional and aesthetic outcomes. METHODS: We retrospectively reviewed 23 patients with full-thickness hand defects after r

The Scandinavian Displaced Lateral Clavicle trial (ScanDiLaC): a study protocol for a randomized clinical trial.

BACKGROUND: Evidence regarding the treatment of displaced, extraarticular lateral clavicle fractures is scarce. No study has shown clinically significant differences between surgical and nonsurgical treatment, but the sample sizes have been small, as it has been difficult to include enough patients with this relatively uncommon fracture type. PURPOSE: This study aims to compare outcomes after surgical and nonsurgical treatment for displaced lateral clavicle fractures. METHODS: This is a pragmatic, noninferiority, preference-tolerant, randomized controlled trial (RCT). A total of 100 patients

Compensatory activation of the deltoid and remaining rotator cuff muscles in patients with rotator cuff tears using real-time tissue elastography.

INTRODUCTION: Rotator cuff (RC) tears are a prevalent condition leading to dysfunction and shoulder pain. Spatial changes in the behavior of the deltoid and remaining RC muscles during arm elevation in patients with RC tears have not been investigated. The purpose of this study was twofold: (1) to investigate factors related to the changes in the deltoid muscle behavior during scapular plane abduction (scaption) in patients with RC tears, and (2) to examine the functional relationship between the deltoid and remaining RC muscles, particularly remaining infraspinatus (ISP) and teres minor (TMin

Association between patient-reported outcomes and accelerometer-based activity in elderly patients 12 months after proximal humerus fracture: Results from two randomized controlled trials.

AIMS: Patient-reported outcome measures (PROMs) represent outcomes important to patients and are subjective rather than objective measures like mortality or reoperations. Their use has, however, limitations. We aimed to investigate the association between PROMS and accelerometer-based movement in older adults with proximal humerus fractures. METHODS: This is a secondary analysis of two previously conducted randomized controlled trials. The first trial included patients aged 60 years or more with any proximal humerus fracture allocated to nonoperative treatment or operative treatment. The seco

Upper Extremity Return to Sport Functional Testing: A Systematic Review.

BACKGROUND: Upper extremity return to sport (RTS) assessments are not standardized with respect to which metrics to use, if the metrics are appropriate for all populations and levels of competition, and what constitutes good or poor test performance. Subsequently, clinicians may utilize suboptimal metrics to evaluate RTS readiness, or forgo objective criteria altogether. The purpose of this study was to examine the psychometric properties (reliability, agreement/measurement error, hypothesis testing/construct validity, criterion/predictive validity, responsiveness) of upper extremity functiona
