Clinical outcomes of a defect-adapted strategy for free fibular great toe flaps in finger reconstruction: a retrospective series of 35 cases.

BACKGROUND: To evaluate the clinical outcomes of a personalized surgical strategy for small-to-medium finger defects, utilizing either free dorsal toe or plantar proper digital artery flaps. METHODS: We retrospectively analyzed 35 patients with finger defects (March 2021-January 2025). Patients were assigned by defect characteristics to Group A (pulp defects, n = 21; plantar proper digital artery flap) or Group B (dorsal/lateral defects, n = 14; modified dorsal toe artery flap preserving the plantar proper neurovascular bundle). We evaluated operative time, flap survival, Disabilities of the

Outcome analysis of conservative treatment of a distal radius fracture with OPTIVOhand orthosis versus plaster cast: a randomized controlled trial.

BACKGROUND: Distal radius fractures (DRF) are often immobilized using a conventional plaster cast although orthoses offer a time- and resource-saving alternative. This prospective randomized study compared the conservative DRF treatment using an orthosis (OPTIVOhand®) with plaster cast immobilization. Besides the maintenance of reduction result (primary endpoint), functional and subjective outcomes (secondary endpoints) were examined. METHODS/ DESIGN: 53 patients with isolated DRF were randomized to the orthosis group (OG) or the control group (CG). The follow-up examinations included radiolo

PROMIS computerized adaptive testing is effective and efficient for outcome assessment in patients with wrist or hand fractures.

INTRODUCTION: Various patient-reported outcome measures (PROMs) are available to assess functional outcomes in patients treated for wrist-hand fractures. Among the most frequently applied instruments are the Patient-Rated Wrist/Hand Evaluation (PRWHE), Quick Disabilities of the Arm, Shoulder and Hand and Short Musculoskeletal Function Assessment. Computerized adaptive tests (CATs) utilizing the PROM-Information System (PROMIS) offers a standardized alternative. This study compared the psychometric properties of PROMIS CATs with these three legacy instruments and assessed independent factors as

Arthroscopic assisted lower trapezius transfer for the treatment of irreparable posterosuperior rotator cuff tears with nonfunctional teres minor: are the results sustained after more than 7.5 years?

BACKGROUND: Lower trapezius transfer (LTT) has been demonstrated to restore external rotation (ER) in patients with brachial plexus palsies. In certain cases of cuff tear arthropathy, patients exhibit preserved forward elevation but lack active ER. This study evaluates the clinical outcomes of LTT, augmented with a semitendinosus tendon autograft and anchored to the infraspinatus insertion using an arthroscopically assisted technique and, to determine whether pain relief and functional improvement are maintained at minimum 7.5-year follow-up compared to the 11-month follow-up. METHODS: Betwee

Have Commonly Utilised Patient-Reported Outcome Measures Been Validated for the Japanese Language and Culture Amongst Patients with Distal Radius Fractures? A Systematic Review.

Background: Patient-reported outcome measures (PROMs) are questionnaires used to evaluate a patient's functional status, examine outcomes and inform clinical decisions. PROMs have been primarily developed with English-speaking populations and should be validated in the patient population in whom they are being applied. As present, the validity of commonly utilised distal radius fracture (DRF) PROMs in patients who speak Japanese is unknown. The purpose was to evaluate the validity and psychometric properties of common DRF PROMs in the Japanese population. Methods: A systematic review was condu

Management of undisplaced or minimally displaced distal radius fractures in adults: immobilization with circumferential casting versus plaster splinting.

BACKGROUND: Distal radius fractures (DRFs) are among the most common fractures in adults, with a significant proportion being stable, non-displaced, or minimally displaced. These fractures generally have a low risk of secondary displacement. This study aimed to compare the clinical and radiological outcomes of short-arm circular casting and volar splint application in the conservative treatment of stable distal radius fractures. Additionally, the study evaluated complication rates, patient comfort, and the feasibility of these two immobilization techniques in emergency department settings. ME

Comparison of treatment modalities for distal radius fractures in terms of functional outcomes pain management and grip strength.

Distal radius fractures are among the most common fractures in adults and require treatment approaches tailored to fracture characteristics and patient needs. The primary objective of this study was to compare the 6-month Patient-Rated Wrist Evaluation (PRWE) score across three treatment modalities—closed reduction and short-arm casting, external fixation, and volar plate fixation. The null hypothesis was that there would be no difference in PRWE outcomes among the three groups. Secondary objectives included evaluating pain intensity, functional disability, grip strength, and wrist range of mo

Mid-term functional outcomes of arthroscopy-assisted lower trapezius transfer using doubled peroneus longus tendon.

BACKGROUND: In relatively young and active patients without glenohumeral arthritis, massive irreparable rotator cuff tears (MIRCT) can be treated with tendon transfer. Arthroscopy-assisted lower trapezius transfer (LTT) for MIRCT needs an allograft such as the Achilles tendon, to bring its attachment to the greater tuberosity. The availability of allografts is limited, particularly in developing countries. The objective of our study is to describe the functional outcomes of arthroscopy-assisted lower trapezius transfer with the peroneus tendon autograft in MIRCT. MATERIALS AND METHODS: This i

Optimising the utility of patient-reported outcome measurements during recovery after orthopaedic extremity fractures: protocol for a multicentre prospective cohort study.

INTRODUCTION: Recovery after extremity fracture is influenced by injury-related impairments, functional capacity, symptom severity and psychosocial well-being, emphasising the need for patient-reported outcome measures (PROMs). Despite increasing adoption of Patient-Reported Outcomes Measurement Information System (PROMIS) and legacy PROMs in orthopaedic research, substantial knowledge gaps remain regarding their measurement properties, comparability and clinical utility, particularly in fracture populations. Our aim is to develop meaningful guidance to clinicians regarding the use of PROMs in

What factors influence outcomes in olecranon fractures in older adults? A cohort study of operative and nonoperative management.

BACKGROUND: Olecranon fractures occur more commonly in older individuals. For patients aged ≥70 years, operative management is often considered standard of care, although recent evidence has supported nonoperative treatment in frail and/or elderly patients. With evolving treatment indications, more granular evidence is beneficial to guide patient-specific decision-making. The aim of this investigation was to explore patient and treatment factors associated with outcomes after displaced, closed olecranon fractures in older individuals. METHODS: This retrospective cohort study with prospective
